The most common thing that catches medics out with the unconscious patient is not checking their blood sugar to see if they are hypoglycaemic says Dr Katherine Henderson, consultant in emergency medicine, Guy's and St Thomas' NHS Foundation Trust.
Once you have done the obvious checks and made the patient as safe as possible, you have to play detective because you cannot get a history from them. ‘You must ask yourself, I've given them sugar and they haven't woken up, so what could be the cause and how am I going to prove or disprove the potential diagnosis?’ says Dr Henderson. You will need to consider a wide differential diagnosis. Many of the tests you need are going to be fairly fundamental. The chances are you're going to request some brain imaging but you need to think about the diagnosis carefully to make sure that you get that the maximum benefit from the test and choose the right modality.

Point of care ultrasound (POCUS) is revolutionising acute care in the UK by improving the speed and accuracy of the diagnosis of the acutely unwell patient at the front door.
He will be illustrating his lecture with some interesting case studies of both normal findings and some of the more interesting pathology that the machines can identify. ‘I will be presenting cases where point of care ultrasound has been used in acute and emergency care to change the management of the patient and has potentially saved their lives,’ he says. Dr Smallwood developed a specialist interest in POCUS during his training and became a member of a working group which developed Focused Acute Medicine Ultrasound (FAMUS), a bespoke POCUS curriculum for those looking after acutely unwell medical patients. He will be explaining to delegates how people can do the training.
