Burnt out doctors struggling to find time for essential training, GMC survey shows By Mike Broad, HospitalDr Nearly a quarter of trainee doctors say their work makes them feel ‘burnt out’, and almost one in three say they are often ‘exhausted’ in the morning at the thought of another shift, according to a GMC survey. The nationwide poll of trainees shows that nearly half report regularly working beyond their

rostered hours, and around one in five say they often feel short of sleep while at work. Forty per cent described the intensity of their work as ‘heavy or very heavy’. Charlie Massey, Chief Executive of the GMC, said: “Doctors are working in highly pressured environments and they are telling us that as a result they are struggling to find time for essential training. This is a major concern for us. Training must be protected and it

What will your talk at Acute & General Medicine be about?

I will be talking about landmark new treatment for acute stroke. The biggest advance in the last 18 months has been the use of using state-of-the-art imaging to see how much of the brain has been affected by a stroke and which areas are still salvageable. Studies have demonstrated that if you use imaging appropriately you can identify people who may be having a big stroke but in whom only a small part of the brain is irretrievably damaged. If you can pull out the clot quickly, you can significantly improve their outcome and maintain their independence. This is going to revolutionise stroke treatment.

Dealing with delirium. I will focus on the latest evidence on the assessment and management of delirium and I will be giving some tips on how to manage it well. Delirium is common medical emergency. It’s an acute state of confusion which happens particularly to older people when they are medically unwell and is a common reason for a change in behaviour.

Why is this topic so important? At the moment this procedure is only being carried out in a few major centres but it will eventually be rolled out across the country and medical professionals in the widest sense (not just doctors) will need to collaborate in order to make this happen. Who will benefit from attending this presentation? This talk is for clinicians in acute medicine and emergency departments who might use our stroke services, particularly people who might see a patient overnight and need to know about the treatment possibilities. GPs will also benefit from knowing what treatments are available. Why did you choose to specialise in neurology? I was inspired at university by my teachers at Trinity College, Cambridge, which has a very proud tradition in neuroscience and boasts several Nobel Prize

winners in neuroscience. I decided to specialise in stroke because at that stage it was rather neglected. It is a very common and devastating disease but it was one of the few neurological conditions carefully applied treatment and prevention could really make a difference. What do you enjoy about your work? I enjoy the challenge of making the diagnosis, justifying every decision I make with the available evidence and having to do that very quickly, sometimes flying by the seat of my pants! What keeps you awake at night? I worry about the future of stroke care in the UK. It sometimes feels as though stroke doesn’t have a home within a particular specialty. We have fantastic collaboration between specialists from neurology, geriatrics and acute medicine but there only needs to be a little change in emphasis in all three disciplines and all of a sudden the supply of stroke specialists could dry up. This is a concern because stroke is a common cause of death and the commonest cause of adult disability. Therefore, we need enough people contributing to the service for it to be sustainable and to improve the health of the nation.

Why is this topic so important? Two thirds of patients in an acute hospital at any one time will be over the age of 65 - half of those will have some form of cognitive impairment and a large proportion will have delirium. Wherever you work in the hospital you will come across patients who have delirium, and there will often be a medical reason for the patient’s change in behaviour so it’s incredibly important to pick it up and treat it. Another reason is that the mortality rate from delirium is high and it can make it very difficult to provide care at home or in the care home because people can become very agitated and confused. Who will benefit from attending this presentation? Generalists. Many people hearing this talk for the first time will recognise patients that they have looked after as having delirium. This will also be a good revision session for consultants who are working with patients across the hospital. What do you enjoy about your work?

In my work I have to problem-solve on the hoof then put in place plans that will have an impact on somebody’s wellbeing. I will follow that patient right through to discharge from hospital. What keeps you awake at night? Sometimes I worry about the patients who have been discharged and I wonder if the arrangements I have made for them in the community are working out.

But it will take investment to solve the issues doctors are telling us about. Those responsible for allocating healthcare funding across the UK must ensure proper provision is made for education and training.” The GMC is now analysing the results in more detail, and working with education providers to make sure improvements are made where training falls below expected standards. Dr David Evans, Vice President for Training and Assessment at the Royal College of Paediatrics and Child Health, commented: “This survey raises serious concerns about the wellbeing of trainee doctors. We know that for paediatric trainees, workforce pressures and falling recruitment numbers have a major impact on morale. Almost 1 in 5 paediatric trainee positions are currently vacant, leading to severely difficult working environments where young doctors feel stressed and lack time for vital training and education. “We need funding for an increase in the number of paediatric trainee places to 465 in each training year for the next 5 years in order to see a

sufficient expansion of the workforce. We also ask that the wellbeing of our trainees is valued and the quality of their training is enhanced; protected time for training is essential to ensure they gain the educational opportunities they need to progress in their careers. This survey should be a wake-up call for governments across the UK, who must act without delay to support trainee doctors and ensure a safe, sustainable health service for everyone.”

It will cover the diagnosis, staging and treatment of NAFLD and delegates will learn how to identify the people who have the most severe form of the disease who need urgent treatment. Dr McPherson will also discuss some new drugs currently in development that can reduce inflammation in the liver and reverse liver fibrosis. Around 50 compounds are currently in phase one to three trials at the moment and the first drugs could be licensed by 2020. However he says even when these new drugs come on stream the first line treatment for NAFLD patients should still be lifestyle intervention. “If a patient does not have cirrhosis of the liver their

condition can be reversed if they can lose 10 per cent of their body weight. The problem is that only 10 per cent of patients manage to lose 10 per cent of their body weight. About half of patients will lose some weight, but not enough to actually reverse the condition. However even some weight loss will reduce liver fat and inflammation and slow progression of the disease. “My view as a clinician is that it is much better for someone to get a bike and exercise and lose the weight, because that has many more benefits than taking a drug that only targets the liver,” he says. This talk will be relevant for doctors in both primary and secondary care.

The aim of NBC is to place all patients on a ward based on their medical needs regardless of their age to ensure specialist staff can provide the most appropriate care around the clock, every day of the week. With the increased level of dedicated care and monitoring, NBC will help reduce the risk of patients deteriorating over evenings or weekends when there is no or consultant cover and will therefore significantly reduce the length of stay for many medical patients. The introduction of NBC will see a complete overhaul in the way care is provided resulting in numerous benefits for patients. Most importantly, by ensuring patients have access to the most appropriate medical team 24 hours a day, seven days a week will provide much improved continuity of care which is directly linked to a reduction in patients’ length of stay in hospital. Specialist medical teams will immediately be appointed to patients after admission regardless of age, psychological or physical needs, to ensure that the most appropriate treatment and diagnostics can

begin as soon as possible. All of this will improve patient experience. NBC will also create a large number of new patient facing roles. The L&D are recruiting teams of physiotherapists, occupational therapists, consultants, nurses and pharmacists to work as part of this fantastic new initiative.
