Welcome to Best Practice 2016, the NAPC’s Annual Conference to support and inspire all those engaged in building stronger, sustainable, patient-centred primary care. The challenges to achieving this are colossal, the opportunities boundless and the consequences of failure, inconceivable. As this conference opens its doors for the fourth time on October 19, England will be celebrating FAB Change Day 2016 – when the entire NHS workforce takes collective action to improve our treasured service. The coincidence of these two events couldn’t be more fitting. The NAPC is an organisation committed to driving positive change, as demonstrated by its work in developing the innovative Primary Care Home (PCH), launched at Best Practice last year by NHS Chief Executive Simon Stevens. The Lisa Thomlinson PCH is a unique model of out-of-hospital care focussed on strengthening and redesigning primary care around the health and social care needs of local communities, drawing on the skills of a multidisciplinary workforce. A year on, you will be among the first to hear about the rapid progress that is being made and next steps. This time last year, the NAPC launched its Primary Care Home model of care at Best Practice 2015 with endorsement from Simon Stevens, as a solution to engaging primary care in its development of delivering care at a local level. It’s been a big year of change, not only for NAPC but the 15 Rapid Test Sites selected to test the model.
I welcome you to visit the NAPC Zone and hear more from our Primary Care Home Rapid Test Sites who will be sharing their journey and learning and will also feature in the New Care Models stream. We also showcase how our innovation projects are progressing and the real difference they are making to patients but also in building strength across a multidisciplinary workforce. And finally I welcome representation from the Practice Managers Network that NAPC now have the great privilege of hosting. So with recent comment from the NHS that we need to “get on with the task” of reshaping the services in line with the Five Year Forward View, NAPC are at the practical forefront of that ethos and getting on with walking that walk of integrated care provision.

Indication: Topical treatment of psoriasis vulgaris in adults. Active ingredients: 50 µg/g calcipotriol (as monohydrate) and 0.5 mg/g betamethasone (as dipropionate). Dosage and administration: Apply by spraying onto affected area once daily. Recommended treatment period is 4 weeks. The daily maximum dose of Enstilar should not exceed 15 g, i.e. one 60 g can should last for at least 4 days. 15 g corresponds to the amount administered from the can if the actuator is fully depressed for approximately one minute. A twosecond application delivers approximately 0.5 g. As a guide, 0.5 g of foam should cover an area of skin roughly corresponding to the surface area of an adult hand. If using other calcipotriol-containing medical products in addition to Enstilar, the total dose of all calcipotriol-containing products should not exceed 15 g per day. Total body surface area treated should not exceed 30%. Safety and efficacy in patients with severe renal insufficiency or severe hepatic disorders have not been evaluated. Safety and efficacy in children below 18 years have not been established. Shake the can for a few seconds before use. Apply by spraying, holding the can at least 3 cm from the skin, in any orientation except horizontally. Spray directly onto each affected skin area and rub in gently. Wash hands after use (unless Enstilar is used to treat the hands) to avoid accidentally spreading to other parts of the body. Avoid application under occlusive dressings since systemic absorption of corticosteroids increases. It is recommended not to take a shower or bath immediately after application. Contraindications: Hypersensitivity to the active substances or any of the excipients. Erythrodermic and pustular psoriasis. Patients with known disorders of calcium metabolism. Viral (e.g. herpes or varicella) skin lesions, fungal or bacterial skin infections, parasitic infections, skin manifestations in relation to tuberculosis, perioral dermatitis, atrophic skin, striae atrophicae, fragility of skin veins, ichthyosis, acne vulgaris, acne rosacea, rosacea, ulcers and wounds. Precautions and warnings: Adverse reactions found in connection with systemic corticosteroid treatment, e.g. adrenocortical suppression or
impaired glycaemic control of diabetes mellitus, may occur also during topical corticosteroid treatment due to systemic absorption. Application under occlusive dressings should be avoided since it increases the systemic absorption of corticosteroids. Application on large areas of damaged skin, or on mucous membranes or in skin folds should be avoided since it increases the systemic absorption of corticosteroids. Due to the content of calcipotriol, hypercalcaemia may occur. Serum calcium is normalised when treatment is discontinued. The risk of hypercalcaemia is minimal when the maximum daily dose of Enstilar (15 g) is not exceeded. Enstilar contains a potent group III-steroid and concurrent treatment with other steroids on the same treatment area must be avoided. Skin on the face and genitals are very sensitive to corticosteroids. Enstilar should not be used in these areas. Instruct the patient in the correct use of the product to avoid application and accidental transfer to the face, mouth and eyes. Wash hands after each application to avoid accidental transfer to these areas. When lesions become secondarily infected, they should be treated with antimicrobiological therapy. However, if infection worsens, treatment with corticosteroids should be discontinued. When treating psoriasis with topical corticosteroids, there may be a risk of rebound effects when discontinuing treatment. Medical supervision should therefore continue in the posttreatment period. Long-term use of corticosteroids may increase the risk of local and systemic adverse reactions. Treatment should be discontinued in case of adverse reactions related to long-term use of corticosteroid. There is no experience with the use of Enstilar in guttate psoriasis. During Enstilar treatment, physicians are recommended to advise patients to limit or avoid excessive exposure to either natural or artificial sunlight. Topical calcipotriol should be used with UVR only if the physician and patient consider that the potential benefits outweigh the potential risks. Enstilar contains butylhydroxytoluene (E321), which may cause local skin reactions (e.g. contact dermatitis), or irritation to the eyes and mucous membranes. Pregnancy and lactation: There are no adequate data from the use of Enstilar in pregnant women. Enstilar should only be used during pregnancy when the potential benefit justifies the potential risk. Caution should be exercised when prescribing Enstilar to women who breast-feed. The patient should be instructed not to use Enstilar on the breast when breast-feeding.
Side effects: There are no common adverse reactions based on the clinical studies. The most frequently reported adverse reactions are application site reactions. Calcipotriol: Adverse reactions include application site reactions, pruritus, skin irritation, burning and stinging sensation, dry skin, erythema, rash, dermatitis, psoriasis aggravated, photosensitivity and hypersensitivity reactions, including very rare cases of angioedema and facial oedema. Systemic effects after topical use may appear very rarely causing hypercalcaemia or hypercalciuria. Betamethasone (as dipropionate): Local reactions can occur after topical use, especially during prolonged application, including skin atrophy, telangiectasia, striae, folliculitis, hypertrichosis, perioral dermatitis, allergic contact dermatitis, depigmentation and colloid milia. When treating psoriasis with topical corticosteroids, there may be a risk of generalised pustular psoriasis. Systemic reactions due to topical use of corticosteroids are rare in adults; however, they can be severe. Adrenocortical suppression, cataract, infections, impaired glycaemic control of diabetes mellitus, and increase of intra-ocular pressure can occur, especially after long-term treatment. Systemic reactions occur more frequently when applied under occlusion (plastic, skin folds), when applied on large areas, and during long-term treatment. See SmPC for a full list of side effects. Precautions for storage: Do not store above 30°C. Extremely flammable aerosol. Pressurised container. May burst if heated. Protect from sunlight. Do not expose to temperatures exceeding 50°C. Elleste SoloTM MX 40 mcg transdermal patch containing 1.25 mg estradiol hemihydrate and Elleste SoloTM MX 80 mcg Transdermal Patch containing 2.5 mg estradiol hemihydrate and delivers 40 and 80 micrograms of estradiol respectively per 24 hours. Elleste DuetTM 1 mg film-coated tablets containing estradiol hemihydrate 1 mg (white tablets) and estradiol hemihydrate 1 mg and norethisterone acetate 1 mg (pale green tablets). Elleste DuetTM 2 mg film-coated tablets containing estradiol hemihydrate 2 mg (orange tablets) and estradiol hemihydrate 2 mg and norethisterone acetate 1 mg (grey tablets). Prevention of osteoporosis in post-menopausal women at high risk of future fractures who are intolerant of, or contraindicated for, other medicinal products approved for the prevention of osteoporosis (Elleste DuetTM Conti; Elleste DuetTM 2 mg; Elleste SoloTM 2 mg; Elleste SoloTM MX 80 patch). DOSAGE AND ADMINISTRATION: Elleste SoloTM 1 mg and 2 mg film-coated tablets: One tablet daily in hysterectomised women; in women with an intact uterus, progestogen should be added for 12-14 days each cycle. Elleste SoloTM MX 40 and 80 patch: initiate treatment with Elleste SoloTM MX 40 in women with menopausal symptoms. If menstruating regularly, start within five days of bleeding. See SmPC for details of switching from other forms of HRT. Apply one patch twice weekly; in women with an intact uterus, progestogen should be added for 12-14 days during each cycle. The dosage may be increased by using Elleste SoloTM MX 80. Elleste Duet 1 mg film-coated tablets: One white tablet taken daily for 16 days followed by one pale green tablet taken daily for 12 days, then begin a new cycle without a break. Elleste DuetTM 2 mg film-coated tablets: One orange tablet taken daily for 16 days followed by one grey tablet taken daily for 12 days, then begin a new cycle without a break. Elleste DuetTM Conti: One tablet taken daily. CONTRA-INDICATIONS: Pregnancy or breastfeeding. Known, past or suspected breast cancer. Known or suspected oestrogen-dependent malignant tumours. Undiagnosed genital bleeding. Untreated endometrial hyperplasia. Active thrombophlebitis. Previous idiopathic or current venous thromboembolism. Known thrombophilic disorders. Active or recent arterial thromboembolic disease. Acute liver disease or history of liver disease as long as LFTs are abnormal. Dubin-Johnson or Rotor Syndromes. Hypersensitivity to the active substances or excipients. Porphyria.
WARNINGS AND PRECAUTIONS: HRT should only be initiated for symptoms that adversely affect quality of life. In all cases, a careful appraisal of the risks and benefits should be undertaken at least annually. HRT should only be continued as long as the benefit outweighs the risk. Before initiating or reinstituting HRT, take a complete personal and family medical history and perform appropriate physical examinations. Advise women about what breast changes should be reported. Closely supervise women with the following conditions or a history of them: leiomyoma or endometriosis; history of, or risk factors for, thromboembolic disease; risk factors for oestrogen dependent tumours; hypertension; liver disorders; diabetes mellitus; cholelithiasis; migraine or severe headache; systemic lupus erythematosus; endometrial hyperplasia; epilepsy; asthma; otosclerosis. Discontinue therapy if a contraindication is discovered and in the following conditions: hepatitis, jaundice or deteriorating liver function; significant increase in blood pressure; sudden severe chest pain; sudden breathlessness; unexplained swelling or pain in calf; severe stomach pain; prolonged immobility after surgery or leg injury; new onset migraine-type headache; pregnancy. Risk of endometrial hyperplasia and carcinoma are increased when oestrogens are administered alone for prolonged periods. The risk is reduced with the addition of a progestogen for at least 12 days per cycle in non-hysterectomised women. Investigate breakthrough bleeding. An increased risk of breast cancer has been reported that is dependent on the length of treatment. HRT can increase the density of mammographic images which may affect radiological detection of breast cancer. HRT is associated with an increased relative risk of venous thromboembolism (VTE) or pulmonary embolism (PE). Risk factors include personal or family history of thrombosis, severe obesity, systemic lupus erythematosus, immobilisation, major trauma and major surgery. Consider discontinuing HRT 4-6 weeks before elective surgery requiring immobilisation. Therapy should be discontinued if VTE develops after initiating surgery. There is an increased risk of cardiovascular morbidity during the first year of use of HRT. HRT is associated with an up to 1.5 fold increased risk of stroke. Long term use of oestrogens in hysterectomised women has been associated with an increased risk of ovarian cancer. Oestrogens may cause fluid retention. Women with pre-existing hypertriglyceridemia should be followed closely (risk of pancreatitis). Certain endocrine tests may be affected. No evidence for improvement in cognitive function. Increased risk of gallbladder disease. Liver tumours leading to intra-abdominal haemorrhage have been reported. Patients with rare hereditary disorders of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption should not take this medicine. Elleste Duet 2 mg film-coated tablets and Elleste Solo 2 mg film-coated tablets contain sunset yellow colouring (E110) which can cause allergic reactions. May interact with other medicines. See SmPC for potential interactions. SIDE EFFECTS: Common side effects (>1/100): Erythema, itching; uterine bleeding;
breast tenderness and enlargement; increase in size of uterine fibroids; nausea; abdominal pain; headache; weight increase / decrease; oedema; change in mood including anxiety and depressive mood; change in libido. Other side effects include: Vaginal candidiasis; vomiting; gallbladder disease; pancreatitis, gallstones; dizziness; migraine; increased blood pressure; leg cramps; alopecia; hirsutism; rash; itching; venous thromboembolism; thrombophlebitis; thrombosis; endometrial neoplasia; dysmenorrhoea; aggravation of endometriosis; changes in cervical eversion, production of mucus and erosion; cystitis-like syndrome; endometrial cancer; breast cancer; bloating; myocardial infarction; stroke; liver tumours; cholestatic jaundice; chloasma; erythema multiforme; erythema nodosum; muscle cramps; vascular purpura; steepening of corneal curvature; visual disturbances; intolerance to contact lenses; sodium and water retention; reduced glucose tolerance; aggravation of porphyria and probable dementia. Easyhaler Budesonide is suitable for once daily dosing in adults and children aged 6 years of age and older with mild to moderate asthma - see SmPC for more details.
Prescribing Information Easyhaler® Budesonide 100 micrograms, 200 micrograms and 400 micrograms/dose inhalation powder (budesonide) Indication: Prophylactic management of mild, moderate or severe persistent asthma. Dosage: Adjust according to individual patient need including transfer from other inhaler devices. Mild asthma: Adults (including the elderly and adolescents 12-17 years) and for children 6-11 years: 200400 mcg/day, up to 800 mcg/day in two divided doses. Moderate and severe asthma: Adults: up to 1600 mcg/ day. Maintenance dose: Twice daily dosing adults: 100-400 mcg twice daily. During periods of severe asthma, daily dose may be increased up to 1600 mcg in two divided doses and reduced when asthma has stabilised. Children aged 6-11 years: 100-200 mcg twice daily up to 800 mcg in two doses. Dose can be reduced when asthma has stabilised. Once daily dosing in mild to moderate asthma – In steroid naïve patients: 200-400 mcg. In patients already controlled on inhaled corticosteroids: up to 800 mcg, children aged 6-11 years: 200-400 mcg. The once daily dose should be taken in the evening. Transfer of patients treated with oral corticosteroids to the inhaled corticosteroid and their subsequent management requires special care – see SPC. Rinse mouth with water or brush teeth after inhaling to minimise risk of oropharyngeal candidiasis and hoarseness. Contraindications: Hypersensitivity to budesonide or milk

proteins. Warnings and precautions: Not for treatment of acute dyspnoea or status asthmaticus. Must be used regularly and should not be stopped abruptly. Patients who have required high dose emergency steroid therapy or prolonged treatment with high doses of inhaled steroids may be at risk of impaired adrenocortical function and may need supplementary systemic corticosteroid during periods of stress. Paradoxical bronchospasm may occur. Systemic effects may occur with high doses of inhaled corticosteroids. Titrate dose to lowest effective level. Reduced liver function affects the elimination of corticosteroids. Avoid concomitant ketoconazole, HIV protease inhibitors or other potent CYP3A4 inhibitors. Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose galactose malabsorption should not take this medicine. Regularly monitor height of children receiving prolonged inhaled corticosteroids Fertility, pregnancy and lactation: Balance benefits against risk Undesirable effects: Common - Cough and throat irritation, oropharyngeal candidiasis, difficulty in swallowing. Rare - Hypersensitivity reactions including rash, urticaria, contact dermatitis, angioedema and anaphylactic reaction, hypocorticism, adrenal suppression, growth retardation, depression, behavioural changes (mainly in children), restlessness, nervousness, hoarseness, dysphonia, bronchospasm, pruritus, erythema, bruising. Very rare -Cataract, glaucoma, decreased bone density. Not known- Psychomotor hyperactivity, sleep disorders, anxiety,
aggression, irritability, psychosis. Legal Category: POM. Get your CPR & Anaphlylaxis accreditation while at Best Practice in Nursing! Red Cross first aid trainers will be demonstrating how to help someone who is unresponsive and not breathing (CPR), along with the recognition and treatment of someone who is having an anaphylactic reaction. The workshops will run 4 times a day and will feature a presentation and practical assessments in accordance with the latest guidance. Due to the change in policy in CPR administration this year, you must attend a 15 minute presentation before undertaking your practical assessment. After the presentation, you will be handed a time card for your practical assessment. Running in parallel to the Best Practice conference for GPs and practice managers, Best Practice in Nursing brings you keynote speakers from all the major nursing organisations – the NMC, the RCN, NHS England and Health Education England ¬– as well as leading academic institutions. The conference opens with keynote addresses from among others, Donna Kinnair, Director of Nursing, Policy and Practice at the RCN, Sara Kovach Clark, head of Revalidation at the NMC, and Crystal Oldman, Chief executive of the Queen’s Nursing Institute. Our expert-led programme provides a broad range of in-depth clinical topics as well as quick updates and refreshers, and covers national policy as well as personal and professional development. It also offers a unique opportunity to meet the majority of your annual mandatory training requirements, all in one go. The conference will tackle the pressing need to recruit and retain nurses in primary and community care head on, with workshops exploring new ways of working and new models of service delivery.
