NHS ENGLAND INVESTS IN LARGE SCALE HEALTH AND RECORD SHARING NHS England has invited regional health and care collaborative communities to bid for national investment to develop shared health and care records.

The LHCRE programme is intended to advance large scale information sharing across a geography of a 2-5 million population area.

Each regional LHCRE will be expected to build on existing local work on shared records to further develop joined up

regional health and care information reference sites, focused on improving direct patient care.

The aims of the LHCRE programme are to support direct patient care then extend to integrating health and care and improving care co-ordination. This will provide a foundation for future health analytics and population health management and while also enabling patient engagement and activation.

An electronic health record (EHR) built by Leeds Teaching Hospitals, which connects over 35 IT systems across health and social care, has created an integrated care record for 2.8 million patients.

Leeds is one of three NHS trust which have built their own EHR rather than procuring one.

Developed over 15 years, the EHR is improving efficiency and safety enabling clinical and care staff to provide personalised, place based care more quickly and effectively.

The system, known as PPM+, has been designed from the outset

DIGITAL CHAMPIONS ARE NEEDED TO BREAK DOWN NHS BARRIERS Deploying new technology in the NHS is a challenge that must be overcome to ensure that the system harnesses all the benefits that innovation can bring for patient care.

This is the view of Professor Mike Hannay, Managing Director of the East Midlands Academic Health Science Network (AHSN) and Chair of the national AHSN Network, who will be addressing this issue in a debate at the Digital Healthcare Show.

He says the key to introducing innovation is to present the technical evidence in a clear and simple way that engages clinicians, financial managers and commissioners.

Healthcare organisations can bid to become one of five new Local Health and Care Record Exemplars (LHCRE) and receive funding of up to 7.5m over 2 years, to be locally match funded.

Three areas will be chosen for Digital Innovation Hub status.

Then champions are needed at all levels of the system to ensure the innovation rolls out and embeds at the clinical coalface.

There is also a need to carefully introduce digital technologies into clinical pathways which can be very disruptive. For example the East Midlands AHSN has been supporting a programme that is introducing 3D cameras to assess diabetic foot ulcer wounds in primary care which means routine assessment and management of foot ulcers can be done in the community with all the data transmitted back to the hospital. This means introducing new infrastructure into primary care and redesigning the whole referral system.

Clinical coding is done from within the EHR and PACS is embedded in the system. A mobile app with proximity capability enables clinicians to pinpoint exactly where their patients are located.

All 102 GP practices across the city can access a virtual ward where they can see information about their patients when they are in the acute hospital. They can request and receive test results from within the EHR and receive discharge letters. The system can also be accessed by community and mental health staff.

Later this year a person held record will go live, enabling patients to access some of the information in their EHR, add information and communicate securely with their clinician.

The digital team has created a digital vision for the trust, known as the Leeds Digital Way, based on the culture of the trust defined by a staff engagement programme using crowdsourcing technology.

“There is a digital fabric over everything we do and the three CCIOs who work in the trust have pushed clinical engagement right to the forefront. So whenever any changes are needed they are based on the input of the clinicians rather than on IT or commercial decisions,” says Mr Corbridge.

AI: THE ART OF THE POSSIBLE Artificial intelligence (AI) has huge potential to help the NHS become more efficient and deliver service transformation and better outcomes for patients. But there are a number of barriers currently obstructing implementation of the new technology.

This is the conclusion of a report, Thinking on its own: AI in the NHS, by the think tank Reform. It says AI could be used to: support the delivery of the NHS’s Five Year Forward View; improve health and wellbeing by predicting which individuals or groups are at risk of illness; reduce the care and quality gap by giving health professionals and patient’s access to diagnostics and treatment tailored to individual need and improve efficiency by automating tasks.

Eleonora Harwich, Head of Digital and Technological Innovation, Reform, a co-author of the report, says one of the key challenges for the NHS will be to improve the infrastructure for collecting, sharing and accessing data. It will be important to collect the right type of data in the right format, increase data quality and ensure that it can be securely accessed. Data standards will have to be uniform across the NHS and AI algorithms must be accurate and fair.

If there was one piece of advice you could give our delegates for the next 12 months, what would it be?

Professionally, what inspires you? What do you enjoy most about your job?

Focus on culture and staff development. Create a happy fun environment with a clear direction and enable all of your staff to bring their whole selves to work and link each and every one to the staff and patient needs they help to meet.

Share with others; what you have learned, what you are working on, what is stressing you out. Share.

The nurses that I work with! They have embraced our eHospital programme and are now our digital care experts!

Professionally, what inspires you?

I love to learn and make connections with people who can give me a new perspective. I also love to help others learn.

What do you see as the biggest challenge for the sector in the next 12 months?

How to balance the urgency of increasing demand and reducing budget whilst thinking strategically.

Why is it important that delegates come to your talk?

Attending a conference is a luxury many in our industry don’t have. I hope to make sure delegates get practical insights into what they can do to make the most of their most valuable asset – their teams.

What are you looking forward to the most at Digital Healthcare?

Learning!

Don’t focus on the technology, think about the socio-technical system (with your index technology as one node in that wider, dynamic system).

Professionally, what inspires you?

Ideas. And the freedom (sometimes) to pursue them.

What do you see as the biggest challenge for the sector in the next 12 months?

The mismatch between policy expectations and what can actually be achieved. It was ever thus.

Why is it important that delegates come to your talk?

I hope this session will be fun, refreshing and inspire new thinking.

It is important for your personal life as well as in business as everything is about making ourselves and/or others happier.

To do that we have to solve a problem, no matter how big or small it is; selling a product, getting investment, going on holiday with the family. We all have problems, we all have solutions, by sharing we help reduce those problems and increase the solutions.

Professionally, what inspires you? What do you enjoy most about your job?

I love to see people happier. It is one of the reasons I love to teach about healthcare UX because all my students learn something and feel inspired or empowered, which is exactly what happened to me when my eyes opened to the wonderful world of UX.

What do you see as the biggest challenge for the sector in the next 12 months?

Overcoming the significant lack of meaningful UX work within the healthcare sector. This has always been a problem, but there are more digital health projects now than ever before, so the problem is only getting worse. Companies know they need great UX resources, but too little time and money is invested, or worse, it is wasted on the wrong type of resource at the wrong time. A proper understanding of UX will change this however, including getting UX professionals working on projects from the outset, not just tacked on the end of a project.

Why is it important that delegates come to your talk?

If you are working on a new or existing digital health project and want to ensure people have a great user experience, my talk will give you many gems of advice and guidance. The talk will be of particular benefit for those who are new or only briefly familiar with UX. What are you looking forward to the most at Digital Healthcare?

Networking with delegates and fellow speakers. There are so many exciting projects happening with incredible people and I will get to meet them at the conference.

Be discerning, don’t fall for the hype around certain healthcare technologies. The projects that will have most impact will probably: take time to implement, there will be loads of preprocessing work to be done before you get to the sexy snazzy stuff, they won’t be sold as a panacea or easy to implement solution.

Professionally, what inspires you?

What inspires me is the idea of being helpful and useful to others.

It’s why I decided to work in policy. What I enjoy the most about my job is when project I’ve worked on has helped policy makers or other stakeholders to shape their thinking or as a point of reference. This is really my personal holy grail.

What do you see as the biggest challenge for the sector in the next 12 months?

Creating the right infrastructure to innovate whilst making sure that money isn’t wasted on gimmicky tech.
