COMMUNITY PHARMACY – CLINICAL SERVICES; BRIDGING THE FUNDING GAP It has been a very challenging time. We now have a flavour of the ‘pharmacy cuts’ and they hurt!

Community Pharmacies also have been working hard to achieve the Community Pharmacy Quality Payments to minimise that pain. I think it would be fair to say that NHS England has got the attention that they needed – are community pharmacies paying attention? We suspect that we are in for a period of calm before the next storm and the baton has been passed to leaders, owners and individual pharmacists.

The future is clinical. Community Pharmacies are convinced that the future is less in dispensing and they are busy making efficiencies to free up pharmacist and staff time.

Significant investment is going into robots and other technology, you will see examples of these at The Pharmacy

Show. Pharmacists are excited about the opportunity that working in a GP practice might give them – an opportunity to develop additional clinical and diagnostic skills and you are welcome to visit the GP Practice hub. Community Pharmacies are being asked to help manage the incredible demand on General Practice and to work closer with urgent and emergency care. But all pharmacists are clinical pharmacists – Ash Soni tells us that all the time – but what of it?

The Community Pharmacy business model needs to change. We can no longer rest on our laurels and depend purely on dispensing income. We can no longer rely on Local Authorities and CCGs to commission services from us when some are failing to award contracts and decommissioning services. PHARMACY SHOW HAS AUTOMATED DISPENSING SOLUTIONS FOR EVERY SIZE OF BUSINESS Multiple pharmacy operators are pushing ahead with hub and spoke dispensing models, widening the gap between independents and multiples.

Pharmacy chains such as Well pharmacies see hub and spoke dispensing is a way maximise efficiencies and increase capacity for income generating services such flu vaccinations and medicines use reviews.

Dispensing covers a number of different processes such as the receipt of a prescription, clinical and accuracy checks and sourcing, preparation, assembly and supply of medicines. Traditionally, all these different processes are done in a single pharmacy. In a ‘hub and spoke’ model, however, parts of these processes are undertaken in another pharmacy.

dispensing, which is currently only legal within the same company, widen the gap between independent and multiple pharmacy, and potentially put dispensers’ and technicians’ jobs at risk, as technology replaces work previously delivered by a person. Pharmacy contract negotiator Pharmaceutical Services Negotiating Committee has called for a “level playing field for community pharmacies irrespective of ownership”.

NHS England’s invitation to see almost 2,000 pharmacists working in general practice by 2020-21 provides a much-needed fillip to the pharmacist labour market.

A key tranche of work within NHS England’s Five Year Forward View programme, the clinical pharmacists in general practice programme has the backing of over £100 million provided to cofund GP employers of clinical pharmacists for three years. At the end of the first wave of applications over 730 sites, covering nearly six million patients in England had signed up to appoint a practice pharmacist, with a second wave of applications expected to close on May 12.

We can look at the minute clinics in America and similar clinical initiatives elsewhere. We could look at New Zealand and their integrated pharmacy service in the community, based on creating patient powered health and wellness. Consumers in New Zealand will be able to understand what their community pharmacists can do for them and what services are provided to improve health. How exciting to be part of a conversation that ends with ‘pop into the pharmacy – they do lots more than you think’.

The future is actually people. Community Pharmacy will provide a wide range of clinical and non-clinical services to people and their local community. They will be the community health and wellness hub and reach out, interact and support people. Just as we arrive at Healthy Living Pharmacies we must evolve again into Health Crating Pharmacies. They will be the first port of call and a routine referral point for up to 20% of NHS 111 calls. All pharmacies will be clean and professional with large consultation rooms that have the look and feel of GP offices with equipment that supports diagnosis.

They will be leading the way in point of care solutions and disease surveillance. All pharmacists will work through a suite of PGDs or be independent prescribers. And every pharmacy will have two pharmacists at all times.

They have to deliver NICE recommendations and be part of NICE guidance. They have to compete – but not pharmacy to pharmacy, but pharmacy together - #oneprofession.

The future is about creating a professional clinical retail healthcare environment from which we deliver a wide range of clinical services. It is patient powered and cocreated; it is both NHS and private; it is clinical.

So why don’t you come to the The Pharmacy Show. It is the biggest conference for community pharmacy and there will be hundreds of sessions to choose between that covers everything from national and local strategy, examples from pharmacists delivering new and innovative services and the support that might be available. Get ready to meet colleagues, bounce around ideas, learn what they are doing and share experiences.

There are so many barriers to implementation – both internal and external. We will change attitudes and build confidence. We will create evidence and push the boundaries; we will go places where community pharmacies have never been before works. We will have to be bold and adventurous, we may fail and have to rethink and do it differently. We have to learn and introduce new clinically advanced services. But most of all we have to learn how to reach out to people, create awareness and understand their needs. We are building our own history and creating our own future.

However, ongoing legal restrictions on the independent sector do not stop independents thinking outside the box of normal labour-intensive dispensing routines to find similar efficiencies and capacity gains as the multiples. Automation in dispensing does not have to mean a shed-like facility in some anonymous and isolated industrial estate; indeed, PSNC has warned pharmacies against adopting remote dispensing models that bypass direct patient contact at the point of dispensing.

A signature characteristic of The Pharmacy Show is its wealth of technology supplier exhibitors, which use the event to showcase robotic dispensing systems suitable for a wide range of patient-facing business models. This means even the smallest independent will find a system to suit their business model. Available systems can include small dispensing robots, MDS automation systems that reduce manual input to this very labour-intensive activity, and automated collection points that extend the community pharmacy service beyond traditional opening hours. Other systems in use in pharmacies include patient appointment management systems that allow staff to reduce patient noshows, and plan the business day more effectively, improving the patient experience and bolstering the reputation and vitality of the pharmacy sector. Case studies and networking opportunities at The Pharmacy Show 2017 will also enable would-be process innovators to share good practice and learn from others’ hindsight of experience.

RPS ANNOUNCES NICE ACCREDITATION The Royal Pharmaceutical Society is pleased to announce the process used to develop RPS professional guidance and standards has been accredited by the National Institute for Health and Care Excellence (NICE).

Accreditation by NICE demonstrates that RPS professional standards and guidance are developed to a high standard, supporting pharmacists delivering services and care to patients and the public.

NICE accreditation, which is seen as the ‘gold standard mark’ will be valid for five years and is retrospectively applicable to standards and guidance developed using the new 2016 NICE accredited RPS process manual. Currently, the accreditation covers the RPS Competency Framework for all Prescribers, however, as the RPS develops new standards, the NICE accreditation badge will be applied, this includes standards and guidance currently in development and any revisions or updates to existing standards and guidance.

NICE accreditation of RPS guidance: • Helps health and social care professionals identify the most trusted sources of guidance developed using critically evaluated high quality processes • Drives the quality of information used by health and social care professionals in decision making •

Helps to improve patient outcomes through providing robust evidence for NICE quality standards.

It involves a rigorous application process and so achieving recognition is a substantial achievement. Our processes align to those of other royal colleges and demonstrates that the RPS are developing standards and guidance to the highest standards.”

NICE has accredited the process used by Royal Pharmaceutical Society to produce its professional guidance and standards. Accreditation is valid for 5 years from 17 February 2017.

APTUK RETURN TO THE PHARMACY SHOW 2017 As the pharmacy landscape continues to change, we again ask ourselves, what does the future look like for the pharmacy technician profession?

Currently work is in progress to support us with our future practice and extended roles. On the 20th October 2016, it was announced that as part of the Community Pharmacy 2016 and beyond review, NHS England declared a £42m Pharmacy Integration Fund (PhIF) to support pharmacy to transform how it operates across the NHS for the benefit of patients over the next two years. The announcement indicated that PhIF will support community pharmacy as it develops new clinical pharmacy services, working practices and digital platforms to meet the public’s expectations for a modern NHS community pharmacy service’.

December 2016 saw a plethora of consultations released and much of APTUKs work has been formulating and responding to these. We believe that responding and giving advice, information, expert knowledge, professional views and our opinions is a crucial professional leadership activity.

We have responded to the GPhC consultation for the standards for the initial education and training of pharmacy technicians; a significant consultation for the profession, as pharmacy technician roles move forward to a more clinically focused environment. We eagerly await the GPhC evaluation of the responses and how this possibly will change the proposed standards.

APTUK were also consulted on the Pharmacy Services Assistant Apprenticeship Standards.

This related to the first standards drafted for pharmacy assistant training; exploring whether they were fit for purpose for both the job role and for use across all pharmacy sectors.

The recently launched GPhC consultation on the revalidation framework proposes a number of changes that will affect all registrants and APTUK will be consulting with our members and responding to this.

QUALITY PAYMENTS: WHY WAS IT SUCH HARD WORK? When we take time to break down the different aspects, from the gateway criteria to the quality criteria, to some degree we were meeting them all before. So why was it all such hard work to complete?

To qualify for any payments it was crucial that we all meet the gateway criteria. Delivering advanced services such as MURs & NMS you would think after many years of being established wouldn’t be an issue. However, those community pharmacies, that due to structural restrictions, who don’t have the space for a consultation room, immediately failed to meet the criteria; despite the high quality of work they deliver to their communities everyday. Furthermore, if those pharmacies didn’t get their NHSmail account activated, they’d fail to register to deliver NUMSAS. Like many others, I kept my NHS choices pages up to date.

The frustration hit when NHS Choices decided to change their layout, and my team had to react swiftly to rectify this.

Let’s not go down the road of EPS. Pharmacy were ready a decade ago in the anticipation of GP surgeries switching to electronic prescribing overnight …

SCR registration was almost impossible. Being allocated only a few training dates in your locality meant that if you missed the training you don’t get activated. LPCs all around worked hard to try and resolve as much as they could to help all contractors.

All these hurdles, and we haven’t yet got to the quality criteria!

By now we all are fluent in the official table 2 that sets out the quality criteria. I found the hardest part of putting this together was changing my methods, habits and systems of delivering quality, and instead spent time in the preparation of tables that showed evidence like a tick box protocol. Is this how we deliver quality?

NEW RPS VIDEO TO HIGHLIGHT THE ROLE OF PHARMACISTS IN PATIENT CARE

Quality Payments has been such painful work for one reason.

A reason that really needs all the bodies to get together to resolve. I put it down to systems & IT.

The RPS has announced a new video promoting the varied role of pharmacists, creating greater recognition of their integral contribution to the wider healthcare team and the public, called ‘here for you.’

Community Pharmacy has always lacked in data collection.

Why? Simply because we don’t have a universal method of easily collecting data from the care, advice, and interventions we do everyday.

The key message of this video is to promote pharmacists and the important work they do for healthcare, aiming to promote a feeling of pride and satisfaction for the service they provide to patients on a daily basis within all sectors.

Having an electronic recording system for every aspect of the quality criteria available for all contractors on one platform would have made this whole process easier. Even with the support and guidance of all national bodies to help us get through, it was too much work in such a short space of time.

A variety of pharmacists’ across all sectors tell their own stories, sharing their stories of the care and compassion provided to every patient, their role as part of a wider healthcare team, what attracted them to become a pharmacist and what advice they would give to those wanting to work in pharmacy via career profiles.

Community pharmacy will always fail due to the lack of unity and collection of data. We can only grow with evidence; but we go nowhere without data first.

Pharmacists make a huge difference to millions of lives on a daily basis, we’re proud to support the hard work that they do.

It’s a cliche and I know it … but it’s now or never! We must unite to build pharmacy and show it off as the heart of every community.

FLU JAB TRAINING AT THE PHARMACY SHOW After successfully delivering flu jab training at the The Pharmacy Show 2016, Rx Advisor has again teamed up with The Pharmacy Show to bring to you face-to-face flu jab training in 2-hour workshop sessions which will be running throughout the two days of the show. This will allow delegates to plan their attendance at the show so that nothing is missed!

It is an NHS requirement that all pharmacists that vaccinate under the NHS flu vaccination service receive face-toface training at least every two years. This is part of the requirement for completing the declaration of competence, which must be completed prior to delivering the NHS service.

The flu jab training at the pharmacy show will allow delegates to receive face-to-face vaccination training which will include practical injection technique training on: intramuscular injections, intradermal injections and subcutaneous injections, and, the course will also cover administration of the intranasal flu vaccine. Practical training for undertaking basic life support and cardiopulmonary resuscitation (CPR) will also be provided, where delegates can practice the correct technique on CPR dummies. A comprehensive online

course on management of anaphylactic reactions with adrenaline will be made available together with a course on the influenza vaccination itself and information about the NHS service specification and Patient Group Direction for pharmacies. Pharmacists will receive information and website links to all the materials they will need to deliver a robust NHS service, this will include: information on how to complete your declaration of competence (which must also be completed every two years), template standard operating procedures, and, video presentations of all the content of the course!

Clinical pharmacists work as part of the general practice team to resolve day-to-day medicine issues, and consult with and treat patients directly.

This includes providing extra help to manage longterm conditions, advise on multiple medications and facilitate patients’ access to health checks.

NHS England says the role has the potential to reduce demand for GP appointments by 30 per cent: having a clinical pharmacist in GP practices means GPs can focus their attention on diagnosing and treating patients with complex conditions.

Karen Acott has been a clinical pharmacist at the Wallingbrook Health Group in Devon since 2004.

She is a full partner in the group, which covers practices in Chulmleigh and Winkleigh.

The GP Practice Hub returns this year after As a prescribing pharmacist, Karen sees patients an incredible debut stream last year. in clinics and delivers telephone consultations, handling all aspects of medication management. Karen says: “My focus is on patients whose conditions and medication are reasonably well controlled and stable and [the] aim is to improve self-management. Within my consultations I also devote time to help patients learn how to self-care and give them confidence and information that empowers them.

“Pharmacists know medication inside out … there’s always something in the drug library that will suit somebody. We can create a bespoke treatment regime and a personalised plan to ensure better patient experience and I now get patients calling and specifically asking to see me rather than the GP.” From her experience within general practice, Karen is adamant that general practice pharmacy demands a different skill-set to community pharmacy, and she urges clinical pharmacist newcomers to use opportunities such as the Pharmacy Show 2017 to develop their competence. The GP Practice Hub was an incredibly popular addition to the programme in 2016 and has been expanded this year with an incredible line up planned.

A prescribing role is crucial, she says. “Being a prescriber in my own right means I can look after a caseload and be totally accountable for what I do.” Practice pharmacists will also need to have a ‘can-do’ attitude, and be willing to learn, as general practice case-load is likely to be clinically challenging for a pharmacist who has previously only worked in a high-volume community pharmacy. Pharmacists will also need to master clinical risk management skills, as well as management of specific operations such as practice minor illness clinics and telephone medicine consultations.

THE COMMUNITY PHARMACY REVOLUTION It is the middle of May and the Community Pharmacy Quality Payment deadline has now passed. We hope that all community pharmacies have taken the necessary action. We might now have 11,000 plus healthy living pharmacies that have joined the NHS Urgent Medicines Supply Advanced Service (NUMSAS), are using Electronic Prescription Services, are using the digital Summary Care Record (SCR), are dementia friendly and checking out people with asthma.

People bring their prescriptions to us; people buy medicines from us and we provide services to people. The thing that links a community pharmacy to their community is people – individuals and families. It is time to pay attention to people, to build relationships and develop awareness and loyalty. Try changing the question from ‘what is the matter with you’ to ‘what matters to you’.

The patient experience is important. However many may feel that the existing questionnaire is a little naïve and the requirement of completion just once a year is not enough.

Of course people want to visit the pharmacy of their choice and have their medicines dispensed quickly and safely, but this is not the only thing that the pharmacy does. We need to know what a customer expects to see when they walk in a pharmacy, how they want to be treated and the quality of the service that they require. Why don’t you just try it yourself?

The NUMAS, however is a pilot over two years and we are only entering the final phase of recruitment. There is little doubt that is community pharmacy doesn’t engage and push this service that trust will be broken and further national services may be in doubt. It is time for leaders, owners and pharmacists to pull together and show NHS England what we can do.

The masterstroke of pure genius is the question about the Community Pharmacy Patient Questionnaire. Have you done one and have you made it publically available on the NHS Choices page. This simple question is key to the change in business model that is required in community pharmacy.

Community Pharmacy should be all about the Community – the hint is in the name – the people who come in and the ones that do not. We are a people profession and I am sure many pharmacists entered the profession to help

people. Community Pharmacies are located in the heart of communities and where people go and meet and shop. As post offices and banks close there is even more need for pharmacies to hold communities together – to be the centre of communities – he anchor business in the high street.

Awareness is a major issue. Many people don’t know the services that are available from the pharmacy and marketing is often just a poster in the window. Many pharmacies don’t know what their customers or their local community need.

Only when we can raise awareness and match community pharmacy services to individual, family and community needs will community pharmacy achieved the necessary change in service model.

RALLY AROUND CORE BELIEFS The NPA is inviting views on a ‘statement of core beliefs’ about community pharmacy, to provide a reference point for developments across the sector in the years ahead.

If community pharmacy is to move forwards, fulfil its potential across health and wellbeing and thrive professionally and commercially, the sector requires unity of purpose based on a shared passion for pharmacy and agreement on fundamental points of principle. That is why the NPA is asking everyone with an interest in the success of community pharmacy to consider this brief list of core beliefs; a simple, but hopefully meaningful statement that we can all rally around and use as a reference point for everything people say and do in the name of community pharmacy.

We have unique strengths and deliver immensely valuable benefits to patients, communities and the NHS. The network of local pharmacies must remain the beating heart of pharmaceutical care in the community.

Community pharmacy can do so much more. Our premises are conveniently located health & social care assets in neighbourhoods across the country. Our skilled people have the capability to do so much more if liberated to do so.

Face-to-face care is vitally important. Even more in this age of increasing automation and digitalisation, the face to face relationship between health professionals and patients matters.

A pharmacy without a pharmacist is not really a pharmacy at all. The pharmacist herself or himself is central to the operations of a pharmacy, indeed to its very identity.

Nonetheless, the delivery of excellent care is a team effort and convenient access to a team dedicated to pharmaceutical care is a key strength of community pharmacy.

One involves mass automation, centralised dispensing and pharmacists working predominantly from GP practices or remotely. This second path is the only path built on solid foundations.

Following NHS England’s announcement in April that it will be reviewing low value prescription items, England’s clinical commissioning groups (CCGs) are up and running with separate reviews of their own. Several London CCGs have recently finished consulting on new guidance for GPs that removes from the local prescribing formulary several prescription items also readily available without a prescription, and others are likely once the pre-election purdah period finishes. Items under review include gluten-free foods, lactose-free and soya infant formulas, and medicines for minor ailments including chloramphenicol eye drops.

GPs have been told that unless there are exceptional clinical reasons to the contrary, patients requiring these items should be directed to their nearest pharmacy or supermarket to source the items there. Nationally, NHS England believes it spends £128 million a year on items that it describes as “ineffective, unnecessary, inappropriate for prescription on the NHS, or indeed unsafe.”

As part of their consultations, CCGs will be obliged to gain feedback from patients, and there is little doubt that, for many patients, reductions in prescription availability will force a change in the way they manage their healthcare. And, as one of the NHS’s most accessible healthcare professionals, pharmacies are likely to find themselves first in line to answer patients’ queries and concerns about ongoing product supply, as well as health concerns that previously would be directed to a GP. Those pharmacy teams best placed to reassure these patients and help them become more self-care aware will be those who attend this year’s Pharmacy Show and are up to speed with the latest in clinical education and best practice tips.

Self-care, as a discipline, covers everything from living a healthy lifestyle to knowing how to access advice and support for acute and long-term conditions when these occur. Self-care follows the principle that even if a patient does require support from a

healthcare professional, the patient will still need to take care of their own health 99 per cent of the time. When a patient is empowered to do this successfully, healthcare outcomes for that patient will improve.

PSNC believes pharmacies have a key role to play in supporting patients to develop the self-care agenda, particularly, as people look beyond their GP for help with clinical or practical queries about their medicines. PSNC says: “Community pharmacy’s traditional role in supporting people to self-care for minor illnesses is an important way in which to manage demand for other NHS services, in particular general practices. Pharmacies effectively act as healthcare walk-in centres where people, live, work and shop.”

For pharmacies faced with punitive NHS funding cuts, increased self-care awareness is good news. Queries about a medicine can translate into a medicines use review or other advanced pharmacy service, or public health service such as flu or travel vaccinations. Counter staff will experience renewed interest in the OTC medicines counter and the advice that they can offer in support of a purchase.
