SURVIVE AND THRIVE

WHAT DOES YOUR FUTURE HOLD? Many pharmacies are looking towards survival. There is little doubt that things are tough and some pharmacies are struggling to break even. Real-term funding has been eroded and is set against increasing workload and increasing costs to deliver a service to patients. We may even be facing a workforce shortage in the near future. Many pharmacy owners have responded by consolidation, reducing staff and altering working practices to drive efficiency and paying more attention to their buying practices. This may work in the short term, to achieve survival, but pharmacies must think about how they transition their business in this changing environment to head for growth and ultimately thrive. This is a transition that requires a deep understanding of the new drivers in the NHS and the opportunities for clinical service development. As always, the Pharmacy Show is there, with an excellent range of speakers and hundreds of companies exhibiting their products and services. The Pharmacy Show is a great place to meet colleagues and discuss issues, create solutions and discover innovative services. The Pharmacy Show allows you to ask that important question – are we in good shape to move from survival to

The expanding role of clinical pharmacists in community As the size of the ageing population in the UK continues to rise and the cost of healthcare treatments and technology become increasingly expensive, the NHS is under increasing pressure to change, to enable it to continue to offer cost effective and high quality patient care. The NHS long-term plan summarises the ambitions of the NHS and the significant improvements they want to make to the current healthcare system over the next 10 years. The improvements recognising the need to ensure the healthcare professional workforce is being used efficiently, to enable it to continue to offer great patient care. As a key healthcare professional within this workforce, community pharmacists have an important role in

supporting this ambition, working collaboratively with other healthcare professionals to deliver more patient focused services. They are ideally positioned to take on a more active role in supporting their local communities to remain healthy, helping to prevent avoidable illnesses and to offer ‘supported self-management’ of chronic conditions.

WHAT DOES YOUR FUTURE HOLD? Continued from p1 buzzword for the future of pharmacies. Delivering the NHS contract is essential. NHS England certainly sees a role for community pharmacy in urgent care and minor ailments. Services such as the NHS Urgent Medicine Supply Advanced Service (NUMSAS), are based on the ability of community pharmacy to take referrals from NHS111. The concept of referring from one service to community pharmacy and back again can form the basis of a variety of new services, both national and local. The connection and integration of services is a major element of the NHS long term plan and community pharmacy must embrace and engage with the digital revolution. Changing the relationship with customers and patients is required. Community Pharmacy needs to adopt more clinical services and drive this interaction onto a different level. This will mean enhanced examination and diagnostic skills as well as increased access to point

of care diagnostics and medicines through independent prescriber qualifications or PGDs. Many pharmacies will have to take a critical look at their premises and ask whether they are adequate for the delivery of a service based delivery model. Pharmacies will need to look to the future. We will watch the Primary Care Networks (PCNs) develop and be required to deliver more services to their patients. Pharmacies will need to align with their network and then look to integrate services. There is no doubt that the PCNs will struggle to deliver all that they will be required to do and a new partnership with community pharmacy will help in the achievement of local targets. The underlying question is always ‘am I ready?’. If you can survive then you can adapt and thrive. Where better to come and ask that question than the Pharmacy Show.

Primary Care Networks – Will they be the game – change for community pharmacy? There are around 1,300 geographical networks of GP practices covering populations of approximately 30,000-50,000 people in England. Of course, nothing is this simple and some PCNs will be bigger, some smaller and some will stretch over traditional CCG boundaries. Where the GP practices will retain their own contracts, there will be a new network contract. It is through this contract that the NHS hopes to deliver important aspects of the NHS long-term plan and they plan to invest significant amounts of new money through these PCNs. and distributed. It would be sensible for the group of community pharmacies to come together and discuss how they will work together and how they will engage with the network. Each network will have an accountable clinical director appointed by the members of the network. It would be sensible for the LPC to facilitate that introduction of the lead community pharmacist to the clinical director and begin the conversation around what they could do and would want to do to support the network in their first five national services.

It is all about size, and the thinking behind the PCN is simple. A single practice may not be large enough to have the capacity and expertise to deliver everything on their own. Encouraging practices to work together, means that they can share expertise and resources and bring in a wider set of staff roles than would be available in a single practice. Practices will sign a network agreement, a legally binding agreement between the practices setting out how they will work together. Networks can use this agreement to set out wider objectives and record the involvement of other partners including community pharmacies. Although they will be encouraged to work with local partners, it will depend a lot on the willingness of those partners to engage and explain what they can do. Funding is already available to support the infrastructure of the PCNs and over time significantly more money will be available to support NHS national and local objectives. The networks will be required to deliver five national service specifications, starting in April 2020: structured medication reviews, enhanced health in care homes, anticipatory care (with community services), personalised care and supporting early cancer diagnosis. A further two services will start by 2012 including cardiovascular disease case-finding and locally agreed action to tackle inequalities.

There is no clear direction to involve community pharmacies as partners, although many people suggest that they will. Money will definitely be available for PCNs to employ their own pharmacist and NHS England expects a PCN to employ up to six pharmacists over the next 5 years. But what should community pharmacies do to ensure that they are trusted partners within the network and are paid to deliver additional clinical services? Co-operation will be the buzzword for success. Local Pharmaceutical Committees (LPC) should start the process of mapping pharmacies to their networks and ensuring that this information is discussed, agreed

The community pharmacies could start with discussing the services that they currently provide such as the NHS Urgent Medicine Supply Advanced Service (NUMSAS) and the NHS 111 Digital Minor Ailment Referral Service (DMARs). The PCN may want to support the expansion of the DMARs to include their GPs as a source of referrals. The opportunities seem endless, but always remember that they are not mandated. If you are looking for simple advice then consider this: • If you are considering developing private clinical services then go ahead and see how they might fit into PCN objectives later on •

Form your own community pharmacy network within the boundaries of the PCN, talk about what you can collectively deliver and make contact with the PCN clinical director – co-operate and collaborate.

Connecting Online Players with Brickand-Mortar Pharmacies Like many other sectors, the world of pharmacies is undergoing some fascinating transformations, evolving to fit the needs of 21st century clients. With home deliveries becoming a key differentiating factor in the last few years, 79% of all pharmacies have started to offer the convenience of home delivery. Moreover, 84% of pharmacists have reported an increase in demand for home delivery in the past year. With housebound patients, as well as those who might not be able to make it to the pharmacy during weekdays due to longer commutes and 9 to 5 opening hours, home delivery is a positive change for all. However, for pharmacies this might be more of a headache, adding to their already full workload. Certified organic aloe products Aloe has been used in pharmaceutical and cosmetic products in the modern world, for a long time. Trends and fads come and go, but aloe is one of those ingredients that has stood the test of time and is as popular today as it ever was, even back in ancient times. Flu Jab Training at the Pharmacy Show After successfully delivering flu jab training at the Pharmacy Show for the last three years and training thousands of pharmacists since 2009, Rx Advisor has again teamed up with the Pharmacy Show 2019 to bring to you an updated, CPD accredited, and, more efficient face-to-face flu jab training workshop. This 1-hour workshop session is combined with comprehensive online learning to ensure pharmacists are well prepared to deliver the 2019/2020 NHS flu vaccines. Private PGDs are also covered as part of the training and for an additional fee pharmacists can purchase a private flu PGD too. The course is now approved by the CPD accreditation service and is updated to exceed the Public Health England core curriculum and national minimum standards. It also covers the important new vaccines to be used in 2019/2020 and exceeds the requirements of the Royal Pharmaceutical Society and General Pharmaceutical council. Pharmacies to receive £14 per consultation for NHS GP referral pilot Most DMIRS pilot sites are due to go live in July. Pharmacies will be paid £14 for every patient consultation completed following a digital referral from a GP in the next stage of NHS England’s DMIRS pilot. NHS England announced in June that the next stage of its Digital Minor Illness Referral Service (DMIRS) pilot will see GPs refer patients with low acuity conditions – such as rashes, constipation and vaginal discharge – directly to community pharmacies via online triage systems. The commissioning body has since told C+D that pharmacies will receive £14 for each consultation completed following a GP referral – the same amount received for completing referrals from NHS111. The payments cover participating in the pilot, completing documentation, taking part in the evaluation and completing system training, NHS England explained.

One pharmacist involved in the pilot, who wished to remain anonymous, told C+D they are concerned there may be “a surge of phone calls at 9am” from patients looking to book their appointments, “as the surgeries are open [earlier] than us”.

The commissioning body estimates that 6% of all GP consultations – 20.4 million appointments per year – could be safely transferred to a community pharmacist if the service is rolled out nationally.

However, Mr Irvine said the pilot service “shouldn’t be a hindrance” and pharmacies in his area have shown an interest in trialling it.

AIM to host Superintendent Forum The Pharmacy Show AIM, the Association of Independent Multiple Pharmacies, is making its final preparations for its bi- annual Superintendent’s Forum which again will be hosted at the Pharmacy Show. This highly sought-after meeting, which is by invitation only, starts with a breakfast and networking session at 9.15am, then the formal presentations start at 10am and are followed by peer to peer discussions. “As an organisation, AIM is committed to supporting its members and recognises that the role of Superintendent is a key one for Independent Multiples. To allow this important group to meet and discuss key topics twice a year is one way we can fulfil our commitment to members and for one of these meetings to coincide with the Pharmacy Show is an added bonus,” comments Richard Wells, Chairman of the AIM Superintendent Forum AIM is a membership organisation representing community pharmacy businesses with multiple pharmacies. The NHS Long Term Plan The plan was published in January 2019 to unlock the new funding settlement of a 3.4% average real-term growth for the next 5 years. The NHS needed the additional funding as performance had declined, waiting time targets were being missed, providers overspent by almost a billion and there were significant staff shortages. The big questions remain will the plan deliver the required improvement in performance and what does that mean for community pharmacy? The main structural change is in the establishment of over 1,300 primary care networks. These are collaborations of GP practices covering populations of approximately 30,000 to 50,000 people who will work together through a network contract. These contracts represent the mechanism by which the improvement in population health and certain specified health conditions will be achieved. The network may choose to engage with their local community pharmacies, commissioning services in order to deliver the required outcomes. They will progress to form the basic unit for delivery of a fully integrated community-based healthcare service. The plan was launched with a bold claim to prevent up to 150,000 cases of heart attack, stroke and dementia over the next 10 years, to improve maternity and neonatal services as well as mental health and cancer care. Delivery of these targets will require the development and delivery of new services to better prevent and treat these conditions and to improve the care of patients. These services may involve community pharmacies, supporting them to deliver wellness based services. There is a significant focus on urgent and emergency care services. A commitment to clinical streaming may support direct referrals to community pharmacy and, as NHS 111 expands, the role of community pharmacy, accepting and managing referrals should increase. This focus should support the development of more clinical services in community pharmacy and bring a link to minor ailment services which have been missing for

years. Much of this will be dependent on existing and new technology that links community pharmacy digitally to other parts of the NHS. To free up beds in hospitals, there will be a focus on delayed transfer of care. A number of innovative schemes have used community pharmacy to manage medicines and to support patients as they arrive home from hospital. The pressure on hospital beds can only increase and the focus to avoid admission through same day care and to speed up the discharge process will

probably involve community pharmacies in the supply of medicines and associated care. Funding and achieving financial balance remain an important element. A shift from item of service payment to a population based payment system will again leave winners and losers; some organisations will have money to spend and others will not. This is not an unusual situation for community pharmacy and variable commissioning of services will remain. The plan recognises the growing workforce issues in the NHS. Vacancies in General Practice have supported the recruitment of pharmacists to perform clinical face to face services. Plans to increase intake in medical schools and more accessible routes into nursing seem unlikely to deliver the right skill mix in a short timeframe. There is little doubt that the NHS will have to look at community pharmacies differently; firstly as a place where healthcare can be delivered and secondly, to look at the services that could be delivered by the pharmacist in their community pharmacy. The plan is just that – a plan, but with very little detail. There is no doubt that community pharmacy needs vision and good local and national leadership to see the opportunities. Commissioners and other healthcare professionals often do not know what Pharmacies can deliver and Pharmacies often do not know what they want to deliver. There will be plenty of opportunities for community pharmacy to have greater involvement in the delivery of healthcare.
