therapy professionals and key names in the therapy world came together at Birmingham’s NEC, on 23rd & 24th November at the hugely successful Therapy Expo 2016. Here, delegates enjoyed an exciting trade exhibition and world class education conference, split over a two day period for plenty of networking, accredited CPD education, practical advice and product sourcing. Delegates, including Physio’s, Chiropractors, Osteopaths, Sports Therapists and many more, from across the NHS and private sector, listened to and learnt from industry leaders such as Jon Graham, Clinical Director, PhysioFunction; Joanne Elphinston, Physiotherapist, Performance Consultant and International Lecturer; and Professor Chris McCarthy, phD, Manchester School of Physiotherapy, with other key education leaders to explore the latest developments within the therapy industry. Thanks to FIVE education streams, visitors took advantage of over 40 hours of accredited CPD training and development – saving hundreds of pounds, compared to the cost of accessing the same quality and quantity of education elsewhere. The exosuit made for walking The idea of using an exoskeleton to enhance a human body has been around for a while but for many reasons they are impractical so how about exosuits? Small and soft exosuits may be the way forwards for those who are unable to or struggle to walk. The major fault of skeletons and suits has always been related to efficiency. There is no benefit to using a skeleton or suit if, due to factors such as weight, the metabolic penalty outweighs the efficiency gained i.e it costs more energy to use it than energy saved by using it. There is also no benefit to using a system like this if it feel unnatural to move around or creates an altered gait pattern. A study in Science/Robotics has performed an experiment using a new type of multi-joint soft exosuit investigating if this new model saves energy during normal walking. Assistance was applied to the ankle and hip joints by the motor and tether mechanism. Seven participants walked on a treadmill at 1.5 meters per second under one unpowered and four powered conditions, where the peak moment applied at the ankle joint was varied from about 10 to 38% of biological ankle moment (equivalent to an applied force of 18.7 to 75.0% of body weight). Overall this type of exosuit showed to make walking more efficient in terms of energy

Think of the sensation as sluggish and heavy legs”. This is encouraging news as currently people with movement or neurological disorders have to rely on clunky or passive/dynamic orthoses (similar to the image above) which can be exhausting to use or require the user to alter the way they walk to maximise their effects. This suit however saved 23% of energy.

Kinesiology Tape, Stretch, and Pain By far the most common reason therapists apply kinesiology tape to patients is in an effort to reduce pain. There are lots of opinions out there on how tape should be applied to optimise its pain reducing effects. Whilst there are plenty of RCTs that support the fact that tape can provide clinically meaningful, drug free, pain relief. There is, frustratingly for clinicians, much less evidence to lean on when it comes the nitty gritty of how tape should be applied. This has allowed opinion the run rife in the absence of hard facts. Depending on which taping course you attend, or book you read, you may have been told that intricately cut shapes or directionally specific applications will lead to better outcomes, however these ideas are not borne out in the research. It is also widely taught that by applying kinesiology tape with some added stretch, it causes convolutions to form in the skin and this is vital to tape producing pain relief. Whilst this might be the case in certain clinical conditions a recent paper adds weight to the idea that this is not the case all of the time

usual activities. The tape was then removed and the test repeated.

This Brazilian study from 2015 compared the pain reducing effect of kinesiology tape applied with 25% stretch, with an identical taping applied without any stretch. They recruited 85 healthy, young University students and divided them into 2 groups. One group were to have the stretched tape applied to the back of their non-dominant hand/wrist and the other group were to have the same application without stretch. Firstly, both groups completed a baseline test of pain induction by cold water immersion. Their hands were placed in 0-2°C water and the time taken to report pain was recorded, along with the total time they were able to keep their hand in the water. The subjects also rated their pain on a VAS scale.

So, whilst it appears that kinesiology tape reduced the cold-induced pain, the authors state that the effect would need to be explained by some other mechanism than the convolutions formed by the stretched tape. The exact mechanism by which tape works to reduce pain remains somewhat speculative. It would seem plausible that tape applied to the skin stimulate some mechanoreceptors and free nerve endings regardless of whether or not it has stretch applied, particularly in a very sensitive area such as the hand.

The subjects were then taped according to their group allocation – stretched tape or unstretched tape. They left the tape in situ for 24 hours whilst performing their

The study found no differences in the pain relieving properties between the stretched tape group and the unstretched tape group.

Perhaps if the tape had been left in place for the second immersion, there may have been some difference in results? It is also possible that the improvement in pain tolerance was due to the learning effect, as the order of testing was not randomised, adding a control group (no tape) would have improved the robustness of the study.  Continued on page 7

It was great to catch up with some familiar faces and a great opportunity to meet many new ones! We spent the 2 days updating delegates on their CPR and AED skills, assessing their competency in accordance with the Resuscitation Council (UK) 2015 guidelines and providing certifications to everyone who passed. We were also able to give an update regarding the recognition and treatment of anaphylaxis. We look forward to returning in 2017 to help the delegates meet their CPD requirements by providing an annual CPR/ AED update.

I enjoyed Therapy Expo very much and a big part of that was the CPR Zone, where I could get my CPR training done for free. Exercises for Medial Tibial Stress Syndrome (AKA ‘Shin Splints’) It’s easy to talk generally about rehab, ‘strengthen x,y and z’, improve movement control etc but we don’t often expand on what this means exactly. This is a brief look at an exercise programme I used recently for a patient with Medial Tibial Stress Syndrome. The patient, is a young male middle-distance runner called Ben. He is, at this stage, pain free with all daily activities and running up to 40 minutes with no symptoms. He’s progressed nicely from the initial session where even jogging on the spot was painful, however, longer runs over 45 minutes cause some discomfort in the medial tibia. This has been a recurrent issue and Ben’s aim is to return to full training without these symptoms. He is gradually re-introducing high intensity sessions with guidance from his running coach and managing well. Assessment reveals mild weakness in Soleus, Glute Med and the posterior chain. Control of single leg balance and single leg dip is good and equal left and right. Broadly speaking our aims are as follows:

Single leg soleus bridge - Note this bridge is done with the forefoot on the edge of a step, with the aim to lengthen the lever to challenge the posterior chain & to work the soleus.

6. Straight leg calf raises - These will strengthen Gastroc and Soleus.

That said this study does suggest that perhaps we can be less concerned about the exact amount of stretch applied, and likewise patients can more confidently apply their own tape without being as pedantic with the tape stretch.

STA Conference 2016 – “The Exceptional education at the SMA Annual Conference biggest and the best”

The Sports Massage Association (SMA) Conference 2016 appealed to many therapy professionals as they came to meet and hear from top experts in the field of sports massage. Designed by top SMA speakers Nick Mooney and Paula Deacon, the insightful demonstrations covered the assessment of the sacroiliac joint and an introduction to the treatment of sacroiliac joint dysfunction.

There was an excellent variety of sessions and the quality of the speakers are high. I’ve been able to learn about the latest evidence and it gives you the reassurance that you are doing the right clinically.” The Sports Therapy Association will be back at Therapy Expo 2017, be sure to keep an eye-out for conference programme highlights later in the year.

Throughout the conference the theatre was packed with delegates, who were treated to fabulous demonstrations and a thought provoking lecture on the issues surrounding the assessment of sacroiliac joint dysfunction. For the SMA the increased awareness is invaluable and it proved just how many other specialisms look to massage within their treatment regimens”.

She was put on extracorporeal life support but she kept deteriorating and her condition eventually progressed to multi-organ failure, something radical needed to happen to treat the infection. In April 2016, a team of 13 operating staff, including 3 surgeons, over the course of 9 hours, removed both of her lungs. Usually at this stage the new lungs are then transplanted into the body, however this didn’t happen. She was placed on ECMO for 6 days until donor lungs were available. Incredibly, even without lungs in her

body, her condition stabilised and they were able to stop all vasoactive support before leaving the operating room. Her condition steadily improved and after an uneventful transplant the ECMO was weaned. The illness left her muscles unable to even support her head, but thanks to a carefully planned physiotherapy programme she can now walk again.
