In September 2014, PhysioFunction was contacted by Papworth Hospital to see if their Neurological Physiotherapy services could support Victoria and facilitate her discharge. PhysioFunction worked collaboratively with Papworth Hospital and the CCG to ensure Victoria’s wish came true and she was able to return to her home. When Victoria was initially discharged from hospital, she required 24 hour care as well as Oxygen and intravenous drug therapy. It was clear that Victoria’s exercise tolerance was very low and very limited. She was only able to capable

of performing a limited number bed exercises. Michelle, her Physiotherapist from PhysioFunction, set an initial goal of strengthening her legs sufficiently for her to stand with the assistance of three (one therapist and two carers). She taught her care team how to assist Victoria to safely and effectively carry out these exercises so that she could do them regularly throughout the day as she felt able. By April 2015 with the support of Michelle, her family, her care team and her own sheer determination and motivation, Victoria was able to use a slide board to transfer from her bed to her wheelchair and achieved her initial goal of standing

Kinesio taping For 35 years Kinesio has been dedicated to further our understanding of how Kinesio Taping affects the human body, and aid in the continuous development of the Kinesio Taping method. So, when Can A. Yucesoy and Uluç Pamuk’s research into how Kinesio Tape can affect targeted tissues was published at the end of 2015, there was great interest in what they discovered. that Kinesio Taping not only affects the directly targeted tissues, but also those that are non-targeted, although the magnitude of shape change in the latter was smaller.

Can A. Yucesoy is the director of the Biomechanics Laboratory at the Institute of Biomedical Engineering in Boğaziçi University, Istanbul. He has a BSc and MSc in Mechanical Engineering, as well as a PhD in biomechanics from the Biomechanical Engineering Group at the University of Twente. His research projects focus on muscle mechanics, the role of fascia-muscle integrity and muscle function. Both researchers are fascinated by human physiology and how it is manipulated by mechanical stimuli. They believe the effects of Kinesio Taping are a promising research field for physiotherapy, sports science and biomechanics. Using advanced magnetic resonance imaging (MRI) techniques they assessed the changes in tissues after Kinesio Tape was applied, with the aim of identifying the mechanism of the tape’s effects on a level that had never been achieved before. After taping they found the skin to be “crimped” partially lifted and partially compressed - which they had expected to be the outcome, with Kinesio applications aiming to improve lymphatic flow. In addition, they found

Can says that there is much more to be discovered to bridge the gap between the expected physiological outcomes of Kinesio Taping and the actual mechanical load imposed by Kinesio Taping, but he believes the results lead to a better understanding of Kinesio Taping and better outcomes for patients and practitioners. The initial results are very promising and the success of their study has encouraged both Can and Uluc to pursue further research in this field.

Good Neuro Rehabilitation can quite literally be life changing!  Continued from page 1

Hydrotherapy enabled Victoria to do aerobic exercise, helping her to learn to control her breathing again. It allowed Victoria’s body to overcome the fear of falling because she felt safe enough to “let go of her balance”. This was a really important factor for Victoria after losing her sense of midline and balance following the long stay in hospital. Having weekly Hydrotherapy meant her confidence improved which positively impacted on other areas of her rehabilitation. Victoria attended PhysioFunction’s Injury Rehab Centre once a week for her Physiotherapy sessions. Victoria says her experience at the Centre was ‘incredible’. She describes how “you feel good to be at the centre because everyone seems happy and everyone is here to get better” Another aspect that has been valuable to Victoria’s rehabilitation was the building the relationships with her carers because they were all able to participate in the hydrotherapy sessions with her.

This allowed Victoria to build trust in her carers and vice versa. Victoria looked fragile following her illness and working together in the physiotherapy and hydrotherapy sessions gave Victoria’s carers more confidence to support her in other situations away from the water as she got stronger and more robust. The knowledge and positive attitude of the team has allowed Victoria to build her confidence in the team which has allowed her to progress further in her rehabilitation journey. Victoria’s enjoyment in the Hydrotherapy pool meant her sessions are fun which in turn means she consequentially works harder during her physiotherapy. Now that Victoria has built up her strength and improved so much and her carers are able to take her swimming during the week. She still attends for Hydrotherapy with PhysioFunction as she continues to make gains with her balance and walking. She can now walk around her house independently and takes walks outside with a little assistance. Victoria is maximising her independence and enjoying a more independent life. Her goal is to be well enough to find full time work, be fully independent and able “to pop out for milk” on her own.  Continued from page 2

She had tried swimming since her discharge but found that the cold water of a conventional pool made it difficult for her to breath. Michelle suggested she tried Hydrotherapy at PhysioFunction’s Centre of Excellence at Moulton, Northampton. The heat of the thermal pool provided her with an extremely positive experience. Hydrotherapy became an integral part of her recovery process. Victoria could work harder in short bursts then relax and float to recover. It also meant that Victoria now looked forward to her physiotherapy sessions where she had the ability to enjoy the water and have fun.

Dry needling and acupuncture for sports injuries – what’s the point? Although sports injuries occur through a variety of tissues and in different areas of the body, they do possess common denominators; namely pain, trauma and soft tissue involvement. As a result of these common denominators so to speak, physiological factors, such as the inflammatory process which influences healing and recovery, are also shared. Acupuncture as a modality has been shown in a variety of studies to positively influence pain and inflammation without significant side effects. This makes it an ideal intervention when managing sports injuries. complex. This stimulus causes the release of chemical neurotransmitters such as endorphins and monoamines that then ‘block’ the subsequent pain messages. These neurotransmitters consist of met-enkephalin, beta-endorphin, norepinephrine and serotonin to name but a few.

How does it work? A variety of studies have shown that the analgesic pain killing mechanism of acupuncture in both humans and animals is considered to be opioidergic. The insertion of an acupuncture needle into skeletal muscle tissue has been shown to stimulate local nerve fibres that send impulses to the spinal cord and these in turn activate three neural regions; the spinal cord, midbrain and supraspinal pituitary hypothalamic

Evidence suggests that acupuncture and dry needling techniques themselves are generally most effective when conducted at traditionally recognised acupuncture points. There is considered to be a direct correlation between myofascial and neural structures and traditional acupuncture points although the exact level of that correlation remains under investigation. There is also thought to be a close relationship between the location of traditional acupuncture points and ‘trigger points’. Some researchers have suggested a 90% crossover between acupuncture points and trigger points and areas of

myofascial pain. This, and acupuncture’s lack of significant side effects, makes it a useful modality for treating both myofascial pain and dysfunction. What can it treat? The range of sports injures that research has shown to benefit from acupuncture include plantar fascial pain; medial tibial stress syndrome; acute low back pain; patellofemoral pain; and rotator cuff tendinopathy. From a clinically reasoned perspective however, the potential exists for it to be used for the treatment of any myofascial pain and dysfunction. Is it safe? A number of extensive systematic reviews covering thousands of acupuncture treatments have concluded that while it is an invasive procedure and therefore carries an inherent level of risk, it is actually a very safe modality in skilled, trained hands. As a result, practitioners should

be appropriately trained and always respect the client, the anatomy and the technique during every treatment. Who can practise the techniques? This is the type of acupuncture that is most commonly employed by physiotherapists and sports therapists at all levels, from treating keen amateurs to elite Olympic athletes. Is it all about the crack? A new model justifying the use of spinal manipulation for facilitating locomotor function using a sensory motor approach. For far too long spinal manipulation has only been associated with the management of painful spinal conditions, and the joint cavitation has been viewed as the holy grail of expert practice. Spinal manipulation has a well-established evidence base in contemporary musculoskeletal practice. However, its clinical objectives are all too frequently attributed to the restoration of joint position or movement of specific spinal segments believed to be misaligned or hypermobile. This limited outlook does not embrace increasing evidence supporting neurophysiological mechanisms affecting pain and sensory motor control after manipulation. Normal human movement is reliant upon effective sensory motor control. The sensory motor system is responsible for the regulation of posture, joint stability and movement. Manipulation is an effective treatment tool for gaining entry into the sensory motor system to promote normal movement. The capsuloligamentus and myofasical tissues contain mechanoreceptors, which are capable of stimulation by the forces applied through manipulation. Several authors have reported clinically significant changes in post intervention proprioception immediately after lower quadrant joint manipulation and mobilisation. Absent, reduced or inaccurate sensory information will compromise the sensory equation of the sensory motor system. This system is critical for all components necessary for individuals to move and experience the environment in which they wish to function. All activities of daily living and athletic performance are reliant on afferent input from the periphery to the central nervous system. The spine has a rich source of sensory receptors that convey proprioceptive information from each spinal level about the positional status of the individual. Insult or dysfunction of spinal structures whether due to previous pain or poor postural habits can compromise its mechanical contributions to sensory

motor control of the kinetic chain. Distorted sensory information can affect the planning and organisation of motor behaviours at its higher centres. Poor sensory input at the receptor level has the potential to disrupt processes that are reliant upon its inclusion for voluntary movement, postural adjustments and motor learning. For example, using this approach it would not be unreasonable to assess the spinal articular system of patients presenting with persistent ankle instability. Most therapists would examine the ankle in detail but not consider the regional interdependence of motor control for the whole system. Spinal manipulation with a goal ordinated motor relearning programme provides a rich prioprioceptive training environment. Although, repeated practice is necessary to facilitate long-term motor recovery, an integrated approach should include targeted manipulation, mobilisation and exercise therapy. This concept of treating the whole person is not new, but is often forgotten in the sprint to rehabilitate the patient. All regions of the body are interdependent both in terms of physiology and anatomy. The effects in one location are often related to distant locations associate with the patient’s primary complaint. Conceptually, regional interdependence has an abundance of evidence including case reports, descriptive reports and controlled randomised trials. There have been several reports of patients complaining of primary low back pain with hip or knee involvement. Many studies report clinically significant changes in symptoms and movement behaviors when treatment is directed at distant anatomical locations. In the upper quadrant, studies have reported post intervention treatment success after manual therapy for the thoracic region of patients presenting with primary neck pain and shoulder impingement.

stimulating mechanoreceptors and facilitating normal movement behaviours. Therefore, an integrated approach for the clinical management of disordered movement should include spinal manipulation and joint mobilisations employed to prepare patients for exercise therapy and functional activities.

Speeding up rehabilitation process with Hypervibe… Chris suffers with Chronic Obstructive Airways Disease (COPD), he doesn’t have any cancerous disease but represents a group of patients who need palliative rehabilitation to extend their active lives. The COPD was diagnosed in 2009 although Chris felt that this had been present for the previous four years. By 2011 things had worsened and he was placed on long term oxygen therapy. His NHS rehabilitation began in earnest with his attendance at the PACE (Pulmonary Rehabilitation) course in 2011. He managed to improve his shuttle test from 70-270 metres on oxygen. However Chis was still experiencing frequent exacerbations of bronchitis that required hospital admissions, and in 2012 he was diagnosed with type II respiratory failure and was placed on night time BIPAP ventilation.

A 30 minute physiotherapy session could save the NHS £2,000 per lumbar surgery patient Patient Education from a physiotherapist pre-surgery is very likely to reduce costs and improve outcomes according to a recent research review by the Spine Journal. The research review published on physical therapy site Co-Kinetic.com based on a study published in Spine Journal, revealed that patients who had received pain education Tor Davies, Co-Kinetic founder rated surgery as significantly more satisfactory on several levels compared with those who had not received pain education from a physiotherapist. Patients that had received pain education also spent 45% less on medical tests and treatment, despite having similar levels of pain and disability. The one-time physical therapy session resulted in a saving of more than £2000 in follow-up appointments and further treatment per patient. The study consisted of sixty-seven patients, scheduled for surgery, who were randomly assigned to receive either the usual surgeon-led pre-operative education, focusing on procedures and anatomical description of the surgery (control group) or the pain education (experimental) group.

The pain education group had one 30 minute physical therapy session which involved information about the neurology of pain including the decision to have surgery, the nervous system’s physiology and pathways, peripheral nerve sensitisation, calming the nervous system and issues affecting nerve sensitivity. The session was accompanied by a £5 neuroscience education booklet for patients to read in their own time before their operation. However despite these findings, at one year, there were no actual statistical differences between the two groups in regards to back pain, leg pain, fear-avoidance, pain catastrophisation and function except a saving of £2000 in further treatment.
