New Opioid-Associated Life Threatening Emergency Algorithm: Updates American Heart Association (AHA)

he must be continuously monitored until help arrives; if he stops responding CPR should be started and naloxone be given again. If the victim does not respond to naloxone, CPR must be continued and AED should be applied once available until advanced help arrives.

A keen healthcare provider must have a high index of suspicion when it comes to patients who are at higher risk of having cardiac arrest secondary to opioid overdose. While naloxone has no definite role in controlling cardiac arrest, empiric administration of naloxone to all unresponsive opioid-associated life-threatening emergency patients may be reasonable as an adjunct to standard first aid and non–healthcare provider BLS protocols. However, standard cardiopulmonary resuscitation procedures and emergency medical services initiation should not be neglected for naloxone administration.

In the past year, I have observed at least 5 deaths of people in this community who have experienced death due to opioid overdose. Their loved ones, family members, and friends felt like they could have done more. AHA has included this life-threatening algorithm in hopes to sustain the life of some or all victims involved.

Opioids are compounds taken from the opium poppy and other synthetic analogues that has the same action to the body. In higher doses, opioids may cause respiratory depression and death, with risks increased upon combination with alcohol. Drugs classified as opioids include morphine, hydrocodone, fentanyl, methadone, oxycodone, tramadol, and heroin. Some specific effect of opioids includes torsade’s de pointes and cardiotoxicity, credited to methadone and propoxyphene.

Opioids are routinely used for pain management postoperatively. However, respiratory compromise, often due to opioid administration, is a contributor or main cause of more than 1/3 of hospital cardiopulmonary arrests. In 2013, the recorded casualty attributed to opioid prescription overdose in the United States amounted to 16, 235 while 8257 were attributed to heroin overdose. Opioid overdose was the leading cause of unintentional injurious death among people aged 25 to 60 years in 2012 both in Canada and the United States.

Given the dangers of opioid overdose and the increasing number of deaths attributed to it, indeed there is a need to establish recommendations for the management of such cases.

Threatening Emergency Algorithm for The Opioid-associated Life Threatening Emergency Algorithm for 2015 begins with assessment and activation. Upon encountering a victim with indefinite or absent pulse who may be in cardiac arrest, the patient must be assessed for unresponsiveness. Help must be sought and someone should be sent to call 911. AED and naloxone should be prepared and breathing should be observed.

If the victim is unresponsive, without or only grasping for breath, cardiopulmonary resuscitation should be started. If the rescuer is alone, CPR must be done for approximately 2 minutes before leaving the victim to call 911, get naloxone, and AED. What is Narcan™ (naloxone)?

Narcan™ (naloxone) is an opiate antidote. Opioids include heroin and prescription pain pills like morphine, codeine, oxycodone, methadone and Vicodin.

Narcan™ (naloxone) only affects people who are using opioids. If a person is not having an overdose but has been using opioids, Narcan™ (naloxone) will put them into immediate withdrawal. This can be very uncomfortable for the person, but is not life threatening.

Narcan™ (naloxone) is a prescription medicine that blocks the effects of opioids and reverses an overdose.

It cannot be used to get a person high. If given to a person who has not taken opioids, it will not have any effect on him or her, since there is no opioid overdose to reverse.

Narcan™ (naloxone) can be given by intramuscular (IM) injection - into the muscle of the arm, thigh or buttocks - or with a nasal spray device (into the nose).

Nasal spray use is less common, but some large cities in the U.S. use the nasal spray version and it can be prescribed.

Once available, naloxone should be given 2 mg intranasal or 0.4 mg intramuscular. This could then be repeated after 4 minutes. Should the victim respond,

n the United States, the Food and Drug Administration (FDA) does not require companies to list the ingredients contained in sanitary napkins and tampons, yet the average woman uses more than 15,000 sanitary napkins, or has a tampon inside of her, for more than 100,000 hours over her lifetime.

The manufacturing process of these menstrual aids has been known to cause serious damage to the environment. The toxic by-products of producing tampons include poisons like dioxin and furan.

In fact, conventional sanitary pads can contain the equivalence of about four plastic bags! Synthetics and plastic restricts air flow and traps heat and dampness, potentially promoting yeast and bacterial growth in a woman’s vaginal area. Conventional sanitary pads can also contain other potentially hazardous ingredients, such as odor neutralizers and fragrances.

How do tampons and pads get that ultra-white “clean” look? Using chlorine bleach, it can also create toxic dioxin and other disinfection by-products. According to an EPA draft report, dioxin is a serious public health threat that has no “safe” level of exposure!

One infamous risk is Toxic Shock Syndrome (TSS), caused by poisonous toxins from bacteria. TSS is a life-threatening condition and mostly affects women under 30. While any woman using tampons can get TSS, more than 60% of fatalities caused by TSS happen in women ages 15 to 24.

Could Gum Disease and Alzheimer’s Disease Be Linked?

eople with poor oral hygiene or gum disease may be at a greater risk of developing Alzheimer’s disease, according to a study published online in the Journal of Alzheimer’s Disease.

Researchers at The University of Central Lancashire (UCLan) School of Medicine and Dentistry in England led by Drs. StJohn Crean and Sim K. Singhrao examined brain tissue samples donated by 10 patients without dementia and 10 patients with dementia. They found gum disease bacteria lipopolysaccharides (the surface of the bacterium) in the sample from four of the people with dementia and none of the people who did not have dementia. Bacteria can enter the bloodstream through everyday activities such as eating, chewing and toothbrushing.

Once in the bloodstream, the bacteria can be carried to other parts of the body.

The researchers hypothesized that when the bacteria reach the brain, they may trigger an immune system response (like they do in the mouth), killing brain cells. This immune response could be one mechanism that leads to changes in the brain, which is typical in Alzheimer’s disease. It could play a role in causing symptoms such as confusion and deteriorating memory.

“This new research indicates a possible association between gum disease and individuals who may be susceptible to developing Alzheimer’s disease, if exposed to the appropriate trigger,” said Dr. Crean, who is the dean at the School of Medicine and Dentistry. “Research currently underway at UCLan is playing an active role in exploring this link, but it remains to be proven whether poor dental hygiene can lead to dementia in healthy people, which obviously could have significant implications for the population as a whole. It is also likely that these bacteria could make the existing disease condition worse.”

This was a small study examining only brain tissue samples from only 20 people. Because of that, the association between Alzheimer’s disease and gum disease in this study could have occurred by chance. It is also possible that people with Alzheimer’s disease have worse oral hygiene than do people without dementia. Therefore, the bacteria in the brain tissue may be the result of Alzheimer’s disease, not the cause. More research is needed to determine whether or not having gum disease increases the risk of developing Alzheimer’s disease.

There are countless diet programs on the market promising magical quick results to give you the “perfect” body you have been dreaming about. However, the truth is that diets just don’t have the long-lasting effects that a healthy lifestyle does. According to the Institute for the Psychology of Eating, 95% of all dieters will regain the weight they lose within one year.

Other credible health sources show that the deprivation of restrictive diets may even lead to a diet-overeat or a diet-binge cycle. According to the Centers for Disease Control and Prevention (CDC), obesity is common, serious and costly. More than one-third (36.5%) of U.S. adults have obesity. Obesity-related conditions include heart disease, stroke, type 2 diabetes and certain types of cancer. The estimated annual medical cost of obesity in the U.S. was $147 billion in 2008. The medical costs for people who are obese were $1,429 higher than those of normal weight. The CDC’s information is surprising to some, but not to Reggie. This is why he

sees his work as more of a purpose. His sees first-hand what can happen when you combine physical activity with proper nutrition. He has not only seen the results on his own personal journey, but through the transformation of his clients. Reggie C Fitness produces real results with real work. Having a cheat meal late at night before you go to bed is just going to sit on your stomach all night, turning into fat.”

A

s March is Colon Cancer Awareness Month, I want to coach you on the importance of preventative care and maintenance of the Colon. Also, this is the perfect time to get a colonoscopy, as recommended doctors and healthcare professionals.

As a certified health coach and nutritional counselor, I am always promoting the importance of detoxing your colon to help rid the body of parasites and buildup in the colon walls. There was a reason when I was growing up that my family would always make sure we were going to the bathroom regularly. They would sometimes slip us a teaspoon of some old family remedy they would mix

up to make us go to the bathroom.

Well today, especially since we are consuming more fast food and processed food, it is all the more necessary to treat our colon with kindness. According to BlackDoctor.org, African Americans have the highest rates of colon cancer (also known as colorectal cancer). We face a 20 percent higher risk of developing colon cancer and a 45 percent higher mortality rate than any other race. Although this disease is the third cause of cancer-related deaths in the Black community, it’s highly preventable. BlackDoctor.org spoke with several medical professionals to see what can be done to lower the risk of colon cancer.

Dr. Strick Woods, gastroenterologist based in Bridgeport, CT, suggests that screening is the first step towards Black men lowering their risk. The American College of Gastroenterologists recommends Black men be screened starting

at age 45 – five years earlier than whites.

The reason? Colon cancer is often diagnosed in African Americans at a younger age. Dr. Woods says the current compliance rate for colonoscopies is at a mere 38 percent. African American have a notably low screening rate which puts us at adverse risk for developing colon cancer.

We all know that there is inconvenience and discomfort to detox the colon and a colonoscopy is not really fun. However, if it helps keep you healthy, then follow the advice of the experts. A colonoscopy allows your doctor to look at the inner lining of your large intestine (rectum and colon). He or she uses a thin, flexible tube called a colonoscope to look at the colon. A colonoscopy helps find ulcers, colon polyps, tumors, and areas of inflammation or bleeding.

Detoxing is very beneficial to clean and clear out the impurities in the body.

There are so many detox systems on the market. I can recommend several of the 5-day, 7-day and also daily detoxes.

Fruits and vegetables are essential to the regularity of our digestive system. Vegetables are great roughage and fruits are great toxin relievers.

Roasted Salmon with Thyme and Honey-Mustard Glaze Ingredients

• 10 thyme sprigs • 1 (3-pound) skin-on salmon fillet (preferably sustainable), pin bones removed • 1/4 cup country Dijon mustard • 2 tablespoons honey • 1 teaspoon white wine vinegar • 2 teaspoons fresh thyme leaves • 1 teaspoon kosher salt • 1/2 teaspoon freshly ground black pepper • 1 lemon, thinly sliced

How to Make It

• Preheat oven to 450°.

• Line a baking sheet with parchment paper. Arrange thyme sprigs in a long row on parchment. Place salmon, skin side down, on top of thyme.

• Combine mustard, honey, and vinegar in a bowl. Brush mixture evenly over top of salmon. Sprinkle salmon with 2 teaspoons thyme leaves, salt, and pepper. Arrange lemon slices over salmon.

• Bake salmon at 450° in center of oven 26 minutes or until desired degree of doneness. Serve hot or at room temperature.

9 ounces white whole-wheat flour (about 2 cups) 1 1/2 teaspoons baking powder 1 1/2 teaspoons baking soda 1 1/2 teaspoons ground cinnamon 1/4 teaspoon salt 1/8 teaspoon ground nutmeg 3/4 cup granulated sugar 1/2 cup dark brown sugar 1/2 cup canola oil 3 large eggs 1 cup grated carrot 1 cup grated zucchini 3/4 cup drained canned crushed pineapple in juice Cooking spray 4 ounces 1/3-less-fat cream cheese, softened 1 tablespoon unsalted butter, softened 2 cups powdered sugar 1/2 cup flaked sweetened coconut, toasted

How to Make It

• Preheat oven to 350°.

• Combine first 6 ingredients in a bowl. Combine sugars, oil, and eggs in a separate bowl, stirring with a whisk. Stir carrot, zucchini, and pineapple into sugar mixture. Add flour mixture to sugar mixture; stir until moistened. Spoon batter into a 13 x 9-inch metal baking pan coated with cooking spray. Bake at 350° for 33 minutes or until done. Cool on a wire rack.

• Combine cream cheese and butter in a bowl; beat with a mixer until smooth. Add powdered sugar; beat just until smooth.

Spread evenly over top of cake. Sprinkle with coconut flakes. Cut into 16 pieces.

id you know approximately 30 million American adults have kidney disease, but most don't even know it? March 1 marks the beginning of National Kidney Month, a time to raise awareness about your kidney health and generate support for those affected by such conditions as kidney stones, kidney infections and kidney disease. It's also a time when the Urology Care Foundation, the nation's leading nonprofit urological health foundation, encourages the public to make direct, positive and healthy changes in their lives to keep their kidneys healthy and happy.

The kidneys are a pair of bean-shaped organs that are about the size of a small fist and sit on each side of the spine, above the waist. Their primary job is to clean the blood and produce urine to rid the body of waste. They also help make red blood cells, maintain a balance of salt and other nutrients in your body, keep your bones healthy and help control your blood pressure.

Your kidneys can become damaged with little to no warning. Kidney disease is known as the "silent epidemic" because if often shows no signs until it is more advanced. High blood pressure and diabetes are two leading causes of kidney disease. Other risk factors include heart disease, obesity, high cholesterol and a family history. Older adults, Hispanics, African-Americans and American Indians are at a higher risk for developing kidney disease.

"Most people don't realize how important the kidneys are to our well-being, which is why it's important to look after them," said Richard A. Memo, MD chair of the Urology Care Foundation.

"If you are at risk for kidney disease, talk to your doctor about having your kidney function checked as this disease is one that can be prevented or slowed down if caught early."

Kidney Stones: One in 10 Americans will develop a kidney stone at some point in their lifetime and are the reason for more than 1 million emergency department visits per year.
