Changes to screening age for colorectal cancer (CRC) Over the past few decades, incidence of CRC has been increasing in younger adults.

Unfortunately, the data collected on these cases is incomplete, leaving younger patients in their 30s and 40s without the benefits of research and trending data available to those 50 and over who fall within the recommended screening age range.

Who’s at risk? Screening for average-risk African-Americans is now recommended to start at 45.

According to the Multi-Society Task Force (MSTF) on Colorectal Cancer, this recommendation is based on a high incidence rate and early mean age of onset. Mortality rates for CRC are even greater for African Americans than same-aged people of other racial backgrounds.

Despite the lack of available data, the Multi-Society Task Force on Colorectal Cancer has identified two groups that they recommend receive screenings before age 50 - average-risk African-Americans, and people with first-degree family members who have a CRC history (see captions for more detail on these groups).

Future research efforts may indicate that additional groups be added to this list of early screening recommendations.

Who’s at risk? People with a first-degree family member (parents, siblings, offspring) with a documented CRC or advanced precancerous lesion before age 60 should begin screening at age 40 or 10 years younger than the youngest affected relative - whichever comes first, per the MSTF. Screenings should continue every five years when the exam is negative.

Until more is known about shifting health trends related to colorectal cancer, there are existing recommendations and tools in place which are now more important than ever to ensure that patients receive preventative care that applies specifically to them.

First is the recommendation for average-risk individuals who are not African-American to begin screening promptly at age 50. The age is not a random selection: studies show increasing incidence between ages 50 and 54 - which means putting it off by even a few years is a risky proposition.

Next is the importance of having a complete history of cancers and polyps.

This information can help determine if a patient is in an identified high-risk category, and screening can begin at the correct age, and continue at appropriate intervals.

According to the MSTF, providers should consider colonoscopies when treating younger patients who present with non-specific gastrointestinal symptoms.

Most average-risk adults age 50 - 75 should get a colonoscopy every 10 years. Other forms of screening may be recommended by your provider.

Says MSTF member Mark Pochapin, MD, “I have heard too many stories from individuals in their 40s, 30s, and even 20s, telling me that they went to the doctor with gastrointestinal symptoms and were misdiagnosed with and treated for hemorrhoids or irritable bowel syndrome for years, only to be diagnosed later with inflammatory bowel disease or advanced colorectal cancer.”
