BY DEAN OLSEN | GATEHOUSE NEWS SERVICE ebbie and Larry Weber's five daughters started playing competitive basketball in Taylorville, Ill. Four of them suffered a type of serious knee injury that plagues many female athletes.

They each tore their ACL, or anterior cruciate ligament, which is a part of the knee that helps stabilize the joint during twisting maneuvers and changes in direction that are common in sports like basketball and soccer.

Biology, hormones and neuromuscular differences make female athletes more likely than males to tear their ACLs, but doctors say research is helping point out specific risk factors and refine techniques that can almost totally eliminate the disparity.

"While we perform very successful surgery and can get the athlete back to high-level competition, it is better to prevent the injury in the first place," said Dr. Diane Hillard-Sembell, an orthopedic surgeon at Springfield Clinic in Illinois who has conducted extensive research on ACL injuries and hopes to begin publishing the results in medical Alfred, NY 14802 journals this year.

Emerging research Hillard-Sembell can empathize with injured athletes because she was a standout high school and college basketball player who underwent ACL surgery in her late 20s and early 30s on both of her knees.

She began a study in 2002 to examine theories surrounding ACL risks in females and learn what works best to prevent injuries.

Hillard-Sembell and her colleagues found that the relatively weaker hamstrings in females put them at increased risk of knee injuries.

"The hamstrings are a protector of the ACL," Hillard-Sembell said.

In males, the quadriceps, muscles at the front of the thigh, typically are stronger than the hamstrings, at the back of the thigh, by a ratio of 3 to 2. But in females, the quads may be twice as strong as the hamstrings, HillardSembell said.

But her research uncovered something new, which has since been confirmed by other researchers: the importance of muscles at the side of the hips. If those muscles are weak, the pelvis will collapse inward, making it more likely for the thigh to angle inward, she said.

A report released from The Children's Hospital of Philadelphia and State Farm Insurance Companies shows the impact of teen driver crashes extends far beyond teen drivers' families and friends. In 2008, 681,000 people were involved in crashes where a teen driver was behind the wheel. More than 40,000 were injured, and nearly 30 percent of those who died in these crashes were not in cars driven by teens.

"When most people think about those affected by teen driver crashes, they think of the teens behind the wheel. We must also consider the significant impact of these crashes on ... occupants of other vehicles, pedestrians, cyclists and other road-users," says Dr. Dennis Durbin, coauthor of the report. "Reducing speeding and alcohol use, increasing seat belt use, and eliminating distractions for teen drivers are the four calls-to-actions we see in this report that would have great impact on reducing injuries and fatalities for all road users.”

Disc disease may be genetic Symptomatic lumbar disc disease, a condition caused by degeneration or herniation of the discs of the lower spine, may be inherited, according to a new study published in the Journal of Bone and Joint Surgery.

"Although excess risk in the immediate family might indicate evidence of a genetic contribution, it could also simply indicate shared environment risks or household exposure that may be contributing to the disease," noted study author Dr. Alpesh A. Patel, assistant professor of orthopaedic surgery at the University of Utah School of Medicine. "Conversely, excess risks in second- and third-degree relatives strongly support a genetic contribution to disease, given the measurable genetic sharing in these more distant relatives and the relative absence of shared household risks."

It's different for girls An estimated 150,000 ACL injuries occur in the U.S. each year, generating more than $500 million in medical costs, according to the American Orthopaedic Society for Sports Medicine. One ACL tear can cost about $30,000 when all the costs for surgery, hospitalization and rehabilitation are included.

The 30 percent of ACL injuries that occur when athletes collide and land afterward can't be prevented in most cases. But when it comes to "non-contact" injuries, females are four to eight times more likely than males to injure their ACL, Hillard-Sembell said.

The peak ages for injuries in females are 14 and 15, then again at 19 and 20.

The main factors that put females at increased risk of ACL injuries are anatomy and biomechanics, Hillard-Sembell said. Females' relatively wider hips and pelvis — built for childbearing — create more potential for "knock-knee" landings that can put special pressure on the ACL. Females also tend to be more flatfooted, and the anatomy of their knees is slightly different from males.

How to prevent ACL injuries ■ Plyometrics is a rapid, powerful movement

that first lengthens a muscle (eccentric phase) then shortens it (concentric phase).

The length-shortening cycle increases muscular power. An example would be an athlete jumping off a small box and immediately jumping back into the air after contact with the floor.

■ Balance training commonly involves the

use of wobble or balance boards. On-field balance exercises may include throwing a ball with a partner while balancing on one leg.

■ To improve single-leg core strength and

stability, athletes perform exercises such as jumping and landing on one leg with the knee flexed and then momentarily holding that position.

Hazards of Texting While Driving While driving recently, I caught myself trying to answer an e-mail on my cell phone that I thought, at the time, was urgent. As I was attempting to type a response, shifting my attention from the road, to my Blackberry, and back to the road again, the incredible stupidity of what I was doing struck me.

The National Highway Safety Administration recently reported that almost 80% of all auto crashes are caused by distracted drivers. Further, 37% of teens in a recent study rated texting while driving as “extremely” distracting or “very” distracting, yet over 90% of all teens do so.

According to a recent study conducted by researchers at Eastern Virginia Medical School and Children’s Hospital of the Kings’ Daughters in Norfolk, driving while text messaging or fiddling with an MP3 player is an even greater hazard than talking on a cell phone while driving.

In the study, the researchers had 21 teenagers between the ages of 16 and 18 take part in a series of simulated driving experiments. Donald Lewis, MD, chief of Eastern Virginia Medical School’s department of pediatrics and co-researcher, said that each teen was asked to get behind the wheel of a simulated vehicle and drive through simulated scenery, in rural, then urban settings, in 10-minute intervals.

In the tests involving distractions that required finger movement, the participants steered the virtual vehicles erratically, weaving in and out of lanes and running over virtual people. Lewis said that the worst driving came when the participants were text messaging, likely due to being required to look down in addition to moving their fingers.

Set a good example for your kids and keep your phone off while driving. No one wants another car accident tragedy caused by texting while driving.
