he new year is well and truly upon us and whilst COVID-19 is still hanging around, it at last seems to be moving in By Joseph Allbeury the right direction. New years are of course a time for making resolutions and equally, for looking back at the year gone by. Whilst there are both positives and negatives to be drawn from 2021, there are a list of things that I would like to see the back of... like endless images of politicians on TV and the rantings of the disaffected on [anti]social media - If you’re having a bad day or someone hurts your feelings, Google “resilience” and move on... No need to record the moment in history for all to judge you by in the saner times ahead. And TikTok is officially now both the most visited website on earth and the single biggest threat to the advancement of the human race. Don’t watch–Don’t contribute! Mainstream media has divested itself of all credibility and the need to constantly post on websites to try and lure visitors in with “click bait” means that any “tweet” is hot news! I’m over Zoom calls and webinars and I will celebrate the day I no longer have to “check in” to shops and restaurants or wear a mask in public. And I could go on. 2022 is the time to return to core business, to being productive and to getting things done. To setting personal goals that are real, not ethereal. To clearing our mind of the fog of the pandemic and focussing on what matters most. Get your mind healthy, get your body healthy, distance yourself from all the noise of the world and focus on the things that matter most to you. Professionally, 2022 is the time to regain pace - put your pedal to the metal! Grow your business and grow your skills. riginally slated for 8-10 April, IDEM 2022 will now take place between 7-9 October 2022 instead. The organisers of IDEM, Koelnmesse and the Singapore Dental Association, made the decision to postpone the event in light of current travel and safety restrictions in the Asia-Pacific region. As the leading exhibition and conference for the dental industry in the Asia-Pacific, the aim of IDEM is to provide a comprehensive experience and bring together exhibitors and attendees from all over the world. With this in mind, the 12th edition of IDEM will take place between 7-9 October 2022 at Sands Expo and Convention Center; Marina Bay Sands instead of the originally scheduled dates of 8-10 April 2022. “IDEM 2022 is intended as an in-person event where practitioners can exchange valuable knowledge, network with other professionals and view new exhibits with the in-person element playing a central role in the overall IDEM experience,” said Mathias Kuepper, Managing Director of Koelnmesse Pte Ltd. “The decision to postpone IDEM to the latter half of 2022 was not taken lightly despite receiving strong interest and bookings from exhibitors and country pavilions. Before restrictions were tightened due to the Omicron variant, IDEM would have been on track for the in-person event in April. Ultimately, our goal as the organisers of IDEM is to bring together the Asia Pacific dental community to experience the full offerings of IDEM in a safe manner.” IDEM Singapore is the leading dental exhibition and conference in the Asia-Pacific region. Given the precise shading of the material, an accurate match with the A–D shade guide is easily achieved by polishing or glazing the IPS e.max ZirCAD Prime Esthetic restorations after they have been milled and sintered. This is all-ceramics at its most efficient. The successful combination of two zirconia materials has resulted in a material that offers an ideal
balance between stability and aesthetics. IPS e.max ZirCAD Prime Esthetic is available in a comprehensive range of 16 A–D and 4 Bleach shades and in 3 disc thicknesses (14mm, 16mm, 20mm) with a diameter of Ø 98.5 mm each.

steps. Two zirconia raw materials are combined in a unique manufacturing process to produce a single material that incorporates the best qualities of both. In the case of IPS e.max ZirCAD Prime Esthetic, this innovative combination consists of a robust, highly translucent 4Y-TZP zirconia raw material to achieve the required stability in the dentine area and thus to offer an attractive spectrum of applications and an exceptionally translucent 5Y-TZP zirconia raw material to provide the necessary translucency in the incisal area for a natural looking enamel. This blend ensures the lifelike appearance of restorations. An innovative filling technology is used to provide the material with its excellent seamless and layer-free progression of shade and optimised translucency. Visible layers, which are occasionally found in other multi-zirconia discs, are thus avoided with IPS e.max ZirCAD Prime Esthetic. Advanced powder conditioning is utilised to adjust the sintering kinetics and ensure a uniform shrinkage behaviour across both raw materials. This process results in a material that enables an exceptional accuracy of fit. In the final manufacturing step, the discs are compressed from all sides at high pressure in a state-of-the-art Cold Isostatic Pressing (CIP) process to improve their microstructure and optimise their translucent properties. This process also primes the material to be sintered in shorter cycles.

W
ith new ceramic pastes Fresco Gingiva and Fresco Enamel, monolithic and slightly reduced zirconia structures can be quickly and easily veneered in the gingival and vestibular regions. Especially in the gingival region, a characterisation with stains is no longer necessary, since a natural gingiva reproduction can be achieved by purely applying the ceramic pastes. Compared to conventional ceramics, Fresco pastes remain moist for a longer time and can be modelled for several hours. The consistency of the pastes can be further personalised by adding the two mixing liquids, Fresco Liquid and Fresco Gel, depending on the preferred working method.

Due to their high primary density, the pastes exhibit very low shrinkage. This means that one Fresco firing cycle is sufficient to finish the tooth shape and the gingiva after the first firing. With this firing process, zirconia structures of different sizes can be fired simultaneously. No parameters such as holding time and/or temperature need to be changed: heating and cooling rates are the only ones to be adapted, since they are directly related to the size and mass of the zirconia structure. If a correction firing has to be carried out, this can be done with the same parameters - this means that with a very simple but efficient firing process stable results can be achieved. IoT functionality for the new Programat® furnaces from Ivoclar

ith the introduction of its new series of Programat furnaces, Ivoclar has brought the Internet of Things (IoT) to the modern dental laboratory. The latest generation of press and ceramic furnaces feature smart IoT functionality, offering dental technicians comfort and reliability. The Ivoclar group, one of the world’s leading dental companies with a comprehensive range of products and system solutions for dental professionals, introduced a new generation of Programat furnaces in June 2021. With their uniquely innovative features, forward-looking enhancements and modern design, the press and ceramic furnaces have earned the approval of customers quickly. The Internet of Things connects physical objects to the virtual world. By opening a user account, customers can tap into the full potential of IoT technology. They can subscribe to a free report to receive automated monthly usage statistics. These reports provide users with a variety of relevant information, including usage metrics (e.g. operating and firing hours), remaining service life of the heating component, vacuum quality and the date on which the furnace was last calibrated. A traffic light system indicates the level of urgency with which individual steps for maintenance and optimisation should be carried out. Easy-to-read diagrams provide an overview of the furnace’s utilisation measured in the number of firing processes and the proportional shares of the different types of materials used. By logging in to their customer account, dental technicians can easily manage their devices and services and directly clarify any questions they may have. All data that is transferred to the Ivoclar server is encrypted and protected from unauthorised access by third parties with the highest degree of security.
This means that dental technicians can benefit from additional comfort and high operational safety when using their IoT embedded Programat furnace. Monthly reports keep them informed of the current state of their device.
Model variants and availability he Programat ceramic furnace is the smart specialist for high-quality firing results. This furnace is available in three variations: P710 G2, P510 G2 and P310 G2. The Programat press furnace is the reliable partner for smart pressing processes. This model is offered in two variations: EP 5010 G2 and EP 3010 G2. All of the above model variants come equipped with advanced IoT functionality and have been available on the market since June 2021.

he new M2 milling unit comfort line exemplifies precision and flexibility. With the M2 Wet Heavy Metal, the M2 Teleskoper, the M2 Dual Wet Heavy Metal, the M2 Dual Teleskoper and the M2 Dual Double Teleskoper, the comfort line comprises five milling units with fully automatic 5+1 axis simultaneous milling technology. The highlight in terms of flexibility is the extra-large Teleskoper Orbit (ø 125 mm): in combination with special interchangeable holders, it allows the processing of all common soft and hard material blanks of ø 95 mm, ø 98 mm, ø 106 mm or even ø 125 mm. With the Teleskoper Orbit, blanks can be removed and reinserted back into the orbit at the same position with high precision in micron range. Digital dentures: The next big thing An insight into everyday practice with Michael Wiernek
T
here is hardly an area of life that has not yet been affected by far-reaching technical change in recent years: “Digitalisation” is on everybody’s mind and, of course, dentists and dental technicians and their day-to-day practice have not been left untouched. In the current digital age, a solid computer-aided design and manufacturing (CAD/CAM) system is essential for cost-conscious and customer-centric dental practices. The following interview looks at the opportunities and challenges of digital dentures based on the experience of Michael Wiernek, a prosthetist from Denmark with more than 20 years of experience.

A personal yet holistic view of digital dentures and digital transformation in dentistry by Michael Wiernek enerally speaking, digital transformation has reached the dental industry and will continue to disrupt it and change dental treatment processes. According to Wiernek, the process of digitalisation in dentistry can and will pick up speed significantly as more and more interest in digital treatment processes is shown by dentists. “Going digital allows me to improve all aspects of the patient treatment: My aim is to offer my patients the highest quality of products possible and to guarantee a

READ ME FOR CPD pleasant, comfortable treatment procedure,” he said. “An additional bonus is that the number of follow-up treatments drops significantly, as the products are much more accurate than ever before.” From Wiernek’s point of view, analog denture treatments show advantages and disadvantages in comparison with digital denture workflows. In the analog treatment and manufacturing process, it is difficult to register the smallest details and take them into account in the design. Therefore, it is not easy to maintain consistent quality. Often, multiple impressions and dentures have to be made and the patient has to come to the practice for multiple fittings. Digitalisation has significantly reduced

spectrum | NEWS the steps as well as the time involved and therefore offers clients treatments that are state-of-the-art and provide superior customer satisfaction. Wiernek himself states that a main argument for using digital dentures is their high manufacturing quality and precision. “In the past, when relying on analog treatments you just hoped that the prosthesis would fit when it was ready. Today, I don’t have to hope anymore, since I know that it’s going to be perfect! And it’s like that every day. I can predict that every denture will fit. Thanks to digital dentures, I always end up delivering the best quality, state-of-the-art treatments and consequently satisfying my customers entirely.”
Do you only work digitally or with conventional dentures? e work only digitally; never with conventional dentures. I consider us to be an almost all digital laboratory. My colleagues and I were one of the first dental technician teams in Scandinavia to use digital dentures. As a result, we have created a laboratory that almost entirely eliminates the need for gypsum for producing dentures. With the digital denture method, we have reduced 10 hours of lab work to approximately 1.5 hours. At the beginning, we couldn’t believe it ourselves, but it’s true.

if you do everything with a digital denture system. I told them to be patient, to wait and see. Now I can tell them that digital dentures help you to improve patient care, treatment quality and efficiency and that is the truth.
What convinced you to manufacture your dentures with Ivoclar Ivotion? or me, the most convincing argument was the fact that with Ivotion you have a full denture plate in one, which no one else is able to provide. In addition, speed is an enormous advantage, since you don’t

Significantly improved software, increased flexibility and processing speed as well as better manufacturing tools support the digital transformation process in dentistry. But it doesn’t mean the end of dental technicians specialising in denture fabrication. Indeed, it is a chance to extend their expertise to a digital platform. We talked to Michael Wiernek and he gave us a very personal insight into his new digital work experience.

When and why did you decide to take this step into digitalisation? ack in 2017, I was in Liechtenstein visiting the headquarters of Ivoclar. This was the first time I really experienced what digitalisation could look like in the dental industry. When I experienced the new technology, I thought that this is how I wanted to work in the future. I then attended a small event at 3Shape in Copenhagen in January 2018, where the new scanner was presented. They told us that everything is set for digital dentures from now on. Two days later, I took the chance to do something different from what my colleagues were doing by entering a new era of dentistry. In March 2018, I bought the scanner, the software and a computer and I have been using them ever since.

Did you go through a big learning process or was it easy and intuitive for you? or me, going digital has been a unique yet tough journey, which I started in March 2018. The following 6 months involved truly hard work because no one (in Denmark) knew how to do it. After the summer of 2018, I had acquired a lot of know-how, but still I couldn’t do everything I wanted to. So, I went outside Denmark (US, Holland and Germany) to network and exchange knowledge with experts in the field. It took me many evenings and nights trying to figure things out at home, but after a year I fully committed to it and it paid off. I know almost everything about digital dentures now and even when I don’t, I always find a way to make it work. In 2018, my colleagues told me, Michael, you’re crazy. What are you doing? It will be the end of your business

have to glue the denture base and teeth together. The biggest difference between a conventional denture and a digital denture in my opinion is that I can significantly reduce patient visits by working with Ivotion. We can cut them down from 5-6 visits to 2-3 visits per denture and we’re fully done with satisfied and smiling patients.
How were your first experiences with Ivotion in your everyday work? fter having gained a deep understanding of the digital process and its steps, it was quite easy to design a denture with Ivotion. In addition to saving a lot of working time, this method may also help you to lower costs in the future. It was a great experience for me to be the first in Scandinavia to work with Ivotion.

Not only did it fit perfectly, but it also had great aesthetic appeal for the patient. I was overwhelmed - as this was a real breakthrough for the industry.”
How did the clinical and manufacturing process change atients have become much more relaxed since I can assure them that the result will fit them perfectly. I have also been able to take away the fear of patients who were anxious about dental interventions. The finished product simply fits better than in the past. The feedback is great because they only have to attend one or two appointments before the denture fits. The patients are much more satisfied than in the past. With Ivotion, they no longer have the same pain level as with the conventional method, since it is not only much better and faster, but also fits them perfectly. In addition, the reproducibility of the created prosthesis is simple and straightforward. Digital dentures are an absolute win from the patient’s point of view.

When it comes to the PrograMill PM7, I thought about it for two years, because of the high investment amount. However, I knew I couldn’t resist due to the fact that the PM7 is a very capacious mill for a high productivity. If I’m investing in something new, I’m going to go with the

The future of digital dentures ental treatments in the future will reflect your dentistry standards. It will become normal for dentists to work with digital dentures and patients will also increasingly ask for them in the future. Digital dentures will help you to create a positive emotional relationship with your patients. They will be enthusiastic about the method, as they will be even more involved in the process and able to follow the procedure themselves. Going digital will ensure and secure your dental business in the near and distant future. If you ask me, if I can recommend the Ivotion system, my answer is clear: Always. And I do recommend it to my colleagues all the time. Of course, it is an expensive system to invest in, but if you want to do high-end dentures, then the only way is to go digital. But it is not only about the quality: I would say if you do not invest in digital dentures now, you may have to close your business in the future.”

I have never regretted it, as for me it is the best available machine. And it pays off.

Digital dentures: The next big thing An insight into everyday practice with Michael Wiernek here is hardly an area of life that has not yet been affected by far-reaching technical change in recent years: “Digitalisation” is on everybody’s mind and, of course, dentists and dental technicians and their day-to-day practice have not been left untouched. In the current digital age, a solid computer-aided design and manufacturing (CAD/CAM) system is essential for cost-conscious and customer-centric dental practices. The following interview looks at the opportunities and challenges of digital dentures based on the experience of Michael Wiernek, a prosthetist from Denmark with more than 20 years of experience.

enerally speaking, digital transformation has reached the dental industry and will continue to disrupt it and change dental treatment processes. According to Wiernek, the process of digitalisation in dentistry can and will pick up speed significantly as more and more interest in digital treatment processes is shown by dentists. “Going digital allows me to improve all aspects of the patient treatment: My aim is to offer my patients the highest quality of products possible and to guarantee a

pleasant, comfortable treatment procedure,” he said. “An additional bonus is that the number of follow-up treatments drops significantly, as the products are much more accurate than ever before.” From Wiernek’s point of view, analog denture treatments show advantages and disadvantages in comparison with digital denture workflows. In the analog treatment and manufacturing process, it is difficult to register the smallest details and take them into account in the design. Therefore, it is not easy to maintain consistent quality. Often, multiple impressions and dentures have to be made and the patient has to come to the practice for multiple fittings. Digitalisation has significantly reduced

Clinical stages Analysis of the facial aesthetics using the Digital Smile Design (DSD) approach indicated that the patient’s nose was asymmetrical. This aspect was omitted and the analysis focused on the remaining reference points. The incisor midline was regular. Only small adjustments of tooth length in relation to the lip line were needed (Figure 3). Following the DSD design approach, a suitable colour was chosen. The patient’s initial selection was Bleach 2. This would have created excessive contrast against the darker natural teeth. After further discussion, shade Bleach 3 was chosen as the body shade and Bleach 4 as the cervical shade.

fter reduction of the maxillary teeth and removal of a crowded displaced mandibular incisor, the teeth were scanned using a CS 3600 intraoral scanner (Figures 4 to 5). A traditional putty-wash PVS impression was also taken. From the digital scan, the OBJ and STL files were sent to the dental laboratory and used to create virtual models within exocad DentalDB software. The intent was to use acrylic (PMMA) for the temporary. This would make occlusal adjustment easier and simplify the process try-in to verify the patient’s preferences in terms of tooth shape and shade and overall tooth alignment (Figure 6). The PMMA temporary was milled from PMMA using a VHF 4K4 milling machine. This was then tried in, the occlusion adjusted and photographs taken (Figure 7). The lower teeth were prepared, digitally scanned and a putty-wash impression also taken (Figures 8 and 9).

Using scans of the adjusted PMMA temporaries, as a pre-op scan, we used exocad software and determined how much additional tooth reduction and preparation was needed. This allowed the final preparations of the upper teeth to be planned and completed (Figures 10 and 11), followed by the lower teeth, as a separate step in the process (Figures 12 and 13). The framework was milled from zirconia (Sagemax NexxZr T OM2) using a VHF 4K4 milling machine. After removing connector residuals, frameworks were cleaned and prepared for sintering. After sintering, their fit was checked against the plaster models poured from the PVS impressions taken earlier. After this check was completed, they were delivered to the dental surgery for try-in. Bite records were taken so the models could be mounted in an articulator (Figures 14 to 16), to check the frameworks.

The zirconia was then etched using the Zircos-e system in our laboratory and the ceramic work commenced using Creation ceramics by Willi Geller.
Veneering or Bleach shades, it’s important to achieve a natural appearance of the tooth. In this case, for the cervical and approximal regions, Bleach Dentine BD-A was used, while on the remaining areas, Dentine BD-B was used. A mamelon effect was enhanced with Transpa pink TI-03. Space separating, finger-like structures were filled with equal amounts of Clear window UC and Transpa TI-05 materials used alternately. A thin layer of neck-transpa HT-52 khaki was applied on the palatal dentine surfaces. Neck-transpa HT-51 iridescent was used as the final layer on cervical areas, while bleach S-AB enamel was applied on the remaining areas, with a 20% addition of clear CL-O to increase enamel transparency. A “halo” effect was obtained using enamel PS-0 white. Texturing of the surfaces was undertaken. The fitting of the whole assembly was checked for the last time. At this stage, some final adjustments of the occlusion were undertaken (Figures 17 and 18). After fusing the porcelain, the restorations were inserted and final cementation was completed. The situation at the final visit is shown in Figures 19 to 21.

Summary his case shows the use of a digital workflow, with traditional models used to check fit and occlusion. This brought out the best from both approaches, giving certainty about proper fit and correct occlusion, making the procedure more predicable.

Enhancing aesthetics simply and naturally: The Aidite protocol By Umberto Scorza
T
hese days metal-free restorations are the mainstay of most dental laboratories and among the many materials involved, zirconia is arguably the most common. There are many types of zirconia produced by many different companies, so it’s important to have a solid understanding of the differences in materials and select the optimal products to establish a productive workflow. The Aidite company has released their latest generation of zirconia, 3D PRO ZIR. When this product is combined with their new Biomic stain kit, excellent aesthetic outcomes are achieved. 3D PRO ZIR is a multilayer and multistrength monolithic zirconia disc that offers up to 57% translucency in the incisal/ occlusal areas, with a multi strength structure from 700mpa up to 1050 mpa . These properties are important to help make the material suitable for most aesthetic indications. It has many advantages such as highly aesthetic characteristics that closely mimic layered porcelain; longevity, as given the crystalline structure of zirconia, it is very stable and the percentage

Aidite Biomic stain kits. of chipping measured under load testing is very low; laboratory efficiency: for example with these universal zirconia discs we can minimize the need to stock other kinds of zirconia discs in the inventory as it’s a “do all” material. 3D PRO ZIR also has a different layering structure as it shows a natural gradient from the dentine to the enamel without steps (Figure 2). To complete the aesthetic characterisation of the zirconia, Aidite has also introduced the Biomic stain kit for gum and aesthetic colouring, that is also usable with all types of ceramic. This combination of materials optimises the workflow to create simple procedures that give great results.

Full contour zirconia onolithic zirconia is now useful for all types of restorations from a simple single crown to full arch “All on X”. When creating large span restorations like “All on X”, however, one of the main problems that can be encountered for a porcelain layered framework is chipping. Patients tend to have a different sensitivity in masticatory movement and lack proprioception, so massive forces can be transferred to the prosthetic. For this reason, the restoration is always subject to differing pressures at certain points, which frequently causes chipping of layered porcelains. To minimise the risk of chipping, using a highly aesthetic monolithic zirconia like Aidite 3D PRO-ZIR is an effective solution. Figures 3-6 documents a case that combines a titanium bar with a monolithic zirconia suprastructure externally characterised with Biomic stains. To achieve the best results, the zirconia surface requires the correct treatment. When preparing the surface to accommodate

the stains, we typically hand polish the entire surface, which is fundamental to enhance the translucency of the material. This phase is carried out using burs, rubbers and diamond paste. Subsequently, the zirconia overlay will be sandblasted and steam cleaned in order to fully prepare the surface to accept the stains. The number of firings for the stains can vary, however we usually attempt to finalise the restoration in a maximum of three firings - Two for the characterisations; and one for the glazing which can be applied as normal fluo glaze or as 3D glaze paste. The aesthetic of the gum was created by combining Biomic stains and a very thin 0.3 mm layer of porcelain. The 3D glaze pastes contain ceramic particles which makes the restoration have a different light impact and also gives more depth. This concept can be utilised in different situations. The next case shows this protocol can also be useful for anterior restorations where we have to mimic natural teeth in the patient’s mouth. Same simple procedure same high aesthetic outcomes.

Anterior restorations s mentioned previously, this concept can also be used for anterior restorations when teeth characteristics allow us to choose a monolithic solution. The Zirconia blank for the case shown in Figures 7-9 was chosen in shade A4 and the aesthetic characterisations of the restoration were consequently processed in three bakes. Two for the stains and one for the 3D glaze paste which was completed using 3DN glaze. As we can see with simple procedures we can give high quality aesthetic results.

Microlayering technique on 3D PRO ZIR n order to achieve teeth characteristics on difficult shades, the microlayering technique offers a very useful solution. The microlayering technique consists of creating a thin porcelain layer on the buccal zone of the restoration combined with internal and external stains. The common cut back thickness varies from 0.3 mm to 0.5mm, therefore it is known as microlayering.

About the author As per the case documented in Figures 10-13, on our left the shade was quite particular, therefore we have finalised the restorations in 3 steps; Internal stains - Porcelain layering - Glaze/ External stains. To achieve the best outcomes, the choice of the zirconia’s shade has to be accurate in order to gain a significant aesthetic base to minimise the layering. In this case, the zirconia blank was chosen in shade A3.5 and layered with different powders like dentine enamel and opalescent which combined with some internal and external stains to achieve the outcome.
