By Jim O’Shea Anxiety. Sleeplessness (contd.). If you have a high level of stress hormones such as cortisol, you will have difficulty in sleeping and staying asleep. This hormone affects the metabolism of glucose, regulates blood pressure, controls insulin release for blood sugar maintenance, and affects the immune function. Your cortisol level may have become raised during the day due to some stressful event or situation, and may still be elevated when you go to bed. You can get some supplements, which will reduce your cortisol levels. It would be advisable to consult your GP when using supplements. Sleep experts have many suggestions to promote refreshing sleep, and, as I have mentioned, these would be explored in sleep clinics. These experts, for example, recommend having a fixed time for going to bed (rather than be dictated by TV programmes!!). It is also beneficial to relax before going to bed. This would mean avoiding TV programmes that might make you tense, or reading serious books. In that context, it is also advisable to avoid exercise within 4 hours of going to bed. So, while exercise is strongly recommended to reduce stress and anxiety, it can be counterproductive if taken too close to bedtime. Having a good sleeping environment can also help. This means not having your bedroom too hot or too cold. It also means using the bedroom for what it is intended – sleep (or other romantic activities!). Have heavy curtain and/or special blinds that keep out the light. Noise can also interfere with sleep. If we live near a busy road, the sound of cars or airplanes can disrupt our sleep, although we eventually get used to them. I am not so sure if we can get used to our partners’ snoring. I think not. By the way, it is a myth to believe that only men snore. It is a real torment for many people. As we get older, we tend to snore more. There is mutual blame, especially among elderly partners, because of snoring. I understand that there are many devices now to counteract snoring. Sometimes wearing earplugs can help, although I find them intrusive because I am so aware of them, and no longer use them

when I am away from home. Double glazed glass can reduce noise (apart from snoring!) Make sure that you have a comfortable mattress (not too hard and not too soft), and, while expensive, it is worth getting a good pillow. It is best to avoid napping during the day if you can. Better to go for a walk if you are feeling tired or sleepy. As mentioned, it is advisable to have a regular time for getting up in the morning and going to bed at night. Irregularity in both these areas (which many of us are guilty of) can disrupt what is called the circadian cycle. The circadian cycle comes from the Latin words circa meaning round, and dies meaning day. Recent research has shown that adults have a built-in day, which averages about 24 hours, preferably 8 hours in bed and the remainder on the move. Incidentally, indoor lighting affects circadian rhythms. Most people attain their bestquality sleep during their chronotypedetermined sleep periods. The term “chronotype” is used to characterize a person's sleep patterns. There are three main chronotypes: morning, evening, and mid-range. In addition to influencing someone's sleep patterns, one's chronotype also appears to have a role in a person's more general physiology. For example, depending on whether one is a morning or evening person, sleep reduction can have different effects. Morning people, sometimes called larks, tend to get up much earlier than the general population, and they go to sleep earlier. Evening people or owls, on the other hand, stay up late and therefore sleep in later. People who are indifferent or mid-range can vary their circadian rhythms, or they fall into an average sleeping and waking pattern, which is between the two possible extremes. Involvement in shift work can therefore contribute to sleeplessness and anxiety. Sleep experts offer a few other pointers that help to relieve insomnia, for example, having a regular sex life can also be beneficial, because sexual intercourse releases endorphins, which promote relaxation and calm. We are also told that it is unwise to have a big meal just before bedtime. We are advised that we should have our evening meal before 6 pm, although

This is an amino acid which helps one to sleep, and is to be found in bananas, fish or turkey. Sleeping tablets help to solve the problem for a while. They are not, however, the answer in dealing with chronic sleeplessness. If they are taken for a long period, they become less effective, although there is a psychological aspect insofar as people who take them think that they are working. Just shows how important our thoughts are in the context of sleeplessness! The evidence would suggest that a combination of sleeping tablets and the measures mentioned earlier work well, although I am very reluctant to tell anyone to take medication, unless it is really necessary.
