General Conditions 1. Unless otherwise required by the agent, the system installer must backfill (cover) a system within 10 days after issuance of a Certificate of Satisfactory Completion for that system. 2. In accordance with Oregon Revised Statute (ORS) 454.665, this certificate is issued as evidence of satisfactory completion of an on site sewage disposal system at the location identified. 3. Issuance of this certificate does not constitute a warranty or guarantee that this on site disposal system will function indefinitely without failure. 4. This Certificate of Satisfactory completion is valid for a period of five years for connection of the system to the facility for which it was constructed. After the five-year period, an Authorization Notice, Alteration permit, or construction-installation permit may be required under OAR 340-071-0160, 340-071-0205, or 340-0710210. 5. A permittee may appeal the denial or revocation of a Certificate of Satisfactory Completion in accordance with ORS 183.3110 through 183.550 and OAR Chapter 340, division 11. Special Conditions • No livestock, structures, utilities, vehicular traffic or parking within 10’ of the initial or reserve drain field areas. • Septic tanks are not designed to support traffic. No vehicular traffic within 5’ of tank(s). • Seed drain field area with shallow rooted vegetation, a dry ground grass seed mix or approved equal. Do not plant shrubs or trees on or near drain field areas. • Any alteration of the site including but not limited to compaction, fill, excavation or excessive irrigation will shorten the life of the drain field. • Any alteration of the soils in the reserve area adversely impacting the ability of the soils serving the reserve drain field area may render the site evaluation null and void.
