Features of the Plans •

A large provider network: BCBSD has the largest network in the state and provides EPO members with access to the nationwide BlueCard® network, with 90 percent of hospitals and 80 percent of doctors in the United States.

Health Reimbursement Arrangements (HRAs) and Health Savings Accounts (HSAs): Two of the DSCC health plans available are HSA-or HRA-compatible, giving consumers a greater control in the decisions affecting how they use their health care dollars. For more information about HRAs and HSAs, please contact a broker who is a member of the DSCC.

Valuable discount programs: BCBSD offers discounts on a variety of alternative therapies, as well as wellness and vision services. To receive the discounts members simply show their BCBSD identification cards to participating providers. What You Should Know

With this in mind, the DSCC and BCBSD have partnered to bring you health benefits plan options that are well suited to meet small business owners’ needs. These health plans are exclusive to the DSCC.

The DSCC health plans are only available to companies that are members of the DSCC.

These plans are competitively priced to be attractive to the small group market.

Available Plan Options The options available to you through the DSCC are a mix of EPO and IPA benefit platforms. They have various deductibles and include options compatible with Health Savings Accounts (HSAs) and Health Reimbursement Arrangements (HRAs). All of these plans are competitively priced to be attractive to the small group market. Here is a quick look at the unique plan offerings:

Prescription Drug Plans •

With a DSCC health plan, you can take advantage of some of our most popular small group Rx offerings.

Dental Benefits •

Medical options for the DSCC health plans include a preventive and diagnostic dental benefit, which is exclusive to the DSCC. This dental plan provides coverage for X-rays and teeth cleanings at 100%, as well as topical fluoride treatments for children.

Groups have the option to add enhanced basic dental coverage to their dental benefits, which would cover fillings at 80% with no deductible.

• Blue Care® Independent Practice Association (IPA) — $15/$25 80%

• BlueAdvantage® Exclusive Provider Organization (EPO) 100% Plan with a Health Reimbursement Arrangement (HRA) — $1,300/$3,900 Individual/Family deductible

• BlueAdvantage® EPO 100% Plan with a Health Savings Account (HSA) — $3,000/$9,000 Individual/Family deductible
