think much of it at first. When she went to the bathroom, she noticed some clots in her pad— again, it’s normal to have spotting after giving birth … but there
shouldn’t be large clots, and new moms shouldn’t go through pads quickly. As a safety precaution, Kimmel called her OB-GYN, who told her to monitor the bleeding

t was exactly one week after Kristen Kimmel’s healthy baby Kate was born. The in-laws were over; her husband, Tom Mathews, was making lunch; and earlier in the week, she had even taken the dogs for walks. Everything was fine. As Kimmel sat down to breastfeed, she felt some unusual cramping.
and then call the office back if the situation worsened. It did. Kimmel says within 20 minutes, she began feeling heavy and ran to the bathroom. When she looked behind her, she saw a trail of blood. “I passed a clot, but it was a clot the size of a softball,” she reveals. She called her OBGYN again, and this time, she was told to go straight to the office. As Kristen stood up, she almost passed out. Her husband helped her to the car, and they rushed to the doctor. How was it possible to be feeling perfectly fine one moment, and bleeding excessively the next? WHAT IS PPH? One of the leading causes of maternal mortality is postpartum hemorrhaging, or PPH—that is, if it is left untreated. But only 3–5% of women experience PPH, according to Dr. Brian Rinehart, the director of the maternal highrisk program at Texas Health Presbyterian Hospital Dallas. What exactly is PPH? It’s defined as excess bleeding after delivering a baby—some professional organizations say 500 cubic centimeters (or 500 milliliters) after a vaginal birth or 1,000 cc after a C-section; however, the World Health Organization suggests that figure is 1,000 cc or greater of blood loss in the first 24 hours. Rinehart explains that, ultimately, how much blood is too much will vary from patient to patient. There’s also late or secondary PPH, the rarer form, which occurs after the first 24 hours. That’s what Kimmel experienced. There are three main reasons that PPH occurs. These are uterine atony (when the uterus doesn’t contract after the baby is born); lacerations, or tears, at birth; and retained placenta or placenta fragments. We know the immediate causes of PPH, and research suggests that certain conditions—including placenta previa, overdistended uterus, hypertensive disorders of pregnancy, obesity and having multiple babies—may increase the chances of PPH. But in the majority of PPH cases, there are no underlying risk factors, says Rinehart—so
technically, there’s no sure way to prevent it. “There is no magical dietary supplement or routine to make yourself healthier to avoid [PPH],” he says, adding that significant bleeding is harder on someone who is unhealthy. “The best way to prepare is to be in the best shape you can be during pregnancy, so that if this should occur, you can tolerate it.” BE PREPARED Kimmel was still bleeding when she and her husband arrived at the doctor’s office. “I was like, ‘You guys are going to want to have me come back right away because I don’t want to leave blood in your waiting room,’” she says. Paramedics were called; Kimmel was taken to the hospital. She remembers asking if she was OK, and the doctor saying only, “You’ve lost a lot of blood.” At Texas Health, patient teams are prepared for PPH through regular drills, Rinehart says, and there are medications to treat PPH in every labor and delivery room. Having the appropriate medications, uterus clamps, blood banks, anesthesia (if necessary) and medical procedures down to a science can save precious minutes. “It’s not about preventing it because it’s not preventable, but how to deal with it in a timely fashion, so Mom doesn’t get sick,” Rinehart explains. He adds that the vast majority of patients respond to manual stimulation of the uterus (to make it contract) or medication; surgery is considered a last resort. “Most women recover from postpartum hemorrhage within the normal recovery time for birth,” he says. “Those that require surgical intervention and transfusion of large amounts of blood may stay in the hospital for a few extra days.” For moms with secondary PPH, treatment is a little different. Kimmel was quickly hauled in for a dilation and curettage (or D&C) to remove part of her placenta that was still inside the uterus. This condition is rare, but when it does occur, it can become life-threatening, according to the American Pregnancy Association. The surgery went well, and by the end of the day, Kimmel was on her way home. She was given
medications to help increase her iron levels, and she says it took about a week for her face to gain color again. “Don’t be shy about asking those questions [about PPH],” she shares. “Trust your gut.” You know your body better than anyone else—if you suspect that something’s wrong after you’ve given birth, call a health care provider immediately to save time and ensure your safety. And if you’re pregnant and concerned about PPH happening to you, find out what precautionary measures your hospital has in place. WHAT IF PPH HAPPENS A SECOND TIME? Since recovery time is usually brief, the aftereffects of PPH may be primarily mental or emotional—for example, offers Rinehart, PPH can interrupt bonding or breastfeeding, which is hard on some moms. Losing a large amount of blood is obviously very scary and stressful as well. Some women may therefore be fearful of having another child and potentially going through the trauma of PPH yet again. “We had always kind of thought we would have multiple children, and after what happened, I was very scared for my health,” Kimmel admits. Though you are more likely to experience PPH if you’ve gone through it previously, the reoccurrence risk is only 15%, says Rinehart. If you’re worried about another delivery after PPH, make sure your provider knows you had it last time, and that they’re equipped to handle it if it happens again. Despite their worries about having a second child, Kimmel and her husband finally “decided to go for it,” she says. They had moved, and she felt very confident in her new health provider, who listened and took extra precautions. Baby Jorie arrived, and Kimmel says she breathed a sigh of relief when the first week after the birth passed without incident. “This is not a topic I’d ever want to scare new mothers about,” Kimmel reflects, “but at the same time, I do think that it’s something important for women to know about. I think it’s good to be aware.” dfwchild.com / The Well Guide

GUT RESPONSE easing kids’ stomach woes

hortly after her oldest daughter’s birth, an Argyle mom we’ll call Leah got her first taste of the chronic digestive troubles to come. “My daughter had terrible acid reflux,” recalls Leah (who asked to remain anonymous for her daughter’s privacy). “I tried different formulas in an effort to ease her discomfort. She was put on Zantac as early as she could be, around 5 or 6 months, so she could hold formula down. Once she reached the ‘food eating’ stage and transitioned off formula, it improved, but she had severe food aversions.” 18

Leah’s little girl would eat only bland foods—“nothing with seasoning and no veggies. Very few fruits,” her mom says. Then, at age 4, Leah’s daughter developed a serious body odor problem. “That really threw me,” Leah recalls. “No way a child should smell that bad. I didn’t realize at the time that both of those things were signs of gut issues.” Gut issues can have a variety of signs and causes. If your child is suffering, you’re not alone. A PEDIATRIC EPIDEMIC “I often joke in my clinic that everyone is constipated until proven otherwise,” says Dr. Alina Olteanu, who practices pediatric integrative medicine at Whole Child Texas in Frisco. “No day goes by when I don’t treat a child or a baby with tummy issues, ranging from infantile colic and feeding difficulties to chronic constipation and chronic abdominal pain.” Other conditions associated with poor gut health may surprise you: Olteanu lists allergies, asthma, eczema and other skin issues, anxiety, attention difficulties and sleep problems. According to Dr. Constantine Kotsanis, an otolaryngologist who is the medical director for the Kotsanis Institute for Functional Medicine in Grapevine, even autism can be traced back to digestive issues. “Autism spectrum disorders are primarily gut disorders that affect thinking and emotions,” he suggests, adding that “almost every child we see has moderate to severe gut issues.” So even if your child isn’t showing traditional digestive symptoms, you may benefit from talking to a professional. “The health of the child starts with healthy nutrition and a healthy gut,” explains Olteanu, who uses both traditional and alternative treatments with the children who come through her office. Some parents whose children have gut problems say they’ve had trouble getting more mainstream pediatric practices to take their concerns seriously. “Our pediatrician did not believe any issues were serious enough to look into further,” says Leah. “I knew in my heart something was going on and had to go mostly on my own to figure things out. I searched a very
long time to find a doctor who would do more than just look my daughter over and say nothing was wrong.” Olteanu has a more optimistic view of the mainstream medical community but notes the full implications of gut health aren’t always understood. “Lots of pediatricians use probiotic supplements for a variety of common conditions,” she points out. “However, more work needs to be done to increase awareness of the gut-brain connection, especially how stress can affect gut health and how improving nutrition and gut health can have a positive impact on a child’s mood and development.” Kotsanis advises that gut health be addressed “very aggressively” during a child’s first eight years. “The reason is the fact that 70% of the immune system lives in the gut,” he says. “Also, 80% of brain hormones are made in the gut. A healthy gut influences the brain very positively; an unhealthy gut is reflected in an unhealthy brain.” GET GUT-HEALTHY In Leah’s case, it unfortunately seems like her daughter’s initial treatment led to even more trouble. “The doctors I found who actually believed something was going on have agreed this was caused by the use of Zantac at such an early age and for such a long time during the development of her digestive processes.” Even now, at age 13, Leah’s daughter has lingering gut problems. The teenager has a stalled metabolism; she also has a very limited palate and remains unwilling to try new foods that aren’t bland. But there have been some improvements. “We discovered a few years ago that the extreme body odor was being caused by milk. At the recommendation of one great doctor”—coincidentally, that was Olteanu—“we removed milk from her diet, and within a week we noticed a significant decrease in the odor. It was amazing, and I felt terrible that I hadn’t thought of that earlier,” shares Leah, who adds that her daughter also has fewer bathroom emergencies. And she has also developed a better understanding of what foods support good digestion. However your child’s
gut issue presents, there are recommendations to ease the problem. Maintain a healthy lifestyle, emphasizes Olteanu. Don’t underestimate the value of enough sleep, physical activity, fresh air, happy relationships and stress management. In addition, be thoughtful about what your child eats. Kotsanis says “toxic fast foods, sugar, wheat, corn, soy and milk products” are all causal factors of gut problems. You can also improve gut health through diet; Olteanu notes that an anti-inflammatory diet can be extremely helpful. The diet’s focal points include fruits, veggies, whole grains, plant-based proteins (beans and nuts) and healthy fats. Stay away from, or minimize, foods that are highly processed, greasy, very sweet or have artificial dyes. “Some additives can act like or become neurotoxins,” warns Olteanu. Dessert isn’t a complete no-go; it should just be a special treat. “Help your child develop a palate for dark chocolate and fruit-based desserts,” Olteanu suggests. With fruits and veggies, encourage your child to eat a “rainbow”—a variety of colors. For children who have trouble with cow milk, there are dairy-free alternatives for calcium intake and vitamin D. Talk to your doctor before eliminating any food groups, though. You can serve fermented foods with natural probiotics, such as pickled veggies and sauerkraut, and prebiotic-rich foods (apples and bananas, to name a couple). And have your children drink water. “Insufficient water intake can slow down everything from cellular function to your metabolism and your ability to fight off infections,” Olteanu says. While changing an eating regimen is undoubtedly hard, “the anti-inflammatory diet can be enjoyable,” she adds. “But it’s the health benefits and the visual improvement of your child that’s most rewarding.” Of course, your child doesn’t need to be currently experiencing a gut-related condition for your family to adopt these recommendations. As Olteanu notes, “It’s always much easier to prevent any illness than to treat it.”

ailas was a teeth-grinder for as long as his parents could remember. He also wet the bed more nights than not. “He was already 7 years old and we had to have him on Pull-Ups,” says his mom, Angela Brennan.
He had attention issues. Breathing issues. Growth issues—doctors said at age 8 he had the bones of a 5-year-old. Thanks to his tongue, of all things. “From the tip of the tongue to the tip of the toe is one fascial tissue,” explains Dr. Dunia Korous, Kailas’ dentist, who practices at Millennium Smiles in Frisco. “When the tongue is restricted, it pulls everything together. When the lower jaw gets narrow, the upper jaw follows, and when the upper jaw follows … the airway also gets a little narrower.” In Kailas’ case, the restriction is ankyloglossia, or tongue-tie. A pesky strip of tissue anchors the tip of his tongue to the floor of his mouth, limiting the tongue’s movement and, unfortunately, kicking off a conga line of other symptoms. You might associate tongue-tie with latching problems and nipple pain during breastfeeding. As more mamas get their infants’ troublesome tissue cut or lasered, some doctors have labeled the treatment a fad—at best, uninformed scaremongering, and at worst, a scheme by practitioners to profit from unnecessary procedures. Then there are kids like Kailas, and adults like our magazine’s creative and content director, whose tongue-ties have contributed to problems with sleep, breathing, posture and focus, among other woes. Tongue-tie appears to have compounding effects over time—literally from head to toe—yet dentists and pediatricians may not look for it. “I didn’t even know that it was an issue as you got older,” says Brennan. “I thought the only time kids had issues with tongue-ties was with breastfeeding.” dfwchild.com / The Well Guide

SNIP IT IN THE BUD The fix for tongue-tie sounds simple (snip, snip!) but experts

say the brief procedure is best with a side of myofunctional therapy—that is, a retraining of the mouth muscles. Patients are prescribed tongue exercises in order to learn to swallow correctly, increase their range of motion and prevent the clipped tissue from regrowing. These glossal workouts might include touching the upper lip and licking a Popsicle. Korous believes in starting therapy before the tongue-tie release so that the tongue knows what to do once it’s free to move about the mouth. Though Kailas hasn’t had his release procedure yet, for months he stood in front of the bathroom mirror alone, three times a day, to move his tongue. “He gets a little embarrassed doing the facial exercises with people watching him,” Brennan reveals, adding that toys and other rewards motivated her son to keep it up. Cole recommends facial exercises too but says the release is most important—even without therapy, kids can adapt. “It’s better to do it poorly than to not do it,” he says. Here’s the thing about tongue-tie: Even dentists who agree on its significance don’t agree on exactly how it ought to be fixed or the extent of its impact. For example, Korous

believes that tongue-tie can lead to sleep breathing disorders like sleep apnea; Cole says that tongue-tie may exacerbate airway issues but isn’t the cause. Both absolutely agree that correcting tongue-tie can change people’s quality of life, even in small ways. After doing thousands of tongue releases, Cole went under the laser himself. “I thought, You know, when I get that released, I’ll be leaping tall buildings,” he remembers. “Nothing. I could not tell a thing, except it was sore for a few days. About three weeks later, I thought, You know, I can turn [my neck] to my left and my right and it doesn’t catch anymore.” Over many months, Kailas has been doing facial exercises and wearing mouth appliances to expand his jaw. Though his tongue is still tied, he’s already out of Pull-Ups. “I think he’s more confident,” Brennan says. “He would have a lot more temper tantrums; now it’s very minimal. His teeth look significantly better.” Brennan is helping spread the word about tongue-tie to her friends and neighbors with children. “If you don’t start it and correct it early,” she says, “then it’s gonna get even worse as they get older.”

THE MORE YOU KNOW How is it that many parents and professionals are in the dark about the effects of untreated tongue-tie? Dr. Stacy Cole of Clear Fork Healthy Dental Care in Fort Worth says dental programs have a lot to cover in order to satisfy certification requirements, and there’s simply no more room in the syllabus. “A lot of our education is after school—it just kind of gets us to where we don’t kill people,” he jokes. “And then we get out and start realizing more and more what we don’t know.” Korous says the symptoms also evade detection because parents don’t know to ask, and kids don’t think to tell. “They don’t even talk about it because it’s the norm—it’s what they’ve been dealing with all their life,” she explains. Ideally, tongue-tie is caught on day one; in fact, Brazil requires every infant to be examined for tongue-tie before leaving the maternity ward. When it’s not caught, kids and adults may show a range of symptoms, like Kailas, or just one or two. Thankfully, more professionals are beginning to recognize the signs and recommend treatment.

irst there were tantrums. Then uncooperativeness. “She couldn’t transition between activities well either,” says Keri Stoner Davis of Lewisville, whose daughter Kaylee was in preschool at the time. “They told us she would hang onto her anger for days.”
Jennifer Nintzel, a Mansfield mother, has dealt with different behaviors attached to the same diagnosis: anxiety. Her son Liam seemed increasingly apprehensive. Sleepovers became a nightmare. He’d want to be with friends, then in the middle of the night Nintzel would get a panicked plea for pickup. 40

“Even at home, when new people were invited to the house, he’d ask all these questions about who they were and why they had to come over,” Nintzel reveals. Nintzel brought her concerns to their pediatrician but says the doctor “pretty much discounted the whole situation” and labeled it as a phase. “It’s hard because other people don’t understand,” she continues. “It’s sort of like an invisible thing.” INSTINCTS ON OVERDRIVE Invisible though it may be, anxiety is pervasive: According to the Anxiety and Depression Association of America (ADAA), anxiety disorders affect 25% of children ages 13–18, and the National Institute of Mental Health puts this figure closer to 32%. If untreated, experts say, kids are at higher risk of performing poorly in school and engaging in substance abuse as well as missing out on important social experiences. Nicholas Westers, a clinical psychologist at Children’s Health and assistant professor at UT Southwestern Medical Center, says parents often miss anxiety indicators because it can manifest differently in children. “Because younger children don’t have the emotional vocabulary yet to express when they’re anxious or scared, the child might be acting physically aggressive or breaking things at home, or even hiding under the bed when it’s time to go to school,” he explains. Ardis Lo, a counselor and play therapy specialist at Therapy on the Square in McKinney, says children commonly exhibit signs of separation anxiety as early as ages 3 to 4. “By the time they’re in second to third grade … kids start becoming more aware of and anxious about the academic expectations,” she explains. “We tend to see social anxiety becoming more of an issue for pre-adolescents, although that can start younger too.” Westers notes that anxiety in and of itself “is not a bad thing. Emotions serve functions, and our fight-or-flight response can keep us safe. Anxiety is only too much if it begins to impair a child’s functioning.”
ANXIOUS PARENTS, ANXIOUS KIDS Someone with a parent or sibling with social anxiety disorder is reportedly two to three times more likely to develop the disorder than someone without a family history. Some say anxiety may seem to run in the family due to passed-on behaviors. “When parents are hypervigilant, anxious, worried and on edge, kids are too,” explains Kathryn Everest, director of counseling services for Fort Worth ISD. “If Mom gets stressed out when she watches politics on TV or is always complaining about drama with friends or co-workers, that rubs off on the children.” Everest theorizes that devices are also a factor. “Think about all the gadgets and electronics our kids have,” she says. “Everything’s louder, screens are larger and the action is fast, fast, fast. It gets way more intense than what previous generations dealt with.” Westers says a child might be genetically predisposed to shyness or anxiety and then begin to experience “clinical levels of anxiety” as a result of bullying, perhaps, or trauma. That doesn’t mean it’s possible to avoid anxiety disorders by avoiding stress. “A better strategy,” notes Westers, “is to help them learn to tolerate and have strategies in place for dealing with stressful situations.” Everest notes that parents sometimes struggle to understand. “They think, ‘What do you have to be anxious about? I feed you, I clothe you, I get you where you need to be,’” she explains. Parents may attach a stigma to anxiety disorder and be reluctant to seek treatment, or they might view anxiety as a phase. “It’s not uncommon for parents or even teachers to say things like, ‘Everybody has emotions like that’ or ‘He’ll grow out of it,’” Westers says, adding that adults may respond by saying, “Just relax.” For a child who has anxiety, such phrases can invalidate their feelings. ADDING TO THE MIX Davis took the teachers’ concerns seriously and signed her daughter up for play therapy. It helped Kaylee, and helped her parents
understand how she expressed anxiety. She went on to cognitive behavioral therapy (CBT). The CBT approach is “something like, ‘I know you’re scared. That’s OK. We can get through this. It’s not fun, but that’s the anxiety talking,’” Westers says. “The next step is to teach them how to talk back to the anxiety. … It helps them become aware of [it,] and they have some strategies in place and ways of coping.” Davis believes CBT made it possible to delay medication for her daughter’s attention deficit hyperactivity disorder (ADHD). Anxiety often co-exists with ADHD, depression and similar disorders. By the time Garland mom Christina’s son was 4, he had been diagnosed with three conditions, including anxiety disorder. (She asked to keep her last name private.) “It was so stressful—even doing homework was combative,” Christina recalls. “We’d get calls from school because he would withdraw, act out or freak out.” A BUMPY ROAD AHEAD Christina’s son became happier after transitioning to a private school that specializes in serving students with these conditions. He is medication-free, although they tried different prescription drugs. “Those either aggravated his symptoms or he was totally out of it,” she shares. The Nintzels started Liam on a very low dose of antianxiety medication. His mother says she’s keeping an eye on it because antipsychotics and antidepressants, often prescribed for anxiety, can make symptoms worse or cause other health problems. Counseling has helped Liam. Still, it’s a struggle for families such as theirs to maintain coping strategies as kids age and anxiety triggers change. “And it’s distressing that he thinks there’s something wrong with him,” Nintzel says. “We have to constantly tell him there’s nothing wrong with him—it’s just who he is.” Liam’s parents coined a phrase to help their son cope: “Anxiety just doesn’t make any sense.” “The fear is irrational … but that doesn’t matter,” Nintzel points out. “It’s how he feels, so it’s very, very real.”
