ow many times have you sat at a restaurant enjoying a delicious, healthful salad and commented to your lunch date that if only someone would make you yummy salads every day you would eat much better and be able to diet more successfully? If you’re like most busy women, the answer is many, many times. These days almost every restaurant offers a complete salad section on their menu with one exotic and enticing option after the other. There are Asian salads and Mexican salads, salads with tofu, mango, Terra chips and more. It’s easy to stay clear of the paninis and pasta when the salads are chock-full of interesting ingredients and sound so appealing and diversified. Years ago a salad at a restaurant meant some iceberg lettuce with a slice of tomato and perhaps a few rings of cucumber on top. And that’s cucumber with giant seeds, not the kirbies or Persian variety we favor today. You would never go to a restaurant to order a salad. It was simply an uninspired side that arrived with the “real” food, be it a steak, chicken or piece of

fish. Today, we look for restaurants and cafes that feature a salad bar, where we can mix and match amongst a myriad of veggies, toppings and dressings. And for those who like their salads ready-made, the choices are plentiful at most restaurants. So why can’t we serve ourselves these imaginative green dishes and not have to wait for a special occasion where we “go out” to treat ourselves to a nutritious and delicious meal? Truth be told, there is more to it than just the salad. There is the whole dining experience - be it at a five star restaurant or a neighborhood diner - that affects how we feel about what we are eating. For starters, when eating out there is no set up or clean up on our part. So much about eating a healthful meal - especially a fresh salad - is related to the prep work. The washing, peeling, chopping and dicing. As women we spend countless hours in the supermarkets and in our kitchens in managing the feeding of our families. Once in a while it’s nice to be on the receiving end.

Some of our kitchens are just as nice, if not nicer, than those in the local eateries but when we are in our own homes we rarely sit down at a table with a placemat, cutlery, a glass of ice water and an elegantly prepared meal. We are the masters of eating on the run and the inhalers of mealtime leftovers. We taste while we cook, we sample while we serve and we’re the shovelers of forkfuls of the family remainders on our way to the trash. Even if we are home alone we are incapable of relaxing and concentrating on our meal with undivided attention. In between hasty bites of food we sort through mail, pack lunch bags, pop in a load of laundry, anything to continuously cross

those endless tasks off our to-do lists. At even the most casual of restaurants at least we sit down and can, for once, dine uninterrupted. And one more thing. While most of us make sure to have all the necessary ingredients on hand in preparing our family’s favorites dishes, we are usually not as wellstocked with the diverse herbs and additions which many of those yummy salads require. Over the years we’ve all added olive oil, slivered almonds and balsamic vinegar to our list of cupboard staples but roasted beets, spicy chick peas, pine nuts, sun-dried tomatoes? Not likely. So say we finally have that day when no one is around, the chores are done and we’ve got the time to whip up something special just for our well-deserving

selves. What are the chances that when we peer into our fridge and open our pantry we’ll find all of the necessary enticing fixings to put together a sensational salad? Not great. Well I’ve been trying. Like most women, “exercise more” and “eat better” are always on my list of resolutions - at our New Year, the secular new year and just about every other day of the year as well. Some years I am more into the fitness aspect. I’ve had periods of playing tennis and racquetball and a serious stint at a local health club where I utilized the treadmill, elliptical machine and spin bike several times a week in addition to putting in time on the weight machines. Serious,

but brief. Some years I was more successful on the dieting end - counting calories and even incorporating a cleansing fast occasionally into my routine. Rarely have I been able to mesh the two together successfully or for a sustained length of time. At my age I know how important the exercise part is, especially weight-lifting drills. I try to walk when I can, stretch in the mornings, practice some yoga, but things hurt. I was loving a Sunday morning Zumba class for several months until I finally made the connection and realized that the exhilarating morning jumping and shaking were responsible for the excruciating

STAYING FIT AND HEALTHY By: Rachel Rosenberg
I
t’s not easy being a lover of good food and drink and maintaining a healthy and fit body. After a recent birthday and with the upcoming summer months, I realized that those extra winter pounds are not going to go away on their own and it was time to take action. Considering that last month’s article focused on working women and why we have no time for the gym but plenty time for pizza and wine, this was not an easy decision for me. As a foodie, I enjoy a great steak, duck gnocchi, lamb riblets, penne a la vodka and of course all the matching wine and cocktails. As a working woman, I usually get home late, eat whatever’s on hand and fall into bed exhausted. After trying many diets and gyms, I found something unique. I recently joined a local Five Towns “gym”, AVID Fitness, run by Avi Dagan with the assistance of his wife Mandy. It can’t really be called a gym as they are not open all day but rather just for a select few spin classes and small group training. These limited classes are perfect for the working woman, with spinning at 8pm followed by boxing or boot camp at 9pm. AVID’s studio is targeted at changing people’s lives, not just giving them a place to work out. There are no treadmills or elliptical machines, simply a small spin studio and a room for small group training. What makes this place unique is the people. After my first spin class, I received a text message from the instructor asking how I was feeling and telling me how great I had done. That’s what AVID is about. A group that works together, supporting each other to get stronger and healthier every day. After a couple of weeks of working out at AVID, Avi Dagan suggested that a group of us AVID women get together and try the OHEL OXC Challenge, a 5 mile obstacle course where participants raise money for OHEL while challenging their bodies and testing their limits. AVID offered free training for anyone signing up for the OHEL Challenge. So on Monday nights, Thursday nights and I can’t believe I woke up that early on Sunday mornings, we got together at a local park and practiced the mon-

key bars, ran carrying each other, and wheel-barrowed our bodies across fields. We had a blast doing it too. That’s why I think I have found a fitness home with AVID. The small groups and awesome instructors really pushed me to get stronger while having fun. Now of course, I still love dining out. The key is to make healthy choices. My workout friends at AVID would give me on target suggestions of what to order at local restaurants, helping me stay on track. At Prime Bistro on Central Avenue in Lawrence, I ordered the Chopped Salad, a hearty salad including string beans, avocado and peppers in a light lemon vinaigrette. On my friends’ advice, I asked that they add grilled chicken, making it a filling meal. Of course I was envious when my fellow diners ordered desserts of cakes and meringues. So instead I ordered the Portobello Mushroom salad as my dessert. For Mother’s Day, I dined with my sons and husband at the Coffee Bar, also on Central Avenue in Lawrence. The chef at The Coffee Bar, Albert Bijou is a true gourmet chef and it was hard to pass on some of his less healthy menu options. I compromised and started with the extraordinary fish and cheese platter. This wooden board of assorted cheese and smoked fish is probably not the healthiest meal option, but certainly better than my usual mac n’ cheese ball. For my entrée I ordered the spaghetti squash sautéed in olive oil and mixed with tomatoes, black olives and feta cheese. This was a healthier version of the pasta I really craved. Eating healthy and staying fit will never be as much fun as pizza, steak and fries, but as they say “everything in moderation, including moderation.” Rachel Rosenberg, a practicing attorney and name partner at Rosenberg and Steinmetz PC, is known amongst friends and family as an accomplished chef and daring foodie with a critical eye and discerning palate. MIND OVER MATTER:
Pain – Is It Real or All in Your Head?

ho has not experienced physical pain at some point in their lives? Pain is the most common reason that people see doctors. According to the Institute of Medicine at least 100 million adults in the United States suffer from chronic pain. Chronic pain affects more Americans than diabetes, heart disease and cancer combined. What is the difference between chronic and acute pain? Acute pain begins suddenly and is usually sharp in quality. It serves as a warning of disease or a threat to the body. Acute pain might be mild and just last a few moments or it might be severe and last for weeks or months. Acute pain will disappear once the underlying cause has been treated or has healed. When pain is not relieved it can turn into chronic pain. Chronic pain persists even though the injury has healed. Pain signals remain active in the nervous system for weeks, months or years. Physical effects include tense muscles, limited mobility, lack of energy and changes in appetite. When physical pain continues it can lead to emotional effects such as depression, anger and anxiety. When we seek out doctors we expect them to diagnose and treat the pain we are experiencing. Unfortunately there are times when the doctor is unable to find an underlying cause for the pain or the cause is poorly understood. This pain is often referred to as Medically Unexplained Physical Symptoms (MUPS). Patients who suffer from these seemingly unrelated complaints stump general practitioners and specialists alike. These patients may suffer for years until they are diagnosed, if at all. Lastly they may hear the words no patient wants to hear and that is “it’s all in your head”. On the average, U.S patients with rare undiagnosed or misdiagnosed conditions wait a little over 7 years for a diagnosis. The longer they wait the longer they feel uncertain about their health and their future. Another thing that happens as time goes on is that people become skeptical about their illness and eventually they too begin to question “is it all in my head?” I’ve had clients who were in chronic pain for various reasons. They have gone from doctor to doctor, some from hospital to hospital, and no definitive diagnosis has been made. Because of this treatments they received may help only some or not at all. One can only imagine the frustration and feelings of hopelessness they experience. According to a Dr. Mel Pohl all pain is real. A person’s

experience of pain is unique to the individual and it can’t be measured from the outside with the exception of some sophisticated tests. He goes on to say that since there’s no way to know how much pain a patient is experiencing he chooses to always believe his patients. All pain is regulated by the brain. Whether it is pain whose source can be seen or not, it is your nerve fibers sending messages to your brain that cause the sensation of pain. (If you are interested in learning more google Chronic Pain Loop). So what does it mean when one is told the experience of pain they are having is emotional? Emotions - just like pain - are the creation of the physical brain, specifically the midbrain. Neurotransmitters are involved in the experience of pain as well as emotions. The main area of the brain where we form and register emotions is the limbic system. This is a set of midbrain structures surrounding the thalamus which is the pain processing center. This center is responsible forSfiltering and prioritizing all the SALE impulses the brain receives. When a person lives with chronic pain it inevitably develops an emotional component. Dr.Pohl continues to suggest that pain is protective and when we experience pain we experience a set of aversive emotions so we can move away from whatever is causing it. “As pain becomes chronic the sensory components become less important and the emotional and behavioral components tend to take on more importance”.

Continued from page 62 Based on this information he concludes that chronic pain is strongly influenced by emotions. This also helps us understand why different individuals have different experiences of injuries/pain. In doing my research I came across an online medical information resource called “Crowdmed.” In their article “What to do when doctors say it’s all in your head” they discuss disorders that are psychological in nature but that often lead to physical symptoms or complaints. Depression is an example of a psychological condition that is comorbid with hundreds of ailments. Likewise there are many physical disorders (that are difficult to diagnosis), which present with symptoms of depression (or other psychological symptoms). We know there is a relationship between depression and fatigue, stress, pain and other complaints. Depression is an example of how mind and body affect each other. Often when a medical diagnosis cannot be made it is blamed on depression. The mind-body connection continues to be a complicated one. So how can a person be helped when they feel like there is no light at the end of their tunnel? One of the first things I suggest is that they become part of a support group. A support group will be vital to one’s mental health. It’s not uncommon for people who are close to you to become skeptical of your situation after a certain amount of time has passed. It’s important to have the support of people who understand your plight (that being that you’ve been struggling with pain which at this time has no diagnosis). Another suggestion I make is to work on accepting that a diagnosis may not be forthcoming and that having a diagnosis doesn’t always solve the problem. More importantly, one must treat and live with their symptoms so they can find ease and comfort. According to an article written in the Huffington Post in 2008 the NIH launched an undiagnosed disease program at its clinical center in Bethesda Maryland. In 2014 the program expanded and draws on multi-disciplinary resources to help advance both diagnoses and potential treatments of difficult to diagnose illnesses. What people with who live with pain and illnesses that have gone undiagnosed don’t need is : 1. Doctors/family members saying it’s “all in your head”. 2. Doctors who automatically send them to a pain specialist. 3. Doctors who try to fit a patient’s symptoms into a box. What they do need is: 1. Doctors who listen to the patient. 2. Doctors who say “I don’t know”. 3. Doctors who are willing to look outside the box for diagnoses and treatments. There is help out there for people suffering in silence. If you are one of them don’t give up!!!

A few times a week I force myself to peel and chop, sprinkle and season as I create some pretty appealing salads. My current favorite is a diced Israeli salad with the addition of feta cheese, chick peas, Greek olives and pumpkin seeds. Yum! For now. Years ago women didn’t go to the gym and the word “fitness” probably wasn’t even part of our vocabulary. There was likely a lot more walking and lifting, strenuous physical work that wasn’t done for the sake of keeping fit but did the task all the same. Today, many of us sit at sedentary jobs most of the day and then again in our cars to go on errands. We have machines to do many household chores and computers to save time and energy on shopping sprees. Food then was simpler and more healthful. Fewer additives and artificial ingredients. Progress? Hmmm. So in this issue, devoted to Health and Fitness, we encourage you to take advantage of the warmer weather as well as the many resources on these pages.
