A Labor of Love Guiding You Through Labor And Delivery 101 BY CHRISTA MELNYK HINES Through each step of pregnancy, you’re not only mothering your growing baby, you’re preparing emotionally and physically for the final, most challenging leg of pregnancy’s marathon-labor and delivery. Wondering what to expect? First, discuss any anxieties or fears that you may be experiencing surrounding labor and delivery with your healthcare provider. “They have seen it all and can often offer a lot of reassurance,” says Dr. Tara Chettiar, OB/ GYN. “Remember that you’re going into this as a team and everyone has the same goal: healthy mom and healthy baby.” Considerations before labor. A written birth plan can be a helpful way to think through your preferences and choices surrounding the birth of your baby. Discuss your plan with your provider, as well as any circumstances that might necessitate a medical intervention like an induction or cesarian section. Also decide where you’re going to have your baby. These days, expectant parents are fortunate to have multiple choices of birth centers and prestigious hospitals with highly trained, experienced medical staff. “Consider delivering at a hospital with an experienced labor and delivery team, including nurses, physicians and neonatal nurses who may care for your infant if difficulties should arise,” says Dr. Peter Caruso, OB/GYN. 8 April 2017 | PB Parenting |

How do I know I’m in labor? Many women feel Braxton-Hicks contractions throughout their pregnancies, especially beginning around the 20-week mark. These contractions are more of an uncomfortable nuisance than painful. They’re also characterized as more erratic than active labor, tending to last 10 to 20 seconds every 10 to 15 minutes. If you’re bothered by Braxton-Hicks contractions, make sure you’re getting enough fluids. “The most common cause of false labor is dehydration and pregnant women get dehydrated much easier than non-pregnant women,” Dr. Chettiar says. Hot baths and showers can also provide relief. “Heat is a natural muscle relaxant so a heating pad on your back-not your belly-is a great option,” Dr. Chettiar advises. The first stage of labor. Some signs of early labor include your water breaking (which is the rupture of the amniotic membranes), backache, blood-tinged discharge, menstrual-like cramps and low abdominal pressure. Stay close to home, try to rest and be prepared to head to the hospital as the time between contractions increases. If you choose to deliver your baby in a hospital, one of the first people you’ll meet will be a labor and delivery nurse who will guide you through the process, review your health history and learn about your birth

Schedule tours of the birth facilities where your provider delivers Learn about the facility’s labor and delivery policies and procedures Discuss your preferences with your physician Write a birth plan to bring with you to the hospital Be aware of your options and plan for the unexpected

plan. The staff will also check your blood pressure and monitor your baby’s heartbeat using a Doppler or fetal monitor. Once you’re in active labor, contractions are more painful and rhythmic in nature, lasting 30 to 60 seconds and occurring every five minutes for at least one hour. During this stage, expectant moms feel a wide range of emotions from anxiety and fear about the

labor process to excitement that they’ll soon be cuddling their long-awaited baby. To stay comfortable, try natural pain relief like intermittent walking (unless you’ve dilated beyond 7 cm), sitting on a labor or peanut ball, or asking your partner for a gentle massage on your lower back. “Listening to music for distraction and shallow breathing techniques are (also) helpful,” Dr. Caruso says. Labor and delivery nurse Genna DeBrabander also suggests activities like slow dancing, imagery and visualization, and changing positions from one side of your body to the other for pain relief. “Another great option for pain control would be an epidural,” she says. Time to push. Active labor has advanced to transitional labor when the intensity of your contractions increases and the cervix dilates from seven to 10 centimeters. At this point, you’ve entered the second stage of the labor

process, which can last as few as 15 minutes to three hours. During this stage, you’ll start to push. Shallow breathing techniques during the first part of a contraction and pushing at the peak of a contraction can help you manage any pain or pressure you’re feeling. Dr. Caruso also recommends a squat bar to help ease pain during the pushing phase. Your emotions may run the gamut too. “There are many emotional challenges, such as despair, hopelessness, inadequacy, elation and a sense of accomplishment,” DeBrabander says. While this is the most physically and emotionally taxing part of the labor process, by the end of all of your hard work you’ll be rewarded with a baby in your arms. Almost done. The last stage of labor is the delivery of the placenta. You may barely notice these contractions after giving birth to your baby. The placenta develops during pregnancy. It attaches to

the uterine wall and is connected to your baby’s umbilical cord. This organ supports your baby throughout her time in your uterus, providing oxygen and nutrients and removing waste products like carbon dioxide. If you plan on keeping your placenta, let your doctor know. After the delivery of your placenta, you may experience cramping as your uterus contracts and some perineum pain and swelling. “Topical medications oral medications and ice packs are great to aid these discomforts,” DeBrabander says. As always, if you have any concerns during your pregnancy or questions about the labor and delivery process, be sure to consult with your healthcare practitioner. Gender Reveal Ideas

Waiting up to twenty long weeks to find out the gender of your baby can seem like an eternity! From throwing a party to spreading the news via mail or facebook, Gender Reveals are a hot new trend in spreading the news. Here are some ideas to tell everyone the gender of your baby.

1. Mystery Cake or Cupcakes Trust a close tight-lipped family friend with the sealed results of your ultrasound to take to a local baker and have a special cake made. From the outside it looks like a regular cake but once you cut inside the cake it will reveal the gender to you and your guests. Have a baker friend? You could even have someone make a cake by adding pink or blue food coloring to a yellow cake mix. 2. Photo Shoot Reveal Schedule a photo shoot with a local photographer to announce the gender of your baby using props such as pink or blue balloons, baby shoes, paint, umbrellas. You could also write your message out using scrabble pieces, blocks or drawing it in the sand. Once you get your photos back from the photographer share your news via print announcements and/or social media. If having a party, you can have it printed on canvas and simply cover it up so no one can

peek until you are ready to announce! 3. Older Sibling Reveal If you already have a child, buy or make a shirt that says, “I’m Having a Baby Brother” (or sister). If you are doing a photo shoot have them wear it during the shoot. If you are having a party, have the soon-to-be sibling keep their jacket zipped until it’s time for the big reveal. 4. Pet Announcement Have a fur baby? For all the pet lovers out there get your pet involved in the act! If you have a cat or dog for example, you can organize for them to come out to the guests at announcement time wearing something around their neck to announce the gender. It might be a pink or blue ribbon or bow. 5. Pin the Bow On Your Ultrasound Use your printer or go to a store that makes enlargements. Blow up the ultrasound picture and put it on the wall. Make a cutout of a pink bow and a blue bowtie and put a small loop of scotch tape on the back. When it comes time to reveal the gender you can stick the pink bow on baby’s head, or the blue bowtie at baby’s neck. If you’re not having a party, make small copies of the ultrasound and put the proper decoration on it before mailing it to your friends and family.



25 Things to Do Before Your Baby is Born Finding a doctor for your baby is just the beginning of things to do before your baby arrives. Here are 25 more things to cross off your to-do list.  Pack your hospital bag.  Take a tour and pre-register with your hospital.  Make a birth plan.  Get a crib and set up your nursery.  Pre-wash baby’s clothes  Attend Childbirth and Education classes at your local hospital.  Choose your baby’s names.  Pick a birth announcement design and gather addresses.  Start researching breast pumps and call your insurance to find out how much they will cover.  Use your crockpot and make some

freezer meals to reheat after the baby arrives.  Pack your camera and make sure the battery is charged.  Book and schedule a newborn photographer.  Have a date night with your husband before your baby arrives.  Schedule some me time pampering - a prenatal massage or pedicure.  Install your car seat.  If returning to work, start researching childcare options.  If you have an older child, take them to a sibling education class.  Also arrange childcare for siblings.  Write down your list of people to contact once baby arrives (with phone numbers and email addresses).  Relax! Take a Nap! 6. Confetti Pop Buy a balloon for every person at the gender reveal. Before you blow them up, put some pink or blue confetti inside each balloon. Hand everybody a balloon and a pin. Count to three and have everyone pop their balloon. The confetti will scatter and everyone will know if it’s a boy or a girl. 7. Pinata Party Another fun party idea is to fill the piñata beforehand with pink or blue candies. When the piñata is broken open, your baby’s gender will be revealed. 8. Make It a Game Have guests guess baby’s gender. At the end of the party, announce what it is. Everyone who guessed correctly gets a trinket to take home with them.

9. Silly String Pass out cans of pink or blue silly string to your friends and family. Don’t forget to wrap the cans in white paper so they don’t know


the color inside! On the count of three, have your loved ones spray you and your spouse with silly string revealing your surprise! 10. Create a Custom T-shirt Create a shirt for mom-to-be to wear at the party. Have it say It’s a... With a checkbox next to Boy and Girl. At the party reveal by crossing off the checkbox with a permanent maker. Important Things To Consider Before Your Reveal Party • The ultrasound tech could get the gender wrong, especially in earlier scans before 20 weeks. So be sure you are confident with the results. • If you get a cake or cupcakes made for you, you’re relying on someone else and hoping there won’t be an error! If you want the gender to be a surprise find someone reputable to do it for you.


Bringing Your Baby Home Whether your baby comes home from the hospital right away, arrives later (perhaps after a stay in the neonatal intensive care unit), or comes through an adoption agency, the homecoming of your little one is a major event you’ve probably often imagined. Here’s how to be prepared. Leaving the Hospital - Moms-to-be sometimes pack clothes for the trip home before even going to the hospital — or they may wait to see what the weather brings and have their partner bring clothing for both themselves and the baby. Plan to bring loosefitting clothing for yourself with a drawstring or elastic waist because you most likely won’t fit into your pre-pregnancy outfits yet. Babies are often overdressed for the first trip home. Dress your baby as you would dress yourself. So, if you’d be too warm in a knitted hat during the summer, your baby probably will be, too. In warm weather, dress your baby in a T-shirt and light cotton pants or a baby blanket over bare legs. If it’s cold, put footie pajamas, a hat, and warm blanket over your baby. But be sure to keep all blankets far from your baby’s face to avoid suffocation. Chances are much better that you’ll bring home a calm, contented baby if you don’t spend a lot of time at the hospital trying to

dress your newborn in a complicated outfit that requires pushing and pulling your baby’s arms and legs. If you haven’t already made the arrangements with your baby’s health care provider, make sure to ask when the baby’s first checkup should be scheduled before you leave the hospital. Depending on the circumstances, some premature babies also go home with a special monitor for checking breathing and heart rate, and you may be taught infant cardiopulmonary resuscitation (CPR). But whether your baby is full-term or premature, don’t feel rushed out the door — have your questions answered before you leave the hospital. And if you find yourself wondering about anything — from bathing to breastfeeding to burping — ask your nurse, lactation consultant, or your baby’s doctor. The Car Trip - The most important item for the trip home is a proper child safety seat (car seat). Every state requires parents to have one before leaving the hospital because it’s one of the best ways to protect your baby. Even for a short trip, it’s never safe for one of you to hold your baby in your arms while the other drives. Your baby could be pulled from your arms and thrown against the dashboard by a quick stop. Infant-only seats are designed for rear-facing

use only and fit infants better than convertible seats. The American Academy of Pediatrics (AAP) recommends that infants and toddlers ride in a rear-facing seat until they are 2 years old or until they have reached the maximum weight and height limits recommended by the manufacturer. (If your baby exceeds the weight recommended by the manufacturer before the second birthday, you’ll need to use a convertible seat designed for bigger babies.) Some parents of newborns find that a “travel system” (which includes a stroller and an infant-only car seat that can be attached to the stroller) makes it much smoother to transition babies — especially sleeping ones — from the car to the stroller. If you borrow a car seat, make sure that it’s not more than 6 years old and was never in a crash (even if it looks OK, it could be structurally unsound). Avoid seats that are missing parts or aren’t labeled with the manufacture date and model number (you’ll have no way to know about recalls). Also, check the seat for the manufacturer’s recommended “expiration date.” If you have any doubts about the seat’s history, or if it’s cracked or shows signs of wear and tear, don’t use it. When buying a new seat, it’s important to remember that there isn’t one type of seat that’s safest or best; get one that fits and can be correctly installed in your car. And higher price doesn’t necessarily indicate a seat’s quality — it could simply mean the seat has added features that you may or may not want or use. Make sure to register your new seat so you can be notified of any problems or recalls. The most common problem involving car seats is improper installation (according to the National Highway Traffic Safety Administration, the majority of all car seats are installed incorrectly). Recently, LATCH (Lower Anchors and Tethers for Children) car seats have become standard in the United States,

but a large percentage of these seats are improperly installed too. Don’t trust illustrations or store displays. Follow the manufacturer’s instructions (and keep them handy). Ask your doctor or nurse about local resources where your car seat can be checked by someone specifically trained to evaluate car seat installations. Many hospitals, police and fire stations, and even car dealerships offer this type of service for free. Make sure that the evaluation is done by someone trained and experienced. First-Time Feelings - Don’t be surprised if you have mixed emotions as you bring your baby home, especially if this is your first child. You’ll likely be nervous. In fact, you may actually feel terrified as you realize that you’ve given up a certain amount of control over your life. If your baby wasn’t with you much at the hospital, you may not know what sort of schedule your little bundle of joy will keep. But you’ll know before long — although babies’ schedules do change a lot during those early months. You’ll be less stressed if you don’t overschedule yourself and can go with the flow. Depending upon your labor and delivery experience, you may feel physically drained and sore. Your hormones may be struggling to catch up, too. Meanwhile, your partner may feel a little left out if you’re totally engrossed with the baby. You might have other kids awaiting the arrival of this newest family member. Or you may be dealing with a pet who’s wondering what’s suddenly drawn everyone’s attention. And the expectations of new grandparents, competitive siblings, or friends can also make the homecoming stressful. Your baby’s first extended crying period at home will be difficult. Remember: young babies typically cry for 1 to 5 hours within a 24-hour period, and can’t always be calmed. Crying usually decreases gradually after the

first several weeks. Although it may seem impossible now, in a few months it will be difficult to recall your baby’s seemingly endless crying episodes. The Home Front - Introducing your baby to others at home can be challenging. If you have other kids, be sure to spend some quality time with each of them. Some parents bring home gifts from the new baby for big brothers and sisters. At first, you can expect some jealousy, especially if the main focus of your attention for several years suddenly has new competition. Encourage siblings to “help” you care for this newest family member. If you have a pet, ask your partner to bring home a blanket with the baby’s scent on it and place it near the pet — even before leaving the hospital. Then, when you come home, the pet will already be somewhat familiar with the baby. But remember to never leave pets alone with newborns. Family and Friends - Ask your partner to be the gatekeeper for visitors and to limit the number of guests at first. You’ll be glad later on if you take some time now to rest and become comfortable with your new situation. Although babies typically aren’t shy around strangers for the first 3 months or so, they may become overstimulated and tired if too many people are around. If you have voice mail or a telephone answering machine, consider changing your message to give the vital statistics of your new arrival. You might say something like: “Our newest family member has arrived. Her name is Julia Marie; she was born on Tuesday, and weighed 7 pounds, 10 ounces. We’re all fine and adjusting to our new life.
