Letter From The Publisher Greetings, September is a busy month for birthing babies! According to a University of Virginia graduate study (cross referencing CDC vital records), the most popular days for giving birth are September 11, 12, 18, 25 and 26. It’s a perfect time for our biannual Baby & Maternity issue! PB Parenting has compiled a lot of information in this issue for new moms (and even not so new moms). Check out page 8 where local mom, Samantha Clark, provides a list of newborn necessities. She definitely knows what is really needed as a mom of four kids, including one still in diapers! Another big decision when you have young kids or one on the way is choosing a pediatrician. There are so many options and trust me you will spend a lot of time there, so be sure to check out page 18 for tips on finding a good match. When your child is sick a big question is what fever/pain medication should you choose? Dr. Romig, Medical Director of After Hours Pediatrics Urgent Care, has expert medical advice to help you make that decision. My “baby” is no longer a baby but is now a big boy that started Pre-K and my little princess just started 2nd grade. We have survived the first few weeks back to school and are now ready to start thinking about extracurricular activities. From wipe warmers to sterilization bags, there were many items on my baby registry that were “essential” for baby # 1 but didn’t quite make the cut for baby #2. But for a first time mom, how do you know what your baby will need? Entering the world of motherhood can be overwhelming and confusing, so here is a list of suggestions from one veteran mom to new moms, on what are really newborn necessities.

Swing/Bouncer seat: Everyone loves to hold a sleeping newborn, but sometimes it’s nice to just let a sleeping baby sleep. In that case, you will need a swing and/or a bouncer seat. A comfortable and entertaining swing/bouncer provides visual stimulation and can soothe a fussy baby. Try to find a swing or bouncer that offers a vibration option, that always helped lull my cranky baby to sleep. Don’t feel guilty about placing your newborn in the swing and catching up on one of your favorite shows, it’s OK to have a few moments to yourself. For baby #3 I received a Rock-n-play as a gift and it changed my world. My daughter spent the first 6 months of her life in there. If the bouncer or swing isn’t doing the trick for your fussy baby, definitely try it out. Best part- it is sold at Target and who doesn’t love an excuse to buy something (or everything) at Target. Baby-friendly detergent and stain remover: Newborns have very sensitive skin and it’s important to use detergent and stain remover free of perfumes and dye. I used Dreft detergent with the first 3 babies and then switched to Tide Free and Clear for baby #4 (it was easier to wash the entire family with one detergent, instead of washing the baby’s clothes separately). I absolutely love Dreft’s stain remover and it saved many outfits from “diaper blow-out” catastrophes. Burp cloths: Newborns spit-up a lot, and multiple times a day (and night). If you want to save your clothes and shoulders, invest in MANY burp cloths. Don’t splurge on expensive burp cloths, go with the budget friendly ones. I had a fancy monogrammed one that I never wanted to use because it was too pretty.

Onesies/footed pajamas: You want to keep a newborn completely covered for the first few months, onesies are the way to go. Some people call them footed pajamas, so to be clear, I am talking about the full length body suit that covers the baby’s feet. Helpful hintonly buy the zipper ones. Do you really want to be snapping 15 snaps in the middle of the night?
Swaddles: Swaddling mimics what babies feel like in their mother’s womb, so it can be soothing for a fussy baby. Purchase at least two, so when one is in the wash, you have another one ready to go. If you are like my amazing postpartum nurse, who swaddled my newborn like a little potato with just a blanket--more power to you and skip the swaddle. Diaper bag w/ changing pad: When you finally have enough energy to leave the house, you will need a practical diaper bag with a travel changing pad. Make sure it is large enough to carry around all that you will need (don’t forget an extra outfit like I always did). There are several styles and sizes of diaper bags, so it is best to try them on in person and see which suits you. Stroller/car seat: Finding a car seat and a stroller can be a daunting task. I made

several trips back to the store to test drive strollers and practice folding them. I sometimes felt like a genius figuring out the creative ways some of these strollers folded. Make sure you pick a stroller and car seat that are lightweight and comfortable for you to tote around. Feeding: Everyone has their own opinion on “the best” bottle, but most importantly, you want to have a bottle that eliminates gas. If the first bottle you try doesn’t work out, it is ok to try another style or brand. Bottle fed babies will also need a healthy supply of formula. In the first few months it might take a couple of tries to find the best formula for your baby, so don’t go overboard on your formula supply. If you are breastfeeding, nipple cream is a life saver. Trust me. Lanisoh and Medela both make safe and effective cream. Pediatrician: It is very important to trust your pediatrician and feel as though he or she has your baby’s best interest at heart.
In the first year of your baby’s life, you will feel like you are living at your pediatrician’s office. So make sure you like the office, the front desk staff and the nurses. You can also set up an interview and tour of the doctor’s office to make sure it is the right fit for you and your growing family. Don’t feel like you are married to your doctor either; it is ok to “break up” if you don’t feel comfortable for any reason. (For more tips on choosing a pediatrician see page 18). Coffee: Ok, this is really for you and not the baby, but coffee is a necessity. If you are able to nap when the baby naps, take advantage of that pleasure. If you’re brave enough for another baby in the next couple of years, the napping option goes out the window (along with your sanity). Coffee is how I made it through the first 6 months of my baby’s life, and now it’s how I make it through the first 6 hours of my day. I am a mother of four, but by no means

an expert on motherhood. These are the things I wish someone told me I would absolutely need before my baby was born. I would like to also add that it’s important for a new mom to ask for help when she needs to. Whether it is from a friend, family or your spouse, a good support system helps a new mom through those first few hormonal postpartum months. I hope these items help you on your journey through the emotional roller coaster of motherhood. Remember, what worked for one baby may not work for them all. So adapt to the bumps and curves life throws your way because remember, you just grew a human being in your body; you’re a rock star! Samantha G. Acetaminophen or Ibuprofen: Guidelines and Good Practices BY: LOU E. ROMIG, MD, FAAP, FACEP When our children have fever or pain, we naturally want to make them feel better. Tender loving care can go a long way, but sometimes we need a little help from medications. When considering when to give medications for fever or pain and also what to give, the first thing to do is realize why we’re using the medication. Pain is pretty self-explanatory – nobody wants their child to be in pain. Pain is not only upsetting, but it also can increase the heart rate, breathing and blood pressure (although not usually to dangerous levels) and make a child cranky or withdrawn. One of our key jobs as parents and caregivers is to reduce suffering by trying to minimize or prevent pain. Acetaminophen (such as the Tylenol™ brand) and ibuprofen (such as Motrin™ or Advil™) are our main tools to do that. Both are great medications for fever and pain, but ibuprofen has an added benefit of fighting inflammation, which acetaminophen does not. For this reason, ibuprofen is sometimes preferred for pain from injuries or illnesses involving inflammation. Fever is one of the most common reasons caregivers seek medical evaluation for their children. There is much apprehension about fever: some people are afraid for their children when they have fever, having heard stories about fever causing brain damage and convulsions or even death. Couple that

with the fact that children often look and act much sicker when they have fever, and it adds up to a lot of anxiety. Caregivers often turn to acetaminophen or ibuprofen, and sometimes both, in an effort to make the fever go away. There’s a lot of confusion about which medicine to give, how much to give and when to give it. The good news is most stories about the dangers of fever are absolutely false! Fever from infection rarely goes above 106 F. The body temperature has to go above about 107 F before there is any damage to the brain or body. There is such a thing as febrile seizures, but they are limited to children between the ages of 6 months and 5 years. When they occur, they are usually very brief and do not cause complications. They are completely unpredictable and do not occur because the temperature goes above a certain level. They’re far scarier than they are dangerous. A fever that doesn’t go down all the way with proper doses of fever medication or one that comes back before the next dose is due is neither an indicator of infection nor of the seriousness of the illness. The fact that you can’t completely control the fever is not something to worry about, as it means nothing about the infection that’s causing the fever. The fever will go away when the worst of the infection goes away. The real reason to treat fever is to make
your child feel better. Fever itself, regardless of the source of the infection, will increase the heart rate and breathing rate; make your child sleepy, cranky or clingy; and decrease their appetite. If you can get the fever down even a degree or two, your child will likely feel better, start eating and drinking better, and look much more like themselves. Then you’ll feel better too! Both acetaminophen and ibuprofen are excellent medications for fever control. We tend to limit ibuprofen to children older than six months. Children with kidney disease, bleeding problems or a few other chronic illnesses may not be able to take ibuprofen. If your child has a chronic disease, check with your primary care provider to see if he or she can safely take ibuprofen. A few studies have suggested ibuprofen may be better than acetaminophen in helping to treat fevers over 102 – 103 F, while acetaminophen

may be better for children who are also having stomach pain or upset, because ibuprofen can sometimes irritate the stomach. Some children consistently seem to respond better to one medication than the other. Each individual illness may also respond better to a particular medication. If you get a feeling that one medication is working better than the other, use that medication. Many medical providers recommend alternating acetaminophen and ibuprofen for better fever control. Studies suggest there may be a slight improvement in fever control when using both medications; however, there is also an increased chance the child will accidentally be given an overdose of one or both medicines, especially if more than one person is giving the child medication. With this possible safety concern about accidental overdose, there’s little benefit
in using the medicines on an alternating schedule. If you choose to alternate acetaminophen and ibuprofen, alternate them every 4 hours. For example, give acetaminophen at noon, ibuprofen at 4pm, acetaminophen at 8pm, and so on. If more than one person will be giving medications, keeping a written schedule may help reduce dosing errors. There is absolutely no evidence that giving acetaminophen and ibuprofen at the same time helps to control the fever. This practice can also lead to significant medication overdoses thus is not safe. Oral dosing recommendations on the packages of medications are most often given in weight or age ranges. This can lead to under-dosing or slight overdosing. It’s best to get a dosing chart or recommendations from your medical care provider so you can dose your child based on their current weight.

To sum it all up, whether they have fever or pain, we use acetaminophen and ibuprofen to make our children feel better. With a few exceptions, both medications are safe to use when given in appropriate doses and with appropriate timing. Ibuprofen may be better than acetaminophen for injuries or illnesses that also involve pain and inflammation or for higher fevers. There is no need to alternate the two medications for fever. Keep it simple and use which ever medication seems to work better. Consult your primary care or urgent care provider to learn the safest and most effective doses to meet your child’s needs. Lou E. 5 Tips for Choosing a Pediatrician BY SARAH LYONS
After your baby is born, you will visit the pediatrician often. In the first year, your baby will have seven well visits. Studies show that first time parents may visit their child’s pediatrician up to sixteen times before baby’s first birthday (source www.fitpregnancy.com). Your child’s doctor plays an important role in their life and, therefore, you will need to find one that is a good fit for the parents and the child. It can be a challenging to find a pediatrician that is right for your family. Here are some tips to get you started on your search: Start with recommendations As a first time parent, or a family that has just relocated, it is hard to know where to begin your search for a pediatrician. “I asked other moms for recommendations. It’s a great way to see what names came up repeatedly.” says mom, Jill Miller. Stacy Cook, nurse and mom of three suggests asking your OB GYN for a recommendation. “I figured if they were good enough for my doctor’s kids then they were good enough for mine.” Some parents, like Nathan and Wendy Hladky chose the same primary care doctor for the whole family. “All four of us use the same doctor. It is super convenient.” says Nathan.

When is it time to find a new pediatrician? •

If your child cries the whole time, that alone, is not cause to switch. However, if the doctor doesn’t seem to care or make an effort to soothe your child, you may have a problem. If you’re unclear why a certain medication is prescribed or why a test is done. If you can’t come to an agreement, or at least a middle ground, on most issues. If the location and hours no longer work for your family. If the doctor often seems inaccessible when your child is sick. If the doctor is making you feel bad or consistently reproaching you for parenting choices. If you feel the doctor is not listening to your concerns. Before switching, discuss your concerns with the doctor. If a compromise cannot be made, it’s time to look elsewhere. Your child’s health is top priority.

Take these things into consideration as you narrow down your list. Visit the office To find out if you feel comfortable in the pediatrician’s office, you will have to take a tour. Call the office and ask if they have times set up for potential patients to come and visit the office or if you would need to make an appointment and ask if you can interview the doctor at the same time. Remember to ask if there is a charge for visiting. Verify with your insurance company to see if the cost would be covered or if you would be responsible for the fees.
When visiting the office be aware of what the overall environment feels like. Take into consideration if the office staff is courteous, polite, and willing to help. Is the office clean and inviting? Was parking convenient? Do they have separate sick and well waiting areas? Take all of these things into consideration in your decision. If you plan to visit more than one office, be sure to take notes for later consideration. Ask some questions After you have decided that the office environment is a good fit, you will need to interview the pediatrician. Pharmacists, Darcy and Phil King said “After collecting recommendations, I scoured their websites, set up appointments and interviewed them. After the visits, it was very clear which was a good fit.” Start by asking how sick appointments work. How long would it take for a sick child to be seen? Is there a good chance the child will be seen by his own doctor? Do you have similar views on health and wellness such as circumcision, breastfeeding, and immunizations. It is also important to consider what your preferences are. “Do you want a doctor who offers choices and lets you decide which one words best for you? Or would you be more comfortable with one who gives a lot of directions?” (www.babycenter.com) Overall, does the doctor seem genuinely interested in your child or does he seem distracted or rushed? Take your overall impression into account when making your decision. Bedside Manner How the doctor interacts with your child will have a lot of impact on the decision to choose a pediatrician. Do you feel comfortable around the doctor? Does your child? Are they willing to take the
time to listen to your questions and concerns? A good beside manner can go along way in making you and your child feel comfortable. Finding the right pediatrician can be overwhelming, but realize your decision does not have to be permanent.“It’s important to recognize you are not stuck with your decision. You see your pediatrician a lot in those first months and if there is something really bothering you, it’s okay to switch.” says mom, Becky Baldridge. The parent’s goal is the same as the pediatrician, a happy and healthy child. When you find a doctor that is a good fit for your family, you can all work together to reach this goal.

Correct Portion Sizes: How to Keep Portion Distortion in Check With restaurants offering enormous plates of food, drink cups often in “Big Gulp” sizes and snacks sold in king-sized packages, it can be hard to know how much to eat sometimes. It’s difficult to avoid eating bigger at home, too. The size of dinner plates, muffin tins and pizza pans have grown. Cars have larger cup holders to accommodate the drink sizes stores sell. As everything gets bigger, bigger starts to seem like the norm, distorting how we think about a serving size or the “right” amount. One study found that modern portion sizes of popular foods added an extra 50 to 150 calories. While that might not sound like too much, an extra 100 calories per day can pack on an extra 10
pounds of weight in a year! Some meals appearing “average” in size can add up to a whole day’s worth of calories. A large order of french fries can contain as many as 1,000 calories. Add a hamburger and an extra-large soda, and you’re getting more than 2,000 calories in one sitting. And this isn’t unusual. A study published in 2012 found that 96 percent of restaurant meals exceed USDA recommendations for fat, salt and overall calories. If you’re trying to maintain your weight or lose a few pounds, read on for some tips on how to estimate proper portion sizes, keep portions in check even when faced with big plates of food.

What you can do to manage your portions. Not sure what a portion size should be? Make sense of portion sizes by using hand symbols for portions (see graphic on page 30). Learn to read food labels. Pay attention to the number of servings contained in the package, then note the calorie and fat content per serving. If, for example, the label on a large muffin indicates that one serving has 250 calories and 10 grams of fat, and the muffin contains two servings, then you’ll have eaten 500 calories and 20 grams of fat from that muffin. Compare marketplace portions to recommended serving sizes. If you eat a marketplace portion of something, compare its size to what’s recommended by the USDA. No Homework Hassles: 7 Simple Ways To Raise Conscientious Students BY CHRISTINA KATZ
Homework serves many helpful purposes. Students learn how to follow through on lessons taught in the classroom. Homework tests a student’s comprehension of material that has already been covered in class so that learning gaps can be caught early. Finally, homework teaches kids how to focus in locations outside of school. Being able to settle down and get to work on a project is a skill that serves students in school and throughout the rest of their lives. Like it or not, parents, homework is an important part of school and learning. Over an academic career, the amount of work your child brings home will gradually increase as a student progresses through the grades. Homework will also become more challenging and complex as the years go by. Many parents are surprised when they can no longer confidently answer their child’s homework questions, especially in math, but don’t let this throw you. Of course, you are not the person who needs to complete your child’s homework. You are the person who can help create a space at home where your student can apply focused concentration in the completion of assigned tasks. If you want the transition from school to homework to go as smoothly as possible, be forewarned, you are going to need to monitor your

behavior as much as your child’s. If you follow these simple tips you can help all your children avoid homework hassles. 1. Be pro-homework. Whatever you do, parents, do not get down on the idea of homework. If you do, you might implicitly grant your student permission to dismiss it, too. If your child cannot handle the load that is considered typical for each grade, discuss your child’s challenges with the teacher. However a generally negative or critical attitude towards learning, teachers or school will only undermine your child’s ability to prioritize homework. Addressing any concerns swiftly and giving teachers and administrators the benefit of the doubt will serve your student’s highest good, inside and outside the classroom. 2. Make space for each student. If you have more than one student doing homework, try to create a separate space for each of them to work. The goal of homework is to take students out of a group environment and teach them to work independently. If others are always nearby creating distractions or trying to help, students won’t get a chance to see what they can accomplish on their own steam. Of course, some homework is meant to be collaborative. Look to your student for invitations to participate. Otherwise try to give each
student space where they can spread out and concentrate uninterrupted for the appropriate amounts of time recommended for their grade. Declare quiet time in the house until every student has had time to complete work. 3. Participate but don’t take over. Sometimes you will be invited to participate in homework, but more often, your child needs to see what can be accomplished alone. Even when you are invited to pitch in, let students lead the collaboration process. Make sure you are the helper, not the boss. Once you take over your child’s homework, it’s difficult to get them to reclaim responsibility. After all, you can get it done so much faster and more accurately than they can. So be mindful about not taking over in the first place. If your child is lost or confused about homework instructions, seek out teacher input to help get your student back on track.

Don’t Forget Class Participation Another way parents can really help students succeed in school is by encouraging them to participate in class. Teachers can’t stress enough how important it is for children to be involved in class discussions and debates. The key is to start early. Start the pep talks as early as Kindergarten and keep bringing up ways to participate all through elementary school. When asking about the school day, ask about ways your child participated in class. If your child is shy, encourage babysteps that will help them become more involved. If you remember to do this frequently throughout elementary school, the habit will be ingrained by the time kids start middle school. This way your child will learn from experience that students who participate do better academically and enjoy school more overall. 4. Encourage routine. Use the first few weeks of the school year to establish a set routine for getting homework done throughout the year. Your child might be crabby or tired during the first weeks of school as they adjust to new schedules and juggling more responsibilities than they did during the summer. But resist the urge to make exceptions to the expected homework routine in the first few weeks, as these habits set the tone for the rest of the school year. In fact, you will likely find yourself going through a homework adjustment period after each school vacation throughout the year. Stand firm that homework is the first priority when students return home each weekday. Homework for Mondays can be completed on Fridays or Sundays, according on what works best for your student and family. 34 September 2016 | PB Parenting |
5. Take advantage of student-teacher interaction. Some children are more shy than others. Other children may have trouble listening carefully to homework instructions in class. Others may forget to write down assignments or leave needed materials behind at school. Try to see all of these homework pitfalls as opportunities for your child’s growth. Don’t interfere unless you have to. Conspire with your child’s teacher to work together to help your student overcome unproductive habits. Don’t get down on your child. Instead, brainstorm with the teacher about ways to inspire improved academic performance. Teachers always have plenty of experience in this department. 6. Check grades regularly. At some point, your child’s grades will be posted online with the expectation that students and parents will keep up with academic progress. When this happens, it means that you won’t likely hear from teachers beyond parent-teacher conferences and report cards. The onus falls on parents to help students monitor each child’s progress in classes and address any discrepancies in grading. Don’t merely check your child’s quiz and test scores. Students are expected to turn in homework in a timely manner and to participate in class in addition to working hard on quizzes, tests, and projects. Don’t let a few misplaced homework assignments bring your child’s grades down. 7. Use tutors as needed. Despite your best intentions and your child’s best efforts, you may find yourself in need of a tutor during the school year, the summer, or even throughout the year. One of the best things we ever did for our daughter was to say yes to suggestions from teachers to enlist extra help beyond the school day. Academic challenges often show up during the elementary
school years and when they are met with helpfulness instead of judgment, academic frustration can be addressed and improved swiftly, especially when parents and teachers work together. Remember you are not your child and seek the type of help most likely to remedy the situation. If you find yourself taking your child’s disappoints too personally, working with a tutor is your opportunity to demonstrate detachment and humility for your child. Life presents challenges to us all. How we meet them determines how successful we will be in school and in life. However she wishes she had better mastered participating in class and interacting with teachers before she went off to college, where professors were far more intimidating.

The children come to the aid of a green sea turtle replica and Dr. Logger teaches the group about common injuries and illnesses that affect these endangered animals. No pre-registration is required.
