A reason to celebrate! That FotoFinder’s 30th birthday could fall into the time of a worldwide pandemic was as unimaginable as the Eiffel Tower without tourists, at least two years ago. Until the summer of 2021, none of us felt like celebrating anyway. However, the silver lining on the horizon is shining a little brighter and we are happy that we can finally celebrate something again! 30 years – FotoFinder is a child of Gen Y and looks back on great technological leaps, from the tube screen of the first dermoscope systems to the latest smartphone, from Windows 3.0 to Artificial Intelligence. The other day I found old floppy disks in my office cupboard, plastic dinosaurs from a different age. 30 years – In marriage, that’s called a pearl anniversary. Round, dazzling and mature, each milestone in early skin cancer detection signifies one pearl, and each medicam, each expert system, each academy, each device another – the 30-year necklace has many rows. 30 years – Prince and Queen were playing on the CD player when we stood by the window in our first Munich office, in late summer, holding glasses of champagne as we youngsters toasted in a new era. 30 years – Some of us have spent more years with FotoFinder than without. On the one hand, this is frightening, because (life) time has passed by so quickly. On the other, it’s also great, because it has been 30 years of encounters with wonderful people, 30 years with a team that is second to none, 30 years of passion for dermoscopy. 30 years – How many malignant skin tumours could be detected early across the globe with the help of FotoFinder systems, how many lives could be saved – if every single one of them were a pearl, it would be the most beautiful necklace in the world. 30 years – time to press PAUSE for a moment and celebrate. Today, our big thanks go to YOU, our wonderful clients, some of whom have been on the road as long as we have, accompanied by FotoFinder systems of all generations. None of this would have been possible without you! We look forward to the next 30 years with you!

FotoFinder Moments Rudolf and Andreas Mayer laid the cornerstone for FotoFinder Systems GmbH in 1991. Many years have passed since then – but some things have not changed since the beginning. Join us in our travels through three decades full of FotoFinder moments.

Arrow-alt-circle-right In 1995, Rudolf and Andreas Mayer, together with Kathrin Niemela, visited their first trade fair – the CEBIT – to present the “FotoFinder Bildzentrale”. Shortly afterwards, the Munich dermatologist Prof. Reiner Scherer (†) gave the founders the idea of developing a system for monitoring nevi: FotoFinder was born! The first device was delivered in summer with temperatures of 30°C and using a small Fiat Uno.

Then and now, major trade fairs such as the EADV are a particular highlight. To turn a visit to the trade fair into an experience for our clients, we are working on ever more innovative and interactive booth concepts, such as here in Madrid in 2019. Arrow-alt-circle-right

The premises in a converted farmhouse were still quite compact – back then, development and sales were close together. Together with Rudolf Mayer, we tinkered with our cameras in “Gyro Gearloose” style.

quote-left In 2000, I bought my first FotoFinder system. It was also the very first FotoFinder in Brazil! In those days, I purchased the system from Rolf Mayer on his first trip to Brazil. I do not remember exactly which system it was, but I guess it must have been the ancestor model of today's vexia.

I’ve known FotoFinder for longer than 20 years. After working with the technology during my residency at the Charité in Berlin, I acquired my first own FotoFinder for my Potsdam practice in 2005 – a tower station, the forerunner of the ATBM. At that time, there was no classic total body mapping, and I was the doctor with the most extensive progress documentation for nevi in Germany.

Now, many years have passed, and I still work with FotoFinder in my practice. More precisely, with the most modern version of the equipment, the ATBM master. Over the last three decades, I've been following all the developments of the equipment. I worked with all versions of the FotoFinder medicam. I concentrated more and more on the follow-up of patients with multiple nevi, using the indication provided by the body map. My work as a dermatologist certainly changed with the remarkable development of the technology, which made my patients and me more confident over time.

Today, there are two ATBM master and two ATBM classic at the two locations in Potsdam and Berlin.

Working with these systems is not only holistic and logical, but also pleasantly simple – simply fun for me and my team! I am particularly impressed by the “leap in technology” that the cameras have made in recent decades. The images in the Mosaic View and in video dermoscopy are so detailed and the patterns so well recognisable that even the smallest melanomas are detected by FotoFinder, something which would not be possible with the naked eye! My patients also appreciate the FotoFinder devices very much: For one thing, it doesn’t matter what location they come to, images and data are always available through the networking of the Universe wireless app. On the other hand, they can follow the entire examination, which makes the whole thing transparent and strengthens their trust in me.

In my opinion, FotoFinder is a real enrichment for every dermatological practice and its patients.

Thanks to Total Body Mapping, I can make a much more valid statement about which nevi actually need to be excised. The number of surgeries has since decreased and is focussed on those that are really necessary. It’s also remarkable that in my practice the demand for appointments for skin cancer screening increased even during the pandemic – quite different from what many colleagues experienced. The ATBM master with its hygienic distance of 1.6 m is appreciated by the patients and is not least a unique selling point in the region.

quote-left When I started my specialist training in 1991, we usually only worked with ordinary magnifying glasses in the practices to examine the total body status of the patients. Only hospitals and clinics routinely had hand-held dermatoscopes. It was in 1998 that I first came into contact with a FotoFinder device and purchased my first medicam – the medicam 400 model. When I opened my practice at that time, I was the only dermatologist in Baden who worked with this system. Although it was only a small private practice, I had a huge influx of people who wanted to be examined with the FotoFinder. That was over 21 years ago and I haven’t worked a single day without my FotoFinder since. It is not only reassuring for the patients to see what is being done. They can see for themselves which nevus I’m examining, right on the screen. Over the past decades, I too have always grown with this technology and become more confident in my work. By looking at the histology of an excised lesion, one can always revert back to the documentation. This confirms one’s own suspicions – or not. Since April 2021, I have been working with a new vexia system and the medicam 1000s. The images that this camera delivers are a quantum leap. I can recognise vascular patterns and especially the criteria for malignant, non-pigmented or low-pigmented lesions. The introduction of Artificial Intelligence has also significantly improved my daily routine. Even after 20 years of experience, I can always get a quick, sensible and simple second opinion in case of an inconclusive diagnosis – and that impresses my patients as much as it does me! In short: FotoFinder has always been the heart of my exam room and I wouldn't want to miss working with it.

I have been working with FotoFinder systems for over 12 years. We currently work with two ATBM towers, paired in our network, one is the ATBM classic and the other is the ATBM master edition. Our first device was a FotoFinder medicam 500, after that we bought a FotoFinder dermoscope in 2015 and later we updated to automated total body mapping in 2017. As we were the first to perform sequential digital dermoscopy in our region, our early start with FotoFinder placed us in a leading position. Generating trust with dermatologists and colleagues from other specialties allowed a very important flow of patient referrals. The start was gradual, and we currently perform about 300 evaluations per month. The technological advances of the devices, the increasing image quality of the dermoscopic images together with the incorporation of body maps with polarised light, made our task much more agile and more reliable – even for our patients. Initially, our work was mainly focussed on the digital monitoring of lesions in high-risk patients. Although we incorporated the "Two-step Method of Digital Follow-up" (total body photography and digital dermoscopy) as our main approach from the beginning, the body maps were registered manually with the medicam, which implied special dedication to make body maps as similar as possible in subsequent follow-ups and, although image quality was adequate for dermoscopy, it was less adequate for body mapping. With the incorporation of the ATBM classic, the mapping process became easier in terms of time and reproducibility. Finally, with the use of the ATBM master, the polarised maps, the Mosaic View and the Bodyscan software, everything became even easier and much more reliable, because now we received crucial information with body mapping only.

Interview With Rudolf and Andreas Mayer, father and son who founded FotoFinder Systems GmbH together in 1991.

It has been 30 years since Rudolf and Andreas Mayer, father and son, founded FotoFinder Systems GmbH together – at that time still under the name TeachScreen. Their first office was the back room of an Asian shop in Munich, and there was no business plan or any start-up capital worth mentioning. However, they did have a vision: to develop a digital image database that would make photo archives superfluous. Today, FotoFinder is a global leader in skin imaging and the brand stands not only for brilliant images, but also for a family community whose goal it is to give people worldwide access to state-of-the-art skin and hair examinations.

FF By founding the company in 1991, you laid the cornerstone for a long success story. What were your motives back then and how did the start go? AM  We started in a completely different direction. Originally, we didn't think about medical technology at all, but about multimedia. At that time, frame grabber cards were a new technology for transferring moving images to computers. I was still a student in Munich at the time and worked in a young, innovative company. We recognised the potential of this technology and thought that it actually needed the development of applications, as these were not yet available. All one could do was feed the images into the computer, but then we asked what one could do with these images. That's when we came up with the idea of using software to create computer-based training and image databases. RM  At that time I was in sales for an American company. However, we actually started our own company at relatively short notice – with a lot of enthusiasm and very little know-how. We only came to medical technology by chance. FF When you started in 1991, it was just the two of you. Today, FotoFinder employs over 80 people and is the market leader in skin imaging systems. Would you ever have dreamt of this development? What brought you to dermoscopy in the first place? RM  Of course, we did not anticipate this positive development initially. In 1991, the only thing that mattered was the survival of the company. We came to dermoscopy purely by chance – and I always like to tell people about it. The start-up situation was already difficult at that time, until one day we were approached by a company from Dortmund that sold sunbeds for tanning salons. This chain had heard about us through the Munich dermatologist Prof. Dr. Scherer and wanted to carry out some kind of skin check to find out the recommended duration of tanning for different skin types. At the time, that was a very innovative idea.

AM  The idea was to use image and camera technology to gain insights into this topic. This is where we came in, because we were asked to develop the appropriate software. We agreed with Professor Scherer, the project's scientific advisor on dermatology at the time, to get involved and put together a prototype camera system. Unfortunately, the chain went broke before the project could be completed. There we were – without an order and with a development in its infancy. However, Professor Scherer then said, “Well, we dermatologists would also be quite happy about such a camera system.” He encouraged us to continue building our prototype with the prospect of buying a further development from us. That’s how we came to dermoscopy. FF When was the moment you realised you had created something great with FotoFinder? AM  There’s actually no concrete key moment. However, we experienced something special after this very hard initial phase when we sold the first “dermoscope”, giving us a real sense of achievement and making us realise that our business idea could actually be something after all. RM  The device looked very different back then and we had to fiddle around a lot with the configuration. But the fact that our system was accepted by a client made us think, “OK, there is something to it after all, something we can work with.” That would have been sometime around the year 1995. FF What successes are you particularly proud of? What are the big milestones? RM  Of course, our idea with the camera was a milestone. Unlike other cameras, the medicam had a special feature that enabled the capture of both micro and overview images with autofocus and high light intensity thanks to the patented microscope attachment. This was extremely convenient for the user and the quality of the images was also very good. The camera therefore represented an important key component and a unique selling point for us for years – even today. AM  And then we were the first to have this idea with Body Mapping. That was in 2007, with the first models of our bodystudio system. Many dermatologists were excited to be able to look at moles with our video camera. However, the fact that the whole body was now being captured with such a machine was a completely new approach and accordingly, there was a lot of scepticism. At the time, we already wanted to define a different workflow. The fact that we have managed to establish and perfect this over the many years is certainly a milestone. And there’s the FotoFinder AI. Impressions FF Were there moments when you would have preferred to give up? Would you do anything differently today? RM  In the beginning, everything was incredibly stressful. Money was tight because, at the time, there was no such thing as venture capital. We had to finance everything with our savings. Or let's put it this way, we always financed ourselves from our own earnings, we never had outside financing. AM  That's why I started working part-time to earn some extra money. At some point, we had the breakthrough. I was 23 at the time; that was certainly quite a bumpy start to my career. I had no experience, and my father took some surprising risks. But he has always been someone who was quite willing to take risks. You ask what we would have done differently? I would no longer dare to start something like that today. And if yes, I would go ahead in more targeted manner. These days, business models look more professional. We did not have a business plan, as is now common practice. We have always moved slowly, something that is no longer the norm at present, everything has to move much faster. In this respect, FotoFinder 2021 would start completely differently. But for us it was fortunate at the time that we were close to the market with our method of always elaborating our findings. FF Looking back, what tips would you give your 30-year younger self? AM  One was brave at the time and did not know what lay ahead. That’s why we had the courage to go ahead. With today’s knowledge, we probably wouldn’t do that again. [laughs] RM  But perhaps we would approach it differently. But that’s difficult today because so much has changed. At that time, we also entered a period of fundamental upheavals in IT. It was certainly also a pioneering time. Now options are so exhausted, back then the situation was still very manageable. Today’s challenges posed by the abundance of regulations did not exist in this form in the past. In this respect, it is probably difficult to see all this in light of the present day. FF Which moment will you never forget? RM  There are lots of those. When you think back to the wonderful moments, you do recognise some milestones. When we were at trade fairs, at some point with a larger booth, we realised: “Gosh, we used to dream of such a great stand. Now we’ve made it and have one ourselves!” What a sensational feeling. AM  In the beginning, I’ll never forget that, after the first sales, we used to say, “Every time we sell a FotoFinder system, we’ll go out to have dinner at this nice restaurant on Schärding’s town square as a reward.” That would be difficult today [laughs]. Unfortunately, the restaurant has also long ceased to exist. RM  And we would have been overweight long ago! [laughs] 12

FF 30 years after its foundation, FotoFinder is still a family business. What does the “FotoFinder Family” mean to you? RM  For us, it’s an extremely strong element. We are actually able to work without conflict from within the innermost family circle. That's great, as is the knowledge that you can also transfer this feeling of connection and strength to employees, clients and partners. AM  We have repeatedly found that the family atmosphere is very much appreciated and that it is possible to achieve what otherwise would not be possible. It is really special when you feel that our company is not just a place to make money, but also a place of fairness where you enjoy being, and meeting likeminded people. Taking responsibility for each other, that’s what we mean by “family”. Our thanks for this approach is commitment and loyalty. That’s really a wonderful thing! FF How did the industry change in the last three decades and what makes FotoFinder so unique when it comes to global competition? AM  Although the industry has partly consolidated over the last three decades, and there are fewer and fewer players, one does notice that conditions are becoming more difficult. As we can see from the many regulations and all the quality requirements, it is expensive. This means that although there are fewer competitors, the market has become more demanding overall. We tried early on to establish a brand and give it a face, and I think we managed that extremely well. FotoFinder is a household name in many countries, and this is also based on many subtle factors: contacts and friendships, of course, key opinion leaders and, last but not least, our consistent brand policy. This distinguishes us from our competitors and makes us unique. We have very loyal clients who know what FotoFinder is all about. Our image, something extremely important in this day and age, is very good, and we are constantly working on it. This is especially noticeable now with our Online Academy series. If you're not known, you don't inspire people to look at anything online.

FF In a world that is becoming increasingly complex and digital, what gives you strength for your everyday work as a FotoFinder CEO? AM  Of course, I also draw strength from my private life and the fact that we have quite a good work-life balance here in Lower Bavaria. Here you are usually quite grounded and live in a still very relaxing environment, knowing that “here I can rest and recharge”. I think it is something very positive to have a contrast to one’s work activity, something I’m experimenting with at the moment. For example, I started cooking jams – something very simple, down-to-earth, working with my hands. A pleasant change from IT and the digital world. FF Is there anything else you would like to tell the FotoFinder community? RM  It's great that the FotoFinder community exists and I hope that it will continue to develop and that many will get involved, each in their own way. Something, that lives on. I'm sure everyone can contribute and thrive in their role. And I think that's not the worst thing. AM  I would be happy if many others could also see it that way and realise: “Okay, the FotoFinder Family offers me a nice combination of positive professional, but also pleasant interpersonal aspects, something that maintains the fun of it.” That, I think, is the most important thing: that when you are successful, you can be passionate about your work and say: “That’s where I rest within myself, that’s what I like to do.”

FF What are your visions for the future of FotoFinder and what challenges do you expect in the coming years? AM  Total Body Dermoscopy, which we initiated, still has a lot of potential. AI is also exciting, and we are only in the early stages. Scenarios can be devised in connection with what we have already worked out with telemedicine. This results in an overall structure that will of course reach many more dermatologists and also find its way into general medicine. Technically, there is definitely still a lot possible in this field. RM  Yes, there is still a lot of potential. I think we will have a whole range of options in the near future. When you look at how AI already works today, then that’s very promising. Artificial intelligence will, I believe, become a very important part of our lives. INSIGHTS 2021 / 2022

The FotoFinder Feeling

30 years – and it feels like only yesterday that we moved into our first office as a two-man business in the back room of an Asian shop in Munich. Back then, no one would have guessed that we would one day become the world market leader for skin imaging systems in dermoscopy. Or that we would win the Bavarian Export Award and be honoured as Lower Bavaria's top company. Or even that, over the years, we would not only find business partners, but also wonderful friends and FotoFinder Family members all over the world, without whom we would not have been able to paint this “Big Picture”. That's how it went from the FotoFinder System to the FotoFinder Feeling, which can't be put into words. We would like to take this opportunity to thank our great staff, our wonderful customers and the strong global partners at our side. We look forward to the next 30 years full of unforgettable FotoFinder Family moments!

Total Body Dermoscopy in Naples An interview with Prof. Giuseppe Argenziano & Team Giuseppe “Geppi” Argenziano is Professor of Dermatology at the Second University of Naples, Coordinator of the Skin Cancer Unit at the Research Hospital “Arcispedale Santa Maria Nuova IRCCS” in Reggio Emilia and co-founder of the International Dermoscopy Society (IDS). He is globally known for his passionate work, research and teaching in the field of dermato-oncology and has published numerous scientific articles and books. Prof. Argenziano provides some insight into his work together with his team around Dr. Gabriella Brancaccio, Dr. Giulia Briatico and Data Engineer Dr. Marcello Di Benedetto.

FF Geppi, you have more than 20 years of experience in digital dermoscopy. How has Total Body Mapping (TBM) with highresolution images changed the follow-up of high-risk patients? Prof. Argenziano Total Body Mapping is a revolution in terms of the efficiency, accuracy and speed of our daily work. I am pretty convinced that Total Body Mapping will completely replace the digital documentation of individual lesions, which has been our standard procedure for more than 20 years. The future I foresee is just represented by a TBM machine that captures clinical and dermoscopic images of the entire skin in a few seconds allowing to immediately visualize new or changing lesions that require our attention.

FF Gabriella, Marcello and Giulia, how do you select patients for Body Mapping? Dottori When we started with Total Body Photography (TBP) and Sequential Digital Dermoscopy Imaging (SDDI) in 2016, we established some rules to appropriately select follow-up patients. The indications are: Caret-Right Patients with more than 100 common nevi Caret-Right Patients with more than 50 common nevi AND at least 5 atypical nevi AND at least one of the following melanoma risk factors: a) age over 50, (b) phototype I, (c) personal or family history of melanoma, (d) photodamage, or (e) history of more than one non-melanoma skin cancer Caret-Right History of multiple melanomas Caret-Right History of melanoma AND phototype I Caret-Right History of melanoma in a patient harbouring CDNK2 mutation Caret-Right Single suspicious lesion which should be monitored over time (SDDI only)  FF What feedback do you get from your patients? Prof. Argenziano In our department we have a clinic for the follow-up of cancer patients as well as a clinic for video dermoscopy. Fortunately, access to the video dermoscopy clinic is inexpensive for patients because it is covered by the National Health Service. The feedback is extremely positive because we always work in a team of two during the examination and the patient perceives the detection and storage of whole body images as a greater protection of his or her health. Those who have to cancel their appointment for some reason (e.g., due to COVID) try to get an alternate date. Some even ask us if we can also book family members who actually do not need this type of screening. A few anecdotes: Some patients are somewhat anxious during the examination because they know they are prone to malignant lesions. Others follow the examination on the screen to discover new moles, or simply to see whether they have gained or lost weight over time. In short, you could say that, among Neapolitans, video dermoscopy is very popular; they call this examination method “mappatura dei nei” – mapping of the nevi.

FF One question to all of you: Time is limited and the number of patients is on the increase. How can software in general and Artificial Intelligence in particular help physicians become more efficient? Gabriella Brancaccio und Marcello di Benedetto Software tools or AI can help us by improving the detection of mole changes. By significant changes, we mean both increases but also decreases in the size of a lesion (regression). As for our way of performing TBP and SDDI, sometimes the software could be less sensitive. Too many yellow circles can be confusing because they imply that all nevi should be checked dermoscopically, which is what we actually do and what every clinician should do. An inconsistent patient position at follow-up examination may also result in too many yellow or red circles. Automatic determination of dermoscopic changes starting from time 0 would be very helpful. As for Artificial Intelligence, we advise not to rely on AI alone, as it cannot replace an examination by the clinician. On the contrary, AI should warn the clinician if there is a nevus that doesn’t show any suspicious features now but is only suspicious because of changes since the last visit. (You can find a paper published by our group on this topic below). A perfect scenario would of course be to distinguish slight physiological pattern changes from pathological ones, but since that point has so far not been reached, a less ambitious goal would probably be more practical. This is the reason why red circles work well, as they highlight each new spot on the body, regardless of its nature.

Editor's comment To optimise the detection of changes, the Bodyscan is set to ‘very sensitive’ by default. In patients with multiple lentigines, but also in case of weight changes, significant changes in skin colour after sunbathing or inconsistent positioning, the software shows too many changes rather than too few. We recommend using the ‘Magnifier’ function of the software to check the evolution. By the way, yellow or red circles do NOT indicate whether a lesion is benign or malignant. FF Finally, a question asked by everyone who has ever been to Naples: What makes a Pizza Napolitana taste so different from all pizzas prepared outside of Naples? Prof. Argenziano Well, the secret lies in the Neapolitan ingredients: The taste of water, tomatoes, mozzarella, yeast and of course the Neapolitan soul make this pizza very special!

Paweł Pietkiewicz Dr. Paweł Pietkiewicz is a board-certified dermatovenerologist and an enthusiast of dermoscopy. He obtained his medical degree and doctorate from the Poznan University of Medical Sciences in Poland. In 2018, he founded the “Polish Dermatoscopy Group” for further training of specialists in early skin cancer detection. In 2021, he became a Polish board member of the “International Dermoscopy Society” and has given numerous lectures in courses and congresses in Europe, Africa and Asia in recent years. His main areas of interest include dermoscopy, inflammoscopy and artificial intelligence in dermatology. He authored and co-authored over 140 publications in dermatology and oncology. We met Dr. Paweł Pietkiewicz and talked with him about his passion in the field of dermoscopy.

FF Dr. Pietkiewicz, where did your passion for dermoscopy start? Dr. Pietkiewicz  It actually started with skin surgery. I needed dermoscopy to qualify lesions for excision and to check their exact borders. My love for dermoscopy was more of a side effect of my passion for surgery. Then I realised that there was a whole universe behind it, a plethora of lesions with unexpected structures invisible to the naked eye. Dermoscopy is the link between clinic and pathology, as each dermatoscopic structure reflects the architectural morphology of the lesion. FF Please describe your skin cancer screening workflow. Dr. Pietkiewicz  I start with Total Body Mapping and then continue with the handheld dermatoscope to cover all skin areas, including the scalp, mucous membranes, the areas behind the ears, neck, armpits, groin, perianal and perigenital regions down to the soles of the feet, and any lesions that seem suspicious to me or the patient. Then, I mark the lesions in the body maps that were recorded with the ATBM master. FF What are your next steps? Dr. Pietkiewicz  I mark the lesions of interest so that it is clear to the surgeon which ones to remove and what to expect. When I receive the histological report, I look at the micro and total body images again and check whether the findings match my diagnosis. If not, I have to check why. Who was wrong, was it me or the pathologist? The ATBM is particularly useful in the follow-up of multiple lesions: some melanomas are initially impossible to diagnose because there are no clues to malignancy. However, a

change can be observed if the patient later comes in for a followup. This technology enables the detection of morphological evolution or other features that do not match the age, skin type or location. FF In the following case, you already selected a specific lesion during the baseline. Tell us why and how you discovered changes in the follow-up. Dr. Pietkiewicz  The 62-year-old high-risk patient required close monitoring; not only because of the three melanomas diagnosed in the past two years, but also because of an atypical nevus syndrome with multiple lesions that present disturbed architecture. The selected nevus was one of these: a solitary lesion on the medial edge of the foot (which is unusual), slightly larger than the other nevi. It was selected along with others for digital follow-up. Within four months there was a slight change, but I thought it was because I stretched the skin a bit. We agreed on a follow-up after three months. When the patient returned after five months, the change was striking! FF One last question we ask our FotoFinder friends around the world: What is your favourite Polish dish that you would recommend to everyone? Dr. Pietkiewicz  Well, that’s a bit controversial, because this national dish is also claimed by Lithuanians [laughs], where my family has its roots. I recommend “babka ziemniaczana”: a potato pie with chopped onions and garlic, marjoram and bacon cracklings. It is served with sour cream and tastes delicious!

2 years of the pandemic – what now?

Who hasn't heard the saying: “What do I care if a bag of rice falls over in China?” Nobody will hold to this sentiment since SARS-CoV-2 has had the world in a stranglehold for almost two years now. One bag of rice falls and causes the next one to topple over – a domino effect that continues until rows upon rows of rice bags toppled around the globe. The pandemic has shown us how small the world has become – in just a few weeks, the virus had moved from deepest China to even deeper Bavaria. Since then, contagions and lockdowns have been coursing across all continents like Mexican waves. The incidence rates were accompanied by new scarcities for us, be it masks, vaccines, toilet paper or microchips. FotoFinder also had a few toppled bags of rice: In a very short time, we restructured the entire operation – working from home, shift work in production, mandatory masks, online meetings instead of trade fairs. In addition, there were delivery delays – some not until 2021 – and blatant price increases for components and parts. Global supply and transport chains are still far from normal – or maybe this is the new normal? The line by the German poet Rolf Dieter Brinkmann is also frighteningly true: “And the closer they lived together, the further they grew apart.” Never have we been so near and yet so far away from one another, spatially as well as in our minds: Suddenly we could no longer cross the border into Austria, not to mention our beloved long-haul destinations. Suddenly, good friends had completely different views on vaccination. Suddenly, everyone knew someone they had misjudged completely. Some have reinvented themselves, while others have become even greater couch potatoes – the radius of many a contemporary has become as narrow as that of a garden gnome. Many skipped important check-ups because they were afraid. This is one reason why the detected melanomas are now often thicker than before COVID. And yet – the crisis has brought forth some good. The FotoFinder Online Academy was born during the first lockdown. The world’s best speakers have already been our guests, and perhaps you too have already enjoyed one or the other online training course from your home or practice. This has brought our dermoscopy community closer together despite social distancing. It's so nice that the Online Academy will continue to exist in the future. And now? Are we going to celebrate “Freedom Day” soon? Will that be the start of the Golden Twenties? Although the end of the pandemic seems visible on the horizon, we may be facing new ones. The fear of losing our social lives again remains. The fact is: We will have to be flexible, within ourselves and in the world, flexible even without being able to travel. And: Whatever we didn’t miss during the pandemic, we don’t need to lug around with us in the future. But one thing has not changed: our common goal of detecting skin cancer as early as possible. We will continue to be fully committed to this goal in the future. And otherwise? Keep a sense of humour, be considerate and face the issues. In other words, live. Dermoscopy makes us wiser and happier! An interview with Prof. Dr. Aimilios Lallas For almost every dermoscopist worldwide, he is one of the central top experts in the field of dermoscopy: Prof. Dr. Aimilios Lallas, Professor at the First Department of Dermatology at Aristotle University of Thessaloniki (Greece) and newly elected President of the International Dermoscopy Society (IDS) with more than 20.000 members worldwide. Aimilios Lallas has specialised in and deals mainly with the early diagnosis of skin cancer using non-invasive techniques and the management of skin cancer patients. He is the author of more than 340 scientific papers, several book chapters and editor of nine textbooks on dermoscopy. The passionate dermoscopist has received several scientific awards and teaches dermoscopy all over the world. Over the last years, he has established scientific collaboration with colleagues around the globe and trained numerous young dermatologists and fellows worldwide. In our interview, he provides insights into the evolution of dermoscopy and diagnosis as well as the future of the IDS to enhance and strengthen the daily routine of every dermatologist.

FF Prof. Lallas, you are the newly elected President of the IDS. Tell us about your vision for the IDS?

FF The pandemic also had a firm grip on the IDS' activities. How did you manage it?

Prof. Lallas We have quite an optimistic plan and we have a great vision for the society! We are planning to enrich our educational activities a lot, to provide much more educational material to our members. But also, to support exchange and fellowships; to organise several scientific meetings; to practice more teledermoscopy; to continue with AI projects; to initiate projects on skin of colour and on imaging in aesthetic medicine.

Prof. Lallas It was really tough because our spirit depended a lot on the interaction. To adjust, we organised our first Virtual World Congress in June this year, which was a great success with more than 2,800 participants from 90 different countries around the world. We also provided a series of educational podcasts and empowered the function of our social media channels. We will continue with this concept and are already planning the next virtual meeting for 2022. But I cannot hide that what we are looking very much forward to is our 6th World Congress of Dermoscopy in Buenos Aires, Argentina in 2024.

FF Which exciting developments have already taken place? Prof. Lallas For example, we have created a committee for the non-English-speaking community; it is chaired by Raimonds Karls and Gabriel Salerni, who will help translate important educational material and society information for those members who would prefer to learn about dermoscopy in their native languages. There will also be a new scientific programme committee chaired by Zoe Apalla for creating exciting content and inviting fantastic speakers to our international meetings and congresses. We created a new logo for the IDS and modified the membership status as follows: being a member in the IDS will remain free but will require periodic validation. We are also preparing a modern free online e-book, microlearning modules, online discussion panels and a lot of gaming!

FF Let’s have a look at your main field of activity. Please explain how dermoscopy changed the paradigm of melanoma screening.

Prof. Lallas I invite you to travel back with me to the 1980s, the years before dermoscopy. Try to imagine our colleagues at that time: they had – to base their decision – to look on the clinical image, on the clinical morphology only. In that era, this was an unrecognisable melanoma ( 1 ); in contrast, this was a melanoma not recognizable ( 2 ). It was just a guessing game. We cannot imagine today, how they were able to do it without dermoscopy. Dermoscopy changed it all drastically. And not only in the field of melanoma and skin cancer, but also in the field of general dermatology.

Dermoscopy also had an impact on questioning predominant theories and changing mindsets, ways of thinking and attitudes. In that era, the predominant theory was that melanoma is mainly the result of a malignant transformation of a benign lesion. Today we know how early melanoma looks like and easily accept that melanoma is usually not a result of a malignant transformation, but it mainly develops de novo and is malignant from the beginning. This is an example of a radical change in the concept of melanoma genesis. 3

FF What about the evolution of dermoscopy in the years that followed? Prof. Lallas Let’s go back to the year 2000, when dermoscopy started and the proportion of melanomas that we cannot recognise became smaller.

The most important change is the following: Look at the pictures; both lesions are melanomas. But of course, 4 is a melanoma that you can recognise, while 3 is a melanoma that we cannot recognise. In the past, we would force ourselves to doubt if the melanoma we cannot recognise is in fact a melanoma. We would prefer to say that this is a precursor. Today, we have no problem in accepting our diagnostic limitations because we have our previous experience. We know that sensitivity with dermoscopy is not 100 % and we have no problem to accept it.

FF Which impact did the development of dermoscopy have on today’s practice? Prof. Lallas What makes us better clinicians today is not only our improved diagnostic capability but also the fact that we are able to accept our limits. By questioning traditional theories, dermoscopy helped a lot to change the concept. Accepting our limits allows us to adopt and apply strategies in order to improve diagnostic accuracy and to minimise the problem of featureless melanomas, such as total body photography, digital monitoring and sequential follow-up or the comparative approach among different lesions to detect the one that deviates from the signature pattern.

FF Can you explain the step from diagnosis to overdiagnosis? Prof. Lallas After the ABCD rule, dermoscopy was the second revolution in dermatological diagnosis and it made us able to recognise melanomas much earlier. In this case, dermoscopy is a game-changer. Especially in lesions with less than six millimeters but with clear-cut dermoscopic characteristics. Clinically, they even could not be seen – in fact. But dermatoscopically they were quite evident. Of course, the diagnosis of small melanomas raises the issue of overdiagnosis, because many of these melanomas would not eventually harm the patients. The problem is that, for the moment, we have no way to reliably predict the biological behaviour of an early melanoma. In my opinion, overdiagnosis is a problem, but this is a reasonble cost to pay to diagnose really threatening tumours earlier. A much bigger problem is the overuse of unnecessary aggressive and/or costly treatments for tumours with low malignant potential, such as lentigo maligna or basal cell carcinoma. This is what we should address in the years to come. Knowing the truth is never a problem. The way we react to the truth might be.

FF What about the management of these patients? Did dermoscopy change it at all over the years? Prof. Lallas Dermoscopy dramatically changed the reality in dermatology by reducing the number of unnecessary excisions of benign moles. This is something easy to understand. How many benign lesions that clinically might look a little bit ugly become

easy to diagnose with our dermatoscope. There is practically no acceptable alternative way to manage patients with multiple atypical nevi than using digital dermoscopy and sequential monitoring. I don’t even want to imagine or to remember which was the fate of these patients in the era before dermoscopy. An alternative strategy would be only to remove all the nevi of a patient to “prevent” melanoma development, which was a misconception anyway.

FF So, do you think there will be a future with no more excisions of benign lesions? Prof. Lallas In my point of view, excisions and biopsies performed for diagnostic purposes are not a real problem. They are less with dermoscopy and become even less with experience. But, even with dermoscopy, some benign lesions will still be biopsied because of diagnostic uncertainty. The problematic excisions are the ones done not because of diagnostic uncertainty, but because of wrong concepts. In the past, a problematic concept was the one of prophylactically excising nevi to prevent the possibility to have one of them transformed into melanoma. This was another radical conceptual change that occurred in the last decades and dermoscopy contributed to this change: the risk of melanoma does not depend on whether we are going to excise the nevi of a patient or not. The risk of melanoma is in the patient’s genes, not inside the nevi.

FF Is there any reason to apply dermoscopy in easy to diagnose lesions?

FF What about other dermatological diseases? Prof. Lallas By enhancing diagnosis, dermoscopy also facilitated the management of melanoma and non-melanoma skin cancer. This is true for basal cell carcinomas, actinic keratosis and squamous cell carcinomas. But it is also true for several inflammatory and infectious diseases such as scabies or lichen planus. I would say that the dermatoscope is the dermatologist’s stethoscope and we use it for any kind of skin eruption.

FF So, with dermoscopy, a clinical image is no longer needed to diagnose correctly? Prof. Lallas Of course it is needed! Dermoscopy is only one piece of the puzzle of clinical diagnosis! Especially in inflammatory disorders, dermoscopy makes no sense if not interpreted within the clinical differential diagnosis. But if you put it in the clinical context – which is always the case in clinical practice – then dermoscopic criteria become highly informative and enhance the clinical assessment.

Prof. Lallas Of course. Even when the diagnosis is straight forward, looking with the dermatoscope will train our eye. That’s not only for inflammatory diseases, that’s also true for skin tumors. We don’t learn by looking at extraordinary things, we learn by looking at ordinary things and this will help us recognise extraordinary things! I think that dermoscopy makes us wiser and for sure dermoscopy makes us happier!

FF What are your visions for the future of skin cancer screening? Will AI play a role? Prof. Lallas The impact of Artificial Intelligence in our practice will be huge in several aspects, but not by replacing the role of clinicians in diagnosis. The impact will be huge from a different perspective, mainly in fulfilling unmet needs. For instance, it might help addressing problems such as the impaired access to health services in countries with a shortage of clinicians, or reducing the time required for diagnostic procedures. An example: I am looking forward to an AI algorithm saving our time by accurately detecting changing lesions. It would be great if, during a follow-up visit, we could focus our attention on five lesions instead of 100 lesions. I also forsee other kinds of AI application such as precision medicine, predicting personalised treatments or prognostic biomarkers. I'm not speaking only about melanoma, but also about inflammatory diseases. Definitely, AI will be there in the future, and this will be a precious tool in the hands of clinicians.
