Hoist Block Inspection Checklist A monthly crane and rigging informative column for all personnel directly or indirectly involved with crane safety. Each month we will attempt to explain a different technical issue pertaining to crane operations here on Guam, addressing the sometimes over-looked or misunderstood topics by management and operators alike. By Dave Barnhouse This month’s crane topic is hoist block and ball inspection, an item frequently overlooked by the operator when performing his daily or monthly inspections. During my annual inspections I frequently find at least one or more of the more common hook block deficiencies. These include: safety latch weak or broken spring, missing data plate or weight and capacity info, dry swivel, damaged sheave flanges, rough hook surface capable of cutting synthetic slings, missing or damaged cheek plate retaining pins. Some of the common findings that are cause for crane block or ball removal from service until repaired or replaced: • Elongated center pin and hook trunnion holes exceeding 5% of original diameter, • Bent side plates, • Severe corrosion pitting • Corroded hook threads, • Bent or twisted hook, • Welding on hook, • Damaged or dysfunctional hook latch, • Cracks in sheaves, side plates, cheek weights, center pins, hook trunnion, dead end connections and hook. • Material loss due to wear exceeding 10% of original section. • Sheave wobble, • Deeply corrugated (not imprinted) sheave grooves, • Sheave wire rope groove diameter smaller than 2.5% or greater than 10% of the nominal wire diameter, • Missing or damaged retaining nuts, 14 | APRIL2018
snap rings, set screws, cotter pins, tie bolts, hook nut cap screws and lock wire, • Missing or illegible rating and warning tags. • Lack of sheave and hook bearing lubrication. Continuous operation: lubricate bushings every 8 hours and roller bearings every 24 hours. Intermittent operation: lubricate bushings and bearing every 14 days. • Ball swivel end play gap exceeding .08”. Excessive endplay indicates damaged internal set screw. For additional information on any particular block refer to the manufacturer. Block and Ball Monthly Inspection Checklist • If hook throat opening has increased by 5% • Load bearing point has been worn by 10%, the hook must be replaced. • Inspect hook tip. If it is twisted by 10° or more the hook must be replaced. • Check for excessive damage from chemicals and for deformation and cracks. • Check for and replace damaged, inoperative, or missing hook latches. The purpose for the inspection check list is to provide a quick reference for checking the integrity of a block or overhaul ball for continued use. The inspector must be aware of elongation, cracks, fractures, corrosion, unauthorized welding, damage to bearings and have NDT testing performed if necessary to assure product integrity. www.guamcontractors.org
Never use a crane block without a legible identification tag...Crane block tag is required to insure proper block application.” All hook and ball assemblies and load blocks shall be labeled with their rated capacity and weight.”- ASME B30.5-1.7.6. Most block manufacturers recommend a complete tear down inspection of crane blocks and overhaul balls every five (5) years to include a full magnetic particle inspection of the hook or load fitting, unless a more frequent period is required by regulation, standard, policy or indicated by previous inspection. Signs of Fatigue A crane’s bottom block assembly is made of many critical components that are subjected to loads and stresses during each hoisting cycle. While hook failure can occur due to overloading or mechanical abuse of the hook, hooks most often fail due to cumulative fatigue. Like Us On Facebook

These failures usually originate as a fatigue crack in the hook shank. The hook shank, which transfers the load to the hook block, is supported within the body of the block by a rotate bearing and other mechanical components not visible without disassembly. You may not get any prior warning in the case of a fatigue failure. Fatigue failure is dependent on the load, stress level and number of lifting cycles. Heavy process-duty applications, changes in application, overloads, old equipment and the use of below-the-hook lifting devices are all factors that may contribute to cumulative fatigue. Non-destructive Testing for crane hook inspections Island CERTS utilizes proven non-destructive testing techniques such as dye penetrant, or magnetic particle, in addition to the visual inspection and measurements normally performed during a hook inspection. Most importantly, the bottom block is completely disassembled. Then, critical components are examined along with the hook shank, where most fatigue failures originate. Lubrication of Hook Bearings: Anti Friction — Every 14 days for frequent swiveling; every 45 days for infrequent swiveling. Bronze Thrust Bushing or No Bearing — Every 16 hours for frequent swiveling; every 21 days for infrequent swiveling.
ASME B30.10 Inspection Frequency 1. Frequent Inspection– visual examinations by the operator or other designated person. a. normal service – monthly b. heavy service – weekly to monthly c. severe service – daily or weekly d. special or infrequent service as authorized by a qualified person – before and after each occurrence, with records of the operation. 2. Periodic Inspection– visual inspections by an appointed person making records of apparent external conditions to provide the basis for continuing evaluation. a. normal service – equipment in place – yearly (definition: service, normal – service that involves operating at less than 85 percent of rated load except for isolated instances.) b. heavy service – as in normal service, unless external conditions indicate that disassembly should be done to permit detailed inspection – yearly (definition: service, heavy service that involves operating 85 percent to 100 percent of rated as a regular specified procedure.) c. severe service – as in heavy service, except that the detailed inspection may show the need for use of nondestructive type of testing – quarterly (definition: service, severe – heavy service coupled with abnormal operating conditions.)

Morrico Equipment Announces Employee Stock Ownership Plan Morrico Equipment LLC announced that its company has become employee owned. The announcement was made last night during a special employee meeting at the Hilton Guam Resort and Spa. The conversion to an employee-owned company, also called Employee Stock Ownership Plan (ESOP), became effective March 31, 2018. As the company fast approaches 30 years in business on Guam, Morrico’s owners have been exploring how the company can expand its business and be able to better reward its employees, many of whom have worked at the company for more than 10 years. “We took a step back and looked at where we are as a company and how we wanted our company to evolve in the next ten years,” said Allan Morrison, President. “We were able to grow this company on Guam with the help of our hardworking and dedicated employees. The future of this company needed to include ways that would plant even deeper roots while serving the needs of our employee base.” They agreed that by giving the employees ownership of the company, they could help the legacy of the company live on for decades to come.

While it speaks well of Vikraman and his TTGI crew, the global accolades give rise to other underpinnings which may not directly factor into the outcomes of a global award, it does ground the business and its staff in as noble a goal—effective community involvement and charities.
TriStar Terminals Guam, Inc. (TTGI) received Outstanding Achievement Award, late March of this year, which was presented to KK Vikraman-General Manager for TTGI, in Rotterdam, Netherlands. This distinction was awarded by Tank Storage Magazine, a leading industry sector trade publication. Their selection panels are comprised of some of the sector’s leading luminaries from other sector-related enterprises; and, presents these annual awards at a gala award ceremony. Earlier in the year, TTGI was notified of their being shortlisted for two categories: . Most Efficient Storage Terminal Award . Outstanding Achievement Award Here’s a general overview and glimpse of the Guam operation. TTGI owns and operates one of the largest fuel storage facilities in the Pacific, Tristar Terminals Guam Inc, which is spread over 237 acres with 4 million bbl storage capacity. The facility has 26 tanks interconnected to Port Authority of Guam's oil berths, other major customers and storage terminals by means of a 8 kilometer long three underground pipelines. As Vikraman puts it, attempting to give clarity to the uninitiated (like moi), he states, “We’re like a realtor. We lease fuel storage to those operations that need it.” Voila! He continues, “We don’t actually own the fuel. We merely store it.” While that brings some illustrative simplicity to their operation; he sketches, literally, some of the innovation which is part and parcel of some of the initiatives he’s undertaken to the modernize and upgrade the overall facility and operation of TTGI. So, he parlays some of his mechanical engineering background in that explanation, and reveals his penchant for quality control—something which has surfaced as the bedrock for TTGI’s exemplary recognition.

As he lists TTGI’s participations and contributions to a host of local civic and charitable organizations, he takes on a tone of engagement and personal concern. “We do want to give back to our community, and we encourage our staff’s personal involvement in these events and activities that demonstrate it,” says Vikraman. He walks over to the office bulletin board, where many photos and certificates that spell out the company’s plain neighborhood participation—the “goods”. Surely, we are happy TTGI, Vikraman, and his Guam team, garnered world recognition for its Outstanding Achievement among and by Terminal Storage Professionals. Yet, we see and applaud, their Outstanding Community work on Guam, which they’ve built upon since their establishing operations in Sumay in 2009. When Vikraman assumed the position of the General Manager of TTGI in 2010, the facility was in an unkempt and unsystematic state and the Tank capacity utilization was below 25%. The terminal was not being utilized to its optimum commercial offering of 4 million US barrels. Vikraman undertook the task to overhaul the facility and strategically align it for future business opportunities and maximizing the storage utility. He developed a 5 year facility plan to increase storage utilization by initiating tank refurbishment, pipe line repairs and upgrading facility structure. The first task Vikraman undertook was the commissioning the refurbishment of 3 tanks with a combined capacity of 1.2 million barrels in preparation of a long-term storage contract with one of TTGI's most important client, Defense Logistics Agency(DLA). The fuel stored by DLA was of strategic national importance to the US Government and Vikraman was given a target to complete the refurbishment within six months. Vikraman developed a detailed project plan by identifying the works to be conducted on the tank which included replacing the tank bottom with double bottom plates, conversion of the tank roof from floating roof to fixed and also identifying a qualified contractor to complete the project as per design. Though the project was faced with multiple challenges ranging from lack of resources on Guam to logistical issues arising from Guam's remote location in the Pacific Ocean, Vikraman was able to uphold and achieve his target by completing the refurbishment within five months and handing over the Tanks to DLA within the committed timeline. Of the many accomplishments Vikraman has done for TTGI, the few projects that stand out are, the commissioning and completion of 13 major tank refurbishment projects, commissioning and completing the pigging and subsequent refurbishment of four, 6 mile long pipelines, refurbishment of LPG and gantry facility and installation of new truck loading gantry facility. The combined value of all projects completed under Vikraman's leadership is approximately $35 million. Simultaneously to the refurbishment projects, Vikraman spearheaded in the development of a Contract Commissioning Plan, Contract Compliance Plan, Quality Control Plan, Additive Injection Plan and various terminal operations procedures specific to

the contract requirements in place by TTGI's customers. The development of these otherwise non-existent documents made the operation of the facility more efficient for both Tristar and its customers. For example, where the previous facility owner Shell Guam, Inc. conducted operations with an excess of 40 employees, Vikraman led a small group of 30 employees to provide professional services Tristar is best known for throughout the globe. Vikraman has implemented a personnel rotation and cross training program for all field staff. TTGI has competent personnel from all departments capable of conducting tasks which is normally only handle by seasoned Operators. Also implemented is a refresher training program for operations personnel. Operators routinely cycle between operating locations to refresh their knowledge. Staying with Tristar Group's mission to be a responsible business, Vikraman has led and initiated the company to partner with government and non-profit agencies on Guam to promote and support various social welfare and environment programs like adopt a school, road adoption, Bus Shelter adoption, feeding the homeless, protection of wildlife, cancer awareness drives etc. TTGI is today renowned and recognized as socially responsible business in Guam. In 2017, Vikraman diversified and expanded the company's operations by entering into the commercial fuel supply business in Guam. After thorough and successful negotiations, Vikraman was able to secure a deal to supply around 50,000 barrels of Diesel to the local power agency for a sale value exceeding $3million. Apart from driving success for TTGI, he has also introduced and suggested innovative fuel blending methods to the local power agency, thereby bringing down their fuel costs and benefitting the utility consumers and payers. In conclusion, Vikraman is well-deserving for this award. TTGI's capacity utilization rate today is in excess of 80%. Vikraman has made a tremendous difference in the functioning of TTGI and gone above and beyond to uphold the core vision and mission of the company and has time and again delivered growth and value to all stakeholders.

However, every time the awards roll around – at seemingly arbitrary times albeit – it has managed to get a little bit more difficult to compose a lavish ode befitting the winner. The concept of the Excellence in Construction Awards has always been about recognizing industry frontrunners; the companies that set the example, you might say, with safety and training always at the very heart of the awards. Going through the archives, we have names ever so familiar in the community. The award itself managed to set contractors apart from each other, creating competition. It has almost always been about who did what and how well they did it. However, now, there seems to be a shift. An interesting shift that unites instead of divides. The award seems to be bringing companies together to share in the concept of excellence. What once was a singular feat, somehow has become an opportunity for the industry to share in the diversity of excellence. Excellence spans various facets of the industry; contracts, safety, and other notoriety-type of stuff. However, the word itself inspires some subjective connotations. Whichever way you define it, four of Guam’s contractors have their names in the spotlight for their excellent work in the construction industry. This year, we congratulate the Black Construction Corporation, Ian Construction, LMS Guam Landscaping, and Fargo Pacific for the awards they snagged at the Associated Builders and Contractor’s Excellence in Construction awards ceremony held in Long Beach, California. Fargo Pacific won the Associated Builders and Contractors, Inc. 2017 National Safety Excellence Award, more specifically under the NAICS 236 General Contractor Merit Winners. This is partially for their ‘exceptional safety records to rigorous training and education programs.’ More often than not, safety is at the center of awards like this and highlight a company’s commitment to and growth of safety programs in the industry. A couple companies from Guam were also recipients of the 2017 STEP Diamond Award. Hawiian Rock Products and Infratech International LLC both achived the highest level of the STEP program.
IAN Corporation IAN Corporation was selected for a “Specialty Contractor” category for the National Safety in Excellence Award. It required a thorough and stricter review of safety mission and trainings that IAN created and uses for all of their projects, according to Jihyung Chong of Ian Corporation. Chong credits the quality of IAN’s end-product, completing their project within the schedule, and the costs they saved the government, for setting them apart from other participants. On top of that, their 100-percent safety helped them snag the award, as well. In addition to the award itself, the notoriety that comes with it also puts the island in a unique position to strut our stuff. “It puts Guam in the limelight especially with all the issues surrounding the military buildup and H2B applications,” she adds. “But notably sets the ballgame high in the construction industry against our US counterparts even with limited resources.” She Like Us On Facebook

continues by saying that their work does not just impact the construction industry, but the economy and community as a whole, citing their commitment to hire locally and training them to master skillsets needed here at home. Further, Chong states that the award gives them recognition throughout the globe and allows for them to expand their clientele rolodex. “Most of our clients have found us through award recognition,” discloses Chong. When it comes to the concept of awards, the accolades help instill a little more trust from a client unfamiliar with the contractor’s work. “Nothing is more important than having a happy and satisfied client but what is more exciting is knowing that our employees are all involved in making this happen,” Chong adds. Chong shares the award with the employees of IAN Corporation whose mission is “to make Guam a better place to live and work.” Additionally, Chong states, “This award means that there are people out there who notice local contractors in the small business community and that our success is because of the people out there who entrust us to complete their project.” In the same vein as their employees, Chong points to mentorship: a key to “inspire by sharing experiences with other local contractors.” But, also an important part about being in the contractors community in Guam is giving back to the community as a whole. “It’s important to…get involved in the community and donate time and equipment into programs that benefit our community.” Chong indicates IAN Corporations contributions toward fixing up local sports fields for youth sports activities.
LMS LMS Guam Landscaping Services actually walked away with two awards – the Associated Builders and Contractor’s 2017 National Safety Merit Award and the Associated Builders and Contractor’s 2017 Step Diamond Award. According to Rob Salas of LMS, “only companies that have received STEP Diamond, Platinum, or Gold awards are eligible to submit a project for the ABC National Safety Excellence Award.” Salas added that the Merit Award goes to companies that maintain their commitment to safety on the jobsite and have ‘exemplary’ safety performance and programs. The last time LMS was in the CNB, they received the National Safety Excellence Award, and the cornerstone of their company has been – and probably always will be – safety. “Our corporate culture centered around safety distinguishes us from other entries,” says Salas. “We strive to create a culture where each member prioritizes safety…as a part of this safety culture, we emphasize the training and growth of the employee,” he continues. Salas highlights the diverse projects that LMS takes on, and that even with the different projects they have, the company still holds true to safety management. They have several safety programs including their Stop Work and CARE programs. These programs strike at the very core of their business – the employees of the LMS Family by having them take ownership of the safety in all of their projects. “Safety benefits everyone…extends beyond the employee and ultimately affects the customers and the local community as a whole,” Salas emphasizes. This extends to their subcontractors, as well. “For us, safety is not a goal, but a culture, and it’s extremely

rewarding to be recognized for the efforts of our diligent employees,” Salas stresses. “[This award is] a confirmation that our stringent processes and training are worth the time and effort.” Out of nearly 1,200 invitees, LMS was handpicked for the EIC award. LMS echoes the sentiments of fellow EIC awardee IAN Corporation “that despite the small size of our island home, our local contractors are still able to be recognized on a national scale,” says Salas. Recognition is fueled by commitment. It is hard work and effort. He states, “As your work increases in quality, it will simply be a matter of time before your efforts are recognized.”
Black Construction Company No stranger to the realm of Excellence in Construction awards is Black Construction Company. BCC submitted their Chuuk International Airport Runway Rehabilitation Project for consideration. The work began in January 2016 and ended in October 2017 – ahead of time! On the capital of Chuuk State in the Federated States of Micronesia, Weno, the project took flight at a $32.6 million price tag funded by the United States Federal Aviation Administration according to Leonard K. Kaae of Black Construction Company. 10-percent of the funding was provided by the local government. A lot of coordination and heavy lifting went into this project – literally. According to Kaae, the project faced many logistical and natural obstacles. The island’s remote location, with limited local resources required much of the equipment be shipped on two barge voyages. Then, without the cranes at the Chuuk Port required Black to transport their own crane over to the island. Additionally, BCC even shipped nearly 40,000 metric tons of aggregate from the Philippines for the FAA specifications. That took three voyages. But, even in the face of adversity, BCC prevailed, in more ways than one. Besides obviously completing the project, significant relationships with the Chuuk government and businesses were formed. Among other things, they leased property for storage, barracks, office space, and whatnot. But, they also hired local workers and purchased as many of their supplies and food from local vendors. It was a rather big investment into Chuuk State. Many know Chuuk as a tropical destination for diving World War II shipwrecks in Chuuk Lagoon. It only makes sense that rehabilitating the runway would have major economic benefits for the FSM State. Remarkably, they also finished the project without disrupting any air traffic. “Chuuk’s tourism industry is now receiving guests from other points in Asia, Europe and Australia, leading to the expansion of air carriers to those areas, who feel confident landing at this facility,” says Kaae. Like mentioned before, BCC is no stranger to the EIC award. However, this project highlights their strengthened commitment to the region. “Black Construction prides itself as being one of the finest outfits in the Pacific,” states Kaae. Yet, they are always look at ways to up the ante, and the recognition does just that for them. “Such recognition is an aid to Black Construction in expanding our services throughout the region and giving all customers the confidence that they will be achieving the highest possible quality for whatever project in which they are investing.”

Kaae states, “There are no short cuts to proper planning and execution.” She adds “Careful planning, leading to performing a task right, the first time, is usually a recipe for success and profit.” BCC should know. They have been in this position numerous times in the past. However, what makes a project ‘excellent’? It always comes down to that question.
What Makes A Project ‘Excellent’? For LMS Guam Landscaping Services: “An excellent project requires a commitment to a plan to achieve a clearly outlined goal. In order to achieve the goal, the project requires a knowledgeable team that is dedicated to achieving its goal. Management must provide the proper resources and equipment to position the team to accomplish its goal. In the end, we want all of our employees to go home to their families, healthy with no injuries.” For IAN Corporation: “An excellent project involves having a complete understanding of the requirements from project timelines and schedules to cost controls and estimating as well as manpower and equipment resources. IAN conducts weekly briefings with core employees involved in the project and has a complete project cycle involving individuals from Safety, Quality Control, Accounting, Engineering and Project management. These individuals all have a part in the project and our (sic) involved 100% into every aspect of their responsibility which makes for on time completion and budgeting and an excellent evaluation from our clients.” For Black Construction Company: Black Construction’s experience and continued effort in strategic planning and extensive support from the Guam headquarters is one of the keys to success for the Chuuk Runway Project. Black emphasizes the teamwork concept of making sure that all employees understand the project goals and strategy, and build on their own effort, skill and initiative to achieve high quality work. Pride in the inclusion of every individual’s effort at an excellent work product and maintaining a high level of safety pay great dividends upon completion.
For some, maybe excellence means devoting your talents to sharing best practices. For others, maybe it requires contractors to develop a succinct and strong concept that all stakeholders can support. Possibly ‘excellence’ means figuring out how to be the best version of yourself and hoping people recognize you for it. It seems that every winner has their own definition of ‘excellence’, AND THAT IS PERFECTLY FINE. Maybe we don’t need to define it after all.

LED Retrofit Kits LED lighting is not new, but the decision to upgrade an existing facility to LED can be expensive. But, like most things in life, you have more than one option 1) Live with my existing light fixtures or 2) Spend a lot of money and replace them with LED. One alternative to completely replacing older f luorescent fixtures is to retrofit them instead. Several lighting manufacturers, such as Lithonia, have retrofit kits designed to cleanly install into existing fixtures. These retrofit kits are designed to install cleanly and quickly and provide an upgrade to both the appearance of your light fixtures as well as the fixtures performance. There are several benefits to this approach. 1. Time savings. Lithonia retrofit kits are designed to install in just a few minutes. Saving a lot of time over the process of removing the old fixture and installing the new. Kits come with quick connect/disconnect mechanical and 30 | APRIL2018
electrical components and are designed to be installed without changes to your current fixtures wiring scheme. Staging time, installation, and cleanup time is significantly reduced. 2. Lighting retrofits can be done for recessed lighting, downlights, surface fixtures, exterior fixtures, and parking lot fixtures. Most commercial, industrial, and government buildings can benefit from this approach of converting to LED lighting. One more way to operate more efficiently and save your company or agency money.
