Eligibility rules for Medicaid are complicated. In Janna Visconti’s article, “Medicaid Myths”, some of the common questions concerning this issue are answered. Another hot topic in the health field is cosmetic surgery. Is Cosmetic Surgery right for you? New, safer procedures and choosing a physician are important to consider when making such a decision. Dr. Lisa Makrides tells us about the new SlimLipo laser-assisted liposuction procedure for smoother more natural results with less down time, less bruising and less pain than traditional liposuction.

The battle of the bulge
T
The battle over the bulge is never easily won.
There is always the latest diet and/or exercise craze to help you lose those unwanted pounds. Sometimes, though, no matter how hard you fight it there are those few inches that never seem to want to “come off” even with diet and exercise. Often it is a person’s genetic makeup that prevents them from shedding those unwanted small bulges.
Liposuction is an option to remove small bulges that won’t improve with diet or exercise. Body sculpting can be accomplished with very little down time or expense. Liposuction, however, does not remove cellulite, only fat. The best candidates for liposuction are those individuals who are only mildly above their ideal body weight, have firm, elastic skin, are in good health and have concentrated pockets of fat that are resistant to diet and exercise. The methods utilized to perform liposuction have changed drastically since its development in the 1970s. Traditional liposuction is performed under general anesthesia and requires a surgeon to insert a large hollow tube called a cannula into the areas you would like the fat removed. The surgeon then uses the cannula to break up the fat cells in each area by using a firm back and forth motion. Finally, the broken up cells are suctioned out. The technique and instruments of traditional liposuction are more aggressive, leave larger scars, require longer recovery periods and are not ideal for small pockets of fat removal. More recently tumescent liposuction has been performed. Using this technique, the surgeon will inject a solution composed of local anesthesia into the fatty areas to be removed. The solution not only includes a local anesthetic but epinephrine, a drug that contracts your blood vessels. The solution aids the surgeon in removing the fat as well as decreasing the amount of bruising associated with traditional liposuction. The ease in which the fat 6 | spring ‘09 | generations

Feel-Good Retirement Strategies How the ‘Emotion Quotient’ Impacts Your Retirement Equation
H
How you feel about your investments can greatly influence the ultimate outcome.
That’s the finding of a 2007 study exploring the link between emotions and financial decision-making in investors approaching, or in, the critical investment window five years before and after retirement. In its “Behavioral Risk in the Retirement Red Zone” report, Prudential Financial, Inc. sites five dominant emotions and tendencies that can influence investment decisions: fear, regret, inertia, aggressiveness, and susceptibility. The study also found: • Three of four investors — 76 percent — are affected by their emotions to a moderate or high degree. • Emotions can influence investors to react in ways that may not be in their best interest. • Products that provide both accumulation and income guarantees help mitigate the effects of behavioral risk. • If you’re planning for retirement, understanding the emotions that can influence your behavior can help moderate the potential negative effects of behavioral risk. If there’s one thing everyone wants during retirement, it’s financial security. When confronted with market fluctuations and losses on retirement savings, it’s natural to feel some concern or even fear. Problems may arise when short-term losses cause you to hastily switch investment strategies and lose sight of long-term goals. The Potential Advantages of Variable Annuities For those concerned about how their emotions might influence their investment strategy, a variable annuity can offer some help. A variable annuity is a contract between you and an insurance company, under which the insurer agrees to make periodic payments to you, beginning either immediately or at some future 8 | spring ‘09 | generations
date. You purchase a variable annuity contract by making either a single purchase payment or a series of purchase payments. A variable annuity offers growth opportunities through a range of investment options, typically that invest in stocks, bonds, and money market instruments. Advantages include: • Death benefit. Variable annuities offer a guaranteed death benefit. If you die before payments begin, your beneficiary is guaranteed to receive a specified amount — typically at least the purchase amount. • Tax-deferred growth. Variable annuities are tax-deferred. That means you pay no taxes on the income and investment gains from your annuity until you withdraw your money. • Guaranteed income for life. Many variable annuities offer, for an additional fee, the ability to receive periodic payments for the rest of your life (or the life of any other person you designate), which protects against outliving your assets. Annuity guarantees are based on the claims-paying ability of the issuing company, so it’s important to choose one with proven financial strength. When it comes to investing for retirement, emotions can run high. Whether you feel confident, anxious, regretful, or a mix of emotions regarding your investment strategies, now is a good time to meet with a financial professional to discuss all of your options, especially if retirement is drawing near. Investors should consider the contract and the underlying portfolios’ investment objectives, risks, charges and expenses carefully before investing. This and other important information is contained in the prospectuses, which can be obtained from your financial professional. You should read the prospectuses carefully before investing. All guarantees are backed by the claims-paying ability of the issuing company. Guarantees do not apply to
the underlying investment options. Annuity contracts contain exclusions, limitations, reductions of benefits and terms for keeping them in force. Your licensed financial professional can provide you with costs and complete details. Variable annuities are long-term investments designed for retirement purposes. Investment return and principal value of an investment will fluctuate so that an investor’s unit values, when redeemed, may be worth more or less than their original cost. Withdrawals or surrenders may be subject to surrender charges. Withdrawals and distributions of taxable amounts are subject to ordinary income tax and, if made prior to age 59 1/2, may be subject to an additional 10% federal income tax penalty. Withdrawals, for tax purposes, are deemed to be gains out first. Withdrawals can reduce the living benefit, death benefit and account value.

Every large cash expenditure should be supported by receipts. The look-back period is lengthening and it will reach fiveyears in 2011. Currently, Community Medicaid, which includes Adult Day Care as well as Home Care, does not have a lookback period. Is cosmetic surgery right for you?
A
As our population ages, more and more people are choosing to have plastic surgery for a variety of reasons.
Baby boomers faced with loss of vitality in their appearance often turn to cosmetic surgery to match the body and face to the mind. The benefits of plastic surgery, whether cosmetic or reconstructive, can ultimately change a person’s quality of life by improving their appearance, thereby adding selfesteem and confidence. Advancements in technology now produce better results with less complications and side effects. Procedures that are minimally invasive and require less downtime are also becoming more popular. Plastic surgery’s popularity has also surged because of our trying economic times. Women and men are choosing cosmetic procedures to make themselves more competitive
12 | spring ‘09 | generations
in the job market, according to the Aesthetic Society of Plastic Surgeons (ASPS). Because of this demand for instant “youth”, Doctors who are not cosmetic surgeons and other nonprofessionals are performing procedures which they may not be qualified to perform, from actual surgery to injecting Botox and facial fillers. That is why it is exceedingly important to make sure you are going to a plastic surgeon who is aware of the latest and safest techniques to ensure the best results. On the cutting edge of technology is SmartLipo MPX, the latest advance in laser assisted liposuction. This minimally invasive procedure permanently removes unwanted fat from hard to lose areas, such as the abdomen, thighs, neck, and upper arms. It also tightens loose skin and can be used in conjunction with traditional liposuction. Patients who are left with areas resistant to exercise and weight loss are the ideal candidates for SmartLipo. Smartlifting is a new surgical technique for facial rejuvenation and tightening. Downtime is also reduced, allowing patients to return to their normal activities sooner. Non-invasive laser treatments are utilized to combat the ravages of aging and sun exposure, such as brown spots, spider veins, rosacea, wrinkle reduction and unwanted hair growth. Velashape, the newest FDA
approved treatment for reduction in the appearance of cellulite, has given patients excellent results. Facial fillers and Botox injections have become increasingly popular, because of the attraction of immediate results with no downtime. Restylane, Juvederm, Perlane, Radiesse, Sculptra and Evolence, to name just a few, can be used alone or in conjunction to fill wrinkle and frown lines and add volume to skin that has lost its volume. Recovery times vary, depending upon the procedure and individual. What may take a week for one patient may take two for another. A general rule of thumb for a cosmetic surgery recovery period is about a week to 10 days. Rest and limited activity are the norms and are very important to speed healing and reduce recovery time. Your doctor will provide you with the specific post-operative instructions and also monitor your healing process in follow up visits. A consultation between you and your plastic surgeon to discuss your goals is the first step in determining what procedure is best for you and what results you can expect. Ask questions, choose surgeons who are experienced, are credentialed and have knowledge of cutting edge technologies. An on-site accredited operating facility with a board-certified anesthesiologist is essential, and offers the patient convenience and assurance of better patient care. moving money from your checking account into an investment. But perhaps the circumstances of your life have changed and this money could better be used elsewhere. Scrutinize your automatic investments to see if they still make sense for you. Check your beneficiaries. Beneficiary designations on financial documents - insurance policies, IRAs, 401(k)s, etc. - are extremely important, because they supersede even the instructions in your will. Over time, your family situation may have changed, through death, divorce, remarriage or the birth of new children, so you should periodically review all your beneficiary designations. Examine your insurance coverage. When you have a young family, you need a certain amount of life insurance coverage to provide for some major expenses - such as your mortgage, college for your kids, perhaps some retirement funds for your spouse. But when your children have grown, your mortgage is paid and your spouse has decades' worth of retirement savings, your insurance needs may change considerably. At the same time, you may find other uses for insurance. Take some time and review your insurance coverage with your financial advisor. By following these "spring cleaning" suggestions, you can help put your financial house in order for the seasons to follow. how can you keep control of your wallet and your waistline? Have you ever gone shopping hungry and come out of the supermarket in shock, having spent far more than you thought you were going to? Or found yourself staring at products in your home, thinking, why on earth did I put that in my cart? If so, then you've succumbed to supermarket psychology, the marketing tactics that are employed by stores to get people like you to buy more products than you really need. Marketing expertise Supermarkets have been employing marketing experts and psychologists to design their stores for many years. The simple plan is to entice you to buy lots of goodies; whether or not you actually need them is beside the point. Recognize the following devices? An in-store bakery: The smell of freshly baked bread is designed to make us hungry and get us spending. Produce near the front door: Fresh food looks best in natural light, hence you find these areas near the opening to the

Shop Smart Your supermarket employs clever strategies to encourage shoppers to spend more;

supermarket. Hidden staples: Milk and bread are set far apart from the entrance and each other, to encourage consumers to walk through all parts of the store. The "end cap" trick: Special displays at the end of the aisles, known as end-caps, are laden with offers; shoppers notice them more than regular displays. Eye-catching at eye level: More expensive items with higher profit margins are placed at eye level, while the shop's basics range will be on the floor — companies actually pay more to have their products at eye level, as shoppers are considered "lazy" and will see them first. David Lewis, a consumer psychologist and author of The Soul of the New Consumer: Authenticity, What We Buy and Why in the New Economy (Nicholas Brealey Publishing Ltd.) has spent 15 years analyzing how we buy. He says, "Nothing is left to chance. From the width of the aisles (planned so that you are prevented from bumping into other people, but aren't so wide that you can't get your hands on products) to the music (composers even spend their lives writing music designed to entice shoppers to buy more goods), a supermarket is a place where a consumer and his money are meant to part company. Remember that a
supermarket is a bit like a machine; its mission is to get you to spend, so to counter this, you need to keep your wits about you." The supermarkets have vast amounts of data about our shopping habits from point of sale, loyalty card databases and market research data that are given to psychologists and the retail geographers to create an optimum store layout. So can you avoid the pitfalls of supermarket psychology? Yes, if you keep your wits about you and observe the following guidelines. • Go in with a list and stick to it • Eat before you go shopping • Stick to buy one, get one free offers and fresh produce markdowns; don't get sucked into special offers if you can — they are often misleading • Check if generic brand products are as cheap and as good as branded — sometimes they aren't • Consider shopping online — you won't be tempted by in-store marketing

Bone Strength and Health
Up the Intensity to Up the Density
A
As we age, our health becomes a growing concern.
Our chronological age, our eating regimen, and our activity level all play a major role in the process. Getting older is inevitable, but aging is optional. One major concern is bone loss or osteoporosis, especially among women. The body possesses an intricate process for building and maintaining our bones. Osteoporosis occurs when that process falters and the natural breakdown and build-up ratio of the bones swings over from maintenance to a system that is unable to sustain bone density. Osteoporosis is one health issue that a person can take control of through diet and exercise. If the disease is too far advanced, then medical attention may have to be the primary approach – hence, the importance of getting a bone density scan sooner rather than later. As for osteopenia or early osteoporosis, a well-structured resistance exercise 20 | spring ‘09 | generations
program, coupled with an increase in dietary calcium, can work wonders. Studies show that if aerobic exercise is your only source of exercise, it can be detrimental to bone density. Low level activity increases cortisol levels, which breaks down the body. Higher intensity activity increases the hormone levels that build up the body. Increasing the body’s calcium uptake is somewhat complex. It takes proper exercise, plus a number of vitamins, minerals, and hormones – especially calcitonin, to generate this process. Hormones are the key factors in making things happen. Vitamins and minerals are the building blocks for the process. If the exercise that is used is stimulating enough to the bone, then the brain is notified by sensors surrounding the bone to secrete the hormones and locate the materials to start increasing bone strength. Low intensity exercise provides too little tension on the tendon attachment on the bone. High intensity exercise – resistance or
weight training – generates adequate tension on the tendon attachment on the bone to stimulate the sensors to release the necessary hormones for bone building. Increased resistance exercise and diet are an excellent prescription for reversing bone loss, but it must be a joint effort between your medical doctor and your exercise instructor. Research is continually concluding that gym therapy, not drug therapy alone, is a very valuable approach to this. bone loss disease. For further information please contact John L. Both were edentulous (no teeth) and were wearing old, ill-fitting dentures. Their dentures were loose; they had trouble eating foods they would have liked; their faces had that sunken-in look and they were in constant pain. Their previous dentists recommended implants to stabilize the dentures. The problem was that as they were both retired and on fixed incomes they could not afford implants. What to do? Yes, implants are a wonderful service and have revolutionized what we can do to improve lives. I love implants! But due to the laboratory and materials costs involved, they are expensive.
My approach is different from most other dentists. I make new, custom-fitted dentures for the patient and tell them that if the new denture doesn’t fit and function well, we can then add implants. My experience is that the denture patient does very well with the new dentures and rarely decides to go ahead with the implants. Implants are always an option but they don’t have to be the first option. I believe that patients like these can be well served with wellfitting dentures.
