Cryoablation Therapy Helps Ease Phantom Limb Pain new study suggests that cryoablation A may reduce phantom limb pain, a condition that afflicts plagues many amputees. Researchers at Emory University (Atlanta, GA, USA; www.emory.edu) and Washington University School of Medicine (WUSTL) conducted the study in 20 amputees, with each undergoing image-guided cryoablation of nerve and scar tissue in the residual limb stump. During the procedure, a probe was precisely placed through the skin, and the temperature was reduced for 25 minutes to create an ablation zone, shutting down nerve signals. The results showed that while prior to cryoablation, patients reported an average pain score of 6.4, 45 days later the score was reduced to 2.4 points. The study was presented as a poster session at the 2016 annual meeting of the Society of Interventional Radiology (SIR).

Propofol Could Benefit in Heart Surgery new study conducted at the A University of British Columbia (UBC, Vancouver, Canada; www.ubc.ca) suggests that the intravenous (IV) anesthetic propofol may offer an effective approach to protecting the heart from damage during open heart surgery, especially amongst diabetics. The study included 137 patients scheduled for cardiopulmonary bypass (CPB), who were randomly assigned to receive either a highly controlled, increasing dose of propofol for a focused period of time during CPB, or an isoflurane preconditioning regime. The results showed that patients receiving propofol experienced a greater increase in 15-F2t-isoprostane (isoP) levels compared with those receiving isoflurane, but a decreased incidence of low cardiac output syndrome (LCOS). Propofol was also associated with fewer heart failure events. The study was published on December 31, 2015, in the Canadian Journal of Anesthesia.

measurement system adapts to complex anatomies during percutaneous coronary intervention (PCI), thus improving measurement of fractional flow reserve (FFR) pressure differences across narrowed coronary arteries. Clinicians can thus identify which coronary narrowing is responsible for the ischemic obstruction and warrant stenting, resulting in improved patient outcomes and reduced health care costs. The guidewire provides enhanced durability and improved handling, in either a cabled or wireless configuration, and the flexible tip can be shaped and re-shaped throughout the procedure, enabling it to assess multiple lesions. FFR is a physiological index used to determine the hemodynamic severity of atherosclerotic narrowing of the coronary arteries.

Intraventricular Assist Device Measures Blood Flow Directly VAD, a new intraventricular axial flow left ventricular assist device (LVAD) made by ReliantHeart (Houston, TX, USA; www.reliantheart.com), is only 2.5 centimeters in diameter, but provides a 1.2 cm main channel that rests outside the ventricle. The main channel houses a magnetically stabilized impeller, while an adjustable pump depth mechanism is placed inside the heart to provide optimal action. Deep linear channels help transport blood through the pump, away from the dangerous forces of radial shear. The aVAD also features direct flow measurement and 3G cellular-powered remote monitoring, allowing clinical teams to notice problems and make corrections sooner. The entire aVAD driveline is disconnectable just below the diaphragm, so that in the event of driveline infection, it can be removed and replaced with a new one.

Seven Procedures Identified as Riskiest Emergency Surgeries at Brigham and Women’s Hospital R esearchers (Boston, AM, USA; www.brighamandwomens. org) conducted a retrospective review study to define which surgical procedures account for the majority of the U.S. national burden of operative emergency general surgery (EGS) deaths. After ranking procedure groups by contribution to EGS mortality and morbidity burden, a final set of seven operative procedures were identified, collectively accounting for 80% of procedures, 80.3% of deaths, 78.9% of complications, and 80.2% of inpatient costs. The seven procedures are partial colectomy, small-bowel resection, cholecystectomy, operative management of peptic ulcer disease, lysis of peritoneal adhesions, appendectomy, and laparotomy. The overall mortality rate was 1.23%, complication rate was 15%, and the mean cost per admission was USD 13,241. The study was published on April 27, 2016, in JAMA Surgery.

Dramatic Reduction in Hospital-Acquired Conditions new report by the US Department of Health and Human Services (HHS; Washington DC, USA; www.hhs.gov) Agency for Healthcare Research and Quality (AHRQ) shows that the incidence rate of hospital-acquired conditions (HACs) in the United States has dropped by 17% over a four-year period. A compilation of data from more than 3,000 hospitals shows the interim rate for 2014 held steady, at 121 HACs per 1,000 discharges, down from 145 in 2010. The biggest reductions were in adverse drug events (40% of the total reductions in HACs), pressure ulcers (28%), and catheter-associated urinary tract infections (CAUTIs, 16%). Nearly 87,000 fewer patients died in the hospital as a result of the reduction in HACs, with almost USD 20 billion in health care costs saved.

According to the study, in the days between ICU admission and discharge, microbiome composition changed dramatically, creating a bacterial imbalance that worsenes during the patient’s stay in the hospital. The samples from ICU patients showed lower levels of Firmicutes and Bacteroidetes bacteria, two of the largest groups of microbes in the gut, and higher abundances of Proteobacteria, which include many pathogens. The clearance apparatus is advanced into the PleuraFlow Chest tube using a magnetic shuttle. It is then advanced and retracted within the chest tube, breaking up and clearing tube obstructions and cloggings, allowing fluid flow to the drainage canister. The PleuraFlow Chest Tube is available in four standard sizes, with graduated measurements in centimeters from the distal eyelet, and a barium stripe to facilitate radiographic visualization. The results showed that peripheral IV catheter failure was lower with skin glue (17%) than standard care (27%), and IV drip dislodgement was 7% lower. Phlebitis and occlusion were also lower with skin glue, and there were no infections. Empirically, the researchers reported that they found that using medical-grade superglue made IV lines five times harder to pull out. The study, undertaken to determine absolute rates and comparative risk differences of incident HF, CHD, and stroke in CKD patients, revealed that they had a 2.3 per 1,000 person-year greater risk than those without CKD for both HF and CHD. For stroke, the risk difference was 0.8 per 1,000 per person-year in CKD, versus no CKD. Risks were higher for African American and Hispanic participants, with adjusted risk differences of 3.5 and 7.8 per 1,000 person-years, respectively. At the same time, NADH and FAD signals increase, since both molecules are related to the energy cells use; malignant cells, per se, use more energy as they rapidly multiply. And as FLIM detects the natural fluorescence spectrum and lifetime of the molecules, there is no need to use an external contrast agent.

Mammography Safer, More Accurate Breast Screening Technique Developed new Ultrasound Computed Tomography (UCT) A breast screening technique developed by a team of researchers led by the UK National Physical Laboratory (NPL; Teddington, UK; www.npl.co.uk) could provide a safer, more comfortable experience than multistage screening techniques, which involve X-ray mammography, a clinical examination, further X-rays, and ultrasound. Instead, the UCT system uses large-area pyro-electric sensors that create an ultrasound image with far fewer artifacts than existing detectors. The procedure involves passing ultrasound waves through the breast while it is immersed in a warm water bath. The ultrasound system is then rotated around the breast, resulting in a 3-D image of breast tissue that can be used differentiate between normal and cancerous tissue types.

Mammograms Contribute to Overdiagnosis of Breast Cancer esearchers at the Dartmouth Institute for Health R Policy and Clinical Practice (DTI; Lebanon, NH, USA; tdi.dartmouth.edu) used data from the SEER program to calculate size-specific cancer case fatality rate during two time periods: before onset of widespread screening mammography (1975-1979), and during a more recent period for which 10 years of follow-up data were available. The results showed that between periods small breast tumor detection rate increased from 36% to 68%, while large tumor detection rate decreased from 64% to 32%. The researchers concluded that if disease burden was stable, only 30 of 162 additional small tumors per 100,000 women diagnosed would become large, implying that the remaining 132 cases were overdiagnosed, and would never have led to clinical symptoms. The study was published on October 13, 2016, in the New England Journal of Medicine (NEJM).

3D Mammography Helps Detect Breast Cancer Earlier new study by researchers at the University of A Pennsylvania (Philadelphia, USA; www.upenn. edu) concludes that screening with digital breast tomosynthesis (DBT) mammography increases breast cancer detection rates. The retrospective, four-year study evaluated the effect of more than one DBT screening at the individual level. The results showed that the yearly rate of women recalled for additional imaging was consistently lower with 3D DBT than with standard 2D mammography. At the same time, cancer detection rates per 1,000 women increased from 4.6 with 2D mammography to 5.5 in year one of 3D DBT mammography screenings, 5.8 in year two, and 6.1 in year three. Interval cancer rates decreased from 0.7% with the use of 2D mammography to 0.5% with the use of DBT. The study was published on February 18, 2016, in JAMA Oncology.

Annual Review 2016 Radiology Studies Provide Insight on Zika Effects hree now studies conducted in Brazil used CT, T ultrasound, and MRI to assess the impacts of Zika virus. The first study, from Barão de Lucena Hospital (Recife, Brazil), examined 16 newborn babies with congenital Zika virus infection, identified a recognizable pattern of decreased brain volume, simplified gyral pattern, calcifications, ventricular dilatation, and prominent occipital bone in CT images. The second study, held at Federal Fluminense University (Niterói, Brazil; www.uff.br) found common MRI findings that included enhancement of certain spinal and facial nerves in adults and orbital injuries and anatomical changes in brain tissue in newborns. The third study, conducted at Clínica de Diagnóstico por Imagem (CDPI), found that over half of the Zika babies examined had microcephaly, brain calcifications, and loss of brain tissue volume, along with other structural changes. All studies were presented at RSNA 2016.

The researchers conducted a retrospective outcome analysis of 5,225 cardiac surgery patients over a 7-year period, in order to evaluate the consequences of postoperative blood pressure on medical outcomes and resource utilization. The results revealed that hypertensive postoperative patients experienced a higher inhospital mortality rate (4.97%) than matched-case normotensive patients (1.32%) and a longer hospital stay. Serum creatinine levels in the hypertensive patients were also increased, as was postoperative renal dysfunction. The study was published on September 9, 2016, in the Journal of Cardiothoracic and Vascular Anesthesia.

The study, involving 246 patients presenting in the emergency setting with left-sided colonic obstruction needing urgent decompression, were randomized to endoluminal stenting as a bridge to elective surgery or to surgical decompression. The results showed that while 69% of the emergency surgical decompression patients needed a colostomy bag, only 45% of the stenting as a bridge to surgery needed one. Stenting also achieved relief of obstruction in 82% of patients and reduced stoma formation. The study was presented at ASCO 2016.

Computer software is then used to filter out the fluorescent background occurring during Raman spectroscopy. The results are compared to a spectral database that contains data on the nine pure substances that make up 99% of urinary stones.
