Rainbow Hospice and Palliative Care was founded on the premise of neighbors helping neighbors. Although the national healthcare landscape is shifting dramatically and Rainbow must adjust in line with those shifts to remain a sustainable organization, we are committed to maintaining our connection to the communities we serve. In 2015 we worked to strengthen our connections as we implemented new programs and enhanced services in order to meet the needs of our communities and adapt to the changing environment.

DEAR FRIENDS, Rainbow Hospice and Palliative Care is a non-profit, community-based organization. In addition to hospice and palliative care services, Rainbow offers bereavement support and professional and community-based education. Our care teams — physicians, nurses, social workers, home health aides, chaplains and music and massage therapists — along with nearly 350 volunteers are honored to have served more communities in 2015 than ever before. Through a significant amount of growth and transition, the dedicated staff of Rainbow Hospice and Palliative Care spent 2015 honoring the connections we encounter everyday and strengthening our connections for the future. Connections to the communities we are lucky to call home. Connections to our professional colleagues and partners. 2015 saw the construction of a 14-seat Call Center in our Mount Prospect office. This new technology ensures that our patients, families and the physicians we work with are provided the fastest and most fitting response to their needs. The Call Center also serves as our home-base for our Innovation Team — a team dedicated to integrating high-tech enhancements with our existing high-touch services. The LIFE Institute spent much of the year building a more robust education program which allows Rainbow’s community outreach team to help more members of our community understand the importance of healthcare planning and end-of-life care. Together with Presence Resurrection Medical Center, our LIFE Institute faculty launched our first physician fellowship program further advancing our commitment to build the future of palliative care providers. Our connection with Presence Health has allowed us to place palliative care doctors and advanced practice nurses in six hospitals in Des Plaines, Chicago and Evanston, and to extend our reach into Joliet. This program will allow us to serve more patients and families and support them earlier on their journey with advanced complex illness. The impact of these connections is invaluable. The stories of the comfort our staff brings to those we are so privileged to serve would fill page upon page of this report. The amount of gratitude that fills us when we think of the patients and families that have touched us is immeasurable. Although we can’t put a value on it, I hope that the programs and stories we have chosen to share highlight the important work of our staff, the work that our most important connection — our communities, friends and donors — support. As always, thank you for allowing Rainbow Hospice and Palliative Care to be a part of your journey.

Love and Faith in Life and Loss LOUIS & RAMONA VERA

Ramona and Louis were married in August of 1948. He was a Purple Heart veteran of World War II and worked at the same company for over 40 years. She worked the night shift so she could take care of their four children during the day.

...but when Louis walked into her room their 60-year connection was still evident. She woke up, looked right at him and he held her hand as she took her last breath.

Their family endured perhaps greater than their fair share of tragedy. A house fire destroyed everything they had, followed by the loss of their eldest son at age 30 and then their daughter at 35. But their love and faith in each other never wavered. When Ramona was placed into Rainbow’s care she hadn’t been awake for hours, but when Louis walked into her room their 60-year connection was still evident. She woke up, looked right at him and he held her hand as she took her last breath. When Louis got sick, Rainbow was again called. By then he had stopped opening his eyes and was transferred to the Ark Inpatient Unit, where he stayed for three days. At the end, the staff kept reassuring his family that despite his almost constant sleep, he could still hear and it was important to keep talking, playing music and being present for him. On Louis’s last day, with his eyes still closed his son Ron bent down to kiss him goodbye and Louis raised his fist, knuckles out– the sign of their lifelong greeting, a fist bump. He had saved his energy for a final message. The devotion Louis and Ramona had toward each other and their family all through their lives highlights the purpose of hospice. Love and faith, especially during sickness and loss, shows who we are as people. The care that hospice offers makes beautiful moments, like holding a hand or a fist bump farewell, as much a part of the end of life as it is during the rest of life.

An Ending Fitting of the Beginning NATE & MILLIE SMITH

Nate and Millie Smith met in November of 1953. They passed away, more than 60 years later, just days apart. Rainbow Hospice and Palliative Care had the honor of caring for both Nate and Millie for their last days. Together for 61 years, Nate and Millie raised their four daughters, Marla, Rhona, Karen and Andrea, in a 3-bedroom apartment in Rogers Park. Happily, they watched their family grow to ten grandkids and four great grandchildren. Love for this wonderful couple still radiates through each member of this tight-knit family.

Hoping her parents wouldn’t have to continue living separately, Rhona asked Rainbow to evaluate Millie.

As their father approached the later stages of dementia and their mother struggled with ALS and Lymphoma, their four daughters had a difficult time finding a care setting that would allow their parents to live together.

“We told Rainbow what we wanted, and it was done,” Karen recalls. “We didn’t even have to think about it at a time where we already had too much to think about.”

Prior to being at Rainbow, Millie and Nate were in the same nursing home, but in different rooms. Karen, recalling her mother’s fear of death, tells a story of her father comforting his wife during a visit. “Don’t worry, honey,” he reassured her in a rare moment of clarity, “we’re going to go together.” Karen remembers it as another indication of Nate’s unwavering love for Millie, but also feels as though he may have “known what was coming.” The next day, Nate had a serious fall and his family decided to move him to the Rainbow Hospice and Palliative Care Ark Inpatient Unit.

A Rainbow nurse visited Millie and within twenty-four hours, she joined Nate, surrounded by their family, in the same suite at the Ark.

Although it was clearly a difficult time, the entire family was truly comfortable at the Ark. “The Ark staff cared for us just as well as they cared for mom and dad,” Rhona tells us. “We’re a big group — about 20 of us would be there at one time — and they made us all feel at home.” Nate died a peaceful death surrounded by his family and lying next to his wife. Five days later, with all of her daughters and several of her granddaughters around her, Millie passed away. Marla and her sisters were struck by the care their parents received even after they died. “Mom and Dad were treated with dignity and compassion the entire time they were in the Ark,” Marla said.

“We were able to go home and sleep at night because we knew they would be taken care of.” After Millie died, Justina, a Rainbow nurse, sang “Amazing Grace” while waiting for staff from the funeral home to arrive. Nate and Millie were each covered in a handmade quilt and a bell was rung as they were escorted out of the building, a tradition for every patient who passes away at the Ark. The connections the Smith family made at Rainbow stay with them today. They met and embraced several families facing a journey similar to their own – families that hugged each other each time a bell rang, families that will never forget each other. Rabbi Yaccov Dvorin, the Rainbow chaplain that visited Nate and Millie at the Ark, presided over both funerals. Each family member is learning how to move forward in their new reality in their own way, and relying on Rainbow’s counselors for support when they need it. Andrea and Karen sleep with their parents’ quilts every night. Granddaughter Jennifer finds writing about her grandparents therapeutic. She wrote this piece about Nate and Millie shortly after they passed away: Nate and Millie Smith shared a once-in-alifetime love story. Theirs began on a blind date in November 1953. It didn’t take them long to realize that they were meant for each other. My Grandma Millie used to tell us about how this very special story began. While they were out dancing one evening, she said to him, “So, when are we getting married?” Well, on June 6th, 1954, just six months after they met, they were indeed married. I always admired their life together.

Papa worked three jobs to make sure their four girls had what they needed. Papa and Grandma were absolutely blessed with more love than most could even imagine. They loved all of their babies; four children, ten grandchildren and four great grandchildren. Their lives revolved around us. Unfortunately, in November, we had to say goodbye to them, and eighteen hearts broke in two. They met in the month of November and left this world together in November. They never had to know what it was like to live in this world without one another for 61 years. In their last few years, Papa suffered from Alzheimer’s, but he always remembered his family. Grandma suffered from ALS, which took her voice from us. It took more than just her voice — this illness took away her ability to sing lullabies to the babies, and it took away hours of precious talks. She was our best friend. Although I cannot see her anymore, I will still talk to her, forever. Our family will carry Papa and Grandma and their memory in our hearts for the rest of our lives. This amazing family, for as long as we are able, will remember for Papa and speak for Grandma. The Smith family is doing all they can to give back — raising funds for Alzheimer’s, ALS and the Ark. “We couldn’t be more grateful to Rainbow.” Marla explains. “Not only did they provide comfort to our parents and our entire family; they allowed a couple, together for 61 years, to remain together until the end.” Nate and Millie were blessed with an enormous amount of love. It is impossible to miss the outpouring of pride in each picture of the couple with their family. There is no doubt that they continue to be proud of their growing family.

Strengthening Connections for the Future As the healthcare landscape continues to shift, Rainbow is committed to ensuring our partnership with the communities we serve — a partnership that includes hospice and palliative care, grief and loss services, development for healthcare professionals and education in end-of-life care and planning. Rainbow is leading the way in transforming care for our patients and families facing advanced complex illness with a focus on innovation, improving patient care, family satisfaction and clinical outcomes. In 2015 we began building platforms to incorporate high-tech support with our high-touch services and strengthened our connection to the communities we serve through specialty programming.

Palliative Care Partnership In partnership with Presence Heath, 2015 saw a significant expansion of Rainbow Hospice and Palliative Care’s palliative care program. This expansion allows our specialists to provide services to patients and families earlier in the disease process. Through significant medical advances, people are living longer with advanced complex illness. Patients and families now need support well before the last six months of life. Coupled with the baby boomer population significantly increasing the number of people eligible for Medicare, there is a great need for palliative care throughout our country.

Palliative care is specialized medical care for people with serious illness whatever the diagnosis or stage of illness. With a focus on the relief of symptoms, pain and stress, the goal of palliative care is to improve the quality of life for both patients and families. In 2015, Rainbow palliative care physicians and advance practice nurses joined the staff at six Presence Hospitals. In collaboration with attending physicians, these specialists work with patients on goals of care, navigating the disease process and pain and symptom management.

Connecting Future Professionals to Hospice and Palliative Care Rainbow has a long-standing commitment to educating healthcare students. Through our internship and chaplain residency programs students receive hands-on experience in hospice and palliative care. In 2015, Rainbow expanded our educational reach serving as faculty for a palliative care physician fellowship program at Presence Resurrection Medical Center. Recognizing the shortage of palliative care physicians nationwide, Presence and Rainbow launched this invaluable program in July. This program is welcoming two more fellows in 2016. Our physicians are honored to train the next generation of palliative care providers.

Grief and Loss Community Support Rainbow’s Good Mourning Program for Children, Teens and Families uniquely addresses grief caused by the death of a parent, sibling or close loved one. Through monthly meetings and a yearly camp, counselors work to meet the emotional and psychological needs of the whole family. Children participate in ageappropriate groups that utilize a child’s natural ability to learn and grow while parents and guardians attend loss-appropriate groups. In 2015, over 80% of participants in Rainbow’s Good Mourning Program for Children, Teens and Families came from communities surrounding our Mount Prospect office. In addition, our Community Counselors offered 450 hours of sessions with individuals not served by our hospice program. Since Rainbow’s inception, our Grief and Loss Program has served not only our patients and families, but the growing number of communities we are privileged to be a part of — a connection we continue to honor.

Connecting Community and Service — Volunteers

Honoring Connections and History — We Honor Veterans

Rainbow Hospice and Palliative Care has a dedicated group of over 350 volunteers. Men and women who give freely of their time to visit patients, call families who have experienced a loss and support our staff in the office and at events. This increased volunteer force now staffs the front desk at the main office. More volunteers allow us more flexibility when patients and families need additional support — at any hour. Day after day this wonderful group provides countless talents to move Rainbow closer to our mission.

An estimated 25% of the patients we serve are Veterans. Patients with unique stories, lessons and needs. In 2015 Rainbow moved to level one of the We Honor Veterans program. A National Hospice and Palliative Care Organization initiative, We Honor Veterans helps to build connections between hospice programs and veterans, allowing better access to care. Through specialized training, events and honor ceremonies, Rainbow will continue to move through the ranks in order to learn how to meet the needs of this important community.

In 2015, our volunteer outreach team strengthened our connection to community by adding 55 new volunteers to our program, with another 116 people in training.

A Step in the Right Direction Opened in September of 2015, Rainbow’s 14 seat Call Center in our Mount Prospect office provides a home base for our ability to use high-tech to enable more high-touch patient interaction. Staffed by hospice and palliative care intake coordinators, triage nurses and community liaisons, the Call Center also hosted the demonstration of Rainbow’s first Innovation team. New call flowing technology allows us to immediately connect patients and families with the staff member best prepared to answer their questions. Use of GPS technology allows us to dispatch the clinician closest to the patient’s location.

The Future of our Connections After months of planning, Rainbow’s Innovation Team began a trial of a new model of care in November of 2015. Beginning with a small group of 29 patients located a short distance from the Mount Prospect office, the Innovation Team — ­ two nurses, two home health aides, a social worker and a chaplain all in consultation with a physician — met each day in the Call Center. Every meeting begins with a discussion of each patient’s condition. Clinicians then reach out to individual patients who have agreed to be contacted daily, to assess their needs for that day. Together, the team responds to care issues as they arise and determines which discipline can best serve the family each day. This new model of daily contact not only enables timely response, but helps to anticipate issues that may occur in coming days to reduce emergencies. 41% of initial Innovation Team participants agreed to be contacted daily.

Moving Forward Combining high-tech services with community based palliative and hospice interdisciplinary teams, Rainbow is better positioned to serve the growing needs of an aging population. Improved use of technology facilitates service to patients and families and allows us to reclaim a sense of community in which neighbors help neighbors.

The Rainbow Hospice and Palliative Care Community Benefit As a non-profit, community based organization, Rainbow Hospice and Palliative Care has an unwritten contract with the communities we serve. Central to our mission is the benefit we provide to our neighbors through care, education, support and service. Although the connections our organization has with these communities are invaluable, there are several programs we are truly proud to highlight in our community benefit report. **Due to adjustments in our fiscal year in 2015, this report covers April 1, 2015 through December 31, 2015.

LIFE Institute for Learning

Integrative Therapies

Rainbow Hospice and Palliative Care’s LIFE Institute for Learning (Leading Innovators For end-of-life Education) is dedicated to serving our communities through education, connection and partnership.

Rainbow Hospice and Palliative Care’s professionals, specially trained to service hospice patients, offer music and massage therapies to reduce physical symptoms and provide emotional support to patients and families.

The LIFE Institute ensures that health care professionals have access to end-of-life and palliative care education.

Our volunteer run pet therapy services bring such comfort and calm to patients.

Grief and Loss Support Through groups, events and individual counseling, Rainbow Hospice and Palliative Care’s Grief and Loss Services provide support to all members of our community, whether or not their family was served by hospice. Our nationally recognized specialists offer a variety of programs including our Good Mourning Program, the oldest and largest family-focused children’s bereavement program in the area, Grief in the School and Grief in the Workplace. IN 2015: • 511 people participated in support groups and workshops for a total of 2000 contact hours.
