Vasectomy is a good birth control choice for men who are certain they don’t want to father a child.

asectomy is a form of male birth control that cuts the supply of sperm to your semen. It’s done by cutting and sealing the tubes that carry sperm. Vasectomy is straightforward and has a low risk of problems. Before getting a vasectomy, however, you need to be certain you don’t want to father a child in the future. Vasectomy is considered a permanent form of male birth control.

Why it’s done

Vasectomy is a good birth control choice for men who are certain they don’t want to father a child. ªª Vasectomy is nearly 100 percent effective in preventing pregnancy. ªª Vasectomy is an outpatient surgery with a low risk of complications or side effects. ªª The cost of a vasectomy is far less than the cost of female sterilization (tubal ligation) or the longterm cost of birth control medications for women. ªª You won’t need to take birth control steps before sex, such as putting on a condom. Keep in mind, however, that vasectomy only prevents pregnancy — it offers no protection from sexually transmitted diseases (STDs).

For most men, a vasectomy doesn’t cause any noticeable side effects, and serious complications are rare. ªª Side effects right after surgery can include: Swelling, Bruising of your scrotum, Bleeding or a blood clot (hematoma) inside the scrotum, Blood in your semen, Infection of the surgery site, ªª Delayed complications can include: Fluid buildup in the testicle, which can cause a dull ache that gets worse with ejaculation, Inflammation caused by leaking sperm (granuloma), Pregnancy, in the event that your vasectomy fails (rare), Chronic pain (rare) Unfounded concerns Many men worry that a vasectomy could cause serious problems — but these fears are unfounded. For example, a vasectomy won’t: ªª Affect your sexual performance. A vasectomy won’t affect your sex drive or your masculinity in any way other than preventing you from fathering a child. ªª Permanently damage your sexual organs. There’s very little risk that your testicles, penis or other

If you change your mind about having a child, it may be possible to reverse your vasectomy. However, there’s no guarantee a vasectomy reversal will work. The surgery is more complicated than the original vasectomy, can be expensive and is ineffective for many men. Before you get a vasectomy, you should be certain you don’t want to father a child in the future.

Eat Nutritious Food: The quantity and amount of food you eat supply the fuel for your body. In order to ensure adequate nutrition, practice the following: Eat as much food in the unrefined state as possible, Include at least five serving of vegetables and / or fruits each day. Whole grains are preferable to refined ones; fresh produce is preferable to canned or processed. Avoid deep fried fruits, sugar-sweetened food, and alcohol, Drink 6-8 glass of pure water every day. Rest: Most people require seven to eight hour of sleep every night. Studies have shown that people perform better with adequate sleep, yet many people are consistently sleep-deprived. Make it a point to get adequate sleep. Sleep deprivation results in hormonal imbalances which can lead to weight gain or many other problems. Avoid Negative Health Behavior: It may seem obvious, but one of the easy ways to be healthier is to not engage in unhealthy behaviors. Nicotine, excesses of alcohol and caffeine and many drug damage health. These habits weaken the immune system by inhibiting cellular immunity. Get Checked: In order to maintain disease free and healthy life, it is advisable to go through preventive health checks on an annual basis. Overdoing it could cause pain or bleeding inside the scrotum. ªª Use an ice pack periodically to help with swelling and pain for one to three days after surgery. ªª Avoid blood-thinning medications for three to seven days after surgery. These medications can increase your risk of bleeding. ªª Refrain from bathing or swimming for a few days following surgery. ªª Avoid any sexual activity for a week or so. If you do ejaculate, you may feel pain or notice blood in your semen. Don’t have sexual intercourse until your doctor says it’s okay, unless you use another form of birth control. It takes several weeks or longer before sperm are no longer present in your semen. ªª Call right away if you have signs of infection, such as pus or blood oozing from the surgery site, fever, or worsening pain or swelling.

Talking to your doctor

Deciding to get a vasectomy can be a tough decision, since you won’t be able to father a child in the future. Before doing a vasectomy, your doctor will want to meet with you to be sure it’s the right form of birth control for you. If you’re in a relationship, bring your partner to your initial doctor’s appointment. At the initial appointment (consultative visit) be prepared to discuss: ªª Whether there’s any chance at all you might want to father a child in the future ªª How your partner feels about the decision, if you’re in a relationship ªª Other methods of birth control available to you ªª What vasectomy surgery and recovery involves, and possible complications Before surgery: Your doctor will give you instructions on how to prepare. You may need to: ªª Stop taking aspirin or other blood-thinning medications several days before your surgery. These can include warfarin (Coumadin), heparin, and aspirin and other over-the-counter pain medications such as ibuprofen (Advil, Motrin, others). ªª Wash your genital area thoroughly the day of your surgery. Your doctor may ask you to shave the surgery area. ªª Take medication to relax you 30 minutes to an hour before surgery, if your doctor prescribed it. ªª Arrange for a ride home to avoid movement and pressure on the surgery area caused by driving.

What you can expect

A vasectomy is usually done at a doctor’s office or surgery center under local anesthesia, which means you’ll be awake and have medicine to numb the surgery area.

¾¾ Results ªª A vasectomy doesn’t provide immediate protection

against pregnancy. Use an alternative form of birth control until your doctor confirms there are no sperm in your semen. Before having unprotected sex, you’ll need to wait up to eight weeks or longer and ejaculate 20 times or more to clear any sperm from your semen. ªª Most doctors do post-surgery checks at six and eight weeks following surgery to be certain that no sperm are present. You’ll need to give your doctor sperm samples to examine. To produce a sperm sample, your doctor will have you masturbate into a container or ask you to have intercourse using a special condom to collect your semen. Your semen is then examined under a microscope to see whether sperm are present. ªª If you’re not in a committed relationship, use condoms during sex — even after you have a vasectomy. Vasectomy is an effective form of birth control, but it won’t protect you or your partner from sexually transmitted diseases (STDs), such as chlamydia or HIV/AIDS.

During surgery

Vasectomy surgery usually takes about 20 to 30 minutes. To perform a vasectomy, your doctor will: ªª Numb the surgery area by injecting a local anesthetic into the skin of your scrotum with a small needle. ªª Make a small cut (incision) in the upper part of your scrotum once the surgery area is numb. Or, with the “no-scalpel” technique, a small puncture is made in the scrotum instead of an incision. ªª Locate the tube that carries semen from your testicle (vas deferens) on either the right or left side. ªª Withdraw part of the vas deferens through the incision or puncture. ªª Cut the vas deferens where it has been pulled out of the scrotum. ªª Seal the vas deferens by tying it, sealing it with heat (cauterizing) or using surgical clips. Then your doctor will return the ends of the vas deferens to the scrotum.

DENGUE FEVER D

engue fever is an acute febrile disease, found in the tropics, and caused by four types (serotypes) of virus of the genus Flavivirus. It is also known as “breakbone fever” or “Dandy Fever”, slaves in the West Indies who contracted dengue were said to have the posture and gait of a dandy. Infection by these viruses may be asymptomatic or may lead to – ªª Classical Dengue fever ªª Dengue hemorrhagic fever(DHF) ªª Dengue shock syndrome (DSS). It is a mosquito-borne (arthropod-borne) disease, transmitted by Aedes mosquitoes, particularly A. aegypti and A. albopictus. Dengue may also be transmitted via infected blood products (blood transfusions, plasma, and platelets). Dengue fever is usually a self-limiting disease and DHF/DSS outbreaks are mainly seen in areas of high prevalence of Aedes mosquitoes. Dengue fever is generally a tropical disease and is seen in tropical and sub-tropical climates, mainly affecting urban and periurban areas (due to high population density). The WHO estimates that nearly 2.5-3 billion people live in areas where the virus can be transmitted, with around 50 million cases worldwide each year. Dengue fever id prevalent in India, with all 4 serotypes found. The origins of the word dengue are not clear, but one theory is that it is derived from the Swahili phrase which describes the disease as being caused by an evil spirit. Or, it may possibly have its origin in the Spanish word meaning fastidious or careful, which would describe the gait of a person suffering the bone

TRANSMISSION

The Aedes mosquito becomes infective by feeding on a patient (from the day before onset of fever to the 5th day of illness).The virus then takes 8-10 days to develop inside the mosquito, after which, the mosquito can infect a healthy individual. Furthermore, once the mosquito becomes infective, it remains so for life. This is referred to as “Man-Mosquito-Man” transmission. The mosquitoes commonly bite during dusk and dawn, and are more abundant during the rainy season, when they breed.

SIGNS AND SYMPTOMS ¾¾ Dengue fever: ªª It may affect people of any age or sex, with children often having milder symptoms.

ªª Onset is sudden, after an incubation of 3-10 days. ªª Patient may have high fever, rash, chills, intense headache, muscle and joint pains.

ªª Patient may also be weak, lose his/her appetite, and have altered taste sensation.

ªª Other symptoms include constipation, abdominal tenderness, pain behind the eyes with painful eye movements, groin pain, and sore throat, diarrhea, vomiting. ªª Fever has typical periods of remission and is often accompanied by a rash with flushing and pin-point eruptions on the face. The rash may be present all over the body. ªª Fever usually subsides after 5-7 days, and case fatality is very low.

Intravenous fluids are advised for patients with vomiting. ªª Aspirin and non-steroidal anti-inflammatory drugs are avoided as they aggravate bleeding tendencies. ªª The mainstay of management of DSS is to tackle shock due to hemoconcentration and bleeding. Close monitoring of vital signs is critical. Oral or Intravenous fluids are given to prevent dehydration. A blood transfusion or just platelet transfusion is indicated in rare cases if the platelet level drops significantly (below 20,000) or if there are significant bleeding. The presence of melena(Blood in the stools) may indicate internal gastrointestinal bleeding, requiring platelet and/or red blood cell transfusion.

¾¾ Dengue Shock Syndrome: ªª Occurs when patients with DHF lose enough blood to go into shock.

ªª It represents an emergency condition where patients ¾¾

have a fast pulse, low blood volume and blood pressure, cold skin and are restless.

CONTROL MEASURES ¾¾ Mosquito Control

DIAGNOSIS

The diagnosis of dengue is usually made clinically from the classic picture of a high fever with a (petechial) rash with low platelet and white blood cell count. It is also important to diagnose signs of DHF and signs of shock in DSS. Care has to be taken as diagnosis of DHF can mask end stage liver disease and vice versa. Dependable immediate information of the Dengue diagnostics in the rural areas can be performed by the introduction of Rapid Diagnostic Test kits which also differentiates between primary and secondary dengue infections. Serology and polymerase chain reaction (PCR) studies are available to confirm the diagnosis of dengue if clinically indicated. If a patient has persistent fever for more that 2 days and/or constant headaches then they should go for a complete blood check. If the platelet count and WBC count are below than their usual range then a Dengue Antigen test is done. To go for Dengue Test is decided depending on the result of CBC Counts.

There are two primary methods: larval control and adult mosquito control. In urban areas, Aedes mosquitoes breed on stagnant water collections in artificial containers such as plastic cups, used tires, broken bottles, flower pots, etc. Periodic draining or removal of artificial containers is the most effective way of reducing the breeding grounds for mosquitoes. Larvicide treatment is another effective way to control the vector larvae there are some very effective insect growth regulators (IGRs) available which are both safe and long-lasting (e.g. pyriproxyfen). For reducing the adult mosquito load, fogging with insecticide is somewhat effective. Prevention of mosquito bites is another way of preventing disease. This can be achieved by using insect repellent, mosquito traps or mosquito nets. In 1998, scientists from the Queensland Institute of Medical Research (QIMR) in Australia and Vietnam’s Ministry of Health introduced a scheme that encouraged children to place a water bug, the crustacean Mesocyclops, in water tanks and discarded containers where the Aedes aegypti mosquito was known to thrive. This method is viewed as being more cost-effective and more environmentally friendly than pesticides, though not as effective, and requires the continuing participation of the community.

There is no commercially available vaccine for the dengue flavivirus. However, a number of vaccine candidates are entering phase I or II testing. rostate specific antigen (PSA) is a protein produced by the cells of the prostate gland. PSA is present in small quantities in the serum of normal men, and is often elevated in the presence of prostate cancer and in other prostate disorders. A blood test to measure PSA is the most effective test currently available for the early detection of prostate cancer. Higher than normal levels of PSA are associated with both localized and metastatic prostate cancer (CaP).

Clinical significance

Prostate specific antigen (PSA) is a glycoprotein manufactured almost exclusively by the prostate gland ; PSA is produced for the ejaculate where it liquifies the semen and allows sperm to swim freely.It is also believed to be instrumental in dissolving the cervical mucous cap, allowing the entry of sperm.

PSA is normally present in the blood at very low levels; normal PSA levels are defined as between 0-4.0 ng per milliliter. Increased levels of PSA may suggest the presence of prostate cancer. However, prostate cancer can also be present in the complete absence of an elevated PSA level, in which case the test result would be a false negative . PSA levels can be also elevated due to prostate infection, irritation, benign prostatic hypertrophy (enlargement) or hyperplasia (BPH) or recent ejaculation, in which case it may give a false positive . It is a myth that digital rectal exam raises PSA. The rate of increase of PSA (the PSA velocity) may have value in prostate cancer prognosis. Men with prostate cancer whose PSA level increased by more than 2.0 ng per milliliter during the year before the diagnosis of prostate cancer have a higher risk of death from prostate cancer

despite undergoing radical prostatectomy. Most PSA in the blood is bound to serum proteins. A small amount is not protein bound and is called free PSA. In men with prostate cancer the ratio of free (unbound) PSA to total PSA is decreased. The risk of cancer increases if the free to total ratio is less than 25%. (See graph at right.) The lower the ratio the greater the probability of prostate cancer. Measuring the ratio of free to total PSA appears to be particularly promising for eliminating unnecessary biopsies in men with PSA levels between 4 and 10 ng/ mL.

blood sample, and the amount of PSA is measured in a laboratory. Because PSA is produced by the body and can be used to detect disease, it is sometimes called a biological marker or tumor marker. It is normal for men to have low levels of PSA in their blood; however, prostate cancer or benign (not cancerous) conditions can increase PSA levels. As men age, both benign prostate conditions and prostate cancer become more frequent. The most common benign prostate conditions are prostatitis (inflammation of the prostate) and benign prostatic hyperplasia (BPH (enlargement of the prostate). There is no evidence that prostatitis or BPH cause cancer, but it is possible for a man to have one or both of these conditions and to develop prostate cancer as well. PSA levels alone do not give doctors enough information to distinguish between benign prostate conditions and cancer. However, the doctor will take the result of the PSA test into account when deciding whether to check further for signs of prostate cancer. 2. Why is the PSA test performed? The U.S. Food and Drug Administration (FDA) has approved the PSA test along with a digital rectal exam (DRE) to help detect prostate cancer in men age 50 and older. During a DRE, a doctor inserts a gloved finger into the rectum and feels the prostate gland through the rectal wall to check for bumps or abnormal areas. Doctors often use the PSA test and DRE as prostate cancer screening tests; together, these tests can help doctors detect prostate cancer in men who have no symptoms of the disease. The FDA has also approved the PSA test to monitor patients with a history of prostate cancer to see if the cancer has come back (recurred). An elevated PSA level in a patient with a history of prostate cancer does not al 3. For whom might a PSA screening test be recommended? Doctors’ recommendations for screening vary. Some encourage yearly screening for men over age 50, and some advise men who are at a higher risk for prostate cancer to begin screening at age 40 or 45. Others caution against routine screening, while still others counsel men about the risks and benefits on an individual basis and encourage men to make personal decisions about screening.. Several risk factors increase a man’s chances of developing prostate cancer. Age is the most common risk factor, with nearly 70 percent of prostate cancer cases occurring in men age 65 and older. Men who have a father or brother with prostate cancer have a greater chance of developing prostate cancer. African American men have the highest rate of prostate cancer, while Asian and Native American men have the lowest rates. In addition, there is some evidence that a diet higher in fat, especially animal fat, may increase the risk of prostate cancer. 4. How are PSA test results reported?

Despite its relatively small size, the prostate accounts for a substantial and disproportionate health problems in older men, the most significant of which is cancer. Prostate cancer has become the most common cancer of males in several developed countries.

Prostate cancer screening

The U.S. Food and Drug Administration (FDA) has approved the PSA test for annual screening of prostate cancer in men of age 50 and older. PSA levels between 4 and 10 ng/mL (nanograms per milliliter) are considered to be suspicious and should be followed by rectal ultrasound imaging and, if indicated, prostate biopsy. PSA is false positive-prone (7 out of 10 men in this category will still not have prostate cancer) and false negative-prone (2.5 out of 10 men with prostate cancer have no elevation in PSA). The current guidelines of the American Cancer Society(ACS) recommend that men over age 50 should be offered the digital rectal exam and PSA tests yearly. Screening should be offered annually to AfricanAmerican men and those with a family history of prostate cancer upon reaching 45 years. Other racial and ethnic groups, such as Asian- and Hispanic-Americans have a lower risk of prostate cancer, and may not benefit from screening. Screening is likely not useful for men over age 70 or with other significant medical problems and a life expectancy of fewer than 10 years. These guidelines may be changing. A new European study has shown that a thorough screening for prostate cancer every 4 years is adequate. The screening comprises a PSA blood test, a digital rectal exam, and a transrectal ultrasound

1. What is the prostate-specific antigen (PSA) test? Prostate-specific antigen (PSA) is a protein produced by the cells of the prostate gland. The PSA test measures the level of PSA in the blood. The doctor takes a

PSA test results report the level of PSA detected in the blood. The test results are usually reported as nanograms of PSA per milliliter (ng/ml) of blood. Most doctors consider PSA values below 4 ng/ml as normal Higher a man’s PSA level, the more likely it is that cancer is present. But because various factors can cause PSA levels to fluctuate, one abnormal PSA test does not necessarily indicate a need for other diagnostic tests. When PSA levels continue to rise over time, other tests may be needed. 5. What if the test results show an elevated PSA level? A man should discuss elevated PSA test results with his doctor. There are many possible reasons for an elevated PSA level, including prostate cancer, benign prostate enlargement, inflammation, infection, age, and race. If no other symptoms suggest cancer, the doctor may recommend repeating DRE and PSA tests regularly to watch for any changes. If a man’s PSA levels have been increasing or if a suspicious lump is detected during the DRE, the doctor may recommend other tests to determine if there is cancer or another problem in the prostate. A urine test may be used to detect a urinary tract infection or blood in the urine. The doctor may recommend imaging tests, such as ultrasound or cystoscopy. Medicine or surgery may be recommended if the problem is BPH or an infection. If cancer is suspected, a biopsy is needed to determine if cancer is present in the prostate. During a biopsy, samples of prostate tissue are removed, usually with 5a needle, and viewed under a microscope. 6. What are some of the limitations of the PSA test? ªª Detection does not always mean saving lives: Even though the PSA test can detect small tumors, finding a small tumor does not necessarily reduce a man’s chance of dying from prostate cancer. PSA testing may identify very slow-growing tumors that are unlikely to threaten a man’s life. Also, PSA testing may not help a man with a fast-growing or aggressive cancer that has already spread to other parts of his body before being detected. ªª False pos0147itive tests: False positive test results occur..3041when the PSA level is elevated but no cancer is actually present. False positives may lead to additional medical procedures that have potential risks and significant financial costs and can create anxiety for the patient and his family. Most men with an elevated PSA test turn out not to have cancer; only 25 to 30 percent of men who have a biopsy due to elevated PSA levels actually have prostate cancer (3). ªª False negative tests: False negative test results occur when the PSA level is in the normal range even though prostate cancer is actually present. Most prostate cancers are slow-growing and may exist for decades before they are large enough to cause symptoms. Subsequent PSA tests may indicate a problem before the disease progresses significantly. 7. Why is the PSA test controversial? It is not clear if the benefits of PSA screening outweigh the risks of follow-up diagnostic tests and cancer treatments. For example, the PSA test may detect small cancers that would never become life threatening. This situation, called overdiagnosis, puts men at risk for complications from unnecessary treatment such as surgery or radiation. The procedure used to diagnose prostate cancer (prostate biopsy) may cause side effects, including bleeding and infection. Prostate cancer treatment may cause incontinence and erectile dysfunction. For these reasons, it is important that the benefits and risks of diagnostic procedures and treatment be taken into account when considering whether to undertake prostate cancer screening.

Oral lesions are among the first manifestations of HIV infection. HIV/AIDS is important because of its potential diagnostic, prognostic, therapeutic and transmission implications. In this case report we describe how a patient was diagnosed to be in a state of immunodeficiency based on the oral lesions and highlight the importance of detection of HIV/AIDS during general dental practice and how best the oral manifestations of HIVAIDS can be treated. Role of dentist in the diagnosis and management of HIV- AIDS

IDS (Acquired immunodeficiency syndrome) is a deadliest disease, difficult to treat and easiest to prevent. In the present scenario, the AIDS pandemic, though global, is overwhelmingly concentrated in sub-Saharan Africa. The centre of the global AIDS pandemic is set to shift from Africa to Eurasia; it comprises of the three largest countries like China, India and Russia and will likely be home to the largest number of HIV (human immunodeficiency virus) victims in the decades ahead. In this situation a dentist plays an imperative role in the following aspects: 1. Provision of routine dental care. 2. Oral lesions- screening, diagnosis, treatment, of oral lesions and recognition of their significance in prognosis of disease 3. Collaboration with other health care workers and social support systems. 4. Education of other health care workers. 5. Education in the community. This article describes a case reportof a HIV-AIDS patient diagnosed based on oral manifestations and highlights the role of dentist in the detection and management of oral conditions associated with HIV- AIDS patient.

Case report

A female patient of 36 year old came to the out-patient department of Oral Medicine and Radiology with the complaint of burning sensation of mouth and gingival bleeding since three months. The burning sensation increased on having hot spicy food and beverages. She had generalised body weakness since 6 months. Patient had undergone treatment for tuberculosis one year back and since one month she was under medication for typhoid.Patient gave history of vaginitis, urethritis and urinary tract infection since six months and exertionaldyspnea since two months. The patient was weak, pale with sunken eyes [Figure 1]. Submandibular and jugulo-diagastric lymph nodes were palpable, firm and non-tender.Lips were scaly with fissuring at the oral commissurals suggesting angular cheilitis. Tongue wasdepapillatedwith macules of melanin pigmentation seen on dorsal surface of tongue [Figure 2]. A linear white plaque was seen on the lateral border of tongue bilaterally, slightly elevated from the adjacent mucosa measuring about 4cms. The lesion was non scrapable, non-tender and smooth in texture compatible with the diagnosis of oral hairy leukoplakia [Figure 3].Discrete white patches were seen on the right and left buccal Hairy leukoplakia presenting as linear white patch on mucosa interspersed the right (A) and left (B) lateral borders of the tongue.

with erythematous areas at the occlusal level measuring approximately 3*1cm. The lesion on buccal mucosa was tender and scrapable revealing an erythematous base suggesting pseudomembranous candidiasis. With the presence of all the above oral findings in correlation with the history and general health of the patient, a state of immunodeficiency was suspected. Complete haemogram was performed which did not show much alteration in the values except for a mild decrease in the packed cell volume (35.8%) and an elevated erythrocyte sedimentation rate (70mm/hr) by Westerngreen method. Tridot (Rapid) test showed positive HIV 1 which was confirmed by RIBA test. The test was positive for HIV Gp120, Gp41, p31 and P24 and negative for HIV 2 envelope peptide confirming HIV 1 infection. The CD4 count was 203.33/mcl. The patient was started with symptomatic treatment for the oral lesions. Clotrimazole troches 10mg was prescribed to dissolve one troche in mouth fivetimes a day for 14 days. Topical triamcinoloneacetonide0.1% oral paste application and multivitamin capsules were prescribed. The patient was referred to non-government organisation centre for antiretroviral therapy.

Salivary gland enlargement (315±156), dental caries (323±54), angular cheilitis (460±310), and erythematous candidiasis were accompanied by the least CD4 count.[5] Howell et al investigated the prevalence of oral soft tissue lesions in children infected with HIV and the relationship of CD4lymphocyte levels with the prevalenceof those lesions. He found that a declining CD4 lymphocyte count (1357 to 35) was associated with an increasing severity of gingival disease. The majority of the patients with candidiasis showed CD4 counts less than 200/mm2. [6]In the present case the patient had a CD4 count of 203/mcl with pseudomembranous candidiasis, angular cheilitis melanin pigmentation and stomatitis. In a study performed by LalitShrimali to clinically evaluate and document the oral lesions in patients with HIV infections showed that oral candidasis was seen in 72% patients, lymphadenopathy in 42% and angular cheilitis in 36%. Xerostomia in 32%, recurrent aphthous ulceration in 22%, oral hairy leukoplakia in 2% were noted.[7] When it comes to dentists, it is always essential that, the patient diagnosed to be HIV positive, receives appropriate treatment for his/her oral condition to alleviate suffering. Later the patient should be referred to the concerned speciality to start with the antiretroviral therapy. Oro-pharyngeal candidiasis is the common fungal infection observed as the initial manifestation of symptomatic HIV infection[8,9, 10, 11]. Oral candidiasis can be treated either topically or systemically based on the clinical type, distribution, and severity of infection. Topical treatment is effective for small and accessible lesions with clotrima zoletroches, nystatin pastilles and nystatin oral suspension are effective for mild to moderate lesions. Topical amphotericin B can be used in the treatment for resistant candidiasis. Discussion

The prevalence of AIDS in Indian population when compared to worldwide data marks the importance of diagnosing the disease encountered in day to day practice. 30 to 80 % of the patients with HIV-AIDS suffer from oral lesions.[1]There is no particular oral lesion associated only with AIDS but there are certain manifestations which are associated very frequently and are considered as AIDS defining lesions. Hence it is possible to diagnose HIV by correlating oral manifestations with laboratory investigations. The AIDS defining lesions have been included in the clinical classification of HIV by CDC in category B. [2] Current classification of many oral lesions of HIV disease is based on their strength of association with HIV infection (Table 1).[3]The overall prevalence of oral manifestations in HIV-AIDS is changed since the advent of HAART. Hence a new classification of oral lesions associated with HIV was putforth (Table 2).[4] The oral manifestations of HIV- AIDS apart from helping the diagnosis of infection in an undiagnosed person, also act as a prognostic indicator.Reduction of circulating CD4 count is the main criteria for assessing the immunosuppression status in HIV-positive patients. The number of circulating CD4 cells ranges from 600 to 1600 cells/mm³, but the initial signs of immune suppression occur when CD4 count is lower than 500 cells/mm3. [5] In the study performed by Davoodi et al to determine the prevalence of oral soft tissue manifestations and their relationship with the degree of immunosuppression observed in HIV-positive patients, showed that immunosuppression was more severe in patients with pseudomembranous candidiasis

involves the use of ketaconazole and triazole medications. Fluconazole is given in the dose of 100-200mg/ day for 7 – 10 days. Viral infections in HIV infection are more severe and show an aggressive course. Hence an aggressive treatment should be used comprising of Pale face with sunken eyes and both symptomatic scaly lips and specific treatment. Oral hairy leukoplakia, herpetic gingivostomatitis, herpes zoster, HPV infections CMV infections are common. Oral hairy leukoplakiaseen on the lateral borders of the tongue, as vertical white striations, corrugation, or as flat plaques bilaterallyis asymptomatic. OHL has been associated with EBV infection. This condition usually does not require treatment but use of oral acyclovir, topical podophyllium resin, retinoids and surgical removal are successful treatments.

necrotising ulcerative periodontitis, and less commonly bacillary epitheloidangiomatosis and syphilis. Linear gingival erythema is usually asymptomatic and requires no treatment. However oral prophylaxis with chlorhexidinegluconate reduces the erythema and may require prophylactic use to avoid recurrence. In patients with necrotising ulcerative gingivitis and necrotising ulcerative periodontitis oral prophylaxis with debridement of necrotic tissue is indicated. Use of chlorhexidinegluconate 0.12% mouth rinse twice daily with metronidazole 250mg orally qid for 10 days is helpful in healing of the tissues. Neoplasms like Kaposi’s sarcoma and NHL are common in AIDS. When only few lesions exist and are small(<1cm)intralesional chemotherapy with vinblastine or sclerotherapy with 3% sodium tetradecyl sulphate is usually effective. Radiation therapy 9800 -2000 cGy) is required for larger or multiple lesions. Non-Hodgkin’s lymphomais seen in stages with CD4 counts less than 100/mm3. It appears as a rapidly enlarging mass, common in the palate and gingiva. Patients should be referred to speciality clinics for radiotherapy and chemotherapy treatment. However, prognosis is poor; with mean survival time is of less than one year. With the advent of antiretroviral drugs and their continuous usage in HIV-AIDS patients, new spectrums of oral lesions have supplemented the list of oral manifestations associated with AIDS. Xerostomia is seen in patients onnucleoside transcriptase inhibitors, protease inhibitors as well as antihistamines, anticholinergics, antihypertensives, decongestants, narcotic analgesics, and tricyclic antidepressants.[14] Brittle tooth syndrome is one of the complications of xerostomia. Treatment for xerostomia focuses on symptomatic relief by encouraging patients to hydrate themselves frequently. Patients are also recommended to use commercially available artificial saliva substitutes like Sodium Carboxymethylcellulose 0.5% aqueous solution to achieve relief. The use of pilocarpine and bethanechol to stimulate salivary flow can also be useful.Erythema multiforme is a known side effect of non-nucleoside reverse transcriptase inhibitors.[15]The concerned physician has to be informed to replace the drug and oral lesions have to be treated.

Herpes simplex and varicella zoster infections present with oral manifestations are diffuse mucosal ulcerations lasting for several weeks, accompanied by fever, malaise, and cervival lymphadenopathy. In HIV infection, herpes zoster frequently presents with early cranial nerve involvement, sometimes with multi-dermatomalor bilateral involvement and carry poor prognosis. The following medications can be used: Acyclovir 200mg, 1 capsule qid for 2 weeks, famcyclovir 250mg tid for 2 weeks, penciclovir 1% cream, 1% vidarabine ointment to be applied to affected areas qid. In post herpetic neuralgias cases carbamazepine 200mg bid to start; 800-1200mg qid should be used till symptoms subside. In case of focal epithelial and connective tissue hyperplasia caused by human papilloma virus , surgical removal, with or without intra-operative irrigation with podophyllium resin, is the treatment of choice.[13] In verruca vulagaris or condylomaaccuminata surgery or CO2 laser surgery is used. The most common oral lesions associated with bacterial infection are linear erythematous gingivitis,

Selection and storage ªª Most varieties of garlic have same characteristic pungent odor and bite. Pinked – skinned garlic tastes a little sweeter and keeps longer than white garlic. Elephant garlic, a large – clove variety, is milder in flavor than regular garlic and should be used like a leek. But most varieties can be used interchangeably in recipes. ªª Choose loose garlic if you can find it. It’s easier to check the quality of what you’re getting than with those hiding behind cellophane. Its appearance can clue you in to its freshness, white – paper skins are best bet. Then pick up the garlic; choose a head that is firm to the touch with no visible damp or brown spots. ªª Don’t expect the flavor of garlic powder to mimic fresh garlic. Much of the flavor is processed out. Garlic powder, however, may retain some active components. Garlic salt, of course, contains large amounts of sodium -- as much as 900 milligrams per teaspoon, so avoid using it. ªª Store garlic in a cool, dark, dry spot. If you don’t use it regularly, check it occasionally to be sure its usable. Garlic may last only few weeks or a few months. If one or two cloves have gone bad, remove them, but don’t nick remaining cloves; any skin punctures will hasten the demise of what’s left. If garlic begins to sprout, it’s still okay to use, but it may have a milder flavor, just remove the tough, green sprout.

Pudina helps digestion and reduces cholesterol. Pudina tea is good for cough and cold. Decoction reduces fever, Juice of pudina with ginger juice removes dyspepsia. The paste of pudina with turmeric is good remedy for pimples. Pudina juice with lemon juice and honey relieves abdominal pain and distention. If the leaves are chewed after food, it prevents caries tooth and strengthens gums. If pudina is taken in empty stomach it acts as anthelminthic. Gargling the pudina decoction improves the voice and removes throat irritation. Pudina decoction reduces abdomenal pain in women if consumed for a week before the menstrual cycle starts. It reduces the intensity. Pudina juice can be used for the healing of wounds. Caution - pregnant women should avoid the consumption of pudina in excess.

¾¾ OTHERS ªª Peppermint derived from pudina is used in the preparation of medicines and chemicals. Peppermint is also used in preparation of painkiller oils. It is used in lozenges (chewable tablets) m toothpastes. Experiments have shown that peppermint oil is helpful in widening the constricted blood vessels of heart and brain. ªª Since ancient times pudina is used in Russia. Pudina decoction was used in relieving stomach ache and cold. Garden mint oil was used as painkiller. Garden mint leaves are used in flavouring tea It was believed that the flavour of garden mint refreshes the mint and also relieves depression. ªª Garden mint was used to clean tables and chairs.

Mint Aloo Dum:

Mint leaves 1 cup Potato 1A kg Onion 1 Cumin seeds 1 spoon Green chillies 4 Lemon juice ½ spoon Boil the potato. Peel off the outer skin. Fry the boiled potato in oil till it becomes brown. Grind cut pieces of onion, green chillies and mint. Mix this paste with mustard seasoning. Add the fried potato to this paste. Heat this under low flame for few minutes.

Mint & Vegetable Curry:

Capsicum 2 Cauliflower ½ Carrot 2 Mint 1 bunch Tomato 2 Coriander ½ bunch Onion 2 Red chilli powder 1 spoon Garam masala 1 spoon Cumin seeds 1 spoon Salt to taste Splatter mustard in oil. Add all the cut vegetables. Cook till they get softened. Sprinkle red chilli powder, garam masala, cumin seeds and salt. Add cut leaves of mint and coriander. Mint Sharabat : Grind the cut mint leaves into a fine paste by adding water. Add jaggery or sugar. Ginger, cardamom and lemon juice can be added. Add sufficient amount of water. This sharabath can be taken in empty stomach early in the morning to get relief from indigestion, chest pain due to acidity.

Mint 2 bunches Green chillies 2 Grated coconut Salt to taste Wheat flour 4 cups Oil Cut the mint leaves. Grind green chillies and coconut. Make a dough adding these to wheat flour and salt. Make the dough in to small balls. Roll the balls using rollpin. Prepare the parota on hot tawa. Instead of mixing with wheat flour the ingredients can be used as stuff in the regular chapathi dough.

Bengalgram dhal 100 gms Blackgram dhal 100 gms Red chillies 75 gms Grated copra 1 cup Jaggery 50 gms Tamarind Salt to taste Oil 2 spoons Roast the grams till golden brown. Make a coarse powder. Roast cut mint leaves, red chillies, grated copra and tamarind with little oil. Mix all the ingredients with jaggery and salt. Make a thorough mix. Can be eaten along with hot rice and ghee or with dosa, idli, chapathi or roti etc.

Add cut chillies and mint leaves, salt and the washed rice. Roast for a few minutes. Add 4 cups of water. Cook the contents in a cooker. Cut the bread into small pieces. Fry them on low flame in ghee. Add to the cooked palav. ªª Mint Shankarapoli: ªª Maida 1 cup ªª Mint leaves ½ cup ªª Green chillies 4 ªª Ghee 4 spoons ªª Salt to taste ªª Oil for frying Grind mint and chillies. Add the paste to maida. Add salt, ghee and prepare the dough. Take small balls of dough. Roll them flat using roll pin. Cut into any required shape. Deep fry in oil. Rice Mint Salt Chilli powder Cumin seeds Asafoetida Soak the rice in water for one or two days. Dry under shade. Found the rice into fine powder. Grind mint, asafoetida, salt and chillies. Add the ground material to rice flour. Cook this mixture by putting slowly to hot water and stirring till it forms a paste. Make them into small balls. Roll them using roll pin. Dry under sunlight. The papad can be fried when needed.

An inmate of an old age home, he says betrayal prevails everywhere and so it does with our children. “But I never betrayed my parents, he quickly adds. There are ways where aged or elderly people can live happily by actively involving in numerous activities. Elderly people can share their long-cherished career experience with young people in several ways. This will also help aged people to remain active and healthy. Majority of older women feel daughters-in-law are the reason behind separation. But 80-year old Kavitha refutes: “It is not always true. Have regular health checkups even if you feel healthy. ªªEnsure that you have a Family Doctor and avoid change of Doctors frequently. Avoid self medication and over the counter drugs. ªªMaintain a file of all your medical records which can be accessed by your family members whenever required. Carry with you a card containing your name, address, phone numbers of contact persons and your Doctor along with particulars of your drug regime Do not stop your medicines all of a sudden without doctor’s advice. Cultivate good oral care to prevent infection. Do not be averse to artificial dentures. It improves food intake, digestion, clarity of speech and facial features. Keep yourself physically, mentally and socially active. Read and write in sufficient light There is no specific dietary rule for elders. Eat healthy food and avoid over eating. Consume a diet high in fibre, low in animal fat and salt. Have a good and nutritious breakfast, moderate lunch and light dinner. Include lots of fruits and vegetables in your diet. Avoid fatty and spicy foods. Ensure that you drink 6 to 8 glasses of water a day. Ensure that you take a multivitamin tablet daily for better supply of Folic Acid, Vitamin B12, D and E. Do not indulge in vigorous exercises. For elders light exercise like walking is the best. Do not be reluctant to rest and relax. ªª As far as possible avoid emotions such as depression, worry, fear, sorrow, envy, hate, resentment as they can adversely affect your health and cause visible physical disabilities. SPINAL CORD INJURIES The spinal cord runs from the base of the brain down the middle of the back and ends just above the waist. It is made up of nerve cells and long nerve fibers that relay all information between brain and rest of the body. Thirty-three ring-shaped bones called vertebrae form the spinal column (spine) and protect the spinal cord. If these bones are broken or damaged, spinal cord is injured & result in varying degree of paralysis. But broken vertebrae doesn’t necessarily mean spinal cord injury. If such a patient is shifted properly without exaggerating bone injury, spinal cord is protected& hence, life long paralysis is avoided for which patient will remain ever grateful.

HORMONE REPLACEMENT THERAPY (HRT)

A boon for women

Hormone replacement therapy (HRT) is a medical treatment for women who have reached or are nearing menopause or if their uterus and ovaries have been removed due to some disease

HRT prevents and reduces discomfort caused by decrease in level of female hormones oestrogen and progesterone levels. The main types of hormones involved are oestrogens, progesterone or progestins. It is often given as a short-term relief (usually less than five years) from menopausal symptoms (hot flushes, irregular menstruation, fat redistribution etc.}. Younger women with premature ovarian failure or surgical menopause may use hormone replacement therapy for many years, until the age that natural menopause would occur.

Who should not take HRT?

Those women with undiagnosed vaginal bleeding, liver disease, pregnancy, heart disease, clots in blood vessels, and cancer of the uterus should not take HRT. Those women with history of migraine headaches, breast cancer, ovarian cancer and gallbladder disease should be careful about taking this treatment. It is important to weigh the risks of taking hormone therapy against the benefits that the woman might have from taking these hormones.

Benefits of hormone therapy

The main symptoms of menopause that are benefitted with hormone replacement are hot flushes, night sweats, sleep difficulties, vaginal dryness, and anxiety. Hormone therapy may also prevent the development of osteoporosis or weakening of bones.

¾¾ Risks of hormone therapy ªª Blood Clots: It has been long known that taking

oestrogen increases a person’s risk for blood clots especially if the person is a smoker. ªª Breast Cancer: Woman who takes oestrogen therapy for a long period of time has a very small risk for breast cancer this has been highlighted so much that people are scared of taking this therapy. Actually the benefit of therapy far exceeds the slight increased risk of breast cancer. ªª Cancer of the Uterus: The risk for cancer of uterus is more than five times higher in women who take only oestrogen. However, taking progesterone with oestrogen seems to protect against this cancer. ªª Heart Disease: Oestrogen may increase the risk of

heart disease in older women or in those who began oestrogen use, more than 10 years after menopause. Side effects of hormone therapy

Some women taking hormone therapy may have headache, stomach upset, bloating, and diarrhea, loss of appetite and gain of weight. Other side effects are acne, swelling of hands, feet or lower limbs due to accumulation of fluid. Some people may also develop breast discomfort. Changing the dose or form of hormone therapy may help reduce these side effects. Close follow-up with one’s doctor is important when there is unusual bleeding.

Forms of hormone therapy

Hormone therapy is available in various forms. Oestrogen comes in different forms such as nasal spray, pills or tablets (most common}, skin gel, and skin patches, which are applied to the thigh or belly area. Vaginal creams or tablets are available which help with dryness and pain during an intercourse. Non-hormonal medications can be used instead of hormone therapy. These treatments include Phytoestrogens or plant hormones like soy proteins and isoflavones. A woman can take other steps to adjust to the changes in life during menopause. Eating healthy foods and getting regular exercise will improve quality of life.

Follow- up after HRT

Follow-up includes checking blood pressure and breast examination every 6 months. If the woman has irregular uterine bleeding, then she should consult the doctor immediately.

Stopping HRT

Menopausal symptoms (hot flushes and sweats) last on average between 2-5 years. A trial of withdrawal of HRT should be considered in:— ªª Those women symptom-free on HRT after 1-2 years. ªª Women who have been on HRT for longer than 5 years. ªª Women on HRT for premature menopause after the age of 50.
