Appendicitis Appendicitis is a condition in which your appendix becomes inflamed and filled with pus. Your appendix is a finger-shaped pouch that projects out from your colon on the lower right side of your abdomen. This small structure has no known essential purpose, but that doesn't mean it can't cause problems The main symptom of appendicitis is pain that typically begins around your navel and then shifts to your lower right abdomen. The pain of appendicitis usually increases over a period of six to 12 hours, and eventually may become very severe. Anyone can develop appendicitis, but it most often strikes people between the ages of 10 and 30. The standard treatment for appendicitis is surgical removal of the appendix. Symptoms Appendicitis can cause a variety of symptoms that

Dr. Chalapathy Professor of General Surgery Vydehi Hospital Whitefield, Bangalore – 66 Ph: 080-28413381/2/3/4 www.vims.ac.in may change over time: zz Early on, the most common symptom is an aching pain around your navel that often shifts later to your lower right abdomen. zz As the inflammation spreads to nearby tissues, the pain may become sharper and more severe. zz Eventually, the pain tends to settle in your lower right abdomen — near your appendix at what's known as McBurney point. This point is about halfway between your navel and the top of your right pelvic bone. But the location of your pain may vary, depending on your age and the position of your appendix. Young children or pregnant women, especially, may have appendicitis pain in different places. What makes pain worse If you apply gentle pressure to the area that hurts, it will feel tender. As you release the pressure, especially if you do it suddenly, appendicitis pain often will feel worse (rebound tenderness). It will also tend to get worse if you cough, walk or make other jarring movements. This is particularly true if the inflamed appendix is touching the peritoneum — the silk-like membrane that lines the inner abdominal wall and enfolds the intestines. The pain may lessen somewhat if you lie on your side and pull your knees up toward your chest. Symptoms other than pain In addition to pain, you may have one or more of the following appendicitis symptoms: $$ Nausea and sometimes vomiting $$ Loss of appetite $$ A low-grade fever that starts after other symptoms appear $$ Constipation $$ An inability to pass gas $$ Diarrhea $$ Abdominal swelling Causes The cause of appendicitis is not always clear. Sometimes it's the result of: $$ An obstruction. Food waste or a hard piece of stool (fecal stone) can become trapped in an orifice of the cavity that runs the length of your appendix. $$ An infection. Appendicitis may also follow an infection, such as a gastrointestinal viral infection, or it may result from other types of inflammation. In both cases, bacteria may subsequently invade rapidly, causing the appendix to become inflamed and filled with pus. If not treated promptly, your appendix is likely to rupture. Children are more likely to have a ruptured appendix than adults are. Children don't always have typical symptoms of appendicitis, and parents may delay getting treatment. For that reason, it's best not to take abdominal pain lightly. Even if you suspect a "stomachache" isn't serious, call your doctor just to make sure. Older adults also have a higher incidence of ruptured appendix, possibly because of delay in seeing a doctor for abdominal pain. Tests and diagnosis The pain from appendicitis may change over time, so establishing a diagnosis can sometimes be difficult. In addition, abdominal pain can arise from a number of health problems other than appendicitis. Other conditions with abdominal pain that may resemble that of appendicitis include: $$ Ectopic pregnancy. An ectopic pregnancy — one that occurs outside the lining of the uterus — can cause similar pain. $$ Certain ovarian cysts. A right-sided ovarian cyst may produce pain in the same general area as appendicitis. $$ Kidney stone. Occasionally, a stone from the right kidney will pass into the ureter, which runs from the kidney to the bladder, and get stuck there. This causes considerable pain that may mimic appendicitis, especially in adults. $$ Crohn's disease. This condition, which causes chronic inflammation of the digestive tract, also can mimic appendicitis. Making a diagnosis To help diagnose appendicitis, your doctor will likely take a history of your signs and symptoms and perform a thorough examination of your abdomen. When gentle pressure on the painful area is suddenly released, appendicitis pain will often feel worse if the adjacent peritoneum is inflamed. Other signs your doctor may watch for include abdominal rigidity and a tendency to stiffen your abdominal muscles in response to pressure over the inflamed appendix (guarding). In addition, your doctor may recommend the following procedures: zz Blood test. This allows your doctor to check for a high white blood cell count, which may indicate an infection. zz Urine test. Your doctor may want you to have a urinalysis to make sure that a urinary tract infection or a kidney stone isn't causing your pain. If it is a kidney stone, red blood cells are usually seen during microscopic examination of the urine. zz Imaging tests. Your doctor may also recommend an abdominal X-ray or ultrasound scan to help confirm appendicitis or find other causes for your pain. An ultrasound scan uses high-frequency sound waves and computer technology to provide images of your internal organs. More commonly, a computerized tomography (CT) scan is used for confirming a diagnosis. A CT scan is an imaging test that uses a series of computergenerated X-rays to provide a more comprehensive view of your internal organs than conventional X-rays do. It can also help reveal other potential diagnoses if your appendix is not the source of your pain. Because

a CT scan produces radiation, women of childbearing age need to have a pregnancy test before proceeding with the scan Complications The most serious complication of appendicitis is if your appendix ruptures (perforates) and the contents of your intestines and infectious organisms invade the abdominal cavity. This can cause an infection of the lining of the cavity (peritonitis). When your appendix ruptures, you may suddenly feel better. But soon afterward, your entire abdomen may become distended with gas and fluid and will likely feel tight, hard and tender to the touch. You'll also have pain throughout your abdomen, but may not have the severe, localized pain of appendicitis. In addition, you may not be able to pass gas or have a bowel movement because of the inflammation. Other signs and symptoms may include a fever, thirst and a low urine output. Peritonitis is a medical emergency. If you or a family member develops signs of this abdominal infection, go to an emergency room immediately. Even with prompt treatment, peritonitis can be extremely serious. Sometimes, infection and the seepage of intestinal contents may form an abscess, a walled-off area of infection (appendiceal abscess). The abscess may be as small as a walnut or as large as a grapefruit. Regardless of its size, it requires treatment before the abscess itself perforates, causing peritonitis. Treatments and drugs Appendicitis treatment requires surgical removal of your appendix (appendectomy) if you have acute appendicitis. Your surgeon may perform traditional open surgery, using a single abdominal incision, or laparoscopic surgery, which requires only a few small abdominal incisions. In a laparoscopic procedure, your surgeon inserts a laparoscope — a pencil-thin tube with its own lighting system and miniature video camera — into your abdomen through a hollow instrument (cannula). Only a small incision is needed. The video camera then produces a magnified view of the inside of your abdomen on an outside video monitor. This allows your surgeon to see the surgery in detail. To remove your appendix, your surgeon uses tiny instruments inserted through one or two other small abdominal incisions. In general, laparoscopic surgery allows you to recover faster and heal with less scarring. But if your appendix has ruptured and infection has spread beyond the appendix, or if an abscess is present, you'll need a larger incision so that your surgeon can clean the abdominal cavity. You'll receive intravenous antibiotics and will need to stay in the hospital during your recovery. Dealing with complications If your symptoms have been present for five days or more, immediate surgery may present complications. In this case, your doctor may recommend a course of antibiotics first to try to shrink the inflammation and infection that may be surrounding structures near the appendix. If an abscess is present, it may be drained through the skin. If this approach is successful, appendectomy is usually performed a couple of months later. www.bangalorehealthtv.com

Seeing blood in your urine can cause more than a little anxiety. Yet blood in urine — known medically as hematuria — isn't always a matter for concern. Strenuous exercise can cause blood in urine, for instance. So can a number of common drugs, including aspirin. But urinary bleeding can also indicate a serious disorder

There are two types of blood in urine. Blood that you can see is called gross hematuria. Urinary blood that's visible only under a microscope is known as microscopic hematuria and is found when your doctor tests your urine. Either way, it's important to determine the reason for the bleeding. Treatment depends on the underlying cause. Blood in urine caused by exercise usually goes away on its own within one or two days, but other problems often require medical care. zz Symptoms: The visible sign of hematuria is pink, red or cola-colored urine — the result of the presence of red blood cells. It takes very little blood to produce red urine, and the bleeding usually isn't painful. If you're also passing blood clots in your urine, that can be painful. A lot of times, though, bloody urine occurs without other signs or symptoms. In many cases, you can have blood in your urine that's visible only under a microscope (microscopic hematuria). When to see a doctor: Although many cases of hematuria aren't serious, it's important to see your doctor anytime you notice blood in your urine. Keep in mind that some medications and certain foods, including beets, rhubarb and berries, can cause your

urine to turn red. A change in urine color caused by drugs, food or exercise usually goes away within a few days. However, you can't automatically attribute red or bloody urine to medications or exercise, so it's best to see your doctor anytime you see blood in your urine.

Causes

In hematuria, your kidneys — or other parts of your urinary tract — allow blood cells to leak into urine. A number of problems can cause this leakage, including: zz Urinary tract infections.Urinary tract infections may occur when bacteria enter your body through the urethra and begin to multiply in your bladder. Symptoms can include a persistent urge to urinate, pain and burning with urination, and extremely strong-smelling urine. For some people, especially older adults, the only sign of illness may be microscopic blood. zz Kidney infections. Kidney infections (pyelonephritis) can occur when bacteria enter your kidneys from your bloodstream or move up

from your ureters to your kidney(s). Signs and symptoms are often similar to bladder infections, though kidney infections are more likely to cause fever and flank pain. A bladder or kidney stone. The minerals in concentrated urine sometimes precipitate out, forming crystals on the walls of your kidneys or bladder. Over time, the crystals can become small, hard stones. The stones are generally painless, and you probably won't know you have them unless they cause a blockage or are being passed. Then, there's usually no mistaking the symptoms — kidney stones, especially, can cause excruciating pain. Bladder or kidney stones can also cause both gross and microscopic bleeding. Enlarged prostate. The prostate gland — located just below the bladder and surrounding the top part of the urethra — often begins growing as men approach middle age. When the gland enlarges, it compresses the urethra, partially blocking urine flow. Signs and symptoms of an enlarged prostate (benign prostatic hyperplasia, or BPH) include difficulty urinating, an urgent or persistent need to urinate, and either visible or microscopic blood in the urine. Infection of the prostate (prostatitis) can cause the same signs and symptoms. Kidney disease. Microscopic urinary bleeding is a common symptom of glomerulonephritis, which causes inflammation of the kidneys' filtering system. Glomerulonephritis may be part of a systemic disease, such as diabetes, or it can occur on its own. It can be triggered by viral or strep infections, blood vessel diseases (vasculitis), and immune problems such as immunoglobulin A nephropathy, which affects the small capillaries that filter blood in the kidneys (glomeruli). Cancer. Visible urinary bleeding may be a sign of advanced kidney, bladder or prostate cancer. Unfortunately, you may not have signs or symptoms in the early stages, when these cancers are more treatable. Inherited disorders. Sickle cell anemia — a

hereditary defect of hemoglobin in red blood cells — can be the cause of blood in urine, both visible and microscopic hematuria. So can Alport syndrome, which affects the filtering membranes in the glomeruli of the kidneys. zz Kidney injury. A blow or other injury to your kidneys from an accident or contact sports can cause blood in your urine that you can see. zz Medications. Common drugs that can cause visible urinary blood include aspirin, penicillin, the blood thinner heparin and the anti-cancer drug cyclophosphamide (Cytoxan). zz Strenuous exercise. It's not quite clear why exercise causes gross hematuria. It may be trauma to the bladder, dehydration or the breakdown of red blood cells that occurs with sustained aerobic exercise. Runners are most often affected, although almost any athlete can develop visible urinary bleeding after an intense workout. Risk factors Almost anyone — including children and teens — can have red blood cells in the urine. Factors that make this more likely include: zz Age. Many men older than 50 have occasional hematuria due to an enlarged prostate gland. zz Your sex. More than half of all women will have a urinary tract infection at least once in their lives, possibly with some urinary bleeding. Younger men are more likely to have kidney stones or Alport syndrome, a form of hereditary nephritis that can cause blood in the urine. zz A recent infection. Kidney inflammation after a viral or bacterial infection (post-infectious glomerulonephritis) is one of the leading causes of visible urinary blood in children. zz Family history. You may be more prone to urinary bleeding if you have a family history of kidney disease or kidney stones. zz Certain medications. Aspirin, nonsteroidal antiinflammatory pain relievers, and antibiotics such as penicillin are known to increase the risk of urinary bleeding. zz Strenuous exercise. Long-distance runners are especially prone to exercise-induced urinary bleeding. In fact, the condition is sometimes called jogger's hematuria. But anyone who works out strenuously can develop symptoms. Preparing for your appointment You're likely to start by seeing your doctor. In some cases, though, you might be referred immediately to a doctor who specializes in urinary tract disorders (urologist). Here's some information to help you prepare for your appointment, and what to expect from your doctor. What you can do Be aware of any pre-appointment restrictions. At the time you make the appointment, be sure to ask if there's anything you need to do in advance to prepare for common diagnostic tests. zz Write down any symptoms you're experiencing, including any that may seem unrelated to the reason for which you scheduled the appointment. zz Make a list of your key medical information, zz

including any other conditions for which you're being treated, and the names of any medications, vitamins or supplements you're taking. zz Consider questions to ask your doctor and write them down. Bring along notepaper and a pen to jot down information as your doctor addresses your questions. For hematuria, some basic questions to ask your doctor include: zz What are the possible causes of my symptoms? zz What kind of tests do I need? Do these tests require any special preparation? zz Is my condition temporary? zz Will I need treatment? zz What treatments are available? zz Do you have any brochures or other printed material that I can take home with me? What websites do you recommend visiting? In addition to the questions that you've prepared to ask your doctor, don't hesitate to ask questions during your appointment at any time if you don't understand something. What to expect from your doctor Your doctor or health care provider is likely to ask you a number of questions. Being ready to answer them may reserve time to go over any point you want to spend more time on. Questions your doctor might ask include: zz Do you have pain while urinating? zz Do you see blood in your urine only occasionally or all the time? zz When during urination is blood present? For example, do you see blood when you first start urinating or does bloody urine become apparent toward the end of your urine stream? Or do you see blood in your urine stream the entire time you're urinating? zz Are you also passing blood clots during urination? What size and shape are they? zz What medications are you taking? zz Do you smoke? zz Are you exposed to chemicals on the job? What kinds? zz Have you ever had radiation therapy? Tests and diagnosis To find a cause for urinary bleeding, the following tests and exams play a key role: zz Physical exam, which includes a discussion of your medical history. zz Urine tests. Even if your bleeding was first discovered through urine testing (urinalysis), you're likely to have another test to see if your urine still contains red blood cells. Hematuria that occurs just once usually doesn't need further evaluation. Urinalysis can also help determine if you have a urinary tract infection or are excreting minerals that cause kidney stones. zz Imaging tests. Most people with hematuria are likely to have an imaging test that provides detailed images of their internal organs. This might be a computerized tomography (CT) scan, which uses radiation and a powerful computer to create cross-sectional images of the inside of the body; magnetic resonance imaging (MRI), which

uses a magnetic field and radio waves instead of X-rays to produce images; or an ultrasound exam. Ultrasound uses a combination of high-frequency sound waves and computer processing to view your kidneys and bladder. zz Cystoscopy. In this procedure, your doctor threads a narrow tube fitted with a miniature camera into your bladder to closely examine both the bladder and urethra. zz In spite of testing, the cause of urinary bleeding may never be found. In that case, your doctor is likely to recommend regular follow-up tests, especially if you have risk factors for bladder cancer, such as smoking, exposure to environmental toxins and a history of radiation therapy. Treatments and drugs Hematuria has no specific treatment. Instead, your doctor will focus on treating the underlying condition. This might include, for instance, taking antibiotics to clear a urinary tract infection, trying a prescription medication to shrink an enlarged prostate, or shock wave therapy to break up bladder or kidney stones. If the underlying condition isn't serious, no treatment is necessary. Prevention It's generally not possible to prevent hematuria, though there are steps you can take to reduce your risk of some of the diseases that cause it. Prevention strategies include: zz Urinary tract infections. Drinking plenty of water, urinating when you feel the urge and as soon as possible after intercourse, wiping from front to back after urination, and avoiding irritating feminine hygiene products may reduce your risk of urinary tract infections. zz Kidney stones. To help lower the likelihood of kidney stones, drink lots of water and limit salt, protein and oxalate-containing foods, such as spinach and rhubarb. zz Bladder cancer. Stopping smoking, avoiding exposure to chemicals and drinking plenty of water can cut your risk of bladder cancer. zz Kidney cancer. To help prevent kidney cancer, stop smoking, maintain a healthy weight, eat a healthy diet, stay active and avoid exposure to toxic chemicals.

Bed wetting (nocturnal enuresis)

Bed-wetting is not a behavioral problem. Many children do feel embarrassed or ashamed by it. Family members need to be supportive and not critical. By around age 4, children start gaining control over bladder and stop urinating in their bed at night (and stay dry). But in some even beyond the age of 5, control is not developed, especially in boys and leads to bedwetting. Wetting the bed at night (nocturnal enuresis) is commonly observed in many three to eight year old children. With respect to the gender, 60% of all the bedwetter is males and a major percentage of them wet the bed almost every night.

Causes of Bed Wetting Heredity: It is associated with certain genes. That is if one parent or both parents seem to have been faced with this problem in their childhood, then there are around 50% chances that their child will also be a bed wetter. However, the child usually stops at the same age that the parent did. Physical causes that increases the chances of bed-wetting ÂÂ Children with a small bladder than normal suffer from this problem. ÂÂ Problem in valve that controls the flow of urine from the Bladder. ÂÂ Urinary tract infections, ÂÂ Constipation exerts pressure on the Bladder. ÂÂ mental retardation and developmental disorders Neurological causes: In some children, the brain takes longer to develop voluntary control over certain bodily functions. Thus, a child may wet the bed because exercise control over the Bladder may yet not be achieved. External stress: Children who are stressed due to events such as moving to a new home, beginning school, conflict between parents, the arrival of a new child, being abused, etc are believed to be more likely to suffer from bed-wetting. In these cases, a child, who did not wet the bed at night, may have started doing so due to stress. Remedies to stop Bed Wetting During the day, let your children hold the urine for a long time. This will encourage bladder control.

Do not make her very conscious of the fact that her bed wetting is a major problem for you. She may get disturbed and lose her sleep over it – causing some other kind of problems. Wake her up in a few hours and empty her bowels once or twice a night. Once she realizes how nice it is to feel dry and clean for a few days, she may get motivated to try a bit more on her own. A child already going through mental distress due to this problem should be rather comforted and reassured from your end that he is perfectly normal and that you both jointly will find a better way to deal with it. Your child’s frequency of bedwetting may also increase with a sudden drop in temperature. So make sure your child is properly dressed and warm. Cover him with a blanket, if required. Psychological Impact on Children Many children suffer from self esteem problem. They have feeling of failure and start to distinguish themselves from others. Feel guilty about their doing. Those patients who have been physically inactive prior to the heart attack are encouraged to become more active and to take regular exercises like walking, gentle jogging etc. on the advice of your doctors. Those who have been practicing Yoga may restart after the 2nd week. However sudden and severe form of physical exertion like sprinting, football, weight lifting, bull worker exercise, pushing a car etc., must be avoided. Outdoor physical activities are best avoided in cold windy weather. Should you feel tired initially do not get discouraged but try to concentrate on becoming more and more active. Your physical exercise programme will be advised after an exercise test. Many patients find benefit from a nap in the afternoon. If you don’t get good night’s sleep a sleeping tablet might help you. However don’t make a habit of this. DIET: If you are over-weight, you put extra strain on the heart and you should lose weight by going on a low calorie diet. Avoid fatty foods, butter, ghee, fatty meat etc. Avoid coconut preparations and coconut oil for cooking as it has high contents of cholesterol. Oils containing poly-unsaturated fats eg., Sun flower oil, can be used for cooking. Avoid heavy meals. Evening meal is preferably taken couple of hours before your bed time. Alcohol in small quantities example a glass of beer or a peg of whiskey or gin may be allowed on occasions. Avoid regular intake of alcohol or excess alcohol, as it will add to your calories and is also harmful to the heart. SMOKING: If you were a smoker before your

heart attack then you should give it up now. This is crucial in preventing further narrowing of the coronary blood vessels. By continuing to smoke or even reducing the number, you are increasing the risk of further heart attacks. zz BACK TO WORK: Many patients with light office work should be able to return to their previous jobs by two or three months after their heart attack. Those who were doing hard physical work may have to look for lighter employment. A medical fitness certificate from your family Doctor or the hospital specialist may help you in achieving this. zz BUSINESS AND SOCIAL RESPONSIBILITIES: If you have had excessive business or social responsibilities and were taking on too much prior to your heart attack, then this is an opportunity to reassess your responsibilities and consider reducing some of them. zz TRAVEL AND DRIVING: You are advised not to drive vehicle for some time after leaving the hospital. When you restart, try and avoid stressful situations such as rush hour traffic or long journeys at first. If you think it is too much of a strain, you are advised to give up driving completely. zz SEX LIFE: Sexual intercourses are best avoided for about a month after leaving the hospital as it puts extra strain on your heart. However as your physical health improves no undue restrictions are necessary. zz DRUGS: You may need to continue on drug treatment following heart attack often to provide protection for the future. ENDOMETRIOSIS The choice of delaying pregnancy may make pregnancy difficult or impossible at a later time

What is endometriosis? The Word “endometriosis” (Say: “end-oh-me-treeoh-sis”) means a problem with the lining of the uterus. The lining is called the endometrium. Normally, every month when you have your periods, the lining (which is also called endometrial tissue) comes out in menstrual flow. Endometrial tissue is usually only inside your uterus. However, if you have endometriosis, tissue from the lining of the uterus moves through the fallopian tubes and gets on your ovaries, in your pelvis, on your bladder or in the other areas. When you have your periods, this tissue swells and bleeds, just like the lining of your uterus. This is often painful, and scar tissue can form in your pelvic area. Is endometriosis harmful to me? Endometriosis may hurt, but it doesn’t cause cancer or affect your overall health, although it might make it harder to get pregnant.

What will be my symptoms be if I have endometriosis? If you have painful periods, severe cramping and pain during sex, low back pain or rectal pain, you may have endometriosis. The only way for your doctor to be sure is to a laparoscopy (say:”lap-ah-ross-ca-pee”). This is away of looking inside your uterus by making a small cut in your skin and putting a thin tube inside. If your doctor decides to perform this procedure, you will be given medicine so you won’t feel pain. How can we treat endometriosis? A number of medicines might help with your pain. Sometimes it helps to take birth control pills. Another medicine that might helps is along - acting progestin (this is a hormone that comes in a shot; it’s also used for birth control). Other medicine used is a monthly shot of a hormone called a GnRH analog. How do these medicines work? These medicines stop your periods. They may cause side effects like the ones women have at menopause. These include hot flashes and vaginal dryness. Danazol might cause acne and unwanted facial hair. Sometimes the side effect of the GnRH analog goes away if you also take an estrogen pill every day. What is the duration of the treatment? Birth control pills, danazol and GnRH analogs are taken for 4 to 6 months. Pain is usually relieved during

the period of medication. After you stop taking the medicine, the pain may come back. How can it be diagnosed? It can be diagnosed by ultrasound. Ultrasound shows blood filled cysts in the ovaries. Gold standard for diagnosis is laparoscopy. What about surgery? Surgery is also used to treat endometriosis. In this surgery, the doctor removes the endometrial tissue from the abnormal places. If endometriosis is keeping you from getting pregnant, surgery might make it easier for you to get pregnant. However, you might still need other kinds of fertility treatments if you want to get pregnant. What about pregnancy? Patients with moderate to severe endometriosis have been know to have difficulty in getting pregnant. The relation of infertility with mild endometriosis is not well documented. Patients will have to follow a strict protocol of treatment where in the periods will be suppressed for a certain period of time to allow the endometriotic process to subside. Their best chance of getting pregnant is with the test tube baby procedure where, in this subset of people, the pregnancy rates can be high as 50%. What happen during and after menopause? For many women, endometriosis goes away at menopause, when periods stop. Until menopause medicine and surgery may be able to help with symptoms of endometriosis.

Recently this procedure is discouraged by the Government in the interest of maintaining the balance of both the sexes. Interestingly medical physiology says how the foetaltestes begin to secrete testosterone at about 2nd to 4th month of embryoniclife. Testosterone performs three functions in the foetus i.e. Sex differentiation, development of accessory sex organs and descent of the testes. In foetus there are two genital ducts called mullerian duct and Wolffianduct. Mullerian duct gives rise to female accessory sex organs such as vagina, uterus and fallopian tube. Wolffian duct gives rise to male accessory organs like epididymis , vas deferens and seminal vesicles. If testosterone is secreted from the genital ridge of the foetus at about 7th week of intra uterine life , the mullerian duct system disappears and male sex organs develop from the wolffian duct. In absence of testosterone, female sex organs develop from the mullerian duct. Thus testosterone is responsible for the sex differentiation of the foetus. The level of testosterone depends on life style, diet and herbs which fuel testosterone level. During prenatal developmenttestosterone exposure isdirectly responsible for masculising the genitals and brain structures . How my mom looks: Pregnancy causes many changes in my mother. The place where I stay, the uterus is trying to protect me from all dangers. By 9th week there will be a sealing of a mucus plug, which forms in the opening of the cervical canal which acts like a sealfor theuterus. My mother still does not look pregnant though her waist looks a little thickening. One thing which alarmed her was the fuller breasts. She was enjoying it and started taking good care of them by using maternity bras with cuts and style which gave her extra comfort from restricted breathing and movements. Pregnancy can drain out the energy and to reduce the discomfort she did her regular stretching exercises .This helped her body cope better with aches and pains which are part of pregnancy process. Walking improves circulation and swimming can improve abdominal muscles. Exercises help the mother to get through her daily tasks and easily cope up with a demanding schedule because muscles which are strong and toned need less effort to engage in any activity. My grandmother always said that giving birth to a baby is similar to running a race which requires stamina, determination and focus. The better shape you are the stronger you will be during labour and delivery time. It makes sense and she warned -make sure it is done in moderation. Even after child birth this alone is a motivation for many women who have maintained strength and muscle tone throughout pregnancy for the body to bounce back to normalcy after giving birth. may be over whelmed; in high spirits together have anxiety lurking behind. When you exercise it boosts the brain chemicals linked to mood enhancing putting you in better spirits. How I got connected with my mother Every day my mother started a daily ritual to connect with me .She was setting aside 5 to 10 minutes a day to think about me .Just after waking up and before going to sleep in the night she would sit quietly and gently rest her hands on her belly which made me feel pampered .After this she focused on her breathing and started thinking about me and that’s the time we got connected to each other. Throughout the day she did talk to me so many times but the time she spent exclusively with me, I always felt was a great way to initiate the bondage process – the bonding of mother and the baby. Did my mother enjoy her sleep When anybody is carrying an extra weight in front of them finding a comfortable sleeping position can be a real challenge for them .My mother would experiment with pillows. and she found that putting pillows under the abdomen or between her legs and a rolled blanket under the small of the back helped her relax and take away some pressure from her back. She also felt sleeping on sides made her more relaxed. She felt left side was better because as the liver is on the right side of the abdomen and lying on your left side helps to keep the uterus off that bigger sized organ. It improves circulation to heart and allows for the Best blood flow to the foetus,uterus,and kidneys and sometimes lying on either side, should do the trick and help take some pressure off back. Sometimes leg cramps awakened her in the middle of the night and my dad was their at her side rubbing her legs with Aswagandha bala lakshadi taila .She also made sure she took enough calcium in her diet with supplements like pravala panchamruta tabs. On days she felt she had disturbed sleep she made sure she took naps during the day to make up for the lost sleep. Common ailments in these weeks Bleeding can be one of the complaints in early pregnancy. Ayurvedic Acharyas specify that use of herbs vrukshadini,payasya,lata, utpala and sariva is prescribed as a drink in this week as a supportive to pregnancy to avoid mishaps. In pregnancy it is easier to pick up a urinary infection so keeping well hydrated during this period and having diet which is soft, sweet, cold, pleasing and gentle is prescribed. All pacifying drugs administered should be soft and be given along with food or drinks. In short pregnant woman should be treated just like a pot filled with oil . Slight oscillation of the pot causes spilling of the oil similarly slight excitement to the pregnant woman can initiate abortion. It is important for babies and young children with albinism to wear glasses to correct refractive error if they are nearsighted, farsighted, or have astigmatism. If a child with albinism who needs glasses doesn’t wear them, that child may tire easily and quickly give up doing certain tasks, and may even affect their development. Short viewing distances Beginning in infancy, people with albinism instinctively hold things closer to their eyes to see them better. Up until the age of five or six, this technique sufficiently compensates for low vision because books for young children already feature large print, or they can sit closer to the TV. As a result of using this desire of moving closer to what needs to be seen, children this age generally do not need low vision aids. Looking closely at objects does not hurt the individual’s eyes, and this strategy continues throughout life. Near vision aids Dome magnifiers, reading glasses, handheld and stand magnifiers, and microscopes are near vision aids that help people with albinism read, look at pictures, diagrams, and maps, and accomplish other tasks that require seeing small details up close. Dome magnifiers are one of the easiest aids to use and arguably the most important for those with albinism. Despite their seemingly modest ability to magnify print (1.7 to 2.2 times), in combination with a short viewing distance many reading tasks can be easily accomplished even in the primary grades of school. These magnifiers are also known as bright field magnifiers, paper weight magnifiers, and Visolett magnifiers. Reading glasses help the user focus on text or other objects while holding the object close

to the user’s eyes. Reading glasses allow the widest field of view for reading compared to other aids. Hand-held and stand magnifiers [Figure 2] enlarge close-up images, allowing the user to see small print and images at greater distances from the user’s eyes. There are many different styles and sizes of magnifiers useful for people with albinism. Microscopes help people see smaller details than magnifiers produce. Microscopes enlarge close-up objects the same way telescopes enlarge far-away objects. Some telescopes and bioptic telescopes, designed for distance vision, also allow the user to refocus the scope for up-close use.

Contact Lens

People choose to wear contact lenses for many reasons. Aesthetics and cosmetics are often motivating factors for people who would like to avoid wearing glasses or would like to change the appearance of their eyes.[6] Other people wear contacts for more visual reasons. When compared with spectacles, contact lenses typically provide better peripheral vision, and do not collect moisture such as rain, snow, condensation, or sweat. Compliance is the key to long-term success for all contact lens wearers. It is even more important for keratoconus patients, since they are almost totally dependent upon contact lenses for all their visual tasks. After patients have been successfully fit with contact lenses, a high percentage of complications and adverse reactions are related to skipping or shortcutting the recommended procedure for cleaning, disinfecting, and storing their lenses. A study revealed that 27% of patients admitted that they don’t clean their lenses daily, and it is likely that an even higher percentage of lens wearers are non-compliant in some way. Some patients have used dishwashing liquid, baby shampoo, or even toothpaste instead of the recommended cleaners; stored their lenses dry, rather than in an FDA-approved disinfecting solution; and used saliva (which contains a host of sight-threatening microorganisms) instead of the sterile solutions that are readily available, to wet their lenses before inserting them in their eyes. Glasses : In the early stages, vision can be corrected with glasses or regular soft contact but as KC progresses they are not able to correct the distortion caused by the irregular corneal surface and more complex contact lens designs are required. Soft Lenses : The role of soft lenses in keratoconus is limited because the soft lens drapes over the irregular corneal surface and the front surface of the lens assumes the same irregular surface as the cornea without trapping a fluid reservoir so the effective refracting surface is no improvement over the original corneal surface. retain more of a rigid shape and may contribute to the liquid lens effect to some extent. There are designs being used for keratoconus, and they are helpful in mild to moderate cases. RGP Contact Lenses : Rigid Gas Permeable (RGP or GP) contact lenses are primary option for correcting KC vision. The rigid lens masks the underlying irregular cornea and functions as the new refractive surface of the eye, with the tear film filling in the space between the back of the contact lens and the front of the eye. “Rigid” defines the type of lens. “Gas Permeable” describes the lens material. There are many different RGP lens designs. “Piggy-backs”: This is a two system: an RGP lens worn on top of a soft lens. The RGP lens provides crisp vision and the soft lens acts as a cushion providing comfort. Hybrid lenses: This is a lens combination that has an RGP center surrounded by a soft skirt. Scleral lenses : These are large diameter lenses that rest on the white part of the eye, called the sclera, and vaults over the cornea. The size can be a scary prospect for some, but scleral lenses have many advantages. Because of their size, they do not fall out, dust or dirt particles cannot get behind them during wear. They are surprisingly comfortable to wear because the edges of the lens rests above and below the eye lid margins so there is no lens awareness. The introduction of rigid gas permeable (RGP) materials has made this design more readily available. Contact Lenses : Eye infections, while infrequent, can be devastating, preventing patients from wearing their contact lenses for days or weeks and sometimes resulting in permanent corneal scarring and loss of vision. A thorough understanding of the role played by each of the rigid lens care products will help to keep lenses clean and eyes healthy. Contact Lens Care Tips zz Always wash your hands with soap and water before

handling your lenses. Dry your hands with a lint free towel. zz To avoid getting your lenses mixed up, always apply and remove in the same order. zz Never use eye drops or solutions that are not specifically for contact lenses. Never use tap water to clean your lenses. zz Never use saliva to try to clean or wet your lenses. zz Do not sleep in your lenses unless you have been prescribed a specific type of lens designed for extended wear or your doctor has

After inserting your lenses, empty the remaining solution out of your case, rinse it with warm water, and let it air dry. Replace your lens case at least every 3 months. Insertion Tips for Soft Contact Lenses Make sure your fingers are clean and dry. Remove your lens from the package or storage case. Inspect the lens to make sure it is not inverted. Inspect the lens for any debris and rinse if necessary. Place the lens on the tip of the index finger of your dominant hand. With the middle finger of your other hand, pull your upper lid up to prevent you from blinking. Pull down your lower eyelid using the middle finger of your inserting hand. Gently place the lens on the center of your eye. Slowly release your eyelid and gently close your eye. Gently blink a few times. Insert the other lens following the same procedure. Removal Tips for Soft Contact Lenses Wash and Dry your hands. Make sure the lens is centered on the eye. Looking up, pull your lower eyelid down with the middle finger of your dominant hand. Bring your index finger close to the eye until you touch the lower edge of the lens. Gently slide the lens down onto the white part of your eye. Squeeze the lens between your thumb and index finger to remove it from your eye. If the lens is a daily disposable lens, simply throw it away. If it is not a daily disposable lens, you will need to clean and disinfect it. Clean the lens by placing it in the palm of your hand with a small amount of solution. Gently rub the lens for 5-10 seconds. Rinse the lens with solution before storing it in its case. Repeat for the other eye. Important Facts to Remember Remove your lenses if you experience discomfort, redness, or decreased vision. Lenses may become contaminated if worn in swimming pools, hot tubs, or saunas. Always use fresh solution. Never reuse solution to store your lenses. Discard your lenses at the interval recommended by your doctor. Remove lenses before using medicated eye drops. In addition, rinsing the cleaner off with tap water just prior to replacing the lenses in your eyes might introduce bacteria or other microorganisms that would ordinarily have been killed while the lenses soaked overnight. If your lenses ever dry out, soak them for at least 4 hours before you wear them again, since they may have warped or flattened in curvature when the liquid evaporated. Do not mix and match solutions made by different manufacturers. Doing so may cloud your lenses, cause them to become gummy, or irritate your eyes. Make sure that the cleaning, soaking, disinfecting, and wetting/rewetting products you buy are all part of the same care system. Always store your lenses in a case with ridges on the bottom. Lenses tend to suction onto smoothbottomed cases and may chip if you try to pry them up at the edges. Replace your case each time you buy a new bottle of wetting/soaking or cleaning/soaking solution. Cases tend to become contaminated. You can clean case with your contact lens cleaner using a brand new inexpensive toothbrush. Be sure to rinse the case well before using it again. If the skin on your hands is rough, ask your contact lens fitter if you can use a manually-agitated device such as the Allergan Hydramat or a mechanical device such as the Clensatron. These devices work like miniature washing machines and may protect the lenses from becoming scratched from your rough skin. If you remove your lenses with a suction cup make sure you know exactly where the lens is before placing the suction cup on the eye. Suction cups should be cleaned with contact lens cleaner and rinsed after each use. They can also be soaked in wetting/soaking or cleaning/soaking solution. If you drop a lens, do not drag it along a surface. Wet your finger with wetting/soaking or rewetting solution and touch it gently to the lens to lift it. www.bangalorehealthtv.com

It was believed all these years osteoporosis can occur only in senile and post-menopausal women but it is found now that it can occur in all age groups. Osteoporosis can be disastrous fractures resulting because of it around the hip and spine results in death in about 20% of people in 1st year the rest of 80% people can become dependent for their rest of their life. Bone is composed of proteins, minerals, vitamins and also has nerve supply and the erythropoietic property (blood formation). Bone has an amazing property of self-healing. At birth there are about 300 bones as the person grows and reaches adult hood there are 206 bones. In severe osteoporosis trivial falls, violent sneezing, and coughing can result in fractures. Osteoporosis of the spine results in softening of the vertebrae and compression fractures and scoliosis and kyphosis. This can result in compression of spinal cord resulting in pain in legs (sciatica). But they play a major role in maintaining healthy bones. them regularly intake of calcium and vitamin-D is necessary. It is essential to stop these medicines for 6months once in 3 years. Bone density can be increased by use of anabolic steroids like strontium, teriparatide. In people suffering from gastro intestinal disorder injection of calcitonin and teriparatide are available, yearly intravenous injections of Zolondronic acid are highly useful. Similarly there are medicines which can be used nasally. Many people are reluctant to continue the medication as it is expensive, takes longer time ,does not have severe symptoms and lack of short term results. Role of surgeryMost of the times osteoporosis is detected after the fractures. The role of surgery in such situations is to get proper alignment of the bones, rapid healing and early mobilization. Surgical management in treating fragility fractures is difficult as it may require different implants. Such as LCP (locking compression plates), interlocking nails. Fractures close to joints are best treated by replacement (arthroplasty) as it allows early mobilization and ambulation. There are certain conditions in which ejaculation occurs but it takes long time (delayed ejaculation). Retarded ejaculation is a disorder where the male wants to ejaculate but is unable to do so. The impotent male is unable to achieve an erection while the male with retarded ejaculation cannot come, although his erection is normal.
