CHICKENPOX What you need to know
What is Chickenpox? Chickenpox is a rash illness caused by a Varicella Zoster Virus (VZV). Once a person is exposed to the chickenpox virus, it takes between 2 and 3 weeks before the symptoms appear. Chickenpox usually occurs in childhood. Adults who contract chicken pox are usually more ill, especially with pneumonia.
How common is chickenpox? Chickenpox is highly contagious and very common, with a secondary attack rate in households of 85 - 90%. It is primarily a disease of young children and is most common in school-aged children between 5 and 9 years of age when approximately 50% of cases occur. Only 1 0% of cases occur in adolescents older than 1 5 years of age and it is estimated that 95% of people have been infected by 30 years of age. Many people are not aware that before a vaccine was available approximately 10,600 persons were hospitalized and 100 to 150 died as a result of chicken pox in the U.S. every year. Signs and Symptoms z Mild fever z Feeling tired and irritable z Itch z Rash. The rash usually starts on the chest, back or face. It then can move to other areas of the body including inside the mouth. At first the rash looks like small pimples. These later become blisters filled with fluid. How it is spread? Chickenpox is very easy to catch (highly contagious). It can be spread by either having direct contact with the person who has chickenpox, from coughed fluids or by touching the liquid from the blisters. Children with chickenpox are infectious 1-2 days before the rash starts until the last blister has dried. What are the serious complications from chickenpox? Serious complications from chickenpox include bacterial infections, which can involve many sites of the body including the skin, tissues under the skin, bone, lungs (pneumonia), joints and the blood. Other serious complications are due directly to the virus infection and include viral pneumonia, bleeding problems and infection of the brain (encephalitis). Who and when should be vaccinated against chickenpox? All infants between 12 and 18 months are vaccinated

as part of their childhood shots. Older children, adolescents and adults who have never been infected should also get the vaccine. Those who may come into contact with chickenpox at work (e.g., nurses) or who live with people with weak immune systems should also receive it. Children on long-term salicylic acid therapy should be immunized and the use of this medication should be stopped for six weeks following the shot. Varicella Vaccine Safety and Side effects: Research has shown chickenpox vaccine to be 7090% effective in preventing disease and 95% effective in preventing severe disease. Varicella vaccine is also very safe. The most common side effects are mild and may include pain and redness at the injection site. A mild rash may develop. As with any medicine, there are very small risks that serious problems could occur after getting the vaccine. However, the potential risks associated with varicella disease are much greater than the potential risks associated with the varicella vaccine. What if my child gets vaccinated and then still develops chickenpox? No vaccine is 100% effective. Some children might not be fully protected against the disease, even though they have been vaccinated. If they get infected, the disease is mild and the child will generally have fewer spots, less likely to have fever and recover more quickly. Chickenpox in children is usually not serious. Why not let children get the disease? It is never possible to predict who will have a mild case of chickenpox and who will have a serious or even deadly case of disease. Now that there is a safe and effective vaccine available, it is not worth taking this chance. Chickenpox and Pregnancy Chickenpox can cause serious problems during pregnancy, especially when infection occurs early in the pregnancy or at the time of delivery. If chickenpox occurs early in pregnancy, several types of fetal abnormalities, including limb abnormalities, scarring of internal organs and neurological damage can occur. Pregnant women who suspect exposure to chickenpox should immediately contact their healthcare provider.

Child abuse Myths and Facts of child abuse
Myth #1: Child sexual abuse occurs only among strangers. If children stay away from strangers, they will not be sexually abused. Fact: Most of the juvenile sexual assault victims know their attacker. Myth #2: Children provoke sexual abuse through displaying seductive behaviour. Fact: Seductive behaviour is not the cause. Responsibility for the act lies with the offender. Myth #3: The majority of child sexual abuse victims tell someone about the abuse. Fact: Most of the child victims do not tell anyone about the abuse for at least a year, do not tell anyone for at least 5 years, but the number of unreported instances is far greater because children are afraid to tell anyone what has happened. Myth #4: Men and women sexually abuse children equally. Fact: The majority of child sexual abusers are men. Men sexually abuse both female and male children. Myth #5: If the children did not want the abuse, they could tell their perpetrator to stop. Fact: Because children are often taught the importance of obeying adults, they generally do not question the behaviour of an adult. Children are often coerced with bribes, threats, or use of a position of authority. Myth #6: All sexual abuse victims are female. Fact: Female children are abused more often than male children..

Myth #7: Family sexual abuse is an isolated, onetime incident. Fact: Child sexual abuse is usually a situation that develops gradually over a period of time and occurs repeatedly. Myth #8: In family sexual abuse, the “nonoffending” parent always knows what has happened. Fact: While some “non-offending” parents know and even support an offender’s actions, because of a lack of awareness, many suspect something is wrong, but are unsure what to do. Myth #9: Family sexual abuse only happens in lowincome families. Fact: Family sexual abuse crosses all classes of society. There is no race, social, or economic class that is immune to family sexual abuse. Myth #10: Non-violent sexual behavior between a child and an adult is not damaging to the child. Fact: Nearly all victims will experience confusion, shame, guilt, anger, and suffer from possessing a poor self image. Child sexual abuse can result in long-term relationship problems as well. The long-term emotional and psychological damage of sexual abuse can be devastating. Myth#11: Sex abusers are dirty old men. FACT: A sex abuser’s average age is 32. Any individual - male or female - can be a sexual abuser. Myth #12: Sex abuse only happens in poor, uneducated socioeconomic groups. FACT: Sexual abuse cuts across all boundaries - socioeconomic status, race, geographic area, gender, and educational level - equally. Myth #13: Sexual abuse victims are “damaged goods” and their lives are ruined forever. FACT: While sexual abuse is incredibly damaging, victims are not “damaged goods.” Healing is easiest when the intervention is immediate and appropriate therapy is provided. For adults who have repressed memories, the recovery process can be lengthy. However, all victims of abuse can become fully functioning, healthy children and adults. There is great hope for us all in facing and healing sexual abuse.

Salient points to remember during vaccination of your child.

Discuss the vaccine with the doctor first and understand the problems (if any) associated with it. Tell the doctor if your child: ÂÂ Has fever or cold ÂÂ Has allergies ÂÂ Has issues with previous immunizations ÂÂ Is on steroid treatment or other drugs affecting the immune system (for cancer or kidney transplants) ÂÂ Has a history of fits or convulsions ÂÂ Has intolerance to eggs; in such a case, your doctor may suggest your baby has his MMR vaccination in hospital ÂÂ Has into intolerance to eggs; in such a case, your doctor may suggest your baby has his MMR vaccination in hospital. ÂÂ Has family members with HIV/AIDS or other immune system diseases. Take the child’s favorite toy or doll along so that he/she can hold on to it for emotional support after the shot. Kinds older than two years of age might throw tantrums at the doctor’s. It is advisable to let the child know about the vaccine a little prior to the visit. Straight forward talk about the shot is best. Tell your child why he/she is getting it and that all kids of her age get it. Stay at the clinic for 15 minutes after the vaccine is given, just in case your child reacts to it. The child may be a little low after the injection and may be irritable. Avoid making any big plans after the appointment. Let the child rest at home. There might be some swelling and redness at the site of the injection. Don’t worry about it. Do not apply any medicine to the area. The child may have slight fever after the appointment. However, if the temperature is very high or if the child has convulsions (or fits), call

your doctor immediately. In the case of fever give extra fluids to drink and do not overdress and infant if hot. What to Do About Side Effects A warm compress applied after the shot can help reduce soreness, as can moving the arm or leg in which the shot was given (you can “bicycle” a baby’s leg to help bring relief, or encourage a toddler to do a few rounds of head-shoulder-knees-and-toes). z A non-aspirin pain reliever such as acetaminophen (after age three months) or ibuprofen (after age six months) can help with residual achiness, as well as with any low-grade fever, so ask the doctor about that before you leave the office. z Some doctors even recommend a dose of a nonaspirin pain reliever before the shot is given, to prevent these mild side effects in the first place. When to Consult a Doctor Though severe reactions to immunizationsare rare, you should call the doctor if your baby has any of the following symptoms within two days of a vaccine shot: z High fever (over 104°F) z Crying for more than three hours at a time (not to be confused with colic) z Seizures or convulsions – often related to high fever z Severe, persistent seizures or major alterations in consciousness z Listlessness, unresponsiveness, excessive sleepiness z An allergic reaction (such as swelling of mouth, face, or throat; breathing difficulties; rash) z Behavior changes -- you know your little one best, so if you notice any type of behavior that’s not normal for your child, it’s always smart to check with the pediatrician -- just to be on the safe side. After the Shots Sometimes children experience mild reactions from vaccines, such as pain at the injection site, a rash or a fever. These reactions are normal and will soon z

go away. The following tips will help you identify and minimize mild side effects. z Review any information your doctor gives you about the shots, especially the Vaccine Information Statements or other sheets that outline which side effects might be expected. z Use a cool, wet cloth to reduce redness, soreness, and swelling in the place where the shot was given. z Reduce any fever with a cool sponge bath. If your doctor approves, give non-aspirin pain reliever. z Give your child lots of liquid. It’s normal for some children to eat less during the 24 hours after getting vaccines. z Pay extra attention to your child for a few days. If you see something that concerns you, call your doctor The barriers against childhood immunizations: a qualitative research among socio-economically disadvantaged mothers. Immunizations are key to preventing patients from developing and dying from certain diseases and infections. All health care providers need to work as a team to increase immunization rates and protect patients. Recommendations for immunizations are constantly changing and new vaccines are being approved. As in all therapeutic areas, health care providers are responsible for keeping their knowledge up-to-date. How Vaccines Work z Vaccines help the body’s immune system prepare for future attacks. z Vaccines consist of killed or modified microbes, parts of microbes, or microbial DNA that trick the body into thinking an infection has occurred. z A vaccinated person’s immune system attacks the harmless vaccine and prepares for invasions against the kind of microbe the vaccine contained. In this way, the person becomes immunized against the microbe z If re-exposure to the infectious microbe occurs, the immune system will quickly recognize how to stop the infection. MYTHS AND FACTS ABOUT IMMUNIZATION Myth 1: Better hygiene and sanitation will make diseases disappear – vaccines are not necessary. FALSE Fact 1: The diseases we can vaccinate against will return if we stop vaccination programs. While better hygiene, hand washing and clean water help protect people from infectious diseases many infections can spread regardless of how clean we are. If people are not vaccinated, diseases that have become uncommon, such as polio and measles, will quickly reappear. Myth 2: Vaccines have several damaging and long-term side-effects that are yet unknown. Vaccination can even be fatal. FALSE Fact 2: Vaccines are very safe. Most vaccine reactions are usually minor and temporary, such as a sore arm or mild fever. Very serious health events are extremely rare and are carefully monitored and investigated. Myth 3: The combined vaccine against diphtheria, tetanus and pertussis (whooping cough) and the vaccine against poliomyelitis cause sudden infant death syndrome. FALSE Fact 3: There is no causal link between the

administering of the vaccines and sudden infant death. It is important to remember that these four diseases are life-threatening and babies who are not vaccinated against them are at serious risk of death or serious disability. Myth 4: Vaccine-preventable diseases are almost eradicated in my country, so there is no reason to be vaccinated. FALSE Fact 4: Although vaccine preventable diseases have become uncommon in many countries, the infectious agents that cause them continue to circulate in some parts of the world. In a highly inter-connected world, these agents can cross geographical borders and infect anyone who is not protected. So two key reasons to get vaccinated are to protect ourselves and to protect those around us. Myth 5: Vaccine-preventable childhood illnesses are just an unfortunate fact of life. FALSE Fact 5: Vaccine preventable diseases do not have to be ‘facts of life’. Illnesses such as measles, mumps and rubella are serious and can lead to severe complications in both children and adults, including pneumonia, encephalitis, blindness, diarrhoea, ear infections, congenital rubella syndrome (if a woman becomes infected with rubella in early pregnancy), and death. All these diseases and suffering can be prevented with vaccines. Myth 6: Giving a child more than one vaccine at a time can increase the risk of harmful sideeffects, which can overload the child’s immune system. FALSE Fact 6: Scientific evidence shows that giving several vaccines at the same time has no adverse effect on a child’s immune system. Children are exposed to several hundred foreign substances that trigger an immune response every day. Key advantages of having several vaccines at once is fewer clinic visits, which saves time and money, and children are more likely to complete the recommended vaccinations on schedule. Myth 7: Influenza is just a nuisance, and the vaccine isn’t very effective. FALSE Fact 7: Influenza is much more than a nuisance. It is a serious disease that kills 300 000-500 000 people worldwide every year. It is the best way to reduce your chances of severe flu and of spreading it to others. Avoiding the flu means avoiding extra medical care costs and lost income from missing days of work or school. Myth 8: It is better to be immunized through disease than through vaccines. FALSE Fact 8: Vaccines interact with the immune system to produce an immune response similar to that produced by the natural infection, but they do not cause the disease or put the immunized person at risk of its potential complications. Myth 9: Vaccines contain mercury which is dangerous. FALSE Fact 9: Thiomersal is an organic, mercurycontaining compound added to some vaccines as a preservative. There is no evidence to suggest that the amount of thiomersal used in vaccines poses a health risk. Yoga and Kundalini are very popular these days. They improve breathing and the nervous system. It is believed that Pranayama helps to increase the activity of nervous system, memory power and intelligence. For this reason the lungs must be kept healthy. Because of its anti-toxic property, Malabar nut is called ‘habit protector’. In 5th century Buddha recommended this plant to be used against lung problems and tuberculosis. Its effect in correcting uterine disorders is almost equivalent to Ashoka (Saraka Indica). The plant grows upto a height of 5-6 feet, the leaves are oblong, wider at base and sharp at tip. Pedicillate flowers are in inflorescence at the tip of stem. PARTS USED: Root, Flower, Bark - all have medicinal properties. CHEMICAL INGREDIENTS: The plant contains oil, fat, an alkali called Vasicin, Adathodic acid, sugar

and gum. The root bark has more vasicin and leaf has only 0.25%. This vasicin is the medicine. MEDICINAL PROPERTIES z The root bark grained and given with honey removes phlegm, in turn cures the cough in children. z Chronic cough and asthma - Fresh juice of the leaves mixed with honey, ginger juice can be used. The decoction of the root bark mixed with finely powdered black pepper can be given twice daily. z One suffering from rheumatism can use the paste of the leaves for local application. Or can take fomentation of the root bark decoction. z Skin diseases: The paste of the leaves mixed with turmeric and cow’s urine, when applied relieves pain and itching. z In leucorrhea, the juice of the leaf mixed with honey gives good result within 15 days. z The leaf juice mixed with honey and powder of piper longum controls the blood during cough. z During anaemia, the leaf juice and honey mixed in proportion 4:1, taken twice daily in empty stomach gives good result. z Piles- Juice of the leaf or the decoction of the root bark cures piles. Fomentation of the leaf to pile mass can be done. z Burning micturition: Decoction or powdered dried flower is useful. z Asthma- People suffering from asthma can use dried leaf as cigerette and smoke, z Conjunctivitis- Flower of the vasa can be kept around the eye. z To reduce fat : 4-6 spoons of the leaf juice before meals is useful. z During first trimester of pregnancy use of vasa is not advisable, z Metrorrhagia : Use of 4-6 spoons of leaf juice, 3-4 times a day is useful. z Fresh leaf juice, removes intestinal worms. 1-2 spoon for children, 3-4 spoon for adult. This is to be given for a week. z Juice of the leaf mixed with honey cures abdomenal pain during menstruation in women, and acts as analgesic. z The filtered water of a handful of vasa leaves crushed and soaked in a glass of hot water ,if given twice daily in empty stomach cures fever. z The crushed juice of the leaf, applied to the foot cures burning sensation. z In hematemesis dried flower powder with honey is useful. Method of juice extraction- keeps the leaves under the steam for a while, then crush it. It gives more juice. Hirsutism has a painful and sometimes even a devastating effect. Hirsutism is constantly stressed by the amount of time and money a women spends for its cure Lifestyle:- Day to day activities like eating, sleeping, exercise, managing stress and the pollutants to which one is exposed determine the amount of hair growth. There are two types of hair removal- Depilation and Epilation. 1. Depilation: Depilation is a process of temporary hair removal. It is the removal of part of hair above skin surface. It can be done by shaving or using creams.

Shaving is inexpensive, painless and provides good results. Creams and lotions chemically dissolve the hair, but they can leave a bad smell that lasts for days and may cause rashes to the skin. 2. Epilation Epilation is the removal of the entire hair from the root. There are several techniques of Epilation. Tweezing involves plucking of hairs and is suitable for small areas of skin. Waxing or sugaring involves applying a sticky paste and then peeling it off the skin, along with the hairs embedded in it. Friction using a mitt with a rough surface. It is used mainly for fine leg hair. Threading involves rolling a twisted thread across the hairs to catch and pull them out. Rotary epilators are similar to electric razors and they pull hairs out. ELECTROLYSIS Electrolysis is one of the permanent hair removal methods that will destroy the hair follicle. It is expensive, painful, tedious and difficult for large amounts of hair. LASER TREATMENT Laser hair removal is a new technique. Lasers use a special light wave that is absorbed by dark hairs and

damages the hair follicle. Laser treatment is useful for large areas of hair. Skin structures can be adversely affected by laser irradiation. MEDICATION Several drugs are available for prevention of hair growth. Some of them are z Androgen receptor blockers: Cyproterone, Flutamide and Spironolactone z Androgen-suppressing agents: GnRH agonists, Estroprogestins (birth control pills), Corticosteroids, and Insulin-sensitizing agents like Metformin. z 5 alpha-reductase inhibitors: Finasteride, Eflornithine hydrochloride. All these drugs reduce androgen levels and have side effects like impaired liver function. OTHER METHODS Surgery may be done to remove tumor that may be present in the ovaries or on adrenal gland. Acupuncture may help in removing excess hair. Psychological support may also be helpful since hirsutism is often a frustrating and embarrassing condition. NATURAL METHODS A healthy diet, supplementation, exercise and stress management will have a beneficial effect for women with hirustism. Vitamins like E and B6 and minerals like chromium, magnesium, zinc, copper, etc normalize insulin and androgens or reduce the effect of androgens. Partially hydrogenated food product should be avoided from consumption. Aerobic exercises improve insulin sensitivity and thus reducing hirsutism. In addition several herbal preparations have been claimed to relieve the symptoms of hirsutism. SIDE EFFECTS OF TREATMENT z Hair removal by most of the above methods is painful and may result in rashes and infection of skin after treatment. z Many of these treatments can result in infection and scarring. z Hair re-growth will occur in some women, in many of the above technique. PREVENTION Regular exercise, stress management and low fatty food can avoid hirsutism.

In this episode baby tells about the 7th and 8th week of its growth and how Pancha maha bhuta energy made it possible for its development. Tejomahabhuta gives rise to metabolism to perform all functions normally. Jala mahabhuta maintains the moisture and liquidity – water metabolism in the body. Pritvi maha bhuta bestows the shape. Akasha maha bhuta contributes to the growth. These elements are always in motion and are constantly changing and interacting with each other. While each element has its own unique nature it cannot assume form without being absorbed by each other .The memory of the five elements is stored in our body. Earth memory is stored in the heart, water memory in the kidneys, fire memory is stored in the intestines, air memory is stored in the lungs, and ether memory is stored in the brain. Earth predominates in the cell membrane giving structure to the cell, water predominates in the cytoplasm, the liquid in the cell, fire predominates in the metabolic process regulating in the cell, air predominates in the gases of the cell, and ether predominates in the empty space occupied in the cell. Nourishing elements z The earth element found within the solid food nourishes the internal organs, muscles skin, bones and all other tissues which are earth predominant. Sweet and sour taste food fuel the earth element. z Water in the liquid food nourishes the mucous membranes, secretions, lymph, plasma, fat, nerves, reproductive system, and all other water predominant tissues. In my 8th week I am able to move constantly though my size is of the kidney bean. It’s because my webbed fingers and toes which have now started sprouting out from my hands and feet are capable of doing movement .My mom is still not able to feel this movement but she will after few days . My eye lids are practically covering my eyes; breathing tubes extend from my throat to the branches of the developing lungs. My tail which always I thought was funny is just about gone. My brain nerve cells are branching out to connect with one another forming primitive neural path ways. Virtually completing 8 wks., I am looking unique, growing strong and vibrant with the help of the five elements... Link between food and elements z All sprouted whole grains, root vegetables, dried fruits, pulses, seeds, nuts, sea salt, minerals etc. all linked with earth element. z All juicy fruits, coconut water, juicy vegetables nut and seed milk and oils linked with water element. z Sour fruits, culinary seeds, spices, thermo genic herbs fermented foods and alcohol etc. are linked with fire element. z All dried fruits cabbage family veggies, leafy greens, sprout; rough veggies beans and legumes are linked with air element, z Herbs like Brahmi, valerian, cocaine, tobacco etc. are linked with ether element. Once a time favourite dish which my mom enjoyed, suddenly left her have aversion and puckish sensation. She did not lose hope and catered to new combinations and tastes. Sticking to food which was bland and boring did improve her nausea and vomiting and improved her feeling for food. Milk medicated with herbs having a sweet taste, cold and liquid diet and milk treated with herbs from jeevaneeya gana – life promoting was prescribed as an elixir for my mother to fight fatigue which is common in this period because her body is still engaged in manufacturing my life supporting system --- the placenta. Her breasts are looking bigger with blue veins clearly visible getting ready for transporting of fluids and nutrients to me after my birth. Another problem my mother was always trying to resolve was of excessive salivation. She used the pulp of bilwa fruit Aegle marmelos mixed with liquid prepared from parched paddy and when it became worst her physician advised her to take Bhunimba –Andrographis paniculata leaves paste with honey. Heart burn and indigestion are symptoms which she commonly experienced .To avoid this she drank sips of water either before or after she ate. She experienced that drinking too much water with food brought about a bloated feeling and aggravated heart burn. Increased mucus vaginal discharge is the nature’s way of protecting the birth canal from infection I heard the physician telling my Mom. But if it is in excess, foul smelling or with associated symptoms of itching, burning, highly coloured take the opinion of your health care provider he said. Occasional headaches did trouble her when she was exhausted, in stress or hungry. She made sure that she took enough rest to drive away fatigue triggered headache and keep fuelled regularly with food and water to keep her headaches at bay and her blood sugar levels from dropping to low levels. Occasional dizziness she knew was common in pregnancy because her circulatory system is trying to coup up with the additional need of the baby. Lying down or sitting brought comfort to her after few minutes. My dad’s supportive role
My father was a big comfort for my mother and kept pumping strength into my mother whenever she had discomfort. Pregnancy he knew would make her feel tired and one way to support her was by participating in few chores around the household. My mom also enjoyed some massages done by my dad when she had cramping in the legs. I personally feel this is a bonding experience as well as a tension reliever for her and a great feeling of being pampered.

They have to face the ordeal of taking care of the child at home as well as find someone to teach the child its daily activities of living. If you find somebody providing such help to the child, the happiness of such parents has no bounds. One can see mentally challenged children in special care homes, Associations for Mentally Challenged Persons, NIMHANS and many care giving centres. But managing a special school to help such children to help them to become independent and self reliant

is a herculean task. One such organization run by dedicated staff under the committed leadership of a social thinker can be seen at Nachiketa Manovikasa Kendra at Vijayanagar, Bangalore. The Centre has come up with the efforts of Sri. N. Narasimha Shenoy, the Honorary Secretary of the Trust. It was Mr. Shenoy’s dream to have an exclusive school for mentally challenged children. And he has done it. His passion to render service to such children and guide them from darkness to light took several years of struggle, time, effort and money. Just an act of compassion may not work here. To establish such a school, one has to struggle to procure the required land, arrange for money, the infrastructure, the confidence of parents of such children – all are essential. With his love towards such children, Mr. Shenoy went on to start one such organization in Vijayanagar a decade back which has now become a reality. Mentally challenged children from the nearby areas are admitted to the School, and provided facilities for their overall development. He has been involved in this activity by devoting his full time and energy. In spite of the attraction of lucrative bank job and the potential promotional opportunities there, he refused all promotions in life and chose social service and remained a clerk in the bank so that he can stay in Bangalore to promote this activity. He has recently retired and devotes full time for his organization. He has no regrets that he did not get any promotion in the bank.

The Centre conducts an I.Q. test of the mentally challenged child by a qualified psychologist before admitting to the school; maintains an excellent record of each child in a file. Mr. Shenoy happily displays files of candidates who have excelled in their various activities and the certificates they have obtained. Some children who did not have even the ability to express their toilet needs are now on their own. Some children who could not stand up are now in a position to walk freely for which the parents feel much relieved and happy, thanks to the Padmashree Institute of Physiotherapy, Bangalore. Children here are taught brushing their teeth, dress themselves, comb their heads, speak words, sing and even dance. Later they are trained in several vocations like candle making, drawing, greeting card preparation, screen printing, tailoring, envelop making etc. during the day. They come in the morning and go back in the evening. For the staff of this Centre, it is a mission to accomplish. The care of the mentally challenged children by this school is laudable that
after retirement from the bank, Mr. Shenoy devotes his whole time for the service of these children. “As long as I live, I shall strive to serve”, he says. Mrs. Shanti, Head Mistress of this Centre has been rendering dedicated service for the last 20 years here and other lady teachers join hands with her. According to another mythic Lord Rama defeated Ravanna and returned to Ayodya, crowned as a king of Ayodya (Pattabhishkam) on this day of Ugadi. Ramarajya means happy kingdom. It should be remained everlasting. So, Ugadi is celebrated. Another beliefs the first day of Ramanavami is Ugadi. Day before Ugadi pre celebration of of the festival begins. Ex. For Worm Problem :- Take some neem leaves one glass of water put together and boil until it becomes 1\2 glass. It is called Kashayam. For this Kasha yam add 2 drops of Asafoetida and shake well. at night 20 ml of Kashayam after the food before going to bed should be taken. Worm problem will be cured. 2. Piles: - Take neem tree cortex (bark) make it powder, add goat milk, drink 1\2 glass twice a day. 3. Skin and arthritis problem:- Neem tree cortex, flower, seed, leaves, root all (together called panchangam.) together make powder (choorna). This powder will be added with neem oil and make small balls like tablet. Swallow along with cow milk. Skin and arthritis problem will be cured. 4. Rashes and small pox:Turmeric and neem leaf should be grained by putting tender coconut water in a mixer. This paste should be applied all over the body. Leave for 15 minutes and take bath in Luke warm water/normal water without using soap. (instead of soap Bengal gram powder may used) 5. Any type of Wound:- equal quantity of neem leaves, turmeric sesame seeds grained with honey. This balm (paste) will be used in different types of wounds. 6. Jaundice: - Neem leaf juice, added with honey. Drink twice a day 1\2 glass. Very helpful in curing the Jaundice. 7. New born babies (above 4 months):- neem leaf Kasha yam 1\2 spoon (2ml) is very helpful in preventing stomach ache, gas etc.,. Also it clears the inconvenience in the stomach. 8. Diabetic:- Take Neem leaves, seeds, flowers and dry for one week under the hot son. Make it powder. 2 spoons of this powder added with hot water or milk before breakfast should be taken. PASCAL (Pattern Scan Laser): Photocoagulator is a new system designed to treat retinal diseases. There are several benefits with the Pascal laser compared to the conventional laser. Most of the laser can be done in one sitting, unlike the conventional laser which requires two to three sessions. This in turn reduces the number of patient-visits to the hospital. The procedure also significantly reduces the discomfort associated with conventional laser, and therefore patient tolerance is much better. Pascal laser can be used in most situations where conventional laser is indicated. z Indirect Ophthalmoscopic Laser Delivery System (LIO): This laser delivery system is mainly used for peripheral retinal lesions, e.g. horse shoe tear (HST), lattice degeneration, retinal holes, Retinopathy of prematurity (ROP) or pan retinal photocoagulation for vascular retinopathy in a hazymedia. z Photodynamic Therapy (PDT): Photodynamic therapy is a specialised form of laser treatment used for patients with age related macular degeneration (AMD). A light- sensitive drug is injected into a vein and travels to the abnormal blood vessels in the macula. This is activated by the laser. The light-activated drug then destroys the abnormal vessels. z Cryotherapy: is a second way of treating retinal tears apart from laser. An extremely cold probe is used to “freeze-burn” a small area on the outside of the eyeball that overlies the retinal tears. The purpose is to seal the tears and create an eventual scar that will “stick” the retina to that z

Fundus Photography: Color photographs of the back of the eye Fluorescein Angiography (FFA) -Spectralis HRA system: A diagnostic procedure that photographs the blood circulation of the retina. This involves injection of a dye called fluorescein into a vein in the arm or hand. Indocyanine Green Angiography (ICG) -Spectralis HRA system: A diagnostic procedure that photographs the blood circulation of the choroid. (Choroid is a layer of blood vessels behind the retina). This procedure is very similar to fluorescein angiography and involves injection of the indocyanine green dye into a vein in the arm or hand. Spectral Domain Optical Coherence Tomography (SD-OCT) – Spectralis: This is a diagnostic procedure similar to the CT scan of the brain. It uses a beam of light and its reflection to obtain cross-sectional images that provide information about the different layers of the retina. Ultrasonography (B-Scan): This procedure involves use of high-frequency sound waves to examine the eye when the normal view is obscured by hemorrhage or cataracts. Electroretinogram (ERG,mfERG, EOG, VEP): Electrophysiological measure of retinal function after light stimulation of the retina. Visual Field Analysis: Measurement of the full extent of the area visible to an eye that is fixating straight ahead. Common OPD Procedures

Warning symptoms following an intravitreal injection: Although rhegmatogenous retinal detachment and cataract are potential complications of intravitreal injection, the most feared complication is endophthalmitis i.e. infection inside the eye (rates typically less than 1%). The warning symptoms of this complication are rapid onset of: ÂÂ Increasing eye pain. ÂÂ Increasing redness of the eye. ÂÂ Greatly decreased vision. You must contact the hospital immediately for advice if you develop these warning symptoms. It is very important to identify and treat this type of infection as quickly as possible. Intravitreal injections for AMD, diabetic retinopathy (including macular edema) and retinal venous occlusion (RVO): In AMD, diabetic retinopathy and retinal venous occlusion, there are increased levels of vascular endothelial growth factor (VEGF) in the eye which gives rise to new vessels and macular edema. To counteract this, an anti VEGF injection is given. The anti VEGF injections are available as Lucentis, Macugen and Avastin (off label use). Anti VEGFs can rarely cause cerebrovascular events in the form of stroke or myocardial infarction (heart attack). Hence in patients who have a risk or history of ischemic heart disease or stroke, Macugen is preferred as it has less chance of causing such events. These injections might have to be repeated more than once, depending upon the response of the eye. z Intravitreal triamcinolone acetonide: Triamcinolone acetonide is a long acting steroid which is given in the eye in cases of macular edema secondary to diabetes, retinal venous occlusion or uveitis (ocular inflammation). You may feel black spots floating infront of eye, which is due to drug deposit in the vitreous. This will reduce over a period of few weeks as the drug is absorbed. Triamcinolone may cause an increase in eye pressure (glaucoma) or cataract (clouding of the lens). The intra ocular pressure can increase in 30% people who undergo the injection. The pressure in your eyes will be checked at every visit and eye drops prescribed if the pressure increases significantly. This increase is transient, and these drops can be discontinued after some time. Cataracts are not a serious problem for the first few months after the injection, but over 50% of treated eyes will eventually develop significant cataract if triamcinolone has to be repeated. z Ozurdex intravitreal implant: This is an intravitreal steroid implant which is approved for the treatment of macular edema secodary to retinal venous occlusion. It has recently been approved by the US FDA for use in eyes with macular edema secondary to uveitis (ocular inflammation). This implant remains in the vitreous cavity for a longer duration compared to the intravitreal injections.

The patient is expected to maintain a certain posture after the procedure for about a week to ten days. Also, he is not allowed to travel by air during this period. z Anterior Chamber / Vitreous tap: A very small amount of fluid from inside the eye is removed in cases of suspected infection or persistent inflammationin the eye. This fluid is then analyzed microbiologically and biochemically to aid in the diagnosis. Intravitreal injections This is an injection into the vitreous, which is the jelly-like substance inside your eye. It is performed to place medicines inside the eye near the retina. Intravitreal injections are used to deliver drugs to the retina and other structures in the back of the eye, thus avoiding effects on the rest of the body. Common conditions treated with intravitreal injections include diabetic retinopathy, macular degeneration, retinal vascular diseases and ocular inflammation. z
Procedure: Once your pupils are dilated, the actual procedure may take around 10 minutes and is carried out in minor operation theatre. You will be made to lie down in a comfortable position and anaesthetic (numbing) drops will be applied in your eye. Your eye will be cleaned with an iodine antiseptic solution. A speculum is inserted and the medicine injected into the vitreous. You may experience a mild discomfort during the procedure. Antibiotic ointment will be applied and the eye padded. Antibiotic eye drops need to be instilled for a week.
The doctor will see you the next day for inflammation or increase in intraocular pressure. Instructions following an intravitreal injection:

Chronic Kidney Disease Early detection and management - the best way to combat CKD, The Silent Killer On the eve of World Kidney Day on 13 March 2014, The Kidney Foundation Mission of Bangalore organised a program to create greater awareness on early detection of Chronic Kidney Disease (CKD) and its management. The focus of the program was the emphasis on early detection of CKD – which has the potential to delay the progression of the disease by at least 15-20 years from detection before reaching the dialysis/ transplant stage. The Kidney Foundation Mission was formed by like-minded individuals for the cause of spreading awareness on CKD and steps to prevent/manage the disease. The Kidney Foundation Mission was founded under the guidance of eminent Nephrologists and the vision of a philanthropic individual who was diagnosed with CKD at a very early stage and went on to manage the disease well owing to the benefits of early detection and subsequently underwent transplant successfully. India is fast emerging as the Diabetes capital of World. This together with hyper tension has known to be the most common cause for CKD. Each
Year thousands of Indians are struck with CKD and unknowingly carry on with their daily life, as the symptoms will appear only during the advanced stage of the disease. The patients thus are often forced to undergo dialysis or opt for a transplant by the time the disease is symptomatically detected. Early detection is therefore imperative and can potentially help in postponing the disease progression to reach dialysis / transplantation by at least 10-15 years. Commenting on the significance of the program, Dr. Gokulnath, Professor of Nephrology at St. Johns Medical College Bangalore and President, Peritoneal Dialysis Society of India said “Our primary objective is to take the message of prevention forward. We have recently conducted the program in a village at Kunigal, with the able support of an organization called Dayabhavan and we hope to shortly conduct screening for the entire district. This will help us map the preventive strategies in a semi-urban and rural area. The objective is to save precious and scarce resources that get spent in dialysis and transplant and to improve quality of life for those diagnosed with CKD”. Dr. Arun K.N., the present Vice President of Indian Society of Nephrology Southern Chapter affirmed that “The International Society of Nephrology is also mainly focussing and channelling its resources for prevention of CKD world wide”. The Kidney Foundation Mission is supported by several eminent Nephrologists from across India and several service minded individuals. As part of its outreach activities the Kidney Foundation Mission voluntarily provides know how, expertise and support to conduct CKD Awareness Programs and conduct Medical camps for early detection of CKD. The mission invites interest from individuals, organisations, volunteers and communities in this humanitarian mission to conduct CKD screening camps with a view to prevent the occurrence and detect the disease in its early stages thereby prolonging disease progression.

Phyllanthus acidus (emblica ) Elements : Protein: 0.7gm, Carboharadiet: 5.9 Gm, Calcium: 6 mili gm, phosphorus 6 mili gm ÂÂ Uses: z Emblica exhibits strong antioxidant activity. It is one of the most important plants in the traditional Ayurvedic medical system as well as in other traditional health systems for immunomodulatory, anti-inflammatory, antiulcer, hepatoprotective, and anticancer actions. However, there is very limited clinical evidence to support the use of emblica for any indication.There are few clinical studies to guide dosages. Diabetes/hyperlipidemia : 1 to 3 g of powdered, dried fruit was consumed daily in 30 mL of water for 21 days in 1 clinical trial. Another trial used amla 300 mg tablets (each containing 50% amla extract and 50% dextrin) 3 times per day z Emblica leaves, too, are taken internally for indigestion and diarrhea or dysentery, especially in combination with buttermilk, sour milk or fenugreek. The milky sap of the tree is applied on foul sores. The plant is considered an effective antiseptic in cleaning wounds, and it is also one of the many plant palliatives for snakebite and scorpion stings. A decoction of the leaves is used as a mouthwash and as a lotion for sore eyes. z The dried fruit yields ink and hair-dye and, having detergent properties, is sometimes used as a shampoo.
