right children who don’t achieve to potential are an interesting puzzle. Many factors contribute to their difficulties. Research has found that vision problems play a role in academic achievement yet are often inadvertently overlooked. Did you know that an untreated vision problem can mimic ADD/ ADHD? LD? and can contribute to behavioral problems? Typically if a child can pass the vision screening, vision is automatically ruled out, because most people think that “20/20” means you have perfect vision. But, did you know that the test that is used in most vision screenings (the Snellen Chart) was designed in the 1800’s? And, only 5% of actual vision problems are caught with that screening? To clarify what “20/20” actually means, the first “20” means you are looking at the eye chart from a distance of 20 feet. The second “20” means you are able to see the size of letter you are supposed to see from 20 feet. That’s all it means! The test doesn’t tell us if the child has all the visual skills required to read (example: following along a line of print easily). Students who are struggling with undiagnosed vision problems will often be very bright yet their intelligence won’t be reflected in their performance on standardized tests. According to Dr. Linda Silverman, an educational psychologist who has worked with gifted children for over 40 years, “A high Verbal IQ combined with a Performance IQ that is 20 points lower should signal the need for an optometric evaluation.” According to Dr. Thomas Unger, a developmental optometrist in Troy, Illinois, “There are over 20 visual skills required for reading and learning besides being able to see the eye chart. Sometimes these vision problems are tough to locate even in a comprehensive vision exam. If your child has any of the above symptoms you want to take your child to a Developmental Optometrist.” Does your child: • Get frustrated trying to read or do homework? • Have trouble keeping attention on reading? • Take much longer on homework than it should? If you answered yes to any of these questions, a vision problem may be contributing to your child’s difficulties. These are just a few of the more common symptoms. imb loss is not something people often think about unless it happens to them or a loved one. According to statistics from the Amputee Coalition, there are roughly 2,000,000 people living with limb loss, and an average of 507 new amputees each day. We often hear of people losing limbs in traumatic accidents or workplace injuries. However, what’s not generally known is that the majority of amputations in the United States are related to diabetes and peripheral artery disease (PAD) and are preventable. Education relating to the risk factors can help people take preventative steps

Replace Sodium and Empty Calories with Wholesome Foods
G
rocery store shelves and restaurant menus are often crowded with foods containing solid fats, added sugars and high levels of sodium. During National Nutrition Month®, the Academy of Nutrition and Dietetics (formerly the American Dietetic Association) is helping Americans understand how to get the most nutrients they need from the foods they eat, all surrounding this year’s theme: “Get Your Plate in Shape.” “When people eat foods that have added sugars and solid fats, they are consuming extra calories they don’t need,” says registered dietitian and Academy Spokesperson Angela Ginn. “These ‘empty calories’ are found in a number of foods and drinks and offer little-to-no nutritional benefits.” Foods high in solid fats (like sausage, shortening and cream) and added sugars (such as regular soda and pastries) should be considered occasional treats rather than regular options. Eating these foods on a regular basis can cause you to consume more calories than your body needs in one day. “Replace these foods with nutritionally sound choices, like fruits and vegetables, whole grains, lean protein and lowfat dairy,” Ginn says. “Eating occasional treats is okay. Just make sure to balance out those treats with healthier options and get plenty of exercise.” In addition to limiting foods high in solid fats and added sugars, consumers should also be aware of high levels of sodium in foods, especially pre-made options like

frozen meals and canned soups and vegetables. Foods containing high levels of sodium are contributors to high blood pressure, heart disease and stroke. “The 2010 Dietary Guidelines for Americans recommend consuming only 2,300 milligrams of sodium per day, which is about one teaspoon of salt,” Ginn says. “While meeting this recommendation may seem hard at first, choosing foods that are lower in sodium is one big step you can take towards meeting this goal. Ginn offers tips to choose healthier options and “Get Your Plate in Shape”: Choose foods and drinks with little or no added sugars. “It is smart to look for foods that have no added sugars, like unsweetened apple sauce or unsweetened whole-grain cereals,” Ginn says. • Drink water throughout the day. For variety, add lemons, limes or cucumbers to your water or try carbonated water. • Choose low-fat or fat-free milk or 100-percent fruit juices. • Eat fresh fruit salad for dessert. Eat fewer foods that are high in solid fats. “Solid fats can increase your risk for heart disease,” Ginn says. “You can reduce this risk by choosing healthier oils and lean meats.” • Instead of regular ground beef, opt for extra-lean ground beef. Ground turkey and chicken are also available in lean options. • Grill, broil, bake or steam your foods instead of frying. • Cook with healthy oils like olive, canola and sunflower oils in place of hydrogenated and partially-hydrogenated oils.
• Opt for fat-free or low-fat milk, yogurt and cheese. Cut back on sodium. “Much of the sodium we eat comes from prepared meals and foods eaten away from home. This can be significantly reduced by eating fresh foods,” Ginn says. • Instead of salt, use herbs and spices to season foods, and avoid salting food before tasting it. • Do not add salt when cooking pasta, rice and vegetables. • Read the Nutrition Facts Panel to compare sodium content of foods such as soups, broths, breads and frozen dinners, and choose the healthiest option. • Eat fresh fruits and vegetables, fresh meats, poultry and fish, beans and peas, unsalted nuts, eggs and low-fat or fat-free milk and yogurt. Ginn also recommends cooking different dishes at home. “This allows you to control what you put in your meal,” she says. hen a loved one is moving into a nursing home, you might think about the kind of nursing care they have, the activity schedule or the meal choices offered. But another important part of the nursing home staff is the social worker. More than 60,000 social workers are in residential and nursing communities across the country, and the field is growing. Social workers help us navigate relationships with ourselves, our family and those around us. Sometimes we carry emotional baggage – pain and anger toward others that make the transition into nursing care even more challenging. But even at its best, moving into a nursing facility is a major change for the entire family. Social workers ease that change by being a voice of reason and understanding. They let patients and families talk their way through concerns, worries and stress to find an agreement or solution. “There are a lot of people involved,” says Melissa Kline, a Passages Hospice social worker. “You have to know what to say, and the right time to say it.” Social workers become even more important at the end of life, when a loved one may be receiving hospice care. Hospice social workers visit each patient on a regular basis and meet with family to address concerns. Then the social worker meets with other members of the hospice care team, to help everyone understand what challenges they might face in caring for the person. Some common concerns for nursing facility residents are maintaining dignity when relying on others for personal care and having less independence than before. In hospice care, questions about death and leaving loved ones behind are common. Social workers help patients and families find their own answers to these questions. Years of education and experience help them sort through sometimes complicated feelings to find out what’s most important. They might make suggestions or give advice, but often they just provide a listening ear. And while they’re listening, they sometimes hear more than worries. They hear wisdom. “I’ve learned more about my own life from working with those near death than I learned throughout all my years in school,” Kline said. Understanding Treatment Options By Dr. Timothy C. Payne, MD Submitted by Med+Plus

ealthy, functioning joints are frequently taken for granted. Every year in the United States, runners train for marathons, overworked executives hit the gym, thousands brace the winter weather to hit the slopes, and families hop on their bikes for a ride around the neighborhood. However, if you are one of the millions of Americans living with osteoarthritis, simple and mundane tasks such as climbing stairs and walking can be a challenge. Osteoarthritis is a disease of the joints. Inflammation, pain and loss of functionality occur when the cartilage protecting the joint breaks down and the two bones rub together. The Centers for Disease Control reports that an estimated 46 million adults in the United States are told by a physician that they have some form of arthritis, rheumatoid arthritis, gout, lupus or fibromyalgia. The most common form of arthritis is osteoarthritis, and in 2005, an estimated 27 million adults were diagnosed with the disease. Arthritis and other rheumatic conditions (AORC) cost the United States billions of dollars each year. In fact, in 2003, reports showed that AORC cost the US $127.8 billion dollars ($80.8 billion in medical care expenditures and $47.0 billion in lost earnings). As a former college wrestler and football player, an avid golfer and a physician with over 30 years of experience in orthopaedic surgery and sports medicine. I’ve seen first-hand the adverse affects of living with osteoarthritis. Many patients are told by their physicians that their only options for treatment are exercise, non-steroidal antiinflammatory drugs, hyaluronic acid injections or total knee replacement surgery. What are the treatment options for osteoarthritis of the knee? Exercise is the number one recommended treatment for osteoarthritis (OA) of the knee because it builds muscle mass, which can help stabilize the knee. Further, with obesity contributing to the progression of OA due to the stress of added weight on the joints, exercise can also help with weight loss, thereby reducing pressure on the knee. Another treatment option for OA of the knee is non-steroidal anti-inflammatory drugs that reduce the inflammation of the joint. However, adverse side effects of pharmaceutical treatments include stomach irritation, possible stomach ulcers, elevated blood pressure or aggravation of heart conditions. Viscosupplementation injections are given in a series of one to five shots over the course of several months (depending on the product used). These injections are ef-
fective in relieving the pain of osteoarthritic knees, but the results may vary from patient to patient. Total knee replacement surgery (TKR) is the most invasive of all treatment options. In this surgical procedure, the end of the femur (thigh bone) and the top of the lower leg (tibia) are replaced with artificial components to create a synthetic joint. Since the surfaces that were previously rubbing on each other have now been replaced and realigned, this reduces the patient’s pain, but full range of motion after this surgery may not be possible if there were significant anatomical changes preceding surgical intervention. Total knee replacement surgery carries risks for patients such a blood clots in the leg or lung, infection, heart attack, stroke and loosening of the artificial knee. Additionally, depending on wear and tear, the joint may need to be replaced at a later date if loosening becomes painful. Rehabilitation time to restore knee function can take several weeks to months. Knee Replacement Alternative In May of 2009, I began prescribing VQ OrthoCare’s OActiveTM knee brace and BioniCare® System, the first non-invasive, non-pharmaceutical, non-surgical treatment option for osteoarthritis of the knee. The treatment delivers a low-level pulsed electrical signal to the knee through two electrodes held in place on the inside of a joint unloading brace, developed specifically to treat osteoarthritis by mimicking the naturally occurring signal present in the healthy knee joint. The low-level pulsed electrical signal is not felt by the patient. Because not every patient is a good candidate for surgery, drugs or injections due to age and other risk factors, the OActive knee brace and BioniCare System alleviates risks associated with surgery, pharmaceuticals and injections, while helping to alleviate pain and allow patients return to their normal, active lifestyles. As a prescribed treatment for osteoarthritis of the knee, the OActive knee brace and BioniCare System patients are under the care of a physician at all times. On average, patents using the OActive knee brace and BioniCare System have reported experiencing positive results after 750 hours. Unlike a standard knee brace, the BioniCare device is integrated into the OActive “unloader” knee brace to reduce the impact of body weight on the knee and reduce the patient’s pain. The OActive Knee Brace is worn undetected under clothing and allows for optimal movement of the entire leg.
Within the first year, I have seen a 70% improvement rate in pain, symptoms and knee fuction with the OActive knee brace and BioniCare System. Ultimately, taking care of ourselves at every stage along the way is the best treatment to preventing disease and illness as we grow older. This begins with exercise and nutrition, and continues by educating ourselves on the best treatment options. Diabetes is a serious disease that strikes nearly 26 million children and adults in the United States, and a quarter of them—7 million—do not even know they have it. An additional 79 million, or one in three American adults, have prediabetes, which puts them at high risk for developing type 2 diabetes. Unfortunately, diagnosis often comes 7 to 10 years after the onset of the disease, after disabling and even deadly complications have had time to develop. Therefore, early diagnosis is critical to successful treatment and delaying or preventing some of
its complications such as heart disease, blindness, kidney disease, stroke, amputation and death. Who should participate in Alert Day? Everyone should be aware of the risk factors for type 2 diabetes. People who are overweight, under active (living a sedentary lifestyle) and over the age of 45 should consider themselves at risk for the disease. African Americans, Hispanics/Latinos, Native Americans, Asian Americans, Pacific Islanders and people who have a family history of the disease also are at an increased risk for type 2 diabetes. Studies have shown that type 2 diabetes can often be prevented or delayed by losing just 7% of body weight (such as 15 pounds if you weigh 200) through regular physical activity (30 minutes a day, five days a week) and healthy eating. By understanding your risk, you can take the necessary steps to help prevent the onset of type 2 diabetes.

I
n our office we have several teen encounters. They range from ages 13 and older. The early pubertal teenagers are usually present with a parent, mother or father, and have concerns that cover perceived irregular cycles to dysmenorrhea. The teen is, usually, terrified of the exam, and the parent is terrified of the child’s budding sexuality. We, as practitioners, are asked to write for birth control to control cycles and symptoms. The visit from the older teenager, generally, covers the need for prevention of pregnancy. In either scenario, our job is critical to educate all involved about normal menstrual physiology, mechanism of action and side effects of desired medications, and prevention of sexually transmitted infections. National Data shows that 87% of males and 79% of female, non-married, teens will utilize birth control during their first sexual experience. The condom, withdrawal, and oral contraceptive pills, in that order, are the top 3 methods chosen. There is, also, an increase in the use of abstinence and the “rhythm method” among female teens. The success of any birth control method is based on the teenager’s ability to achieve a higher level of planning, decision making, and problem solving. Registered Nurse, Licensed Esthetician It is necessary to clear up misinformation and myths about birth control that is provided by peers and even family members that may be functioning under old guidelines from their teenage years. Table 1 shows the current methods of birth control with their levels of effectiveness. One of these misconceptions is that you need a pap smear in order to start a birth control method. This is no longer true. According to the current guidelines, the pelvic exam is not necessary if the teen has no complaints. Another is the thought most young girls have that they have irregular cycles. The art of counting out the length of the cycle, 21days- 35 days, is completely lost on this generation. Most teens are under the impression that each period should land within a calendar month, only once, but if you have a 21

day cycle you get double the pleasure and for the 35 day cyclers, you will skip a calendar month. This little fact of physiology must be clear before any form of birth control is started. That being said, before anyone walks out of the gynecologist’s office, they should be aware of how their method of birth control works. She must be aware of follow-up visits needed to monitor her birth control and possible side effects. Because this generation of teenagers is so internet savvy we can make the assumption that they are aware of more information than they really are. We must also correct wrong information that is obtained through this electronic medium. Lastly, there are so many choices available to today’s teen, and because of this fact, the initial visit with the teenager takes a substantial amount of time. National Colorectal Cancer Awareness Month By American Cancer Society
A
s the nation marks National Colorectal Cancer Awareness Month this March, the American Cancer Society is encouraging men and women 50 and older to make getting tested for colorectal cancer a priority. Colorectal cancer (commonly referred to as colon cancer) is one of only two cancers that can actually be prevented through screening, which allows doctors to find polyps in the colon and remove them before they turn cancerous. Regularly scheduled colorectal cancer screening can help save lives and help achieve the American Cancer Society’s goal of creating a world with less cancer and more birthdays. Screening for colon cancer has been proven to reduce deaths from the disease both by decreasing the number of people who are diagnosed with it and by finding a higher proportion of cancers at early, more treatable stages. Overall, colon cancer rates have declined rapidly in both men and women in the past two decades, due in part to early detection and removal of precancerous polyps. However, only half of the U.S. population aged 50 and older has been tested. “We have an opportunity to significantly reduce death rates from colon cancer through regular screening,” said Maggie Grotefendt, Health Initiatives Representative for the Metro East American Cancer Society. “However, there may be barriers to screening such as a lack of health insurance. We hope that people will use the month of March – National Colorectal Cancer Awareness Month – as an opportunity to make screening a priority and talk to their doctors, family members and friends about getting tested. By doing so, they are taking a key step toward staying well.” An estimated 103,170 cases of colon and 40,290 cases of rectal cancer are expected to occur in the United States in 2012. Of those, an estimated 51,690 deaths from colorectal cancer are expected to occur in 2012, accounting for 9% of all cancer deaths. Colorectal cancer is the third most common cancer in both men and women. Nonmodifiable risk factors for colon cancer include a family history of the colorectal cancer and a personal history of colorectal cancer, colorectal polyps or chronic inflammatory bowel disease. Major modifiable risk factors that have been associated with an increased risk of colorectal cancer in epidemiologic studies include physical inactivity, obesity and high consumption of red or processed meats. In addition to screening, there are healthy lifestyle behaviors individuals can adopt to reduce risk of colon cancer. Studies show that
being overweight or obese increases risk of colon cancer, and people whose diets include a high amount of red and processed meats are at increased risk. The American Cancer Society recommends that adults engage in at least 30 minutes of moderate to vigorous physical activity on five or more days of the week, and consume a healthy diet that includes five or more servings of vegetables and fruits each day, whole grains (instead of processed grains and sugars), limited alcohol and processed and red meats, and controlled portion sizes.
