Support for Caregivers Submitted by Senior Services Plus

oday, there are more than 65 million family caregivers in the United States and growing. There is no one else in a better position to ensure continuity of care for their loved ones. Caregivers are the true strength of the American family. In difficult times, Americans come together to ensure our loved ones are comfortable and safe. Whether caring for a parent, relative, or child, our Nation’s caregivers selflessly devote their time and energy to the well-being of those they look after. However, by devoting yourself to the needs of your loved ones you put your self at greater health risks than the ones you care for. signs of illness, exhaustion, or depression that you are experiencing. But science has now proven what family caregiver advocates have talked about for years; that providing care to someone you love can take a huge toll, both physically and emotionally. When was the last time you stopped to think about yourself? It is now recognized that the stress of care giving results in any number of long-term health effects for the family caregiver. These include: infectious diseases, depression, sleep deprivation, premature aging, and higher mortality rate. Yet millions of family caregivers neglect themselves each year.
Are You at Risk? These findings mean that the cumulative effects of the stress of family care giving are no laughing matter. They mean that all family caregivers really do need to start doing some things to reduce their stress or they—and their loved ones—are going to be in a more difficult situation than they already are. Whatever you do, don’t dismiss your feelings as “just stress.” Self-care is very important, and there is a wide-range of preventive health behaviors that caregivers tend to neglect. These include getting exercise, eating right, having medical checkups on a regular basis, and making time for themselves. But time is precious. We live longer lives and we find that we may require some assistance during those later years. Asking for help from neighbors, your church, social services agencies, and or homecare programs may be necessary. But trusting someone you know little about can be very scary both for the caregiver and their loved one. Picking a homecare company or an individual whom you can trust and ensure they have the skills to help requires some expertise and research. Here are just a few questions that should be asked to get the best possible homecare worker for your loved one: • How long has homecare agency been in business? • Are they licensed and insured? • Does the agency meet with the potential client before services start? • Is the agency available during non-business hours? • An absolute must is: “What are your hiring practices?” Some homecare service agencies may not follow state requirements that exclude applicants who are convicted felons. Other agencies do not require drug screenings.
“You need to know who is coming into your home to provide homecare services so that you can be sure your loved ones are looked after in an appropriate and professional manner,” advises Jonathan Becker, Executive Director for Senior Services Plus. SSP feels strongly that they have a responsibility to take every precaution and safeguard in pre-screening their Homemakers before they are trained and sent into a seniors’ home to provide those crucial in-home services “When we hire an employee we are hiring a friend, caregiver, companion, and someone you can trust.” Theresa Collins, Senior Services Plus Homecare Director, agrees that an individual or family member seeking homecare services through an agency should ask lots of questions and before they hire an individual on their own, they should consider these same critical requirements. SSP thoroughly screens all their potential homemakers. They require a physical, drug testing, fingerprinting, reference checks, and training before they are hired and assigned a client. Following these guidelines will help ensure the safety of the senior and prove that the individual has the required skills to do the job. This is especially vital as 40% of the seniors we serve do not have family living in the immediate area and are dependent on the internet and phone conversations to manage their loved one’s care. Taking that extra step in asking questions may save some heartache and avoid problems down the road. “Super” Snacking Is Easy with Grapes I
n today’s busy world, snacking is an integral part of the American lifestyle, but adults and kids often gravitate toward less healthy choices. Why? According to a recent study on snacking trends conducted by Symphony IRI Group, more than half of consumers said that they opt for taste over health in their snacking choices. Grapes in a Canoe Created by Ellie Krieger Makes: 4 servings 4 large celery stalks 8 teaspoons peanut butter or almond butter 1 cup red California grapes 32 thin pretzel sticks, preferably whole wheat If necessary, make a slice or two on the spine of each piece of celery, so they lay flat on a plate. Spread 2 teaspoons peanut butter or almond butter into each celery stalk. Place the grapes in a row inside the celery, pressing them lightly into the nut butter. s summer gets into full swing, many of you may have started to get back to the golf course and tennis courts for the first time since last season. Tennis and golf are known to place increased stress on the elbow and lead to irritation of the tendons around the elbow. These conditions are commonly called “tennis elbow” and “golfer’s elbow”. They can be so painful that they can cause some to give up their sport. Proper warm up and stretching of forearm muscles help with prevention and can be a great place to start treatment if you do begin to have pain.

It was previously thought that the pain associated with chronic tendon pain or “chronic tendonitis” such as tennis elbow, was due to inflammation. However, recent research has shown the tendon to have very little, if any inflammation. Instead, repetitive stress on the tendon causes an over development of scar tissue which effectively weakens the tendon. This is important when we consider the best treatment approach. While rest, ice, and anti-inflammatory medication may initially reduce the pain, they don’t necessarily address the root cause of the injury. The most effective treatment will help stimulate the tendon to remodel itself and get stronger. What is “scar tissue” exactly? Scar is your body’s natural response to an injury to assist with strengthening an area where damage has occurred. The strength of the scar helps to stabilize the damaged area so you can continue your normal activities. In most cases, scar tissue is helpful. On some occasions it can be laid down in excess or in a pattern that doesn’t allow you to perform your normal activities without pain. When you develop a form of tendonitis whether it is acute or chronic; your body lays down scar tissue to help strengthen the area. All the small injuries add up over time until enough damage has occurred and pain and inability to use the affected extremity are noticed. ASTYM is a therapy approach that addresses problems such as scarring, fibrosis and degeneration that occur in soft tissues throughout the upper and lower extremities. ASTYM is effective for new injuries as well as chronic conditions. ASTYM treatment uses instruments along the surface of the skin to locate these problem areas and start the body’s healing process. What is Compounding? Pharmacy compounding is the art and science of preparing medications that are customized to meet the specific needs of each individual patient. Compounding customizes the drug, strength, size, and dosage form to fit each individual patient specifically. This allows patients with special needs such as difficulty swallowing pills or who have allergies to certain medications, to have their prescription individualized to their specific needs. Both pharmacies offer compounded medications for pain management, bio-identical hormone replacement, podiatry, ophthalmology, pediatrics, dermatology, cosmeceutical, veterinary, and dental use. All compounds are formulated using FDA approved ingredients. With compounding, patients can not only get the exact medication they need, but they can get it in a variety of forms that will best meet their needs. Customized Bio-Identical Hormone Replacement Therapy: Bio-identical hormones have a molecular structure that is identical to hormones that are naturally produced by the human body. The replacement hormones must duplicate the function of hormones that were originally produced by the human body, which means the chemical structures must be identical. Pharmaceutical hormones are synthetic, artificial, and have been chemically altered, and are not identical to the hormones produced by the body that they are intended to replace. These structural differences may be responsible for some of the side effects seen with non-bioidentical hormones. Pediatric Compounding: When a child has an allergy, medications can be customized to eliminate gluten, dyes, sugar, lactose, alcohol and preservatives. If a child cannot swallow a pill, alternatives such as lollipops or transdermal gels can be compounded If the taste is a problem, pleasantly-flavored suspensions, concentrates, gummies, lollipops or lozenges can encourage a child to take their medication. The pharmacists can also combine medications and supplements into one dosage form to simplify administration and improve compliance. Individualized dosing can be compounded for the optimal dose based on age, weight and disease state to provide the benefits while minimizing risks of side effects. our parents took care of you when you were little, and there may be a time when you need to take care of them. Even if your parents are still physically able, you will need to look out for them on multiple levels, as elder abuse can come in many forms—physically, emotional, and financial. Because those over 55 control over 77% of the wealth in the United States, elders can be prime targets for financial abuse. Predators can easily gain the trust of those who may be lonely or isolated from family and friends. Additionally, those elders who have diminished cognitive abilities will be easier to manipulate, especially when the abusers hold a position of trust, such as a friend or family member. Sadly, elder financial abuse can be difficult to detect; it is often unreported because it may not be recognized as abuse. In the case of elders who eventually realize what has happened, they tend not to report the crime due to embarrassment or a feeling of responsibility about what happened. Unfortunately, this leaves the predators free to commit this crime again and again. Moreover, if others, such as lawyers, caregivers, or other professionals, realize that elder financial abuse has or is occurring, they may not report the crime because they do not want to prosecute a colleague or want to avoid adverse publicity. Unfortunately, over 60% of the abusers are family members—an adult child no less—who tend to be financially dependent on the elder in question. While any elder can be a target for this crime, women in particular are targeted due to their longer lifespan and possible unfamiliarity with financial decisions after the passing of a spouse, among other reasons. What signs can you look for regarding elder financial abuse? You may notice unusual or excessive worry about finances, larger-thannormal credit and cash transactions in a short period of time, or odd reasons for, or secrecy regarding, withdrawing money. Aside from financial changes, elders may gain new “friends”, be missing possessions, or undergo abrupt or unexplained changes in behavior, grooming, or health.
How can you protect yourself or your loved ones from elder financial abuse? First, make sure to have a reliable and reputable power of attorney. Keep any valuables and important documents safe and record what they are. Never give out personal information—social security numbers or personal banking information—to anyone over the phone. Try to avoid isolation—make sure that there is a network of trusted friends and professionals around your loved one. Professionals, such as medical personnel, social workers, clergy, and nursing home workers, are required to report elder abuse, financial or otherwise. ecent news articles have gotten many patients nervous about the use of dental X-rays. Many patients are questioning the amount of radiation received from dental X-rays, as well as the necessity of dental X-rays. Dental X-rays are definitely needed to determine if patients have problems with their teeth. The good news is that dental X-rays are very safe.

Why are dental X-rays taken? Typically patients receive a new set of full mouth X-rays when they go to a new dentist. Then patients receive cavity-detecting X-rays every 6–18 months to determine if new problems have occurred. The amount of X-rays and the type and frequency vary by patient based on how susceptible a patient is to getting cavities.
Dental X-rays not only help determine if a patient has hidden cavities in between their teeth; but also help to reveal abscesses, cysts, tumors, bone loss, foreign bodies, impacted teeth, and the presence of periodontal disease. Dental X-rays also help determine the condition of existing crowns, fillings, and bridges. Without dental X-rays, many of these potentially dangerous conditions would go undetected. How much radiation do dental X-rays expose me to? One of the benefits of dental X-rays is that they are very focused and greatly limit the exposure of the patient to radiation. Some patients are very concerned about their exposure to radiation. This is understandable with recent headlines about the dangers of dental radiation, and the high levels of radiation exposure that many medical X-rays have. Compared to most medical X-rays, the exposure to radiation is much less. With modern digital x-ray sensors, a patient is exposed to only six to eight mREM for an entire full mouth series. An mREM is a unit of measurement used for x-ray exposure. For comparison, a person is exposed to 360 mREM per year from environmental factors such as living in brick homes, sleeping next to people, sunlight, driving, and being near electrical devices. A person is exposed to two to three mREM every time they fly in an airplane. A single chest x-ray can expose a patient to over 10 mREM. Government safety guidelines limit workplace exposure to 5000 mREM yearly; that’s the equivalent of over 800 full mouth dental series! When considering the amount of radiation that people are exposed to, it
becomes obvious how little radiation a person is exposed to from dental X-rays. How does new technology limit radiation exposure from dental X-rays? New technologies have greatly decreased the amount of radiation exposure caused from dental X-rays. Digital X-rays have reduced the amount of radiation that patients are exposed to by 80%. This has helped to calm many of the concerns over dental x-ray radiation exposure. Also, special safeguards such as lead aprons and special x-ray units have also reduced the exposure to patients. The best thing to do if you have concerns about exposure from dental X-rays is to discuss your concerns with a dentist. Many dentists will work with patients to reduce the amount of radiation that their patients are exposed to. However, it is important to know that without dental X-rays a dentist cannot give an accurate diagnosis of dental problems, or may not be able to determine if there are dental problems at all. The small amount of radiation that dental X-rays expose a person to are worth the benefits that come from getting dental X-rays. ust like your option to choose your doctor, you also have the choice of where you can go to get your next MRI. Your doctor will make a recommendation on where to go, but is it the right place for you? There are many factors that you should consider when choosing the best provider; namely quality, cost and convenience. In this economic climate, cost is a key factor when considering where to get an MRI. Charges for MRIs can vary drastically from one facility to another. Consider that some outpatient facilities are able to charge less for MRIs and other imaging services because their overhead is less and they have lower contracted rates than hospitals. bout half of American adults suffer with at least one chronic condition, such as heart disease, cancer, or diabetes, according to the Centers for Disease Control and Prevention (CDC). Advances in Prostate Cancer Treatment Submitted by Illinois Oncology, Ltd.
A
ccording to the American Cancer Society, prostate cancer is the second most common cancer in men in the U.S. and the second leading cause of cancer-related deaths in men. Each year one in six men will be diagnosed with prostate cancer during his lifetime, and one in 36 will die from the disease.
PROVENGE One of the latest advancements in treatment options for prostate cancer patients is Provenge, the first-in-class immunotherapy. Provenge is the only FDA-approved treatment for certain men with advanced prostate cancer that uses the patient’s own immune system to fight the disease. This therapy is designed to stimulate a patient’s immune system to identify and target prostate cancer cells. Each dose is manufactured specifically for each patient using his own immune cells through leukapheresis. The leukapheresis procedure itself takes place at an American Red Cross facility. PROVENGE is not a chemotherapy drug and is different from hormone therapy, which decreases the level of testosterone in the body. PROVENGE is an immunotherapy that works by stimulating the natural ability of immune cells already in the patient’s body and may help patients live longer. A typical PROVENGE course of treatment can be completed in six weeks and the most common side effects are mild or moderate. PROVENGE Indication and Safety PROVENGE is an autologous cellular immunotherapy indicated for the treatment of asymptomatic or minimally symptomatic metastatic castrate resistant (hormone refractory) prostate cancer.
PROVENGE is intended solely for autologous use and is not routinely tested for transmissible infectious diseases. The safety evaluation of PROVENGE was based on 601 prostate cancer patients in four randomized clinical trials who underwent at least one leukapheresis. The most common adverse events (incidence greaterthan or equal to 15%) are chills, fatigue, back pain, nausea, joint ache, and headache. Serious adverse events reported in the PROVENGE group include acute infusion reactions (occurring within 1 day of infusion) and cerebrovascular events. In controlled clinical trials, severe (Grade 3) acute infusion reactions were reported in 3.5% of patients in the PROVENGE group. Reactions included chills, fever, fatigue, asthenia, difficulty breathing, hypoxia, bronchospasm, dizziness, headache, hypertension, muscle ache, nausea, and vomiting. No Grade 4 or 5 acute infusion reactions were reported in patients in the PROVENGE group. A
re droopy eyelids affecting your vision? Do your eyes feel tired and are you having difficulty keeping your eyes open to do daily tasks or drive? As we age, it is not uncommon to develop extra skin around the eyelids, known as dermatochalasis. It is not uncommon for lower lids to become affected also as the malar cheek pad descends. You may experience “bags” or excess skin in the lower lid from either a laxity in the skin or a prolapse of the fat pads. This condition will only get worse if not corrected. An upper eyelid blepharoplasty is usually considered a cosmetic procedure unless the heavy lids or detached levator muscle causes a decrease in the field of vision. In those cases, insurance companies may consider the blepharoplasty a necessary procedure. With some local anesthetic, the surgeon will make an incision along the upper eyelid crease so it is essentially invisible with the eyes open. The surgeon will determine the amount of skin and muscle to be removed. If the levator muscle is detached, it is reattached to the tarsus muscle. The whole procedure usually takes less than an hour.
Repair of the lower lids is called a lower lid blepharoplasty and is considered a cosmetic procedure because it does not affect the field of vision. Fat pads and excess skin are removed by making an incision along the crease near the eyelashes. Sometimes the lid is very lax and may need to be tightened at the same time. The eyelid tightening is called a canthopexy. Surgery on the lower lids usually takes 60-90 minutes. Recovery time is usually very quick with minimal discomfort. The surgeon uses dissolving sutures in both upper and lower lid surgery so there are no stitches to be removed. AMH Goes Back to School Through BJC Outreach Program
I
t’s never too early to teach area students a few things about healthy living. Through the help of a $35,000 annual grant from the Alton Memorial Health Services Foundation, Alton Memorial Hospital is able to offer health education programs from BJC School Outreach and Youth Development for several schools in the hospital’s service area. “Each program is designed to address Illinois Learning Standards and target a particular grade level,” says Diana Wilhold, Director of the BJC School Outreach and Youth Development Department. “There are programs in many schools and communities that reach a lot of youth. Unfortunately, many of these programs do not have a formalized method of evaluating the effectiveness of what is being taught. Our programs include pre-post assessment and we have found that there is a 40 percent to 57 percent improvement in knowledge, attitude and behavior change for the various programs.” Programs address the six high-risk health behaviors identified by Centers for Disease Control and Prevention and a comprehensive health career exploration program. Lower Extremity Amputation By Kathy Stephens, PT, Director, The Rehabilitation Center at Anderson Hospital
I
n the United States, there are approximately 1.7 million people living with limb loss. An amputation is the removal of part or all of a body part. In a lower extremity amputation, this could be the removal of the lower leg at the level of the hip, thigh, knee, calf, ankle, foot, or toes.
There are two types of amputations: Non-traumatic: The amputation is the result of a problem inside the lower leg, such as decreased blood flow and or/healing ability in the leg or foot. Common causes are diabetes, peripheral vascular disease, non-healing wounds, tumors, or infections. Risk factors include obesity, tobacco use, alcohol consumption, high blood pressure, and high cholesterol. Therefore, the best way to decrease the risk of having a lower extremity amputation is by preventing problems in the legs. This includes maintaining a healthy weight, eating a balanced diet, getting regular exercise, and practicing good skin care. Traumatic: The amputation is the result of an injury outside the lower leg. Common causes are motor vehicle accidents, gunshot wounds, and war injuries. The loss of function following an amputation is different for every person. This is due to the level of the amputation and the overall health of the person prior to the amputation. Some of the more common problems following an amputation are tightness in the residual limb from lack of use (contractures), pain in the residual limb/ phantom pain (feeling of pain from the missing limb), problems with healing after the amputation, difficulty with balance and walking, increased fall risk, emotional changes or depression. Some lower extremity amputees are candidates for a prosthesis or artificial limb. Learning to use a prosthesis takes time, strength, and patience. What you will be able to do while wearing a prosthesis will depend on the level of your amputation. With a below-knee amputation, doing what you used to do takes 25-40% more energy than it used to. With an above-knee level amputation, it may take 68-100% more energy. Because doing what you used to do will take so much more energy, exercising, and staying healthy are very important. Your strength, overall health, and determination are even more important than the level of the amputation. Technology has greatly advanced in the world of prosthetics, allowing an amputee to perform almost any activity or sport that they want to do. Losing a limb is a life-changing event. After an amputation, the individual will experience different emotions, such as grief, denial, anxiety, frustration, and helplessness. These feelings and fears are normal and part of the healing process. Support groups offer a chance for people to share stories, share feelings, and feel connected to other people who have gone through similar events. What Should I Know Before My First Gynecology Appointment? Submitted by Kimberly Sanford, MD, Maryville Women’s Center
A
t the Maryville Women’s Center, we have young teen girls and women of all ages that brave that first visit to the obstetrics and gynecology (Ob/Gyn) office. The ages range from as early as 12 years to as late as age 28 or 29 years. In other cultures and countries, young women are not necessarily encouraged to see an OB/Gyn unless they are having a baby or having difficulties. Prior to the first visit to the Ob/Gyn office there are several questions that are pretty common among these young women: • When should I have my first gynecologic visit? • Is it normal to be nervous before? • What should I expect at the first gynecologic visit? • What exams are performed? • What actually happens during an external genital exam? • What are the pelvic exam and the Pap smear? • What are common vaccinations given in the Gyn office? • What special concerns can be discussed with the Ob/Gyn? • What can I do to stay healthy?
The purpose of this article is to offer insight into these common questions. The American College of Obstetricians and Gynecologists (ACOG) recommend that young women, age 13 to 15 years, should have their initial visit to the Ob-Gyn. This is about one to three years after the average start of the menstrual period. The patient is, usually, accompanied by her mother, but in some rare instances, her father. That visit is accompanied by much fear and trembling by both parent and child. This can be soothed with the knowledge that they don’t necessarily need an exam. I find that basic education about menstrual cycles, treatment of menstrual symptoms, and the HPV vaccines offers a tremendous amount of material for the first-timer and even their parents. We understand that this visit represents a transition in a young woman’s life. If the patient is requesting hormonal birth control for either cycle control or to prevent pregnancy, this visit is a little more advanced. If the patient is requesting hormonal birth control medications, we will collect a clear and frank sexual history along with the general medical history. At this point we try to make clear to the teen that mom or dad can be asked to step into the waiting room. It is extremely important that all answers to these questions are candid. The exam that is performed depends on sexual activity and the physical complaint that may have brought the teen into the office. If an exam is warranted, the basic pelvic exam (Fig.1) is explained thoroughly then performed with care. The difference between a pap smear (Fig. 2), the cervical cancer screening test, and the pelvic exam is explained in detail. The pelvic exam is the entire Gyn exam, and the Pap smear is a very specific test that is utilized during the pelvic exam. No woman requires a pap smear until she is 21 years old, unless she becomes pregnant prior to that age. The Pap smear is a test that looks for abnormal cells on the cervix that are caused by the Human papilloma virus or the virus that causes genital warts. An abnormal pap smear can lead to cervical cancer, which was a leading cause of death in women prior
to the start of the Pap smear in the 1920’s. It continues to cause high mortality in other developing countries that don’t, necessarily, have access to health care as we do. There are two vaccines on the market that can be given for two to four types of HPV prevention. These four types of HPV cause 70% of the cervical and pelvic disease, but it should be made clear that there are over 80 different types of HPV that exist. Finally, there are many topics that can be covered during that first visit to the gynecologist. Those topics range from cramps and menstrual periods, acne, weight gain/loss, sex and sexuality, birth control, STD’s (sexually transmitted diseases), alcohol drugs and cigarette smoking, and emotional ups and downs. Although we are specialists, the Ob/Gyn falls under the umbrella of primary care. We look forward to meeting the needs of women. alvin May, an Associated Press stringer, is known as Tony La Russa’s First Question Guy for the St. Louis Cardinals baseball team. He also relies on insulin injections to preserve his life. An Edwardsville native, May is excited to be the team captain of Team Red for the Metro East Step Out: Walk To Stop Diabetes. Cardinals’ First Question Guy In 2004, La Russa, then the St. Louis Cardinals’ manager, was looking for May, normally the first to ask him questions in the press-
room after games. May was nowhere to be found, and La Metro East Step Out: Russa missed him. May had Walk To Stop Diabetes. been painting at his home on his day off. The Cardinals’ victory in last year’s World Series was one of the most spectacular moments of his life, said May, who interviewed MVP David Freese after Game 6 when he won the game with a home run. “Who would think what David Freese did in Game Six hitting one out of left field would ever happen?” May said. “It was like a dream come true.” Many don’t know this, but during the last innings of the game, May goes back behind the press box area and injects himself each time with long-lasting insulin. He doesn’t talk about it much, although he said he did speak about it with La Russa on a couple of occasions. May, age 49, has had diabetes since he was a year old. In sixth grade, May gave himself his first injection, prior to a dinner with his basketball team. Before that, his mother had given him the shots. “I had to learn how to give myself the shot, or I didn’t go,” he said. rivate Duty services provide a customized plan of care that can include: nursing visits, home health aides, companion care, and homemaker services. These services are combined and designed to respond to the individual needs of those who require assistance with the activities of daily living— allowing the individual to live independently, often bridging the gap between independent living and institutional care. The need is so great for these services that the private duty segment of in-home health is rapidly growing. ore than one million older adults will lose their spouse this year, the Social Security Administration estimates, joining the 11 million widowed adults living in the U.S. The loss of a spouse can be overwhelming for the remaining partner and for other family members that will help their loved one navigate through the bereavement process. “People who are grieving often feel alone in their grief,” says Passages Hospice chaplain, David Weiss. “To receive the support of others helps them realize that they don’t have to carry the whole burden themselves. There are others who are truly interested and willing to listen.” If you know someone who is dealing with grief, the American Hospice Foundation has some recommendations. Be patient and understanding. Grief will diminish with time, but there is no “right” length of time to grieve. Continue being supportive even if the person seems to be “doing better.” Expect the unexpected. Some people experience delayed grief, especially if the loved one was ill for a long time or there were many
details to arrange after the death. Grief usually surfaces at some point, and he or she may still need help even if it’s weeks or months later. Don’t neglect physical needs. Grief puts stress on the immune system, leaving the body more susceptible to illnesses. Suggest an appointment with a primary care physician who knows that he or she is grieving. “It’s okay to have that feeling of loss, and to recognize it and accept it,” Weiss says. “In essence it’s a reflection of the love we have for that person and their memory.” All hospice providers offer bereavement support for the families of patients for 13 months after the death.
