O
f the many orthopedic issues which bring patients to physical therapy, one of the more common is a total knee replacement. Almost everyone has heard the horror stories from family or friends about physical therapists straight from the Tower of London who mercilessly work to bend and straighten your knee. While it is true that regaining functional range of motion after knee surgery is not a cake walk, thanks to a piece of rehabilitation technology the range of motion aspect of therapy following surgery can be more akin to a social hour than a game of tug of war with your therapist. Isokinetic units (machines which move at a fixed speed that are utilized for joint strengthening) have been around since the 1950s. Not long after their use became more common in rehabilitation and exercise, it was discovered that these machines could be used as a slow and controlled form of joint passive range of motion. Instead of the patient exerting a force against an “arm” moving at a fixed speed, the patient could simply kick back and relax and let the machine do moving for them. The Biodex System is one of the more popular forms of these machines. Many people who have received a knee replacement are familiar with the CPM unit, a machine placed on the knee after surgery to increase range of motion of the joint. The patient lays in bed with their affected leg in the CPM secured with straps at the thigh and shin. The far end of the machine slides on a rail towards the person causing the knee to bend at the “joint” in the machine in an accordion like fashion. For the majority of patients this experience is less than enjoyable. The Biodex System, in contrast, includes a chair, a control unit, and a movable arm. The patient is placed in the chair and then adjusted so that the bend of the knee is roughly aligned with the axis of rotation of the moving arm. After ensuring proper alignment, the machine can then be adjusted to alter the speed of the moving arm, how long the hold time is when the knee is in the maximum amount of stretch, as well as which range of motion to emphasize depending on whether a patient is lacking in bending or straightening out of the knee. There are many features of this process which patients find enjoyable. The controls allow for gradual adjustment into increasing amounts
of bending and straightening of the knee. With varying treatment times, this allows the patient to begin in a relatively small, pain-free range of motion and gradually work into greater amounts of stretch based on their personal level of tolerance. With the user friendly controls, the patient can take an active role in therapy and control the amount of stretch placed on the knee. Another benefit of this type of range of motion is, with consistent set up by the therapist, it provides for a visual measure of progress for the patient beyond the standard verbal milestones given in terms of degree of bend. Returning to prior levels of functioning after a total knee replacement is often times a slow and effortful process. However, the addition of the Biodex System to the early stages of total knee replacement rehabilitation can make the regaining of range of motion in the clinical setting more enjoyable for both the patient and their physical therapist. earing a diagnosis of cancer is frightening, but hearing that its pancreatic cancer is devastating! I know all too well the emotions and the reality of this deadly disease. My father was diagnosed in June 2010, by August he was gone. Everyone diagnosed with any cancer deserves a chance to fight. Early detection is the key to survival. Like most people, when my father was diagnosed I knew very little about the disease. And through my search to empower myself, I learned that it is the least researched, least funded, and has the lowest survival rate of all the major cancers. My question is how can this be since it is the 4th leading cause of death among

the top 10 cancers, and one of the cancers that even doctors fear the most? 74% of people will die within the 1st year; the 5 year survival rate is only 6%. Imagine this, 100 people in a room, 5 years later only 6 people remain. This is a statistic that is hard to ignore. In my search I came across an organization called The Pancreatic Cancer Action Network. This is a national organization creating hope to increase the survival rate for people diagnosed with pancreatic cancer. They’re doing this is in a comprehensive way, through patient support, community outreach, and advocacy for a cure. In Oct. 2010, 2 months after my father died, I knew that

It has been over a year now since my father died; my heart is healing, and the grief is not as deep. My tears have turned into a passion to help fight so that other families will not have to watch their fathers, mothers, sisters or brothers die from this horrible disease. My hope is, someday on a podium somewhere, there will be survivors of every cancer, with numerous pancreatic cancer survivors dressed in “purple” and they will proudly say, “I fought pancreatic cancer, & I WON! Wild Rice Stuffing with Grapes and Hazelnuts
W
hat can possibly make the holidays easier and fun, more delicious and sweet, more festive and bright, and yet traditional all at the same time? The answer is fresh grapes. Luckily, grapes from California are in plentiful supply in all colors — red, green and black — throughout the holiday season. For a dinner or buffet, fresh grapes can be added to two favorite sides, stuffing and cranberry relish, with delightful and satisfying results. There are many familiar stuffing ingredients in Wild Rice Stuffing with Grapes and Hazelnuts, but the distinction in the dish is the wild rice, the toasted hazelnuts and fresh grapes. The grapes add a warm, still-juicy brightness to the stuffing, one that’s perfect with any kind of poultry, from turkey to game hens to goose. In Grape Cranberry Relish, two seasonal fruits work very well together. Grapes provide natural sweetness, so the the amount of sugar that’s added to quiet the strong tartness of cranberries is much reduced compared to similar recipes. The relish is a nice blend of still-noticeable grapes in a sauce that’s remarkably luscious. Yields: 6 1/4 cups 2 cans (28 fluid ounces total) low-sodium chicken broth 1 cup wild rice 4 slices bacon, diced 1/4-inch 1 tablespoon butter, unsalted 1 large onion (1 1/2 cups chopped), diced 1/4-inch 2 stalks (3/4 cup chopped) celery, diced 1/4-inch 1 1/2 cups button mushrooms, sliced 2 cloves garlic, fresh, minced 1 teaspoon thyme, fresh, minced 1/4 teaspoon salt 1/4 teaspoon black pepper, ground 1/4 cup parsley, fresh chopped 1 cup hazelnuts, toasted and coarsely chopped 2 cups California seedless grapes, rinsed In heavy saucepan, bring chicken broth to a boil. Add wild rice and stir. Cover pan and reduce heat to low. Let simmer for one hour, until rice is tender and has popped open. Meanwhile, cook bacon in fry pan over medium until almost crisp, about 8-10 minutes. Add butter to bacon pan, then add onions, celery, mushrooms, garlic, thyme and salt and pepper. Cook over medium heat for 5 to 7 minutes, until onions are translucent. Remove from heat and fold in parsley, hazelnuts and grapes. Add rice with any remaining liquid in pan to vegetable mixture and toss well to combine. Stuffing may be used to fill turkey, game hens, or chicken. Stuffing may also be baked separately in a casserole dish covered with foil; bake at 350°F for 20 to 30 minutes until hot throughout. To toast hazelnuts, place in 350°F oven for 9 to 12 minutes. Rub in a clean dishtowel to remove skins. Note: If you make ahead and refrigerate, cook time will be 30 to 40 minutes until hot.

Yields: 3 cups 2 cups red seedless California grapes 1 12-ounce package cranberries, fresh or frozen 1 orange, juiced and zested 1/2 cup sugar Place all ingredients in heavy saucepan and bring to a boil over high heat. Reduce heat to medium and cook, stirring occasionally for 10 minutes until thick and shiny. Let cool thoroughly, then store in airtight container in refrigerator for up to one week. Serve alongside roast turkey, pork or ham.

Fib is a common condition in the heart that causes the atria - the upper two chambers of the heart - to beat very fast and irregularly. When this happens, blood isn’t pumped through the heart as well as it should be, which makes people with AFib vulnerable to the formation of a blood clot in the atria. AFib is the most prevalent sustained cardiac arrhythmia (heart irregularity) in the United States, affecting more than 2.2 million Americans. In the past 20 years, there has been a 66 percent increase in hospital admissions for AFib, due to the aging population. Looking ahead, it is estimated that AFib will affect more than 12 million people in the U.S. by 2050. What are the symptoms of AFib? AFib can produce a broad range of symptoms, or none at all. Because it is characterized by an irregular and fast heart rate, patients often experience unpleasant palpitations or discomfort in the chest. The inefficient pumping ability of the heart may cause weakness, faintness, and shortness of breath. How do I know if I’m at risk? Age is a major risk factor for AFib; the average age of people with AFib is about 75, and approximately 70 percent of those people are between 65 and 85 years of age. Underlying heart disease and having had prior heart surgery are the most common risk factors of AFib. It is possible to have AFib when there are no other signs of a heart disorder. Other risk factors include diabetes, high blood pressure, hyperthyroidism (too much thyroid hormone), and excessive alcohol consumption.
AFib and stroke risk During AFib, the atria (upper chambers of the heart), don’t pump all of the blood in the ventricles (lower chambers of the heart). Some blood pools in the atria. When this happens, a blood clot can form. If the clot breaks off and travels to the brain, it can cause a stroke. People with AFib are at a five-fold increased risk for stroke compared with the general population. At least 15-20 percent of all strokes occur in patients with AFib. Several factors are considered when assessing the risk of a stroke for a person with AFib. People who have had a prior stroke are at high risk. Additional factors that increase risk of stroke in people with AFib include being 75 or older, high blood pressure, heart failure and diabetes. How can I manage stroke risk with AFib? People with AFib have treatment options to help reduce the risk of stroke, including medication that thins the blood to help prevent blood clot formation. Unfortunately, nearly half of people with AFib do not receive appropriate preventive medicine to help protect against the risk of stroke. Where can I go for more information? A Patient-Friendly Procedure The specialized technology of Smartlipo TriPlex™ helps ensure a quick and easy treatment process with very little patient recovery time. According to Dr. Savells, the procedure’s use of the latest technology, the smallest incisions and only a local anesthetic keep bruising, swelling and downtime to a minimum. Patients can take off work one day and then return to work and their regular activities the very next day, so there’s very little downtime. The procedure generally lasts between one and two hours, depending upon the area of the body that requires treatment. Smartlipo TriPlex™ is performed as an outpatient procedure in the office, eliminating hospital costs for patients. Patients also save money as there is no cost for anesthesia. In our confidential, one-on-one counseling sessions, a certified counselor will assist you with housing-related issues, debt, credit problems, and bankruptcy education. We also offer community workshops on a variety of topics including buying a home, credit and budgeting. Although we are best known for working with clients with severe debt, the majority of our counseling helps people learn how to budget, use credit wisely, prevent becoming a victim of identity theft, or the steps to take before buying your first home. ClearPoint also partners with other like-minded organizations to promote consumer health through financial education. To prepare for your counseling, and have more time to ask your counselor questions, gather your most recent credit card statements and correspondence you’ve had with collection agencies or the I.R.S. and make a list of your sources of monthly income and expenses (such as food, utilities, gas, clothing, gifts and other miscellaneous expenses). Also, prepare any questions you have for your counselor. It also helps to think about your specific financial goals so you can share them and provide for them in your pending budget planning. During your free, confidential session—which lasts about an hour— you and your credit counselor will work together to develop a plan which will address your household budget, creditors, and savings. Your counselor will pull your credit report (with your permission) and conduct a review of your situation and your financial goals. Depending on your personal circumstances, your counselor could provide you with many various suggestions. He or she may tell you about the debt management program, advise you to speak with an attorney about your debt, send you off on your own to see if you can follow your budget, refer you to other resources, or help you determine how you might increase your income or reduce your expenses. You will usually have time to ask your most burning questions as well.
Once your credit counseling session has been completed, your counselor will give you a written overview of your current financial picture, including an Action Plan detailing your personal goals and corresponding strategies to meet them. most important money management basics such as what elements make up your credit score, personal budgeting forms and assessing your debt-to-income ratio. RETHINKING Holiday Drinking Alcohol-related Traffic Deaths Jump on New Year’s Eve
S
ome New Year’s predictions are, tragically, very reliable. For example, more people are likely to die in alcohol-related traffic crashes on New Year’s Eve than on other mid-week winter evenings. Just look at the numbers. A recent analysis of National Highway Traffic Safety Administration statistics shows that, by the time our country finished ringing in the year 2011, 59 people had died in alcoholrelated traffic crashes in the 12-hour span between 6:00 p.m. on New Year’s Eve and 5:59 a.m. the next morning. Two weeks later, on the same night of the week, the death toll dropped to 13. Myths Persist Even though many of us are aware of the higher rates of alcoholrelated traffic fatalities on New Year’s Eve, myths about drinking and Page 16 — Healthy Cells Magazine — Western Kentucky — December 2011
driving persist—myths that, for some, can prove fatal. Scientific studies supported by the National Institute on Alcohol Abuse and Alcoholism (NIAAA) on how alcohol affects our brains and bodies provide important information that challenges these commonly held—but incorrect— beliefs. These myths are related to how quickly alcohol affects the body and how long these effects can last. Alcohol’s Effects Begin Quickly Many New Year’s revelers get into trouble because they generally do not recognize that critical driving-related skills and decision-making abilities are diminished long before they begin to show the obvious physical signs of intoxication. Initially, alcohol acts as a stimulant and if you drink you may temporarily feel upbeat and excited. But don’t be fooled. Inhibitions and

But certain facts are clear—there’s no way to speed up the brain’s recovery from alcohol and no way to make good decisions when you are drinking too much, too fast. On an evening like New Year’s Eve (or any other evening, for that matter), rapid consumption of large amounts of alcohol is especially dangerous. During a migraine, the blood vessels in the brain expand in a process called vasodilatation. As the tissues surrounding the brain swell, the pain increases. Sleep Tips for
Children and Infants S
leep is vital for children’s overall health and development. That’s why it’s important that children develop good sleep habits, right from the start. “Parents of infants need to know how to help their baby safely fall asleep,” says Robert W. Block, MD, FAAP, of the American Academy of Pediatrics (AAP). “And older children will benefit from an environment that helps them get enough sleep.” Here are some tips for safe and adequate sleep from the experts at the AAP: Safer Sleep Babies up to 1 year of age should always be placed on their backs on a firm surface to sleep. This will reduce the risk of Sudden Infant Death Syndrome, or SIDS, which is the leading cause of death in the United States for babies between 1 month and 1 year old. “While we don’t know what causes SIDS, we have learned how parents can dramatically lower the risks,” says Dr. Block. Make sure the crib, bassinet or play yard meets current safety standards and hasn’t been recalled. And make sure to keep all objects — including soft toys, blankets and crib bumpers — out of the crib, as they can increase the risk of suffocation or strangulation. Consider using a sleeper instead blankets, and make sure the baby’s head remains uncovered. The crib can be in the same room as you sleep, but do not place the baby in the same bed as you. Warm, Not Hot Keep the room where your baby sleeps at a comfortable temperature. In general, dress your baby in no more than one extra layer than you would wear. Your baby may be too hot if she is sweating or if her chest feels hot. You may offer a pacifier, which can help reduce the risk of SIDS. However, other products like wedges, positioners, special mattresses and specialized sleep surfaces have not been shown to reduce the risk of SIDS. If you are breastfeeding, wait until breastfeeding is going well before offering a pacifier, usually around 3 to 4 weeks. Children should be taken out of a crib by the time they are 35 inches tall. Establish Routines If your child or teen seems to be having trouble sleeping, try altering the environment or establishing a routine. For example, see if your child sleeps better in a dark room or with a night light. Do not allow a TV in your child’s bedroom, and make sure he or she doesn’t watch or read anything upsetting or scary within two hours of bedtime. Instead, a bath, warm drink or story time will help a child unwind. For more tips to help your young one get a good night’s sleep, visit the AAP’s website for parents, www.healthychildren.org. ast month we touched upon the idea that grievers develop a relationship to their pain, as if their identity hinged on regarding themselves as unhappy. Many people become so familiar with those painful feelings that they are afraid to let them go. If it were not so sad, and if it did not have such dire consequences, one would be tempted to draw a cartoon of someone clinging desperately to a horrible looking creature called “pain,” terrified of losing it. And yet, that is exactly what it looks like. Some of us have very long-term relationships to pain. We may have unresolved loss-of-trust experiences from childhood that keep us in an almost perpetual state of acceptance of pain as a permanent condition. Many of us keep dragging the unfinished relationships of our past into all of our new relationships, and then acting surprised when they always end the same. We may be ill-equipped to deal with the feelings caused by the end of each new relationship, and we may be unaware that almost all of our past relationships are incomplete or unresolved. Some of us are able to acknowledge that we have sabotaged many relationships. While we have the intellectual awareness that we are the common denominator in the sabotages, we find ourselves unable to change our behavior. If the intellect were the key to successful recovery then we would be able to think ourselves well. We would be able to understand ourselves into better actions. Clearly that does not work. Unresolved grief is cumulative and cumulatively negative. Incomplete relationships create unresolved grief & unresolved grief creates incomplete relationships. Incomplete relationships can cause us to limit our lives, can induce us to sabotage good relationships, and can encourage us to keep choosing poorly. Unresolved grief becomes a relationship to ourselves as unworthy of happiness. We must learn how to grieve and complete relationships that have ended or changed. It may sound simple, and it is simple. Why then, do so many people resist taking the simple and clearly defined actions of Grief Recovery®? The opening paragraph of this article referred to how familiar we become with our pain. Familiarity can create a powerful illusion that change is not necessary, that growth is not possible, and that where happiness is concerned, 20% equals 100%. “Am I equipped for happiness?” Yes, but I am much more familiar with pain. As the direct result of years and years of practice, I am expert at identifying and relating to pain. Happiness is an unwelcome intruder in how I relate to myself. We have all searched desperately for the key to happiness. While it may sound simplistic to say that we held the key, ourselves, all along, it is true. Access to our own happiness is directly linked to our ability to grieve and complete our relationships with people and events, as well as our ability to grieve and complete our relationship to the pain we generate when we are reminded of the unhappiness we have experienced in our lives. Many of us say, over and over, that if only this or that would happen I could be happy. The thing might be love or money or success
or fame. And yet, how often do we get the very thing we wanted and wind up as unhappy as we were before, and even more disillusioned? To rediscover your ability to be happy, you must go back and grieve and complete all of the incomplete relationships from your past. As you do so, you will begin to find your normal and natural desire and ability to be happy. You may have heard people talk about stripping away the layers of an onion; we prefer the analogy of stripping away the leaves of an artichoke, and discovering your heart inside. QUESTION: I have had many painful loss experiences in my life. Sometimes I feel as if there is no way I can ever let down my guard and allow any positive or happy experiences in. Will the principles and actions of Grief Recovery® help me change this fearful habit? ANSWER: In clearly identifying your behavior as a habit, you increase the probability of growth and change. The idea of changing a habit is probably less intimidating than the idea of changing a behavior. In truth, most of our behaviors are habits that we have practiced so well and so often that they seem like our nature. Many of our survival habits were developed when we were quite young. Often we are managing an adult life with the limited skills and perceptions of a small child. As we grieve and complete the events and the behaviors of our pasts, we become open to our ability to be happy. Western Kentucky Residents Called to Contribute to Historic Cancer Research Effort
Involvement Can Answer Critical Questions About Cancer Causes, Prevention Submitted by American Cancer Society
R
esidents from throughout Western Kentucky have an unprecedented opportunity to participate in a historic study that has the potential to change the face of cancer for future generations. Men and women between the ages of 30 and 65 who have never been diagnosed with cancer are needed to participate in the American Cancer Society’s Cancer Prevention Study-3 (CPS-3). CPS-3 will enroll a diverse population of up to half a million people across the United States and Puerto Rico. This rare opportunity for residents from throughout the region to enroll in CPS-3 will take place at the Relay For Life of Warren County, home to Bowling Green, next June 1, 2012. CPS-3 will help researchers better understand the lifestyle, environmental, and genetic factors that cause or prevent cancer. To enroll in the study, individuals complete two steps, one in person and one at home. As part of the in-person enrollment, individuals complete a brief written survey, provide some physical measurements, and give a small blood sample. The enrollment process is complete when individuals complete the more comprehensive baseline survey. Over the course of the study – which is anticipated to last 20 to 30 years – participants will be asked to fill out follow-up surveys every few years that will be sent to their home. “Many individuals diagnosed with cancer struggle to answer the question, ‘What caused my cancer?’ In many cases, we don’t know the answer,” said Alpa V. Patel, Ph.D., principal investigator of CPS-3. “CPS-3 will help us better understand what factors cause cancer, and once we know that, we can be better equipped to prevent cancer.” Page 24 — Healthy Cells Magazine — Western Kentucky — December 2011
Dr. Patel added, “Our previous cancer prevention studies have been instrumental in helping us identify some of the major factors that can affect cancer risk. CPS-3 holds the best hope of identifying new and emerging cancer risks, and we can only do this if members of the community are willing to become involved.” Researchers will use the data from CPS-3 to build on evidence from a series of American Cancer Society studies that began in the 1950s that collectively have involved millions of volunteer participants. The Hammond-Horn Study and previous Cancer Prevention Studies (CPS-I, and CPS-II) have played a major role in understanding cancer prevention and risk, and have contributed significantly to the scientific basis and development of public health guidelines and recommendations. Those studies confirmed the link between cigarette smoking and lung cancer, demonstrated the link between larger waist size and increased death rates from cancer and other causes, and showed the considerable impact of air pollution on heart and lung conditions. The current study, CPS-II, began in 1982 and is still ongoing. But changes in lifestyle and in the understanding of cancer in the more than two decades since its launch make it important to begin a new study. The initial enrollment process takes about 30 minutes at the local event and an additional 45 to 60 minutes at home to fill out the more comprehensive baseline survey. Periodic follow-up surveys of various lengths are expected to be sent every few years to individuals. The voluntary, long-term commitment by participants is what will produce benefits for decades to come. “Taking an hour or so every few years to fill out a survey – and potentially save someone from being diagnosed with cancer in the future - is a commitment that thousands of volunteer participants have already made. We’re looking for more like-minded individuals in Western Kentucky to join this effort that we know will save lives and improve the outlook for future generations,” said Dr. Patel.
