Unode los grandes sueños de la humanidad, especialmente de los defensores de la conservación del medio ambiente, ha sido la posibilidad de utilizar el poder nuclear para generar energías limpias a tiempo indefinido.

La buena noticia es que el Departamento de Energía de los Estados Unidos (DOE) y su Administración Nacional de Seguridad Nuclear (NNSA) anunciaron el logro de la ignición por fusión en el Laboratorio Nacional Lawrence Livermore (LLNL).

Tienen razón ambas instituciones cuando aseguran que se trata de un importante avance científico que lleva décadas en desarrollo y allanará el camino para los avances en defensa y el futuro de la energía limpia.

Un equipo de la Instalación Nacional de Ignición (NIF) de LLNL realizó el primer experimento de fusión controlada en la historia para alcanzar este hito, también conocido como punto de equilibrio de energía científica, lo que significa que produjo más energía de fusión que la energía láser utilizada para impulsarla.

Pero el anuncio hecho por el departamento de energía de Estados Unidos confirma que existe una ruta limpia para la generación de energía limpia y que las inversiones

realizadas durante años han empezado a rendir frutos, por lo que es necesario continuar dotando a esos proyectos de recursos para acelerar la llegada de aplicaciones prácticas.

Más allá de consideraciones políticas, la búsqueda de energías limpias debería ser una política de estado en todos los países del mundo. Both institutions are right when they say that this is a major scientific breakthrough that has been decades in the making and will pave the way for advances in defense and the future of clean energy.

A team from LLNL's National Ignition Facility (NIF) conducted the first controlled fusion experiment in history to reach this milestone, also known as the scientific energy equilibrium point, meaning it produced more fusion energy than the energy laser used to drive it.

As the DOE explains, fusion is the process by which two light nuclei combine to form a single heavier nucleus, releasing a large amount of energy.

“This monumental scientific breakthrough is a milestone for the future of clean energy,” California Senator Alex Padilla told CNN, adding, “While there is more work ahead to harness the potential of fusion power, I am proud that California scientists continue to lead the way in the development of clean energy technologies.”

The reality is that this discovery in the laboratory cannot be used immediately for the purpose of commercial electricity generation.

Several scientific teams in other countries around the world seek precisely to apply this type of nuclear fission technology to materialize projects for the practical use of this energy, but there is still a long way to go, and it is an arduous and complex scientific task.

But the announcement made by the United States Department of Energy confirms that there is a clean route for the generation of clean energy and that the investments made over the years have begun to bear fruit, so it is necessary to continue providing these projects with resources to accelerate the arrival of practical applications.

Beyond political considerations, the search for clean energy should be a state policy in all countries of the world. Unlike other races, such as the arms race, the race to replace fossil fuels with clean energy represents the historic possibility of a sustainable future.

REPORT: HOW SCHOOLS CAN LURE STUDENTS BACK TO COMMUNITY COLLEGE

During the pandemic, California community colleges lost 19% of their enrollment, but as a new report found, a handful of schools bucked the trend.

Researchers with the Campaign for College Opportunity found some schools had smaller losses or even added students by focusing on equity and expanding outreach.

Colleen Moore, higher-education research and policy consultant who interviewed dozens of college administrators for the study, explained financial support was helpful.

"All the colleges I spoke with talked about expanding financial supports in some way," Moore recounted. "Through things like transportation and book vouchers as a means of helping low-income students stay enrolled."

Schools also relied on data analytics to see where students were stopping out and refine course schedules, curriculum, programs and institutional policies to better meet student needs. They also increased partnerships with local employers to offer students a path to a job after graduation, and tried to create a more supportive campus culture.

Moore added many of the schools expanded their efforts to reach out to current, former and prospective students.

"Some people talked about explicit efforts to divide up their list of students to contact based on race or ethnicity," Moore noted. "And assigning the lists of staff that share that background as one aspect of their efforts to make their campus more welcoming to all students."

TIPS TO AVOID THE LATEST HOLIDAY SCAMS

Withthe holiday shopping season in full swing, scammers are hard at work, so the experts have some tips on how to shop smarter.

The most common types of scams these days are online shopping scams, empty gift cards and package delivery scams, according to AARP FraudWatch.

Kathy Stokes, director of fraud prevention programs for AARP, cautioned you should never click on a link directly from social media or an unsolicited email or text.

"If you get an email from, let's say Amazon or Etsy or something, instead of clicking on that link that they give you go and type that address into your web browser yourself," Stokes suggested. "That way, you're sure that the link isn't malicious, you know, it could be a really good fake. "

Scammers are also tampering with gift cards and placing them back on the rack, so the card gets drained shortly after a person pays for it at the reg-

ister. Experts advised pulling your gift card from the middle or back of the rack, and then examining it closely to lower your chances of buying one which has been altered.

Stokes pointed out authorities are also getting more reports of people receiving a scam text message purportedly from FedEx, U-P-S, or the Postal Service, saying there is a problem with your shipment and giving you a link she warned you should never click.

"Quite often what they're trying to do there is capture your keying on your keyboard -- capturing your logins, your passwords, your user account names -- and all of that is intended to steal your money right from your accounts," Stokes explained.

Scammers like to target people who get regular income from the government, so veterans and those receiving disability payments should be especially wary.

Doyou suffer from itchy eyes, a runny nose or a scratchy throat during winter? Do you often find yourself coughing, sneezing and having trouble breathing? Chances are you may have indoor allergies.

Allergens are not just found outside in trees or grass. The indoors can host many allergy triggers like mold related to water leaks, pet allergens, and dust mite and cockroach allergens.

Indoor allergies are a year-round problem, but you'll notice them more during the winter because you spend more time at home. Also, because of the colder weather, you're likely keeping doors and windows shut to keep the cold out. By doing that, you're also sealing allergens indoors with you, and your heating system will recirculate the indoor air and any allergy triggers.

You don't have to accept indoor allergies as an inevitable fact of the winter season. Using these five tips from the American College of Allergy, Asthma and Immunology (ACAAI), you can be proactive and manage your winter allergies.

1. Clean regularly

It may seem like a simple solution, but cleaning house is one of the easiest ways to deal with indoor winter allergies. Dust, mold, pet dander and dust mite and cockroach droppings can hide anywhere and cause sneezing, coughing and congestion.

To limit your exposure to these indoor allergens, keep a regular cleaning schedule. Dust and vacuum frequently, especially under and behind furniture, where allergens can easily build up. Wipe down counters and other surfaces that you use often. During the holidays, wipe off holiday decorations, especially if you've stored them in areas where dust and mold proliferate.

2. Reduce pet allergy triggers

It can be difficult to balance your love for your pets and indoor allergy management. Just like you, your pets are spending more time indoors, increasing your exposure to pet dander and saliva.

While you may enjoy snuggling with your pets before bed, keeping them out of the bedroom will help with any allergies you experience at night that can disrupt your sleep. During the day, keep the door to your bedroom closed, so your furry family members aren't tempted to go in.

When petting your cat or dog, make sure to wash

your hands afterward, so you don't accidentally touch your face and provoke an allergic reaction. Finally, remember to groom your pets regularly to reduce indoor air allergens.

3. Protect your bedding from dust mites

According to the ACAAI, dust mites live and multiply in warm, humid places and are often found in pillows, mattresses, carpeting and upholstered furniture. Even if your home is visibly clean, dust mite allergen particles are too small to be seen.

Use mite-proof zippered cases for your mattress and pillows to limit your exposure to dust mites. Every week, wash and change your sheets, blankets and other bedding in hot water to eliminate any lingering dust mite droppings.

4. Find and prevent indoor mold

Did you know that there are roughly 1,000 species of mold in the U.S.? Many molds aren't visible to the naked eye, and mold spores can cause allergic reactions when they become airborne. Because mold can grow in your basement, bathroom and under the sink, avoiding mold allergens can be difficult.

If you suspect you have indoor mold, it's best to contact a professional who can find and clean up existing mold in your home. To prevent mold related to water leaks, quickly stop and clean up any standing water. Also, make sure to reduce moisture and humidity in bathrooms and kitchens. If humidity control is a problem, consider a dehumidifier.

5. Find an allergist

Consulting an allergist is the best way to identify and treat your winter indoor allergies.

"Allergists are the best trained medical professionals to treat allergies and asthma," said Kathleen May, M.D., ACAAI president. "They can help you get tested, get treated and get better so you can enjoy the holiday season and beyond."

After a consultation, your allergist may perform tests to diagnose your allergies and identify potential causes. An allergist can also prescribe a variety of treatments to give you relief from your allergic symptoms.

Vivir en las calles de California es un asunto mortal. La expectativa de vida de una persona desamparada es de 50 años, según estimaciones nacionales, casi 30 años menos que la del californiano promedio. A medida que la falta de vivienda se sale de control en todo el estado, también lo hacen las muertes en las calles, pero son aquellos cuyas vidas son más frágiles los que tienen menos probabilidades de recibir atención médica.

“Maldición. Feldman, un asistente médico, está buscando a una paciente de unos 40 años con asma grave y no controlada. Ella entra y sale del hospital de manera predecible, y Feldman sabe que pronto tendrá otra hospitalización.

El camino está surcado por el agua de un camión de limpieza y los trabajadores sanitarios con chalecos fluorescentes barren los escombros. Brindar atención médica de esta manera es costoso y no es particularmente efectivo para el paciente o el sistema. Más de la mitad del presupuesto de Medi-Cal de $133 mil millones del estado se gasta en el 5% superior de los usuarios con grandes necesidades, según el Departamento de Servicios de Atención Médica de California.

“Donde nos hemos quedado cortos, especialmente con esta población, es que su realidad es tan diferente a la nuestra que no hemos estado construyendo sistemas basados en la realidad para ellos”, dijo Feldman. “Tienen Medi-Cal. Hace varios meses, la paciente de Skid Row de Feldman sufrió una lesión cerebral por falta de oxígeno durante un ataque de asma. Ahora está confinada a una silla de ruedas y depende de un amigo para sus necesidades básicas, como encontrar comida y usar el baño.

Los medicamentos más nuevos para el asma podrían ayudar a terminar su ciclo de hospitalización, pero hasta hace poco solo su médico de atención primaria asignado, a quien nunca había visto, podía derivarla a un especialista para que la evaluara según las reglas bizantinas de Medi-Cal. Feldman había estado tratando de conseguirle una cita de atención primaria durante más de un año, sin éxito.

Sin embargo, según las nuevas reglas, Feldman podría haber remitido al paciente asmático directamente al especialista en pulmones que necesitaba u obtener una autorización previa para el medicamento, ya que se recomendó durante una estadía en el hospital. En cambio, sin atención médica adecuada para tratar su condición, su vida ha sido alterada irrevocablemente.

Estadísticamente, tendrá suerte de vivir más de unos pocos años más.

“Ella solía ser un elemento básico aquí. Ella conocía a todos”, dijo Feldman. “Ahora no puede caminar, está confinada en su tienda. Se siente sola porque está acostumbrada a ser parte de la comunidad de Skid Row. Tuvo una vida muy plena a pesar de no tener casa”.

Feldman, cofundador y director del programa de medicina callejera de la Facultad de Medicina Keck de la Universidad del Sur de California, dijo que el objetivo de la medicina callejera es devolver cierta autonomía a las personas a las que normalmente les queda muy poco poder en sus vidas.

Menos del 30% de las personas sin hogar con Medi-Cal han visto alguna vez a su proveedor de atención primaria, según un análisis legislativo estatal de un proyecto de ley de medicina callejera vetado por el gobernador Gavin Newsom en 2021. Vínculo entre la falta de vivienda y la salud

Los datos de salud sobre las personas sin hogar son escasos, sin una agencia estatal y solo un puñado de condados rastreando la información, pero está claro que la mayoría de sus muertes se pueden prevenir.

En los condados de Alameda y Marin, la mitad resultan de condiciones de salud agudas o crónicas como enfermedades cardiovasculares, cáncer o insuficiencia respiratoria. En el Condado de Orange , estos representan una cuarta parte de las muertes entre las personas sin hogar. En el condado de Los Ángeles , la enfermedad cardíaca es la segunda causa principal de muerte entre las personas sin hogar, solo superada por las sobredosis.

Incluso las muertes por sobredosis se consideran prevenibles; sin embargo, en San Francisco, las sobredosis causan el 82% de las muertes entre las personas sin refugio.

“Comúnmente vemos condiciones que se verían en una población típica, pero simplemente no se abordan; así que la presión arterial alta fuera de control, la diabetes no controlada… también el uso de sustancias en términos de opioides, vemos mucho más que en la población general”, dijo el Dr. Absalon Galat, director médico del Departamento de Servicios de Salud del Condado de Los Ángeles. El equipo de Galat comenzó su incursión en la medicina callejera en un esfuerzo por repartir vacunas contra el COVID-19, pero los miembros del equipo pronto descubrieron que necesitaban hacer más. El condado usó fondos de ayuda de COVID-19 para comprar clínicas móviles, y los fondos de CalAIM les ayudaron a contratar a 60 miembros del personal.

Tomemos como ejemplo a Danny Doran, de 56 años, quien visitó la clínica móvil del condado de Los Ángeles en Whittier Narrows Park un jueves reciente para recoger insulina. Pasó su carrera como plomero y era dueño de una casa en Bishop. Hace tres años cayó en coma diabético y estuvo hospitalizado durante meses. Un amigo en el que Doran confiaba para pagar sus cuentas mientras estaba hospitalizado vació su cuenta bancaria y desapareció. Doran ha estado sin hogar desde entonces. Hace varias semanas fue golpeado y robado por otro vagabundo, quien lo dejó con una fractura en el cráneo y temblor en las manos.

“Supongo que soy un poco ingenuo”, dijo Doran. “Todos somos humanos y somos propensos a cometer errores, ¿sabes? Así que odio que a alguien le roben su dinero como al mío y termine como yo en las calles”.

En la clínica móvil, Doran dijo que el médico del personal aceptó ser su médico de atención primaria. Su anterior médico de atención primaria dejó de aceptar el seguro de Medi-Cal y, desde entonces, Doran no ha tenido acceso regular a la insulina.

“La doctora aquí, realmente tiene compasión por sus pacientes. Me alegro de que nuestros caminos se hayan cruzado”, dijo Doran.

Schneidermann, quien supervisa un estudio sobre programas de medicina callejera en California, dijo que CalAIM, que también paga los servicios de vivienda, es una oportunidad para que el estado aborde su problema más pernicioso.

“Hasta que podamos poner fin a nuestra

crisis de personas sin hogar… tenemos que encontrar una manera de brindar atención a las personas en las calles”, dijo Schneidermann.

Aparecen nuevos programas

Antes de los cambios en las reglas de CalAIM y del Departamento de Servicios de Atención Médica, los programas de medicina callejera operaban fuera de la atención médica tradicional, financiados por organizaciones filantrópicas o por organizaciones de salud raras dispuestas a perder dinero. Ahora, los cambios del departamento ofrecen alguna esperanza de estabilidad, dijo Feldman.

Al señalar que estos programas nacieron de la pandemia, Feldman dijo que “podrían no existir en unos años si no cuentan con el apoyo, pero tienen todos estos pacientes que dependen de ellos”.

Hace un año, solo existían 25 programas en todo el estado, principalmente concentrados en áreas urbanas, dijo Feldman. Pero desde que se lanzó CalAIM a principios de 2022, se ha topado con más organizaciones que buscan comenzar a prestar servicios. Visitan seis campamentos cada dos fines de semana.

THE HOMELESS HEALTH CARE CRISIS

on the streets of California is a deadly affair. The life expectancy of an unsheltered person is 50, according to national estimates, nearly 30 years less than that of the average Californian. As homelessness spirals out of control throughout the state, so too do deaths on the street, but it’s those whose lives are the most fragile who are least likely to get medical care.

Living

Now, the state Medi-Cal agency is endeavoring to improve health care access for people experiencing homelessness. Through a series of incentives and regulatory changes, the Health Care Services Department is encouraging Medi-Cal insurers to fund and partner with organizations that bring primary care into encampments.

They’re known as street medicine teams. There are at least 25 in California.

“Oh crap. This is where she was, and they just swept that,” said Brett Feldman on a Friday morning in November, looking at a green tent, crumpled and abandoned on Skid Row in Los Angeles. Feldman, a physician assistant, is searching for a female patient in her 40s with severe and unmanaged asthma. She cycles predictably in and out of the hospital, and Feldman knows she’s due for another hospitalization soon.

The road is streaked with water from a cleaning truck, and sanitation workers in fluorescent vests sweep up debris. Parking enforcement and police cruisers line the section of road where a homeless encampment once stood. Nearly 5,000 people live in the half-mile block infamous for the hypervisibility it affords the state’s unrelenting homeless crisis.

Burdened by disproportionate rates of addiction, mental health disorders and chronic disease, people experiencing homelessness are some of the state’s neediest patients, but few receive anything more than emergency services. Barriers like lack of transportation and cumbersome insurance

rules keep most from getting regular health care. Instead, they drift through the emergency room during a crisis, racking up high costs to the system and deteriorating physically in the interim.

Delivering health care this way is costly and not particularly effective for the patient or the system. More than half of the state’s $133-billion Medi-Cal budget is spent on the top 5% of high-needs users, according to the California Department of Health Care Services.

“Where we have been falling short, especially with this population, is their reality is so different from ours that we haven’t been building reality-based systems for them,” Feldman said. “They have Medi-Cal. They’re eligible for all these benefits, but they can’t access these benefits.”

The state’s efforts to bridge the gap between eligibility and access is supported in part through CalAIM, a multi-year plan to revamp the state’s low-income health insurance program. Grants to hire staff or invest in billing or data collection software offer some stability to teams that have historically been volunteer- or charity-operated. The department also issued a rule change in November allowing street medicine teams to tap into and manage homeless patients’ Medi-Cal benefits, meaning providers can be reimbursed for their work.

“One of our core principles of CalAIM is breaking down the walls of health care and meeting people where they are,” said Jacey Cooper, director of the state’s Medi-Cal program. “We really feel like street medicine helps us do that.”

Row patient suffered a brain injury from lack of oxygen during an asthma attack. She’s now confined to a wheelchair and reliant on a friend for basic needs like finding food and using the toilet.

Newer asthma medications might be able to help end her hospitalization cycle, but until recently only her assigned primary care doctor, whom she has never seen, was allowed to refer her to a specialist for assessment under Byzantine Medi-Cal rules. Feldman had been trying to get her a primary care appointment for more than a year, to no avail.

Under the new rules, however, Feldman could have referred the asthmatic patient directly to the lung specialist she needed or gotten prior authorization for the medication since it was recommended during a hospital stay. Instead, without adequate medical care to address her condition, her life has been irrevocably altered.

Statistically, she’ll be lucky to live longer than a few more years.

“She used to be a staple down here. She knew everybody,” Feldman said. “Now, she can’t walk, is confined to her tent. She’s lonely because she’s used to being part of the Skid Row community. She had a very full life despite being unhoused.”

Feldman, co-founder and director of the street medicine program at the University of Southern California’s Keck School of Medicine, said the goal of street medicine is to give some autonomy back to people who usually have very little power left in their lives.

Each day he and a team of providers scour the county streets diagnosing chronic and acute conditions, treating mental illness and substance use, delivering medicine, drawing blood for tests and following up with patients who request a visit. Community health workers hand out food and hygiene supplies and help them navigate hurdles as they try to obtain housing and social services.

“We know that people who are experiencing homelessness have higher mortality, have higher ER utilization, have higher length of stay when they get admitted,” Cooper said. “We really see this as part of a comprehensive approach to ensuring that we have a true continuum of care for people experiencing homelessness.”

The traditional health care system thrives off efficiency: The more patients move through an office, the more the provider gets paid, resulting in brief appointments and little sympathy for circumstances that make patients late. But that setup doesn’t work for unsheltered people who run the risk of getting their belongings stolen if they leave their camp — or who would rather find something to eat than take care of what may seem like a minor malady.

Less than 30% of unhoused people with Medi-Cal have ever seen their primary care provider, according to a state legislative analysis of a street medicine bill vetoed by Gov. Gavin Newsom in 2021. The measure passed the legislature with broad support but was opposed by the state Health Care Services Department for potential duplication of services. In the veto, Newsom directed the department to work with street

medicine teams to fill any gaps left out by CalAIM — one such gap was adjusting billing codes that prevented street medicine reimbursement.

“When you’re focused on those very basic needs, like food, safety, shelter, how are you then able to focus on, you know, managing your diabetes or your blood pressure or some of these risk factors that can lead to more serious downstream effects?” said Dr. Kyle Patton, medical director of the street medicine program at Shasta Community Health Center in Redding.

On a Monday in September, Patton and Anna Cummings, a case worker, trekked through a wooded area on the north edge of town to meet Amber Schmitt, 47, a patient with an infected leg. The ground is muddy from a storm the night before. Schmitt is paying a friend $700 a month to stay in his apartment, but hidden among the trees and rolling hills is her abandoned encampment, along with dozens of others. Schmitt gets $1,000 a month from Social Security, but it’s not enough to afford a security deposit or rent in the area, she said.

The gash on Schmitt’s right shin is mottled and inflamed. She said she scraped it on a fallen branch. Patton cleaned and dressed it for her previously, but she had no choice but to reuse bandages after running out. Now she can barely walk from the pain.

“This is a silver-based dressing, which will kill bacteria in wounds,” Patton tells Schmitt after rinsing the area with a saline wash. “We’ll get you some more dressing too. And then you’ve got some skin breakdown and maceration between your toes. I don’t like the look of that.”

He gives her a fungal cream and a bottle of antibiotics. Schmitt is a leukemia survivor and has had a hip replacement on the same injured leg. Her medical history makes her prone to infections and poor circulation in her extremities, Patton said.

“There’s people that would maybe make the argument that … they have health insurance here in California, they should just utilize the system as is. The reality is because of certain factors within the context of their homelessness, they’re not able to do that,” Patton said.

Although they qualify for comprehensive health coverage under Medi-Cal, the program wasn’t necessarily designed with homeless people in mind. For example, Medi-Cal will pay for transportation to and from a doctor’s appointment, but it requires the patient to provide a fixed address and give several week’s notice to the driver, something most people experiencing homelessness aren’t able to arrange.

Link between homelessness and health

Health data on homeless people are sparse, with no state agency and only a handful of counties tracking the information, but it’s clear that most of their deaths are preventable.

In Alameda and Marin counties, half result from acute or chronic health conditions like cardiovascular disease, cancer or respiratory failure. In Orange County, these make up a quarter of deaths among the unhoused. In Los Angeles County, heart disease is the second-leading cause of death among

people experiencing homelessness, second only to overdoses.

Even overdose deaths are considered preventable — yet in San Francisco, overdoses cause 82% of deaths among the unsheltered.

“We commonly see conditions that you would see in a typical population, but they’re just not addressed; so out-of-control high blood pressure, uncontrolled diabetes…also substance use in terms of opioids, we see a whole lot more than in the general population,” said Dr. Absalon Galat, medical director for LA County’s Department of Health Services’ Housing for Health division.

Galat’s team started its foray into street medicine in an effort to dole out COVID-19 vaccines, but team members quickly found they needed to do more. The county used COVID-19 relief funds to purchase mobile clinics, and CalAIM funding has helped them hire 60 staff members.

In September, the county’s fleet of mobile clinics, complete with fully outfitted exam rooms, began visiting areas where services are sparse. Smaller teams of clinicians and case workers roam encampments to follow-up with patients, treat minor issues and bring patients to the mobile clinic. There’s some disagreement among street medicine providers about whether mobile clinics remove enough barriers because they still require patients to travel to a set location, but Galat said his goal is to improve access, whether it’s by wheel or foot.

“People are dying every day,” Galat said. “So we have to try with what we know best in the medical field right now to limit people who are dying.”

The connection between homelessness and health is inextricable, said Dr. Michelle Schneidermann, director of the PeopleCentered Care team at the California Health Care Foundation, a statewide health policy think tank.

“Either one can lead to the other. A catastrophic health incident or a series of conditions can lead to someone not being able to work, leading to poverty,” Schneidermann said. “We see this all the time, health conditions precipitating homelessness, and the other way around.”

“Until we can end our crisis of homelessness…we have to find a way to deliver care for people on the streets.”

Take Danny Doran, 56, who visited LA County’s mobile clinic at Whittier Narrows Park on a recent Thursday to pick up insulin. He spent his career as a plumber and owned a home in Bishop. Three years ago he fell into a diabetic coma and was hospitalized for months. A friend Doran trusted to pay his bills while he was hospitalized emptied his bank account and disappeared — Doran has been homeless ever since. Several weeks ago he was beaten and robbed by another homeless man, who left him with a fracture in his skull and a tremor in his hands.

“I guess I’m a little bit naive,” Doran said. “We’re all humans and we’re prone to mis-

takes, you know? So I hate for anyone to have their money stolen like mine was and end up like me on the streets.”

At the mobile clinic, Doran said the doctor on staff agreed to be his primary care physician. His previous primary care doctor stopped accepting Medi-Cal insurance, and Doran hasn’t had regular access to insulin ever since.

“The doc here, she truly has compassion for her patients. I’m glad our paths crossed,” Doran said.

Schneidermann, who is overseeing a study on street medicine programs across California, said CalAIM, which also pays for housing services, is an opportunity for the state to address its most pernicious problem.

“Until we can end our crisis of homelessness…we have to find a way to deliver care for people on the streets,” Schneidermann said.

New programs popping up

Prior to CalAIM and the Health Care Services Department’s rule changes, street medicine programs operated outside of traditional health care, funded by philanthropies or the rare health organization willing to lose money. Now, the department’s changes offer some hope for stability, Feldman said.

Noting that these programs were birthed out of the pandemic, Feldman said they “might not exist in a few years if they’re not supported, but they have all these patients that rely on them.”

A year ago only 25 programs existed across the state, primarily concentrated in urban areas, Feldman said. But ever since CalAIM launched at the beginning of 2022, he’s run into more organizations looking to begin services. CalAIM requires Medi-Cal insurers to coordinate patients’ physical, behavioral, dental and developmental care as well as social services — something many street medicine teams already do. The goal is to make the “system hustle behind the scenes rather than making the patient hustle,” California Health Care Foundations’ Schneidermann said.

One such program is run by Anthony Menacho in Sacramento. Unlike USC, Shasta Community Health or LA County’s teams that are staffed full time, Menacho’s street medicine band is composed entirely of volunteers. They visit six camps every other weekend.

The work was funded initially by a $100,000 grant from Health Net, the largest Medi-Cal provider in the state, but Menacho, who trained as a physician assistant with Feldman at USC, wants to be able to do the work full time and hire more clinicians. He’s working to secure money through CalAIM and the Department of Health Care Services.

“We don’t have the academic resources or people behind us to be able to put in a department or infrastructure on the drop of the dime,” Menacho said. “We run on grants, but that’s not true sustainability. We can’t do it ourselves. It has to be a coordinated effort and I think that’s what CalAIM is trying to do.”

Theadvent of the internet and smartphone innovations have brought information to our fingertips. As the number of users skyrockets, advertisers have identified lucrative opportunities to meet people where they are. Google is a household name, holding 87 percent of the market share and hosting billions of users. Polls of Google users show that they trust that it produces credible search results. Seventy-three percent of users believe that most or all of the information they find in a search is accurate and trustworthy; however, 68 percent of users were unable to identify the difference between an ad and an organic search result. This gives a leg up to potentially nefarious advertisers spreading climate disinformation.

Google addressed the problem of pervasive climate disinformation prior to the United Nations (UN) Conference of the Parties (COP) 26th annual meeting in 2021; the internet giant pledged to halt the display of ads that promoted climate disinformation. This commitment came in tandem with other advertisers pressuring Google not to display their products alongside climate disinformation.

The commitment that Google made has come under fire as there are questions about the legitimacy of their pledge. Michael Koo, co-chair of the climate change disinformation coalition at Friends of the Earth, stated, “It appears Google is spreading disinformation about its efforts to fight disinformation”. In the span of two years, Google has accepted nearly $24 million in advertisements from oil giants. Of that $24 million, $10.9 million was spent on greenwashing ads that were seen 58.6 million times, according to the Center for Countering Digital Hate (CCDH). These ads were targeted to key-word searches including: greenhouse gases, renewable energy and information about “eco-friendly” companies. Additionally, since their pledge, Google has accepted $421,000 for ads from climate denial groups with taglines like “climate campaigners hype the risks of global warming,” and “fossil fuels make the planet safer”.

CCDH has outlined steps that Google needs to take to actualize the claims they’ve made on their stance on climate disinformation. Step one would be to halt climate denial ads and ads promoting greenwashing. Additionally, CCDH suggests that Google introduce a transparent library of their advertisements so that the public can examine how ads are being placed and how they may distort search results. Lastly, CCDH suggests that a legislative framework be established to ensure the transparency, accountability and responsibility of not only Google, but all online platforms.

These recommendations are a good starting point, but there is no guarantee that Google will follow them. That being said, users should be armed with skills to identify climate disinformation. Common tactics that proponents of climate disinformation utilize are: false expertise, logical fallacies, impossible expectations, conspiracy theories and cherry-picked data. The best way to combat these tactics is to be a cautious consumer of online information. Be prepared to vet the source of the

information that you find, and get in the habit of developing a mixed-news diet.

DEPRESSION HIGH AMONG LGBTQ POPULATION DESPITE DEAL, RAIL WORKERS SAY CONDITIONS STILL ROUGH

The Census Bureau has released a report that shows LGBTQ people suffered higher rates of depression, anxiety and other mental hurdles than did the general population during the pandemic - and an Iowa survey has dug a little deeper.

The census figures show that 18- to 29-year-olds reported symptoms of anxiety that were almost 18% higher than the general population during the pandemic.

A related survey by advocacy group One Iowa shows that 50% of LGBTQ respondents in Iowa report mental-health issues as their number one concern.

One Iowa project director Malycki Monon-Sosa said the problems don't end there.

"About 40% of the folks," said Monon-Sosa, "the LGBTQ Iowans that took that survey, reported being victims of violence all based on their LGBTQ identity."

The Iowa Health Needs Assessment also showed half of the respondents who reported struggling with mental-health issues are seeking counseling - but that a staggering 55% have considered committing suicide, with many of those saying they had plans to do so in the past year.

Monon-Sosa said Iowa's aging population has contributed to stagnant attitudes about LGBTQ people in Iowa.

He added that One Iowa is working with state agencies and other advocacy groups, to try to end stereotypes that he says are starting to change among young people - as alternative lifestyles be-

come more acceptable.

"Our young population is becoming more and more diverse," said Monon-Sosa, "and if we hold onto the notion that everyone will never change and the way that society looks now will be the way that society looks forever, we start to push people who are more diverse to the margins of our society."

The Iowa Health Needs Assessment will be ongoing for five years, and works under the direction of the state Department of Health and Human Services.

Nick Katich - the Minnesota legislative director for the Sheet Metal, Air, Rail, and Transit (SMART) Union, the largest freight railroad labor organization - said workers aren't threatening to walk off the job right now.

But he said they want people to know that working conditions are draining morale.

"Our folks are just tired, they're fatigued," said Katich. "They can't go on like this. We can't recruit new employees."

The union is calling on President Joe Biden to take executive action to address paid leave.

Beyond that, labor leaders say worker safety is another big concern, as rail companies opt for smaller crews to create efficiencies.

The Association of American Railroads says it hopes to address these concerns, but adds the new deal should help with scheduling issues and deliver pay raises.

On the safety issue, Katich said trains are operating with more cars - making them more susceptible to mechanical failure. He said that creates hazards when some trains operate with just one crew member.

"There's kind of a task of overload," said Katich, "as crew size has been reduced and more technologies have been added."

To enhance safety, the Federal Railroad Administration has proposed a rule to require a minimum of two train crewmembers for over-the-road rail operations, with some exceptions.

That process is now in the public comment phase.

Q&A: BEYOND BABYLON

this: movie stars, tuxedos, jazz music, nudity, alcohol, drugs, an elephant and a party that seems to last forever. That is the introduction we get to the world of writer-director Damien Chazelle’s newest film, Babylon

It is at this raucous party where the two main protagonists first meet, Margot Robbie plays aspiring actress Nellie LaRoy and Diego Calva portrays Manuel “Manny” Torres, a Mexican-American film assistant. Amongst a comically large pile of cocaine, Nellie asks Manny, “If you could go anywhere in the whole world, where would you go?” Manny responds, “I always wanted to be part of something bigger. Something that lasts, that means something. Something more important than life.”

Babylon aims to end 2022 with a massive epic about the wild times of Hollywood’s burgeoning golden age in the 1920’s and the demise of the depravity in the 1930’s. It’s a dynamic reflection of the characters going through extreme highs and fathomless lows that glistens and glows for its three-hour running time.

Led by Brad Pitt, Margot Robbie and Hollywood newcomer Diego Calva, the cast is extensive and the story is ambitious.

The story is set in 1920s Los Angeles where the movie business is still in its infancy and has not yet become the tightly controlled studio system at the height of the golden age of Hollywood that lasted from around the 1930’s-1960’s.

It is a place where lavish parties, illegality and debauchery are the main pastime of the people involved in the movie business, and Chazelle’s epic does not shy away from showing all of it in its worldbuilding.

To find out some behind the scenes details of Babylon, we recently had the opportunity to speak to Margot Robbie and Diego Calva about their experiences working on a movie about actors, what parts of the journey reflected their own careers, how they build up their chemistry as co-leads, and how Robbie’s experience helped newcomer Calva into the world of modern Hollywood.

Babylon is in theaters on December 23, 2022.

To start off, I wanted to know, like, basically, what is it like being an actor working on a film that's about being an actor working on films?

It's so surreal. Yeah, we were literally a film crew filming a film crew making a movie. And that was not just in one scene. That was in a number of instances. I think the best part about it I mean, it's just strange to look at, but it's also just hilarious and wonderful. But the best part about doing that is getting to convey to audiences that feeling that I keep trying to describe to people when they're like, "Oh, is acting fun? Is it hard to learn your lines?" That's actually not even the tip of the iceberg when it comes to being on a film set.

And I wish I could transport everyone onto a film set so that you could see - it's like sitting in the eye of the storm and there's this beautiful, insane chaos around you. And when you capture a moment and everyone's working, it's such a community thing and it's so crazy that everyone just wants to talk to actors at the end of it, because I'm like, "we are a tiny cog in this massive machine." And, yes, we were there, but there were actually hundreds of people that we're all really close with and we're all moving in this, like, synchronized dance to achieve this one magical moment.

And you get to actually see how that happens in Babylon. And then I hope that audiences feel that thing that we feel when you do achieve that on set because there is no greater feeling. And when our characters say in the movie like, "Being on a film set is the most magical place in the world," I really, really, really believe that. And in this movie, audiences get to be on a film set and feel that magic.

Diego Calva

As Margot was saying, [it’s] like, beautiful and very meta, but also the relationship between me and my character put another layer into that kind of metafiction. My first day on set as Diego Calva in a Hollywood set with Brad Pitt, with Margot, with Damien, with 800 extras, it was my character's first day on set. I think I felt like a very, very tiny line between me and Manuel all the time. For example, the emotions you see in Manuel's face…all the emotion was real. So that face, I don't know if he's Manuel or it was me.

Margot

Yeah, there was a lot of those moments. I think for Nellie, she has that moment when everyone's clearly going, "Oh, she's not going to be able to do this right now." And that was always my drive when I'd walk into it before I did The Wolf of Wall Street, after that, everyone expected something great, and that's when actually acting got

kind of scary for me.

But before that, I would just always walk into an audition and people wouldn't be listening or, you know that they're like, "Ugh, look, she's going to be nothing, like, get her out of here," sort of thing. And in my head, it would just get me so fired up and be like, "I am going to blow your mind! I'm going to make you look up from that piece of paper!" And it was such a motivating aspect to my acting to begin with, is that I knew no one expected anything great, and I was just so determined to prove everyone wrong. And Nellie definitely has that. Everyone's like, "Who is this? She's just a random chick from the party." And she's just like, "Just you wait."

So, yeah, there are a lot of moments like that I definitely could kind of harness the energy that I've experienced in my own career and kind of put it into those scenes. And then there's those really lovely moments where Nellie's in a theater watching her film and then watching an audience react to it. And did something similar in Once Upon a Time in Hollywood with Sharon Tate going to the cinema as well. And I've done that, too, where I've gone to the cinema and just it's such a surreal moment. It's completely surreal. So, yeah, there were lots of moments that were very art, imitating life.

Now, I want to talk about your guys' collaboration on bringing Nellie and Manny to life. Just wondering how you guys worked on that chemistry and if you were leaning on Margot because you're a lot newer to the world of American Hollywood. What was that partnership like?

Margot

Brilliant. I mean, I think we worked so well together and we did chemistry reads to begin with and that's how we kind of knew that, A, we would have chemistry, but B, that we could kind of, like, support each other's acting styles in a way that would only get the best work out of us. And I think to do that, you have to kind of know what your acting partner is comfortable with. And with Diego, it's like there were no limits to what we could do or try.

We just had an understanding that we could do and try anything with each other. You never needed to hold back, which is so important to these characters. Like Nellie cannot hold back. I really needed a scene partner that I wasn’t going to tiptoe around in any way. And so I relied on him and needed him to push me so I could push him so we could get to these places. And that's honestly just a credit to Diego as an actor, he's so unbelievably talented, so present and such a supportive costar, like such a supportive scene partner where he's like, "I got it. Try it. Go for it. Do it again. Let's go."

El Día de los Tres Reyes Magos

We could almost do that without even saying anything very quickly into the shoot. We could look at each other and be like, "yeah, okay." We just had an understanding in that way. And I think we got the best - like some of the best acting I've ever done was with you and that's because you are so good, but also because we have that kind of that trust and that limitless sort of no boundaries with our working relationship.

Diego

And really believe that you made me an actor because I have acted before in so many things and everything, but never, like, with this kind of energy. And everything with Margot, I remember thinking so many times since the chemistry reading to our last scene, like "you have to do better. Look what she's doing. Like, just go more and higher and higher." And honestly, after Babylon, I even believe that I'm not a bad actor. I think I've grown a lot doing this.

And the trust, [Margot] just said the trust, like the confidence, that's so important because it was my first movie, it was my first time meeting Damien. It was my first time acting with a superstar. And I'm talking about the chemistry reading, and I was so nervous, so afraid to fail. Like, can you imagine? I just didn't want to blow this opportunity away. And from the first moment, when I realized, "okay, I can yell at her, she's committed." And [Margot] you just said there was something in the eyes, like, "do it, let's play." And I thank you, owe you so much.

Okay, last question. What does the title Babylon mean to you?

Margot

I think when you hear the word Babylon, you think epic. I think epic. You think of like D.W. Griffith's giant sets. And I mean, there's so many different, like, correlations throughout Hollywood history, but Babylon was also considered at that time like a city of debauchery and excess, which obviously, but then there's, of course, Hollywood Babylon, Kenneth Anger's book, which is just a fantastic read if anyone cares to look into it. Yeah, it makes me just think of, like, big, epic debaucherous, decadent insanity and then knowing that ultimately, it's all going to crumble in a way, but also stays forever. It stays forever. Exactly. Like the myth of it ends up being larger than the thing itself.

Diego

Yeah, to me it's that. But also, of course, the relationship with Griffith and everything. But also, I think it's like something burning, like coming to life. It's like the beginning. And Babylon is, for me, also like the beginning of Hollywood. There was nobody down there. It was like all little houses. [Babylon] makes you think about, like a whole castle, like something like Pompeii, like epic.

Para empezar, quería saber, básicamente, ¿Cómo es ser un actor que trabaja en una película que se trata de ser un actor que trabaja en películas?

Es tan surrealista. Sí, literalmente éramos un equipo de grabación grabando a un equipo de grabación haciendo una película. Y eso no fue solo en una escena. Ocurrió en varias ocasiones. Creo que la mejor parte de esto, quiero decir, es simplemente extraño de ver, pero también es divertido y maravilloso. Pero la mejor parte es transmitir al público ese sentimiento que sigo tratando de describir a las personas cuando dicen: "Oh, ¿actuar es divertido? ¿Es difícil aprender tus líneas?" En realidad, eso no es ni la punta del iceberg cuando se trata de estar en un set de filmación.

Y desearía poder transportar a todos a un set de filmación para que pudieran ver - es como sentarse en el ojo de la tormenta y hay este caos hermoso y loco a tu alrededor. Y cuando capturas un momento y todos están trabajando, es algo tan comunitario y es tan loco que todos solo quieren hablar con los actores al final, porque pienso, "somos un pequeño engranaje en esta máquina masiva". "Y sí, estuvimos allí, pero en realidad había cientos de personas con las que todos somos muy cercanos y todos nos movíamos en este baile sincronizado para lograr ese momento mágico.

Diego Calva

Como decía Margot, [es] hermoso y muy meta, pero también la relación entre mi personaje y yo pone otra capa en ese tipo de metaficción. Mi primer día en el set como Diego Calva en un set de Hollywood con Brad Pitt, con Margot, con Damien, con 800 extras, fue el primer día de mi personaje en el set. Creo que sentí como una línea muy, muy pequeña entre Manuel y yo todo el tiempo. Por ejemplo, las emociones que ves en la cara de Manuel... toda la emoción era real. Entonces esa cara, no sé si es Manuel o fui yo.

Margot

Sí, hubo muchos de esos momentos. Creo que para Nellie, tiene ese momento en el que todos claramente dicen: "Oh, ella no podrá hacer esto ahora". Y ese siempre fue mi impulso cuando entraba antes de actuar en The Wolf Of Wall Street, después de eso, todos esperaban algo grandioso, y fue entonces cuando realmente actuar me empezó a dar un poco de miedo.

Pero antes de eso, siempre entraba a una audición y la gente no escuchaba o, ya sabes, decían cosas como: "Uf, mira, ella no va a ser nada, como, sácala de aquí". Y en mi cabeza, me prendía tanto y decía: "¡Voy a dejarte boquiabierto! ¡Voy a hacer que levantes la vista de ese pedazo de papel!" Y fue un aspecto tan motivador para mi actuación, para empezar, sabía que nadie esperaba nada grandioso, y estaba tan decidida a demostrar que todos estaban equivocados. Y Nellie definitivamente tiene eso. Todo el mundo dice: "¿Quién es esta? Es solo una chica cualquiera de la fiesta". Y ella dice: "Solo espera". Entonces sí, hay muchos momentos como ese en los que definitivamente podría aprovechar la energía que he

experimentado en mi propia carrera y ponerla en esas escenas. Solo teníamos un entendimiento de que podíamos hacer e intentar cualquier cosa entre nosotros. Nunca necesitabas contenerte, lo cual es muy importante para estos personajes. Nellie no puede contenerse. Realmente necesitaba un compañero de escena que no iba a andar de puntillas de ninguna manera. Así que confié en él y necesitaba que me empujara para que yo pudiera empujarlo a él para que pudiéramos llegar a estos lugares. Y, sinceramente, eso es solo un mérito para Diego como actor, es tan increíblemente talentoso, tan presente y un coprotagonista tan compasivo, como un compañero de escena tan solidario que dice: "Lo tengo. Y realmente creo que me convertiste en actor porque he actuado antes en tantas cosas y todo, pero nunca, como, con este tipo de energía. Y todo con Margot, recuerdo haber pensado tantas veces desde la lectura de química hasta nuestra última escena, como "tienes que hacerlo mejor. Mira lo que está haciendo. Margot

Quiero decir, hay tantas correlaciones diferentes a lo largo de la historia de Hollywood, pero Babilonia también se consideraba en ese momento como una ciudad de libertinaje y exceso, obviamente, pero luego está, por supuesto, Hollywood Babylon el libro de Kenneth Anger, que es una lectura fantástica si a alguien le interesa leerlo. Margot Robbie Y Diego Calva Hablan Sobre Trabajar En Peliculas Sobre Peliculas, Crear Química Y La Esencia De La Actuación Margot, ¿Hubo un sentimiento similar para ti, esa membrana entre la ficción y la realidad en la que actuabas, pero tu personaje está sintiendo esa experiencia de hacer películas como tú la tienes en tu carrera?
