Lab on Demand
was founded in 2012 when Babar T. Rizvi, MT, MLS (ASCP) CM, MCLD decided to put his longstanding dream into action. Rizvi is the CEO Babar T. Rizvi and director of laboratory services of Lab on Demand and has over 30 years of experience as a lab director in the industry. While holding director roles for many years, Rizvi felt that there was a need for direct to consumer lab services so patients could take control of their health without always having a doctor’s order. “Usually, people only obtain lab tests when they have a doctor’s order to check their health. However, it is a good idea for patients to get lab work done to check their health and wellness to see how their cholesterol levels or vitamin levels are,” said Rizvi. They offer a comprehensive range of lab tests to
screen for arthritis, diabetes, drug use, heart health, hormones, liver function, thyroid function, STDs, cancer markers, and many more. Lab on Demand also can do male and female wellness profiles to ensure optimized health. Students can even obtain titers required for school through the lab and those requiring immigration medical titers can have the entire immigration medical exam done with a designated civil surgeon. With the wide range of services offered here, it really is a one stop shop! Lab on demand allows for patients to come in to the office for lab testing with no long waits or appointments during business hours. Lab on Demand also offers services to those who have doctor’s orders as they can provide affordable lab tests for those who have high deductible plans or are uninsured. “There are several patients who come in for lab tests and have high deductible insurance plans. If someone comes to Lab on Demand, they can avoid costly bills for lab work and save anywhere from 25-85% off of the original cost. Test results within 24-48 hours. Hassle Free Lab No Doctor’s order required. Order your own Lab test,” said Rizvi.

I have represented individuals suffering from serious injuries to the brain and body including but not limited to brain injuries, neck and spinal injuries, eye injuries, burn injuries, and injuries of the limbs including paralysis. I fight for payment of your past and future medical bills as well as compensation for your pain and suffering, permanent disfigurement, permanent impairment, scarring, cognitive impairment, emotional distress, and the overall loss of the quality of your life. PROACTIVE STEPS YOU CAN TAKE TO PROTECT YOUR PERSONAL INJURY CLAIM BEFORE YOU HIRE AN ATTORNEY When someone is injured in an accident, whether it’s an auto, motorcycle, or bicycle accident, a slip and fall, or some other circumstance caused by another’s negligence, that person is in shock. It is difficult to think straight or ask the right questions. Many times, a person is too injured to do anything at the time of their injury other than seek medical attention. But often, that is not the case. Knowing what to do beforehand can help you protect your claim. Here are some simple tips on what to do if you are injured:
Collisions involving moving vehicles, motorcycles, bicycles, trucks: Photograph the scene where the collision occurred before vehicles/motorcycles/trucks or bicycles are moved Photograph the damage on all vehicles/ motorcycles/trucks or bicycles before they are moved Provide a statement to the investigating officer explaining what occurred from your point of view
LINDA H. HAVEL A Trustworthy Laywer Who Cares Obtain a copy of the police report from www.Buycrash.com Notify your auto insurance carrier right away that you have been involved in an accident Don’t agree to allow your motor vehicle to be salvaged by your insurance carrier until you have had a chance to examine your vehicle and take photographs. If liability is in question, you may need the property damage information to help your case.
Slip and Falls:
Photograph the rug, mat, spill, step, hole, obstacle or other item or circumstance that caused your fall. If the slip occurred as a result of a mat or rug, photograph the underside of the mat or rug before leaving the scene Report the fall to a person with authority and provide your own written account - do not sign their written account Ask for a copy of the incident report before you leave the scene Ask for the name of the company’s insurance carrier and request that a claim for your medical expenses be opened pursuant to their med-pay coverage If the fall occurs at someone else’s home, ask for a copy of their homeowner’s insurance policy and/or the name of their homeowner’s insurance carrier
For all situations:
If requested, provide a tape recorded statement to your insurance company only – do not provide a tape recorded statement to anyone else’s insurance company or an investigator Seek immediate medical attention – do not wait and delay treatment Explain how you were injured to the attending physician Follow all of your doctor’s orders and keep a copy of all medical visits, prescriptions and records pertaining to you injury Know your health insurance coverage and keep track of your co-pays and expenses
If you have an injury and believe you may have a claim, I encourage you to contact me on my cell phone at 317-525-7754 or at my office at 317-576-8620 for a free consultation.

Eliminate Your Limiting Beliefs January is the month that has been designated to assess how you are doing in your life and what you may need to do to reach a greater potential! Are there things that have been tugging on you to change and Carol Juergensen Sheets you have been pushing them down to avoid the rigors of moving into unfamiliar territory? Many people come up with lots of reasons or excuses to stay stuck in their old routines. It can feel scary to identify some changes that will alter your life and so it is easier to stick to limiting beliefs that hold you back. When I work with people in coaching, we always work on their limiting beliefs because we all have them. Common limiting beliefs are mindsets that stop you from taking yourself to the next level. They may include:
• • • • • •
I am too old I am too young I don’t have enough money I am not smart enough Nothing ever works out for me No one will help me
These beliefs keep you from taking your life to the next level and as a result you may never change careers, get out of an abusive marriage, get into counseling, start your own business, have a child, ask for that promotion, lose weight, start that fitness routine or go back to school. But there are 3 spiritual practices that I know to be true and so I want to share them with you as assurance that you can move into this unfamiliar territory of change and self actualization.
The first principle is: Belief in Self = Achieving Your Goals. To achieve your goals means that you need to believe in yourself, so you need to invest in the resources and supports that are going to fortify your sense of self. The second principle is that you are not doing this by yourself. When you commit to changing your life for the sake of the greater good, your higher power or the universe will be your co-pilot and help to guide you in your endeavors. The 3rd principle is the toughest one to integrate because it sends you right to the area you are most afraid of in making a change. The principle is that what you fear really opens the door to what you need most in your life to access your higher purpose. So. let’s identify how these three principles work in real time. Let’s say that you want to start your own business but fear failure. It is important to do the internal work to understand what you need to do to create greater belief in yourself to make that happen? How would it benefit the world and what do you need to do to allow your higher power to assist you? What fears do you have that prevent you from taking the risk? Perhaps you fear that you won’t have the energy to make that happen. Could this fear open the door to finding a partner to help you start the business? What do you want to accomplish in your life? Are your limiting beliefs stronger than the belief in yourself? If you knew you weren’t in this alone would it strengthen your resolve that you could accomplish your goals. And if you looked at your obstacles as opportunities, could you have faith that doors were going to open in ways that would pave the way for your greatness?
You were born for greatness… but not until you face your fears and practice the principles!

History Center. The exhibit features dozens of new themes, from holiday staples such as the Sugarplum Fairy and Santa's sleigh to unique concepts such as popping champagne and the fictional Indiana town in Stranger Things. While the mood is festive, there's a friendly competition for fan favorite, with past winners such as the Indianapolis Zoo and Plow Digital keeping this year's idea top secret. As visitors tour the winter wonderland, they also can take part in scavenger hunts for The Elf on the Shelf or the Weihnachtzgurke, 10 pickle ornaments hidden in trees throughout the building. When legs get weary, guests are invited to take a break in the Cole Porter Room, where a singer leads guests through holidaythemed songs, as well as popular Porter standards. Special programs include the return of Twilight Tuesdays, with live music performed by local talent. Families are invited to mingle with their favorite Nutcracker characters at the Sugarplum Breakfast on Sunday, Dec. 2, and channel Ralphie at A Christmas Story Day on Saturday, Dec. 22.
