y now you have heard that Hosparus is Hosparus Health! We officially changed our name in January 2017 to start off the New Year and celebrated in local communities throughout the first quarter. But, before I elaborate on our future, we created this issue of Caring Connections to recap an exceptional 2016 through our Annual Report. For those of you who are regular recipients of our bi-annual Caring Connections, you’ll notice our expanded summer issue. We have so much to celebrate from each part of our organization that we decided to provide highlights from our Grief Counseling Center, volunteers, fundraising events and, of course, our patient and families. Families shared poignant testimonials about how we’ve touched their lives, and community leaders praised our services and pledged their continued support. This overwhelmingly positive response is a testament to the great work we do in all the communities we serve. As you read through this issue, take note of our impressive financial position. It is because of the contributions from our donors, as well as our finance team’s ability to maximize every opportunity for reimbursement of care, that we received an “impeccable” financial review from our auditors. Every program throughout Hosparus Health is managed by professionals that put our mission of improving the quality of life at the forefront of what we do.

VOLUNTEERS AT THE CORE OF OUR WORK. After all, it was volunteers who started Hosparus Health in 1978, and they continue to help improve the lives of our patients and families today. Our Volunteer Visitors, Pet Companions, and Grief Support volunteers help us meet people where they are and honor their choices. Patient care volunteers receive extensive training so that they can offer support to you and your loved ones and may be available on a regular basis or for a one-time request.

In 2016, the total number of volunteers reached 594.... providing 53,283 hours of support to patients and their families. The dollar equivalent of their generous gifts of time was $1,201,539.

GRIEF COUNSELING CENTER YOU ARE NOT ALONE. WE CAN HELP. Our Grief Counseling Center offers the area’s most comprehensive selection of programs and services for children, adolescents, adults and families coping with loss due to death. Individual, group and family services promote understanding of and healthy adjustment to the grief process. Programs and services are led by licensed professionals, masters level student interns working under supervision, and trained volunteers. AT THE FOREFRONT OF INNOVATION
T
hroughout our history, Hosparus Health has been the leader in end of life care. Today, we embrace the opportunity to once again lead in the innovation of advanced illness care. Adding the word “Health” to our name provides hope that there is something more. By adding Health to our name, we’ve added hope for patients and families dealing with a serious or life-limiting illness. We truly believe that the earlier you call, the more we can help. And our expanded focus under our new name now includes supportive palliative and advanced illness care so that hundreds more families can take advantage of our person-centered, multi-disciplinary program of exceptional care well before the end of life. “Hosparus Health is on the forefront of care for serious illness. We’re seeing the future and the direction that health care is moving, and we want to be the leaders for innovations in care,” says Denise Gloede, Hosparus Health’s new Executive Director of Palliative Care.
We know that advanced illness care (some call it palliative care) provides care that optimizes quality of life by anticipating, treating and even preventing disease symptoms to avoid patient suffering. Hospice Care - Just As Important As The New Model Of Advanced Illness Care To access hospice care, a patient must have a life expectancy of no more than 6 months. But in today’s world of new therapies and treatments, it is not always easy to put a time frame on an illness. Advanced Illness Care is not limited by a patient’s life expectancy; it can be provided at any stage of acute or chronic illness and care can also be provided as part of curative treatments. Just like in our Hospice program, each patient in our Advanced Illness Care pilot enjoys an interdisciplinary care team including physicians, nurse practitioners, nurses, certified nursing assistants, social workers, therapists, chaplains, volunteers and grief counselors. Our team strives to coach, teach and empower patients and families to better manage their overall health. To participate in the pilot, patients and families pay a small monthly fee to cover administrative costs. This fee is a fraction of the true cost of care and is available on a sliding fee basis. We are also in negotiations with private insurance companies to cover costs for our pilot on a case by case basis. In the short time since we launched this innovative pilot program, we’ve served a total of 61 patients in our adult pilot and 50 in our Kourageous Kids program. Our goal for 2017 is to serve a total of 75 adult patients and 75 pediatric patients.
This pilot enhances our mission of improving the quality of life and we know without a shadow of a doubt that this type of care changes lives…and saves money on healthcare costs. If we can show positive outcomes through our pilot program, we are confident Congress will take notice and approve an advanced illness care benefit as part of Medicare. Currently there is no insurance benefit that covers advanced illness care.
A Gap in the System Like we said earlier, there is no standard benefit available for advanced illness care through Medicare or private insurance. Hosparus Health’s pilot program fee is well below our actual costs, of about $1,500 per month. The average fee paid by a family in our pilot is less than $400 per month. We are choosing to invest in this type of care because we believe we can close the gap in our current health care system by providing improved health outcomes and illustrating the benefit of covering these services.

support in their homes. Hosparus Health is investing in innovation and testing this new model because we believe it is the future of care for chronic and life-limiting illness,” said Gloede. The Benefits of Care Over 70% of our baby boomers are living with at least one chronic illness, often four or more. Patients with chronic, unmanaged conditions are not eligible for home care, skilled services or hospice care. These patients are often suffering due to poorly managed conditions and a fragmented health care system. They frequently face social and financial challenges that can make their symptoms worse. None of us wants our loved ones to suffer needlessly! Our palliative care team assesses patients and families to identify these challenges and builds a plan to address the gaps. While the intensity of services differs from hospice care, the monthly home visits and regular communication can have a significant impact on the quality of life for the patient AND their families. So far our advanced illness care teams prevented 30 emergency room visits over a 15-month period, saving thousands in costs to the health care system; a nice by-product of improving the quality of life. Of the patients in our pilot program, nearly 80% have
completed an advance directive, compared with the national average of 30%. Patients and families in our pilot are also ranking high in satisfaction scores, averaging 4.9 out of 5. So What’s Next? - How YOU Can Help “The only reason we can test this model is through the generosity of our donors and the community. Our donors understand the importance of taking care of patients earlier, and their support is essential to success of the program,” says Bob Mueller, Vice President of Development. To make palliative services available to the people who need them most, we need your help. In addition to financial support for the program, we need volunteers and we need your voice. We are working with our elected officials to pass legislation that will create an Advanced Illness Care insurance benefit. And, anyone can make a referral to our palliative care program by calling our main customer care line at (800) 264-0521. Palliative Care Program Exceeds Expectations for ALS patient CARREY DEWEY
When Carrey Dewey called Hosparus Health for help, she was primarily looking to ease the burden on her caregivers and didn’t expect to get much out of it herself.
W
hile she says it has proved to be just the blessing to her family she had hoped for, she has been surprised and pleased to discover how much better her own quality of life has become in the months since she was admitted to the Hosparus Health Palliative Care program.

“I was turned away from hospice, but the palliative program offered a wonderful alternative,” Carrey says. She was admitted to the pilot program in August. “I think it’s so great that people who don’t qualify for hospice still have an option.”
“It’s nice to have an objective person who can help me take care of some of these personal and emotional things that I don’t want to ask anyone else to do for me.” Carrey says.
“We can afford to pay the monthly fee, but there is no pressure if we couldn’t,” she adds. “It is not intended to be a financial hardship. Hosparus Health would let us pay what we can, and I really appreciate that.” The Benefits of a Team Approach Carrey receives nursing, social work, counseling and volunteer services through Hosparus Health. She is also receiving various treatments for ALS in addition to supportive palliative care, and her Hosparus Health RN serves as a bridge for all her medical providers. The nurse consults and coordinates with all her doctors to ensure she is getting the most beneficial care possible. Carrey says Marcy, her social worker, helped with 13 advanced care planning and a DNR, “She offered

A recently added CNA visit to help with bathing is “a huge blessing” for Carrey. This one hour a week visit has made a big difference in her quality of life. Just having an extra pair of hands and eyes to make simple suggestions makes life a lot easier, like suggesting she cut her tank tops up the back to make them easier to get on and off and using Desitin ointment to prevent skin rashes. A Hosparus Health volunteer does wonders for Carrey’s mental health by visiting once or twice a week and weeding her flower garden. “It was depressing to see my garden looking so pathetic and not being able to do anything about it,” she says. This gift, again, makes a world of difference to Carrey. Volunteers also help with writing letters to her family to preserve her legacy, and with posting to her Team Dewey: Kickin’ ALS Facebook page. Preparing for Transition Carrey’s care team is helping her live as well as possible for as long as she can. “I feel comforted that I have a team of people who know not just me, but also know my family, that give good advice, and I know I can call anytime,” she says. She adds that, though she is limited, she has a lot of living left to do. “I don’t feel like I’m dying even though, with my diagnosis, it’s clear that I am. I have three kids, and I feel like I have an active life.” Her team knows she plans to transition to hospice when she is ready to give up treatment. “I’m on a ventilator, which hospice does not support,” she says. “Since I am already in the system, it will be an easy transition, especially if things turn quickly, like they tend to do with ALS. Having a team and plan in place gives me a lot of peace and comfort.” And Carrey is eager to share that peace and comfort with others. In fact, she recently recommended Hosparus Health palliative care to a friend with ALS, and his quality of life has already improved.
Carrey, 45, has ALS and is mostly confined to her bed. Her husband, Eric, is her primary caregiver, and the couple has three children, ages 8, 10 and 12.
“It feels great that other people like me can benefit,” she says. “I deeply appreciate that Hosparus Health sees the value for patients like me and plans for the program to continue.” We are honored to support such a courageous woman and her family as they deal with the challenges of ALS. Kourageous Kids Expansion Hosparus Health’s Kourageous Kids palliative care program provides care based on collaboration with the child’s doctor or medical team. Children in this program continue to seek curative treatment. In fact, Hosparus Health’s adult palliative care program is modeled after our highly successful pediatric Kourageous Kids program. We believe we were the first hospice to create a pediatric program, serving seriously ill children in our communities since 1980. As part of our expanded vision, we are putting a renewed emphasis on pediatric palliative services, with the goal of serving 75 children and their families by the end of 2017.

“Glitter and Glitz” was the theme for this year’s event, held at Hardin County Schools’ Early College and Career Center (EC3). Hosparus Health furnished a white tablecloth, white linen napkins and sugar, but it was up to table decorators to provide the rest, including a centerpiece and place settings for eight. We saw some of the most elaborate tables yet, including one that featured an Egyptian vase! EC3 culinary students prepared the food and desserts were donated by local businesses. All teas and treats were served by volunteer businessmen and local dignitaries dressed to the nines in tuxedos donated by USA Bridal/Tuxedo Avenue. Guests also enjoyed bidding on a wide variety of exciting silent auction items. The E-town Tea has been a Central Kentucky tradition for 25 years and it’s not too early to start planning for next year! Greensburg Couple Celebrate 50th Anniversary with the Help of Hosparus Health NELSON & JUDY POSEY Nelson and Judy Posey celebrated their 50th wedding anniversary in style on January 14th with a surprise party thrown by their church and children, but that might not have been possible without Hosparus Health of Green River. Judy says she is convinced that Nelson wouldn’t have been able to celebrate this milestone “if Hosparus Health hadn’t come in.” Nelson and Judy were each diagnosed with lung cancer within a year of each other. While Judy is now in remission, Nelson had been in and out of the hospital for months before his doctor recommended Hosparus Health last year. At the time, though, Nelson wasn’t ready. When we sat down to talk with him earlier this spring, he told us he’d heard all the myths about hospice care, such as that it’s only for the last two weeks of life, and it “would kill him.” He and Judy soon came to realize Hosparus Health offers just the opposite. “They are life savers,” she says. Hosparus Health care allowed Nelson the highest possible quality of life in his final months. He passed away in May.

Every Time it Rains, it Rains Pennies from Heaven.... Not everyone can afford to buy a ticket for a fancy gala, but everyone can afford to donate a handful of change. That’s why Hosparus Health of Southern Indiana hosts Pennies from Heaven each year, a special fundraiser where anyone can make a difference. This year’s fundraising campaign brought in over $5,400 - that’s a lot of pennies!
Record Year for Hosparus Music Festival For the sixth year, local bluegrass enthusiasts hosted a weekend of music and fun to benefit Hosparus Health of Central Kentucky. Breckinridge Bluegrass Music Association (BBMA), along with other bluegrass groups from Hardin and Grayson counties, brought in some of the hottest acts in bluegrass music to raise a record $20,296 for patients and families. That’s more than double what the event brought in last year!
Funds will help us provide hospice care to patients in Clark, Crawford, Floyd, Harrison, Scott and Washington counties, support our Kourageous Kids pediatric program, and provide grief support to those who have lost a loved one. For about 35 years, hundreds of folks have collected bright shiny pennies and other loose change to support Southern Indiana patients and families. We ask the community to collect coins all year. They can bring them to our office any time, or we’re happy to pick them up.
Longtime Hosparus Health supporter Bobby Smith launched the Vine Grove Bluegrass Band Scramble in 2012, and it raised more than $26,000 for Hosparus over the years. In 2016, the event merged with BBMA, was renamed the Hosparus Bluegrass Music Festival, and moved to Rough River State Park Lodge. Held this year March 31-April 1, the weekend was filled with music, fellowship, as well as exciting live and silent auctions. Many local businesses and individuals from Hardin, Grayson and Breckinridge counties offered their support by sponsoring the festival. “It has been wonderful to see the growth of this festival over the years,” added Lisa Sanford, Director of Development for Central Kentucky. “The money raised has made such a difference in helping us provide the highest quality care for our patients and families, regardless of their ability to pay.”
About 25 wonderful volunteers help count change at the reception. Many of our volunteers come back year after year, and we couldn’t do it without them! “This is a great ‘everyman fundraiser’ because it allows people who might not normally attend one of our events to support our mission,” said Vonya Gresham, Hosparus Health of Southern Indiana Volunteer Manager. “It also allows us to meet new people in the community who may need our help or might like to volunteer.”

Hosparus Health Celebrates Vets Through Honor Flight More than 1,800 veterans die every day in America. At Hosparus Health we honor our veterans through We Honor Veterans, a program of the National Hospice and Palliative Care Organization (NHPCO) in collaboration with the Department of Veterans Affairs (VA). We Honor Veterans is an ongoing and growing program to recognize the sacrifices these men and women made for us and to ensure they receive exceptional quality care at the end of life. We rely on a core group of patient care and veteran volunteers to help us serve our veterans at this most important time of their lives. Honor Flight was founded to ensure that every WWII veteran (or veteran of the Korean and Vietnam wars) has the opportunity to visit the War Memorials in Washington, D.C., for FREE. Top priority is given to veterans 406 veterans were supported by Hosparus from any war who have been diagnosed with a Health Volunteers terminal illness. Often those through the We Honor traveling need wheelchair Veterans program. and personal assistance. It was our honor to donate three wheelchairs to Honor Flight to use during trips from Louisville to D.C. as a very small way to honor our local veterans. President and CEO Phil Marshall and SVP/Chief Marketing Officer Gwen Cooper joined hundreds of supporters bright and early at Louisville International Airport a few months ago to thank the vets boarding the flight and wish them well on their trip to D.C. A huge crowd greeted them in Washington, and hundreds more were back at Louisville International to welcome them home that night. A Rewarding Experience Don Parsons is a Korean War Army Veteran, whose daughter, Donna Frank is on the staff at Hosparus Health. Donna suggested that Don become a We Honor Veterans volunteer and he’s been doing data entry for medical records ever since. Don says he was fortunate to travel on the recent Honor Flight alongside about 150 other heroes and their family members, and it was an experience he will never forget. “Outside the birth of my three daughters, it was the best day of my life,” he says. “It was a once in a lifetime event, and you just knew everyone felt the same way. It was like joy at Christmastime.”

He adds that while seeing the war memorials in Washington was a moving experience, he was equally inspired by how many people — most of them strangers — turned out to support him and his comrades in arms. “When we got back, and it was like a national convention!” he says. “It was amazing.” An Opportunity For Hosparus Health Hosparus Health board member, Dr. Todd Schmiedeler, SVP of Employee Services and Foundation for Trilogy Health Services, recently heard about Honor Flight, and thought it would be a great flagship initiative for his company’s Live A Dream Foundation. Over the next year, Trilogy will fund 100 guardians (each Veteran has a personal “guardian” joining them on the trips) to accompany vets living at Trilogy facilities on Honor Flights. Todd chose to be the first Trilogy guardian on the May flight, and he was incredibly touched by the experience. Honor Flight allowed his vet, Art Hardy, “to stand a little taller.” Todd told us that “it was incredible to see how energizing this day was for his resident.” Todd sees the parallel missions between Hosparus Health and Honor Flight. “Our job is not just to care for people, it’s to make sure they feel cared about,” he says. “I believe Hosparus Health does that very well. And Honor Flight is an opportunity for vets to feel cared about.” “Hosparus Health has an opportunity to identify people who might not otherwise have the opportunity to go on a flight, and be able to offer that as a lasting legacy,” continues Todd. “As a board member, I think it’s a terrific opportunity and a great fit for our organization and I look forward to getting more of our patients and volunteers involved.” We agree, and see this as another way for our patients to experience special moments at the end of life that might not otherwise be possible.
