Heartbeat JANUARY / FEBRUARY | 2018
I am proud to say we wrapped up 2017 on a high note, caring for the highest number of patients and families in our history — nearly 7,600. We couldn’t have reached this impressive milestone without all your hard work! It’s clear we’re already riding the positive momentum you’ve created into the new year. Speaking of positive momentum, I was thrilled to be able to share some of the exciting things we’ve got going on with Insider Louisville in January. I couldn’t have written a better headline myself: “Hosparus on forefront of aging care innovation.” It’s so gratifying to be recognized as a leader in our industry. YOUR SPIRITUAL CONNECTION
Chaplain’s Corner
Practice, Practice, Practice By: Marie Ellis, Chaplain
Finally, the frenzy of the holidays has slowed down! Our to-do lists have shrunk, and we are probably just about now making peace with the fact that most of our New Year’s resolutions might be unachievable. This happens every year around this time. We joke about it, saying, “Oh well, I didn’t think I would actually do that anyway,” or “I will try again next year,” or perhaps just resign ourselves to failure, thinking, “I’m just worthless.” Woah! This is supposed to be a motivational article to lift our spirits! What is this talk about worthlessness? What are we doing here? We are supposed to actually achieve these resolutions every year! We are supposed to get fitter, eat better, stop smoking, read more, grow our portfolio, learn a new hobby and learn to dance. Right? But, I ask you, how can we possibly live up to that?
The piles of shoulds and supposed to’s that we heap around ourselves is totally overwhelming. But here’s the good news, should’s are optional! So why not turn around and walk away from all these unreasonable expectations? Great, another person telling me that I am doing things all wrong (eye roll). Normally, when I hear anything I think belongs on a poster with a kitten, I simply disregard it. But I implore you to read on! I recently read Brene Brown’s book THE GIFT OF IMPERFECTION, and she has a new understanding of how to accomplish this herculean task! Brown views the task of caring for yourself as a P-R-A-C-T-I-C-E. ‘Why does this matter to me, or mounds of should?’ you say. Because practice is simply that, it is practice. It does not have to be right, perfect, correct, or even close to on track. It means that
8
you are trying. Practice also implies an opportunity to learn. So if you let go of unreasonable expectations as a practice, then we are afforded the opportunity to try and try again. Brown’s message: that releasing those ‘shoulds’ and ‘supposed to’s’ will eventually free us from them. Because guess what? They are only practice. In this freedom, there is an opportunity to learn from failure, and to reflect on what our practice might look like next time. One of my good friends and fellow chaplains, is fond of asking me, ‘So what is this trying to teach you?’ when I have failed and I absolutely hate it. Well, actually… I begrudgingly appreciate it, because it forces me to think. Now, it also reminds me that I am practicing. I don’t have to be perfect, and failure is not an all-
consuming nightmare that I think it is. It is a part of practicing. It is an opportunity to try again, and do something different, possibly better the next time. Contemplate on why you feel that you ‘ought’ to behave in a certain way, listen for your inner voice or simply take a few minutes to breathe deeply and be present in the moment. This knowledge might change your perspective of yourself from ‘worthless’ and lead you toward a new sustainable joy and hope. I would like to leave you with these words from Brene Brown:
“Authenticity is the daily practice of letting go of who we think we are supposed to be and embracing who we are.’ Let’s take a deep breath and just let that all go….you are worh it!

Grief CounselingHeartbeat “Roots” first session a success
Rooted in Hope “Roots” finished its first session and we could tell you about the program’s success, but we’ll allow the participants to do that for us! “It was so well planned--both relaxed and structured. I really appreciate the consistently open and supportive atmosphere-- I always felt perfectly welcomed to share or just listen. Thank you!”
Kourageous Kids & the Art Therapy Pilot Hosparus Health had a rich history of recognizing and supporting art therapy as a valuable clinical modality. We have always had a board certified and licensed art therapist on staff, but how often we could offer art therapy to patients and families became very limited due to census growth and sharing the art therapist with the bereavement department.
“We felt safe and understood when we expressed our feelings. Very caring staff.” “Teaching my child how to handle grief and anger…benefited our family and for this I thank you so much.”
If you know someone who may benefit from Roots, they can register here: Rooted in Hope Registration.
We are excited to say that the KKids program is starting a pilot to explore the idea of having a dedicated art therapist for KKids. Joe Ferry, a bereavement counselor at the GCC, will be helping re-ignite this program, working with one family at a time and helping shape what this program will look like. More to come!

Palliative Care News
• We are in the process of building a Nurse Practitioner/MD Palliative Care Consult Service which will allow us to provide specialty palliative care visits to patients wherever they call home in both Kentucky and Indiana. The service is being created in order to provide patients with advanced or serious illness more options to help address troubling symptoms, to facilitate advance care planning, and to aid in aligning treatment preferences with the patient’s goals. Our goal is to launch in March 2018. Be on the lookout for more details coming soon! • The Financial Assistance scale for the Adult Palliative Care Program has been modified to offer more generous support for patients with a demonstrated financial need. If you know of anyone who could benefit from our team-based palliative care program who may have been concerned about whether they could afford the program, please have them contact us! Please reach out to Bea Aistrop, Palliative and Kourageous Kids program coordinator, baistrop@ hosparus.org if you have any questions. • We are happy to share that two of our Hosparus Health physicians are providing palliative care consults in innovative partnerships! Dr. Cory Lucas is spending ½ day per week at Hardin Memorial in the outpatient Supportive Care Clinic doing palliative care consults for oncology patients and Dr. Dustin Dillon is spending ½ day per week at Norton Children’s Hospital performing palliative care consults for children.

BlogHeartbeat Hosparus Heath will introduce a new program, Heart Connection Launching its pilot program Spring 2018 By: Stephanie Alvey, HLSW Team Nurse February is American Heart Awareness Month. The first American Heart Month took place in February 1964, per President Lyndon B. Johnson. In 2002, the Red Dress campaign, known as “Go Red for Women” was introduced by The Heart Truth ® , becoming a national symbol to increase awareness of women’s heart disease. The awareness from these campaigns along with alternative medical advancements in cardiac care has affected the length of life for those with cardiac disease. While people with heart disease are living longer, in most cases they live with a poor quality of life.

Many patients in our communities qualify for our services but do not receive them due to lack of public and physician knowledge. Our team will be educating patients, hospitals, physicians and nurses about the care we provide for advanced cardiac disease. The Hosparus Health Heart Connection program will provide in-home visits by our specially trained interdisciplinary team and continue our 24-hour clinical support. In addition, these patients will receive a symptom tracker tool. The symptom tracker is a user-friendly tool designed like a stoplight allowing the patient (and family) to triage their symptoms as they experience them day to day. Patients, in need, will be provided a digital scale to ensure consistent daily weights. Staff will provide the education to prevent frequent readmissions along with coordinating care with their primary cardiologist. Treatment plans will be patient goal oriented to improve quality of life, all while occurring in the patient’s choice of home environment. Advanced cardiac disease cannot be cured, but it can be managed if monitored closely. We look forward to implementing this program throughout the organization and supporting the mission of Hosparus Health. Learn more about American Heart Month and heart health at www.heart.org.
Hosparus Health currently provides care to patients with advanced heart disease but only to a small percentage of the potential population. To further serve more patients we have developed Hosparus Health Heart Connection, launching its pilot program Spring 2018. The goal of the program is to reach the vastly under-served patients that are filling local emergency rooms weekly because they are unable to manage their heart disease symptoms independently at home. As an organization, we want to close the gap in care that has contributed to the $32 billion annual health care cost.
Did You Know? 12
In 2017 Hosparus Health served 1,365 patients with the diagnosis of heart disease or stroke.
