As you know, one of the many ways Hosparus Health provides Best in Class care is by be being there for our patients and families 24/7. We take pride in the fact that anyone can call us with questions or concerns, no matter how small, and we’ll always answer the phone.

overall with our patients, families and supporters, it all starts with solid communication among our team. As our census and care teams both continue to grow by leaps and bounds, open and active communication is more important than ever. It’s easy to take for granted that everyone is on the same page, but for an organization our size, that’s not always the case.

Whether it’s in the middle of the night, a weekend or a holiday, both our stellar customer service team and our on-call staff are there to ease any fears and ensure our patients and families get the support they need, when they need it. I hear from families all the time that the peace of mind this provides is the one of the most important services we offer.

I know we’re all busy, especially as we ramp up for the holiday season, so I want to challenge everyone (myself included) to come up with new and creative ways to share information and ideas across all levels and departments. We’re all in this together! Phil Marshall

Corporate Zip Code Changes

As our patient population has continued to grow at Hosparus Health Louisville, the need occurred again to examine census and resources for our four HL communities. After examining census, talking with our physician staff, and considering the resources available in the communities, the decision was made to reassign some zip codes to better meet the needs of our patients, physicians, and clinical staff. The following changes have been made and are currently in effect: • 40023 and 40243 in the Northeast community (with the exception of The Forum) are now part of the Southeast community. • The Northeast Community will continue to serve The Forum. • Home patients in zip code 40220 (Central Louisville) will be admitted to the Southeast community. • All nursing facility patients in 40220 will continue to be part of the Central Louisville Community. The changes have been made to the Home and NF Team Assignments document on the HUB. Marketing is working on updating maps. See photo to the left for areas affected.

Corporate Staff Bonus Announcement for 2018

2018 has been another incredible year of growth. To say thank you for meeting our mission and for being part of the teams that allowed us to meet our financial and strategic goals, we are pleased to announce we will be paying bonuses to all full and part time non-management staff. Bonuses will be paid in the following amounts: • Full time staff (0.8 – 1.0 FTE) will receive a $650 bonus if hired prior to June 1, 2018 • Part time staff (0.4 – 0.7 FTE) will receive a $350 bonus if hired prior to June 1, 2018 • Full time staff (0.8 – 1.0 FTE) who were hired between June 1, 2018 and September 1 will receive a pro-rated bonus of $150 • Part time staff (0.4 – 0.7 FTE) who were hired between June 1, 2018 and September 1 will receive a pro-rated bonus of $75 • PRN staff, staff hired after September 1, 2018, and employees who receive incentive compensation are not eligible for the bonus. The bonus will be paid via direct deposit on November 26, 2018. To receive the bonus, the following requirements must be met by November 14th at 5 p.m.: • Mandatory education through the 3rd quarter must be complete • Immunization requirements (including flu shots) must be met and supported with any required documentation • Licenses must be renewed and current • Have a signed Handbook Acknowledgment and Arbitration Agreement on file • Anyone currently on a performance improvement plan is not eligible for the bonus While a bonus is not routine and cannot be anticipated, We are very pleased to be able to recognize your contribution over this past year. Thank you for the work you do and congratulations on this bonus!

Open Enrollment 2018: 11/1 - 11/14

Great news – the plans aren’t changing, and our premium rates remain exactly the same. Employees only need to participate in open enrollment if 1. They want to change plans 2. They need to make changes to their dependents 3. They want to participate in an HSA or an FSA (those must be re-elected each year). More information will be available soon. Questions? Grief Counseling

Amy Sloboda’s Farewell This month, we said ‘see you soon’ to Amy Sloboda, Grief Counseling Center Director. The staff signed letters for her and treated her to a special dinner. Happy retirement Amy! The holiday season is upon us. This time of family gathering can be especially challenging for those grieving a loss due to death. The Grief Counseling Center invites bereaved adults and families to Hope for the Holidays. Each session is facilitated by one of our Grief Counselors who address aspects of the holiday season and offer practical suggestions for supporting one’s self and family during this special time of year. Join us for one session or all. Sessions are free to the community. Pre-registration is recommended. Location is the Louisville Grief Counseling Center. Programs occur every Wednesday at 1:00 PM and 6:00 PM. Chaplain’s Corner Thanksgiving Through the Holy Days By: Lawrence Lucas, Chaplain

While the holiday season is a festive time of year for many people, it can be quite the opposite for anyone who has lost a loved one. For our patients and families who are coping with grief and loss, the holidays can be downright gloomy and depressing. For all of us at Hosparus Health, let this season of giving be a time of reflection and introspection. Let’s take a fresh look at how we are called to serve our patients and their families. Let’s all remember it is at their most sacred hour — during their pain and agony, their crucible of suffering, turmoil and doubt, loneliness and fear — that we are called to journey with them. For me, it is at these most vulnerable moments that I sense an awareness of the Divine. I believe that we all can find God or a sense of Holiness in the midst of pain. In fact, I believe that is what we are called to do: to bring healing, comfort and care during despair, and not just during these Holy Days, but every day, all year long. I feel fortunate that my family always provided wonderful examples of generosity and Divine awareness. I watched both my mother and my grandmother provide care and comfort as RNs, but I have to confess that I never knew the extent of their compassionate work prior to my chaplaincy.

Annual Memorial Services

Hosparus Health Remembrance Services provide opportunities to reflect, remember and honor loved ones who have died in the last year.

After my father retired, and as age and illness began to take a toll, I’d call to inquire as to his wellbeing. His response often would be, “Well, every day is a holiday, and every meal is a feast.” Though that may not have been his everyday experience under hospice care during the last months of his life, he always embodied the spirit of Thanksgiving. Through my work, I see many examples of gratitude and Divine awareness in my interactions with patients and families, even during their most painful, difficult times. I have learned that the Divine shows up in many ways, and one is through all of us who give ourselves selflessly as agents of Hosparus Health. This holiday season, may you be able to find the Divine, even through suffering. May you find a spirit of Thanksgiving. Happy Holy Days!

Volunteer Referrals

Year to date in Louisville, we have trained 75 volunteers, with one more training session for 2018 on November 9th. Our last class of volunteers was one of the larger classes, with 11 new individuals! Many of our them come to Hosparus Health because of a personal experience they’ve had, or by the recommendation of another volunteer or staff member. Volunteers attend an 8-hour training session, and go through a background and references check, interview and immunizations, same as employees. To refer someone you know who would make a great volunteer, send them to our website. The volunteer tab has more information about requirements and the online volunteer application.

The following employees have recently been added in leadership roles, give them a warm welcome when you see them!: Mary Pat Stickler, Vice President of Quality and Clinical Excellence - Mary Pat brings over 30 years of clinical leadership experience. She has spent the last 15 years at Kindred Healthcare where she served two years as CEO, and 13 years as Field Director of Quality/ Regulatory Compliance. Mary Pat will be working with the Quality and Patient Safety department to continually improve quality outcomes and patient safety, standardize care to best practice, and promote continuous survey readiness. Tricia Pancner, Vice President of Clinical Services - Tricia has been certified in Hospice and Palliative Care for 20 years. She brings over 18 years of multi-site leadership experience in hospice organizations. She has served as an Area Vice President of Operations in two large organizations, most recently at Kindred Hospice. Tricia will be working with Doug Hedges, Becca Simpson, Kim Thieneman, Amy Hill and Tracy Trumbo in the delivery of care and services to our patients and families.

Tough Questions Q: Why can’t we have paid holidays instead of using our TOPS time? A: Occasionally individuals think TOPS equals only vacation hours but TOPS is “time off with pay,” and represents 3

categories of time off with pay (vacation, holiday, and sick). When we set the accrual of TOPS time, we take into account that our employees will have seven paid holidays from that accrual. If we were to pay the holidays using a designated holiday bucket, we would deduct those seven days from the current TOPS accrual. As some of our staff work on the holidays, this seems the best way to allow most individuals to have a paid holiday while acknowledging that some staff must work. We’ve also had some ask for designated sick days, but the more we segregate the buckets of paid time off, the less flexible it can be to meet an individual employee’s needs. We prefer to keep the bucket of hours less defined, so that you can maximize the flexibility of your time off for your needs.

Q: When are we going to fix the Hub? As a go-to resource for staff in the field, it is not user friendly. You cannot search for

what you need without an abundance of information displaying. A: We agree! The Hub could use an overhaul. We don’t think that we have the internal resources to undertake that type of project, so we’re exploring whether we could find a consultant to help re-organize and update the Hub, making it more user friendly. There are multiple owners (i.e. departments) of the information on the Hub, and multiple individuals add information on an ongoing basis, making it difficult for any single entity to maintain. If we find a consultant to assist in updating and re-organizing the Hub, they would need to work with all departments, which means this can’t be done without the investment from a lot of internal resources. As you may recognize, there is not an easy fix to this need. As we move to Ceridian for our Payroll & HR needs, we can make that portal a “onestop-shop” for all kinds of HR and employment information that you would want to find, also available on a mobile device. This project is a big one, so you may not see immediate movement on it but we are working behind the scenes to begin to address your concerns.

Q: Why should day shift RN have to cover for night shift RN when night shift RN never have to cover for day shift? A: Coverage of our patient care needs is always a priority and can be challenging when trying to cover

for TOPS and other types of staff vacancies. Staffing models may vary by communities. Generally, there are more staff assigned to day shift positions which makes it more likely that a day person may be used to cover a need on night shift, versus the opposite. Often it is more difficult to fill night shift positions and as a result many staffing models do not pull the night nurses to cover days. We encourage staff to discuss ideas to improve staffing models with their managers.

The Over The Top program is designed to show appreciation to a fellow co-worker who has provided excellent customer service and/or support to their peers. The just culture framework requires accountability but is not punitive. While employees are still held accountable for behavioral choices, we also recognize that employees are humans, and to err is human. Further, we understand that employees work within the structure of organizational policies, procedures, and process, which can at times fall short. Our goal is to provide an open and safe environment where employees feel comfortable reporting their own and others’ mistakes or near misses without the fear or stigma of punitive discipline. This will allow us to continually adjust and learn from mistakes and near misses to the benefit of the organization, our employees, and ultimately the patients and families that we serve. To ensure a just culture, we are committed to providing proper training and resources so employees can perform duties safely and correctly. We will also foster a trusting environment where employees feel comfortable reporting concerns. Finally, we will only hold employees accountable for behavioral choices that were within their control. In return, we expect employees to agree to make good decisions and follow established policies and procedures. We also ask that employees report when they have concerns about a policy or suggestions for improvement. We want to know when mistakes and near misses occur, even if there is no adverse outcome. Each mistake is an opportunity to learn and improve our processes. Finally, we ask employees to refrain from engaging in reckless behavior. Reckless behavior will result in discipline up to and including termination of employment.

Cyber Security: Scam Alert Cyber-criminals are hacking into employees’ payroll accounts and rerouting their direct deposits. The scam starts with an email asking a worker to share the login credentials to their employee account. Law enforcement officials say to be overly cautious regarding any emails asking about your payroll, as the fake message reportedly looks very convincing. **Please forward emails that ask for your login credentials to the IT Department ithelp@hosparus.org to make sure they’re legitimate.**


Robyn Mathias CHPNA ABOUT here’s a reason Robyn Mathias has been with Hosparus Health for 28 years - she says it’s a calling. “Why I’m

here is a good question, I believe this is where God planned me to be. I never would have thought that this would be my specialty but now I can’t imagine doing anything else in my life. It’s truly a blessing being able to walk through this part of life with people. It is the most awesome, rewarding feeling to get so many smiles and special ‘thank yous’ each and every day from the patients and families I visit.” Currently, Robyn works on the HL-Southwest team, primarily focusing on Bullitt County. A typical day in her world involves making calls, gathering supplies, and helping patients with tasks such as bathing (assist with all ADLs), changing bed linens, providing wound care if needed, etc. She also spends a lot of time with families, educating them on end-of-life and bed-bound care. Robyn truly believes in her job and completes each day to her fullest. She is always willing to work extra to help others, regularly precepts new CNAs and is devoted to a better documentation system.” In her spare time, Robyn’s biggest passion is riding her motorcycle. As she says, “it’s my relaxing place”. She also has 3 nieces and 10 great-nieces and nephews that are everything to her. She even has the honor of being one of her nieces Godmother.

Ham Days!

Hosparus Health was proud to sponsor the breakfast tent at the Marion County Ham Days this year in the Green River Community! We had a great time making sure people’s stomachs were happy and full to enjoy the festival.

Spencer County Chamber Golf Scramble Hosparus Health was a beverage sponsor at this year’s golf scramble. Kenya Whitfield (Outreach Manager), Kim Thienaman (Community Director), and Dana Fowler (HLSE Nurse), represented us on the Tanglewood green for the day.

Patient Visits the Smoky Mountains Again Patient Georgia Blair, with husband Eugene and her son, was able to visit the Smoky Mountains for one last trip. The family told the nurse that without our staff the trip wouldn’t have been possible. We are so glad we can make special moments like this happen!

Blog A Valued-based Provider in a Payer World

By: Gwen Cooper, MPA, CFRE, Sr. Vice President/Chief External Affairs Officer Published in Medical News, September 2018 The recent American’s Health Insurance Plans (AHIP) conference was full of insights and opinions. In our everchanging healthcare system, providers must understand the mindset of payers to stay ahead of the “new” value-based care curve. The theme: “New Perspectives in What’s Next in Health Care” looked promising so off I went to San Diego. The conference didn’t disappoint, with speakers ranging from the new CEO of the Amazon-JP Morgan-Berkshire Hathaway venture Atul Gawande, to President George W. Bush and numerous short but impactful sessions. I came away with the validation that hospice providers have had it right from the beginning. We are “value-based, person-centered care” personified. Enacted in 1983, the Medicare hospice benefit was the first capitated (value-based) payment model embraced by Medicare, paying providers a single rate to provide complete, integrated, multi-disciplinary care for patients until the end of their lives. For 40 years, Hosparus Health has been one of the largest not-for-profit hospice organizations in the nation, caring for over 1,200 patients-per-day in 37 counties in Kentucky and Indiana. We serve as a crucial safety net for the sickest, most vulnerable patients in the communities we serve. As a mission-driven healthcare provider, we are continually committed to improving access to the highest-quality advanced illness and end-of-life care.

When I attended session after session listening to payers talk about how they are working to “build a system designed to meet patients’ needs,” I sat there thinking: “helllooo, look at us, this is what we do, it’s already created, just tap into the proven method of patient care and imagine how many lives we could improve and how much money we can save the system.” It’s really a no brainer. Right under the nose of literally every payer in the country, are nonprofit hospice and palliative care providers whose mission has always been to wrap our “valuebased” services and complete interdisciplinary teams around

patients and families to provide the most compassionate person-centered care until their very last breath. And now, we continue to wrap our services around patients and families but upstream from hospice care. Many hospice providers have begun to roll-out their own innovative models of advanced illness care upstream from the hospice and palliative care programs as part of their service menu and in partnership with local hospital systems and payers that see the value in providing person-centered care to the most seriously ill. Advanced illness care/palliative care is shown to reduce hospital readmission rates and frequent ER visits for disease symptom management. While hospice is not a substitute for advanced illness care, these programs can feed into a quality hospice program that provides compassionate care at the end of life. By providing the same model of care to patients with an advanced illness while they continue treatment, we are better able to manage their pain and progression of their illness much earlier. Discussing hospice care is often met with “I am not ready to give up.” While we know that providing hospice care is not about giving up but about compassionate pain and symptom management, respite care, grief counseling, spiritual care and so much more, we also know that introducing the same care approach much earlier in one’s chronic illness journey will have lasting, quality outcomes for both the patient and their family, not to mention also lowering healthcare costs. It was nice to see that the payer industry is beginning to embrace our model of person-centered care. A broader understanding of the benefits of interdisciplinary medicine as early as possible to help manage a chronic illness is the healthcare game changer that we’ve been practicing for 40 years. I am hopeful that organizations like ours will have a seat at the table with payers nationwide ready to engage hospice providers in the design of this new payment model.
