welcome Health and longevity, In today's world of "sound bites" and the realization in our media-focused world of Andy Warhol's 15-minutes of fame for all, the term "antiaging" can encompass a wide diversity of ideas to many different people.

For some it means beautification - especially skin antiaging. For others it has a far wider meaning - being able to escape aging's various debilitating guises, and to remain lucid, agile and independent for as long as possible. Some may only be interested in specific areas of physical or mental improvement, and then there are those w h o s e goal is longevity and life extension itself. History teaches us that the social and medical changes that have already happened from sanitation and clean water to antibiotics and some forms of surgery - have not only benefited in their o w n way, but they have also been major contributors to extending life span. As antiaging medicine is the ultimate preventative medicine, seeking to slow, allay and inhibit the problems of old age, could add vital years to life and therefore is also likely to improve our longevity, both as individuals and as a species. For those serious about life extension, longevity is the goal of antiaging medicine. However, I suspect for the majority of people, it is just to improve or maintain a good level of health and performance for as long as possible. This is often referred to as the "optimal health span." This issue of antiaging magazine is all about the optimal health span - 1 hope you find it of interest and use. Optimal Healthspan with Oeprenyl By James South, MA

Deprenyl is a drug that was discovered around 1964 by Dr. Joseph Knoll and colleagues, originally developed as a "psychic energizer." Also known as L-deprenyl and selegiline, it has been intensively researched over the past 40 years with many hundreds of research papers having been published. When reviewing them, it becomes apparent that deprenyl has a unique and exciting profile. It is the only potent, selective MAO-B inhibitor in medical use, plus it is a "catecholamine activity enhancer." In addition, deprenyl has been shown to protect nerve cells against a wide range of neurotoxins, and to act as a "neuroprotection agent" when nerve cells are exposed to damaging or stressful conditions. Today, deprenyl is a recognised treatment for Parkinson's disease, but is also useful in treating depression. In aged animals, deprenyl has proven to be a highly effective "sexual rejuvenator" and also shows promise as a cognitive enhancement agent. Deprenyl has been proven, in four different rodent studies, and one dog study, to be a remarkable life-extension agent.

UNIQUE ACTIVITY It was in 1971 when Dr. Knoll showed that deprenyl was a unique kind of MAO inhibitor— specifically a selective MAO-B inhibitor, without any "cheese effect." Effectively by inhibiting this enzyme, deprenyl can increase the brain's levels of dopamine, and while some authors have questioned deprenyl's ability to increase dopamine levels by this method alone, it wasn't until the 1990s that Dr. Knoll discovered the "main basis" of deprenyl's action.

Deprenyl is one of the mainstays and still one of the most important drugs used in Antiaging Medicine today. It has been shown to have a wide diversity of uses, including slowing and treating the prevalence of dementia (in particular Parkinson's and Alzheimer's Disease), and is also a potent anti-depressive, as well as a libido enhancing agent. Furthermore, it has significantly increased the life span of animals. How can one substance have such a wide diversity of effects? This article attempts to highlight the actions and issues surrounding deprenyl and details some of its uses 4

including the development of tests for pharmacological analysis of tranquillizers and psychostimulants. Plus his analysis of the physiological basis of drive-motivated behavior created new analgesic antiinflammatory drug groups. In the 1960s he was a key-player in the development of deprenyl. His experiments have shown it to slow the age-related decline of sexual and learning performance and prolong life.

ATTENTION, DEPRESSION, LIFE SPAN AND THE CATECHOLAMINES During the 1990s Dr. Knoll uncovered a new mode of action of deprenyi that he believes explains its widespread clinical utility. He discovered that deprenyi (and its "cousin"phenylethylamine) are "catecholamine activity enhancers." Catecholamines refer to the inter-related neurotransmitters of dopamine, noradrenaline and adrenaline. These transmitters activate key brain circuits helping to maintain focus, concentration, alertness and attention. Dopamine is also the transmitter for a brainstem circuit that connects the substantia nigra and the striatum, helping control bodily movement and which partially dies off and malfunctions with Parkinson's disease. When an electrical impulse travels down the length of a neuron, from the receiving dendrite, through the cell body, and down the transmitting axon (see Fig: 1) it triggers the release of packets of neurotransmitters into the synaptic gap. These transmitters hook onto receptors of the next neuron, triggering an electrical impulse which then travels down that neuron, causing yet another transmitter release. What Dr. Knoll and colleagues discovered through their highly technical experiments, is that deprenyi and phenylethylamine act more efficiently to couple the release of neurotransmitters.

Dr. Knoll believes that deprenyl's positive catecholamine effect explains its antiaging benefit. In addition, his work indicates that phenylethylamine is also a catecholamine substance. Phenylethylamine is a trace amine made in the brain that modulates (enhances) the activity of dopamine and noradrenaline neurons. Autopsy studies have shown that while deprenyi increases dopamine levels in a Parkinson's patient brain by 40% to 70%, deprenyi increases phenylethylamine levels by 1300% to 3500%. Phenylethylamine is the preferred substrate for MAO-B, the MAO that deprenyi inhibits. Paterson and colleagues have shown that phenylethylamine has an extremely rapid turnover due to its rapid and continuous breakdown by MAO-B. Thus deprenyl's catecholamine activity has a dual mode of action. At low, non MAO-B inhibiting doses, deprenyi has direct catecholamine enhancing activity. At higher, MAO-B inhibiting doses, deprenyi creates an additional catecholamine effect, due to the huge increases in brain phenylethylamine levels that deprenyi causes, (phenylethylamine also being a catecholamine substance). Many authors have pointed out the probable dopamine neuron activity enhancing effect of phenylethylamine in Parkinson's patients taking deprenyi. Therefore, Dr. Knoll's discovery of phenylethylamine's catecholamine effect now explains this phenylethylamine/ dopamineenhancing effect. THE NEUROPROTECTOR Deprenyi has been shown to protect nerve cells from an ever-growing list of neurotoxins. Some of these neurotoxins can actually be produced within the brain under certain conditions, while others come from the environment or diet. Some of these neurotoxins are noted below: MPTP is a contaminant in synthetic heroin, however, compounds with a chemical structure similar to MPTP, include both natural and synthetic products, (e.g. paraquat), and are used in agriculture!

In other words, deprenyi causes a larger release of transmitters in response to a given electrical impulse. It's like "turning up the volume" on catecholamine nerve cell activity. This may be clinically very useful in various contexts such as Parkinson's and Alzheimer's disease, where the nigrostriatal tract and neural circuits underfunction. It can also be useful in depression, where there may be under-activity of both dopamine and noradrenalin neurons. Dr. Knoll's research indicates that after sexual maturity, the activity of the catecholamine nervous system gradually declines, and that the rate of this decline determines the rate at which a person or animal ages. 6-OHDA, is a potent neurotoxin that can spontaneously form from dopamine in dopamine using neurons. 6-OHDA may then further auto-oxidize to generate toxic superoxide and hydroxyl free radicals and hydrogen peroxide. Dr. Knoll's research has shown that pre-treatment of striatal dopamine-neurons with deprenyi can completely protect them from 6-OHDA toxicity. Even in those not suffering from Parkinson's disease, the nigrostriatal neurons are the fastest aging neuron population in the human brain- with an average 13% loss every decade from the 40s on. Dr. Knoll and others believe that 6-OHDA neurotoxicity is a key cause of this "normal" nigral death, and that deprenyi may be able to retard this debilitating downhill neural slide. DSP-4 is a synthetic noradrenaline nerve toxin. In rodents, deprenyi has been shown to

AF64A is a cholinergic toxin- it damages brain cells that use acetylcholine, (the neurotransmitter primarily affected in Alzheimer's disease). Deprenyi has been shown to protect cholinergic neurons from AF64A toxicity. Deprenyi has also protected human nerve cells from peroxynitrite and nitric oxide toxicity. Methyl-salsolinol is another MAO-B produced endogenous neurotoxin. Salsolinol is produced from the interaction of dopamine and acetaldehyde, (the first-stage breakdown product of alcohol). By inhibiting MAO-B, deprenyi reduces the toxic load on the brain that is routinely produced through the normal operation of MAO-B. MAO-B digests not just dopamine and phenylethylamine, but also tryptamine, tyramine and various other secondary and tertiary amines. The above are just some of the many reports in the scientific literature on deprenyl's versatile neuroprotection. PARKINSON'S DISEASE Parkinson's disease is one of the two major neurodegenerative diseases of the modern world, (Alzheimer's disease is the other). Parkinson's affects approximately 1 % of those over 70, a lesser percent of those 40 to 70, and rarely anyone below 40. Parkinson's is caused by a severe loss of dopamine, using nigrostriatal neurons, with symptoms manifesting after 70% neuronal loss, and death usually ensuing after 90% loss. (See Fig: 2). The role of the nigral neurons is to inhibit the firing of the cholinergic neurons in the striatum. When the nigral neurons fail in this negative feedback control, voluntary movement and motor control is "scrambled," leading to the typical Parkinson's symptoms of shuffling gait, stooped posture, difficulty initiating movement, freezing in mid-movement and the "shaking palsy." By the late 1960s the standard treatment for Parkinson's was the amino-acid precursor of dopamine, levodopa. The levodopa increased the dopamine levels in the few remaining nigrostriatal neurons in Parkinson's patients (80% of brain dopamine is normally located in nigral neurons), thus at least partially restoring normal movement and motor control. After analyzing results of 1052 Parkinson's patients treated over 12 years, A. Barbeau wrote of levodopa that; "long-term side effects are 5


numerous.... although we recognize that levodopa is still the best available therapy, we prefer to delay its onset until absolutely necessary." Deprenyl became a standard therapy to treat Parkinson's by the late 1970s. In 1985 Birkmayer, Knoll and colleagues published a paper summarizing the results of a 9-year study with levodopa alone, or combined with deprenyl in Parkinson's patients. They found a typical 1 to 2 year life-extension over the average 10-years from levodopa onset until death in the combined levodopa/ deprenyl group. The 1996 DATATOP study found that, "to the extent that it is desirable to delay levodopa therapy, deprenyl remains a rational therapeutic option for patients in the early stages of Parkinson's." In a 1992 paper, Lieberman cited 17 studies supporting the claim that; "...with levodopa-treated patients with moderate or advanced Parkinson's... the addition of [deprenyl] is beneficial." Thus, by the 1990s, deprenyl had become a standard Parkinson's therapy, used either to delay levodopa use, or used in combination with it. However, in 1995, a report published in ffie British Medical Journal seriously questioned the use of deprenyl in combination with levodopa to treat Parkinson's. This study stood alone as being detrimental, but since then, it has been recognised that this particular study was deeply flawed, and consequently it can virtually be ignored. ALZHEIMER'S DISEASE Alzheimer's disease is the most widespread neurodegenerative disease of modern times, affecting several million people in the U.S. alone. Alzheimer's is characterized not only by severe memory loss, but also by verbal dysfunction, learning disability and behavioral difficulties, even hallucinations. Alzheimer's is known to involve damage to the cholinergic neurons of the hippocampus, but in addition to the reduction of acetylcholine, alterations have been observed in the activities of other neurotransmitters. In particular, cerebral depletion of dopamine can lead to memory and attention deficits, plus in Alzheimer's there is significant increase in MAO-B. Thus, with its combined MAO-B inhibition effects and catecholamine activity enhancing effects, deprenyl would seem "tailor-made" to treat Alzheimer's. Indeed, that is the conclusion of a 1996 review paper on Alzheimer's and deprenyl. Tolbert and Fuller reviewed 4 single-blind and 2 open label deprenyl trials in Alzheimer's, as well as 11 double-blind deprenyl/Alzheimer's studies. They noted that all 6 of the single-blind/open label studies reported positive results, while 8 of the 11 double-blind studies reported favorable results, typically utilizing a 10 mg deprenyl/ day dosage. In 3 of the single-blind studies deprenyl was compared to 3 Nootropics, oxiracetam, phosphatidylserine and acetyl-L-carnitine, and deprenyl was superior to all 3. Tolbert and Fuller were so impressed with deprenyl that they concluded; "...in our opinion, [deprenyl] is useful as initial therapy in patients with mild-to-moderate Alzheimer disease to manage cognitive 6

behavioral symptoms. In patients with moderateto-severe Alzheimer disease, [deprenyl's] efficacy has not been adequately assessed. However, given the lack of standard treatment, selegiline should be considered among the various treatment options."

"deprenyl was compared to 3 Nootropics, oxiracetam, phosphatidylserine and acetyl-L-carnitine, and deprenyl was superior to all 3" DEPRESSION Deprenyl has been used experimentally as a treatment for depression since the late 1970s. While the causes of depression are diverse, it is now accepted that dysfunction of dopamine and noradrenaline neural systems is a frequent biochemical cause of depression. In addition, the research of A. Sabelli and colleagues has established that a brain phenylethylamine deficiency also seems to be strongly implicated in many cases of depression. Given that deprenyl is a catecholamine activity enhancer, and that deprenyl strongly increases brain phenylethylamine through MAO-B inhibition, deprenyl would seem a rational treatment for depression. Studies with atypical depressives, treatmentresistant depressives and major depressives have shown deprenyl to be an effective, low side-effect depression treatment. However, such studies have often required deprenyl dosages in the 20 mg or even 30 mg range. While these dosages caused little problem in these short-term studies, it is dubious to consider using such very high, non-selective MAO-B inhibition doses for long term treatment, (e.g. months or years). In 1978, Mendelwicz and Youdim treated 14 depressed patients with 5 mg deprenyl plus 300 mg 5-HTP three times daily for 32 days. Deprenyl potentiated the antidepressant effect of 5-HTP in 10 of the 14 patients. 5-HTP enhances brain serotonin metabolism, which is frequently a problem in depression, while deprenyl enhances the dopamine/ noradrenaline activity. Underactivity of brain dopamine, noradrenaline and serotonin neural systems are the most frequently cited biochemical causes of depression, so deprenyl plus 5-HTP would seem a natural antidepressant combination. In 1984, Birkmayer, Knoll and colleagues published their successful results in 155 unipolar depressed patients who were extremely treatment-resistant. Approximately 70% of their patients given 5 mg to 10 mg of deprenyl plus 250 mg of phenylalanine daily, achieved full remission, typically within 1-3 weeks. Some patients were continued for up to 2 years on treatment without loss of antidepressant action. The combination of deprenyl plus phenylalanine enhances brain phenylethylamine activity, while both deprenyl and phenylethylamine enhance brain catecholamine activity. Thus, deprenyl plus phenylalanine is also a natural antidepressant combination.

In 1991 H. Sabelli reported successful results treating 6 out of 10 drug-resistant major depressive disorder patients. Sabelli used 5 mg of deprenyl daily, 100 mg of vitamin B6 daily, and 1-3 grams of phenylalanine twice daily as treatment. 6 out of 10 patients considered that their depressive episodes terminated within 2-3 days! Global Assessment Scale scores confirmed the patients' subjective experiences. Vitamin B6 activates the enzyme that converts phenylalanine to phenylethylamine, so the combination of low-dose deprenyl, B6, and phenylalanine is another biological way to enhance both phenylethylamine and catecholamine brain function, and thus diminish depression. THE ANTIAGING DRUG Four series of rat experiments, as well as an experiment with beagle dogs, have shown that deprenyl can extend lifespan significantly, even beyond the "technical lifespan" of a species! Dr. Knoll reported that 132 Wistar-Logan rats were treated from the end of their second year of life with either placebo (saline injections) or 0.25 mg/kg deprenyl injected 3 times weekly, In the saline-treated group the oldest rat reached 164 weeks of age, and the average lifespan of the group was 147 weeks. In the deprenyl treated group, the average lifespan was increased by 30% to 192 weeks, with the shortest-living rat dying at 171 weeks and the longest-lived rat reaching 226 weeks, (a 38% increase). In a second series of experiments, Dr. Knoll treated a group of 94 "low-performing" sexually inactive male rats with either saline or deprenyl injections, (0.25 mg/kg) from their eighth month of life until death. Dr. Knoll had already established a general correlation between sexual activity status and longevity in the rats. The salinetreated, low-performing rats lived an average 135 weeks, while the deprenyl-treated, lowperforming rats averaged 153 weeks of life. The saline treated, high-performing rats lived an average 151 weeks of life, while the deprenyltreated, high-performing rats averaged 185 weeks of life. In addition, 17 of the 50 highperforming, deprenyl-treated rats exceeded their estimated technical lifespan limit of 182 weeks. Dr. Knoll's experiments were partially replicated by Milgram and co-workers, and Kitani and colleagues. Milgram's group used shorter-living Fischer rats, while still starting deprenyl treatment at 2 years of age, (effectively later in their lives). They also found a significant 16% increase in life span. The Kitani group, also using the shorterlived Fischer rats, started their deprenyl treatment at 1.5 years of age, and recorded a 34% life span increase. Ruehl and colleagues performed an experiment with beagle dogs and deprenyl, administered at 1 mg/kg orally per day, for up to 2 years and 10 weeks. In a subset of the oldest dogs tested (10-15 years of age), 12 of 15 deprenyl-treated dogs survived to the conclusion of the study, while only 7 of 18 placebo-treated dogs survived. By the time the first deprenyl-treated dog died on day 427 of the study, 5 placebo-treated dogs had already died, the first at day 295. Ruehl noted

that; "dogs provide an excellent model of human aging." So this study takes on an added significance. Dr. Knoll has repeatedly emphasized that the nigrostriatal tract, the tiny dopamine-using nerve cluster in the basal ganglia (or old brain), typically dies off at an average rate of 13% per decade starting at around 45 years of age in humans. This fact literally sets the human technical lifespan (maximum obtainable by a member of a species) at approximately 115 years, since by that age the nigral neuron population would have dropped below 10% of its original number, (at which time death ensues, even if in all other respects the organism was healthy). Based on the sum total of the animal deprenyl literature, and the 1985 study showing life extension in deprenyl-treated Parkinson's patients, Dr. Knoll has suggested the following: If deprenyl were used from the age of 40 something onward, and even if it only modestly lowered the nigrostriatal neuron death rate, i.e. from 13% to 10% per decade, then the average human lifespan might increase by 15 years, and the human technical lifespan would increase to roughly 145 years, (fig:2). After 45 years of research, Dr. Knoll has concluded that; "the regulation of lifespan must be located in the brain." His research has further convinced him that; "it is the role of the catecholaminergic neurones to keep the higher brain centres in a continually active state, the intensity of which is dynamically changed within broad limits according to need." Dr. Knoll's research has shown that catecholaminergic nerve activity reaches a maximum at sexual maturity, and then begins a long, gradual downhill slide thereafter. His animal research has shown catecholaminergic activity, learning ability, sexual activity and longevity to be inextricably linked. Dr. Knoll argues that the quality and duration of life is a function of the inborn efficiency of the catecholaminergic brain machinery. In other

His key conclusion is that; "as the activity of the catecholaminergic system can be improved at any time during life, it must be essentially feasible to [transform] a lower performing, shorter living individual to a better performing, longer living one." 1

DOSAGE AND SIDE EFFECTS Dr. Knoll consistently, throughout his deprenyl papers, recommends the use of 10 mg to 15 mg of oral deprenyl per week, starting in the 40s. Dr. Knoll's research clearly convinces him that deprenyl is both a safe and effective preserver of the nigrostriatal tract, as well as a catecholamine activity enhancer. Deprenyl is undoubtedly an important antiaging drug. The Glucose Cross-Linking Problem As long ago as 1942, Dr. Johan Bjorksten was working on what is now referred to as the Glycosylation Theory of Aging. Glycosylation is the product of a reaction between sugar and the free amino group of proteins. This is commonly referred to as cross-linking. The linking of glycosylation by-products to proteins, results in the development of large, cross-linked molecules that inhibit the ability of the cell to function normally. Dr. Bjorksten was a raconteur and often used "stories" to explain complex science. He described cross-linking by likening it to a factory where each day, more workers were handcuffed together. Eventually work would grind to a halt, and in the same way, inter and intra molecular crosslinks inhibit cell performance and output. It is believed that the cross-linking of the proteins that make up the human body, play a significant role in the human aging process. Everyone is familiar with the effects of cross-linking reactions, because the process causes food to turn yellow and become tough, (i.e. an apple that browns after it has been cut in half). Cross-linking may be responsible for many of the problems associated with old age, including senile cataracts, thickening of the arteries, some cancers and damage to the immune system. A damaged immune system leads to increased susceptibility to infection and some cancers may arise from the effects of glucose on DNA. Naturally, DNA contains all the information necessary to create a normal cell, however, it can react with glucose to produce damaged DNA, which in turn causes abnormal cells to be produced. Cross 1 inking, Aging and the Heart Aging is associated with cardiac enlargement and arterial stiffening. This may be the result of the age-related accumulation of Advanced Glycosylation End products, (AGE). One study conducted on animals showed that Aminoguanidine prevented age-related cardiac enlargement. In fact, the membrane surface area was reduced by 30%. Furthermore, the collagen content of their arterial walls was increased by 24% to 30%. Aminoguanidine is therefore acting to improve overall heart and arterial condition and not just by preventing or slowing proteins from cross-linking. It is also decreasing the AGE-induced cross-linking of the extracellular matrix. Studies conducted at the University of Milan over the last 25 years, have shown Aminoguanidine's ability to do two things; Firstly, in tests on animals, Aminoguanidine has reduced the ability of very low density lipoprotein, (the bad form of FAX ORDERING +44 (0)208 I8I 6106

cholesterol) to bind itself to blood vessel walls. In turn, blood platelets are less likely to coagulate and form dangerous clots. Secondly, Aminoguanidine has the ability to treat patients whose blood vessels are constricted by arteriosclerosis. At the University of Milan, 11 patients with peripheral vascular disease were treated with Aminoguanidine. Their blood vessels were so clogged that they couldn't walk for more than 500 meters, but after treatment, their blood flow improved on average by 30% and their ability to exercise improved by 50% to 105%. Diabetes Diabetes is often seen as a form of accelerated aging, and research has provided support for the idea that cross-linking is a cause of aging. This is because the levels of cross-linked products in diabetics are two to three times higher than those found in nondiabetics. It is believed that AGE is increased in diabetics and plays an important role in the development of diabetic complications. As Aminoguanidine acts to bind to sugars, thus preventing them from binding to the lysine group of proteins, it was only a matter of time before various trials began. A number of different studies with diabetic animals indicate that Aminoguanidine administered subjects have significantly superior survival rates, compared to those who are untreated. Diabetic clinical trials with humans have also highlighted Aminoguanidine's ability to prevent oxidative modification of low-density lipoproteins (LDL) and inhibit the formation of atherosclerotic plaques. American trials were conducted with Pimagidine (which is another name for Aminoguanidine). They highlighted that Aminoguanidine can significantly reduce Albuminuria, (proteins present in urine, usually as a result of kidney disease), delay the onset of end-stage renal disease and improve the cholesterol profiles of diabetic patients.

Research has shown that glucose is partly responsible for the cross-linking of proteins, which in turn leads to aging damage. Glucose is found in every cell of the body and is relatively stable, but can join with protein to form a glucose/ protein combination. It is this combination that will continue through a number of steps, to eventually cause active cross-links. The good news is, that the formation of this process appears to be reversible. Glucose/ protein substances stay in the body for months, even years, crosslinking with the proteins around them.

This continuous cross-linking may be prevented by using glycosylation inhibitors, such as Aminoguanidine, to help stabilize the metabolism of glucose. Aminoguanidine is able to join up with substances that cause links, and helps to stop cross-links from developing. Consequently, it may be able to help alleviate or prevent senile cataracts, thickening of the arteries, kidney failure, thinning bones, osteo-arthritis, skin wrinkles and many other signs of degeneration caused by aging. Aminoguanidine's ability to stabilize the metabolism of glucose, help prevent and treat adult onset diabetes, reduce very low density lipoprotein cholesterol, and the evidence that it can improve blood flow, (helping to reverse the conditions of arteriosclerosis and blood clots), indicates that Aminoguanidine can help prevent and treat a number of aging disorders. As all the body's proteins deteriorate with advancing age (and much more so in diabetes), Aminoguanidine has the potential to slow the aging damage process by protecting the proteins that make up the human body. In particular, these are skin proteins (collagen and elastin), eye lens protein, nerve protein and kidney proteins. Aminoguanidine is able to combat some of the adverse effects of diabetes and improve the quality and duration of life. Furthermore, as diabetes is an age-related disorder, and in-fact affects everyone over the age of 30 (because physicians alter the parameter of the test based upon chronological age), Aminoguanidine can be considered, a true anti-aging medicine. It is becoming clear that agents that slow, prevent and even reverse the effects of cross-linking, will have a major impact in the role of preventative medicine. Additionally, products that directly impact upon a theory of aging, such as the Glycosylation Theory of Aging, can indeed claim to be true antiaging medicine.

Is the Recommended Dietary Allowance for Vitamin C Too Low? Got a bottle of vitamin C somewhere in the kitchen cupboard? Most Americans do. A quick examination of the label on the back of the bottle reveals 60 mg provides 100% of the Recommended Dietary Allowance. That's far from what Dr. Linus Pauling suggested to achieve optimal human health. Dr. Pauling consumed 18,000 mg per day, claiming this water-soluble vitamin needed frequent replenishment through the day to achieve optimal health. Now freshly published research confirms many of Dr. Pauling's suggestions, but not without plenty of controversy.

Vitamin C advocates go on the offensive Now, however, vitamin C advocates are about to go on the offensive. In their new book, Ascorbate: The Science of Vitamin C (www.lulu.com), Professors Steve Hickey and Hilary Roberts at the University of Manchester in Great Britain allege the RDA for vitamin C has mistakenly been set too low. For the past year the British professors have been taking researchers at the Institutes of Medicine (IM) and the National Institutes of Health (NIH) to task for faulty science. Drs. Hickey and Roberts claim researchers established the current RDA for vitamin C without recognition of its half-life. In about 30 minutes, half of any dose of vitamin C disappears from the human body. The RDA was established by measuring blood plasma levels of vitamin C 12 hours, or 24 half-lives, after consumption. "To be blunt," says Hickey, "the NIH gave a dose of vitamin C, waited until it had been excreted, and then measured blood levels."

Considerable disagreement has ensued in the past decade over how much vitamin C should be consumed by adults. Vitamin C advocates have continually been on the defensive. The most recent negative news story about vitamin C emanated from Duke University where researchers mistakenly concluded from an animal study that more than the Recommended Dietary Allowance for vitamin C may worsen joint problems. But an earlier large-scale human study showed that adults who consume high-dose vitamin C experience a 3-fold reduction in the risk for progression of their knee osteoarthritis and that supplemental vitamin C reduces knee pain. [Arthritis Rheumatism 39:648-56, 1996] How did an animal study receive so much publicity when a contrary human study had already been published? Then back in 2001 a paper in Science Magazine errantly claimed that high-dose vitamin C might damage DNA. Again, the recommendation was to limit supplemental vitamin C to no more than the RDA. However, this recommendation was again based a test-tube study. Five human studies had already been conducted proving high-dose vitamin C does not damage DNA, but were overlooked by the reviewers at Science Magazine. When I called this to their attention they printed my rebuttal letter which cast doubt on the test-tube study which errantly warned the public away from high-dose vitamin C supplements.

Furthermore, the IM and NIH developed the current RDA using studies of just 15 subjects, not nearly enough to statistically establish an RDA for a population of 280 million people. Hickey and Roberts claim the amount of vitamin C needed for optimal health is likely to be more like 2500 mg per day, nearly 42 times greater than the current 60 mg RDA. Recent studies authored by NIH researchers confirm that the saturation point for vitamin C in blood plasma is not fully achieved even when taking doses as large as 2000 mg per day. Some of these studies now even confirm Dr. Pauling's claims that high-dose vitamin C may be effective against cancer.

here are a new breed of stimulants in town and they call themselves Eugeroics. Taken from Greek, Eugeroic simply means "good arousal," but eugeroics form a unique class of drugs and contain only two at present. They are Adrafinil and Modafinil, (both of which have been developed by Lafon Laboratories of France). The popular press has picked up on their ability as "lifestyle drugs" to improve people's work and concentration ability, you may know some of the trade names such as Provigil, Modiodal or Olmifon. The basis of their uniqueness lies in their ability to only "stimulate when stimulation is required." As a result, the "highs and lows" associated with other stimulants such as amphetamine, appear to be absent with eugeroics. Eugeroics have been designed to treat narcolepsy (sleeping in the day), hypersomnia (excessive sleep) and cataplexy, (a condition of sudden muscular weakness or fatigue). Yet uniquely, eugeroics don't affect normal sleep patterns, nor are they addictive and they have far fewer side effects than the previously prescribed stimulants. Modafinil's Unique Stimulation One animal study suggested that a possible vigilance enhancing property of Modafinil was its ability to inhibit the release of GABA, through an action on 5HTP serotonin receptors. However, the doses used were far higher than normal therapeutic doses. So meanwhile, most clinical studies point to Modafinil as a unique and highly selective agonist of brain postsynaptic receptor sites called, alpha-1 adrenergic. These alpha-1 sites are receptive to the neurotransmitter- Noradrenaline (also known as Norepinephrine). The central function of Noradrenaline is a relatively recent discovery, it appears to regulate alertness and the waking/ sleep cycle and has a role in the maintenance of attention, memory, learning, cerebral plasticity and even has neuro-protection qualities. Central nervous stimulants (CNS), such as Amphetamine or Pemoline are the most widely used drugs to treat narcolepsy, hypersomnia and cataplexy, but these have a number of well documented problems, particularly cardiovascular side effects, as well as interference with sleep, psychiatric disturbances and addiction. Two studies directly compared the affects of Modafinil and the "classic" stimulant, d-amphetamine on sleep. Both double blind controls involved about twelve individuals. One study was undertaken with individuals whose mean 12

age was 68, the other on much younger volunteers. They utilized 100 mg. or 200 mg. of Modafinil, or 10 mg. or 20 mg. of d-amphetamine, or a placebo. All the drugs were administered orally and sleep scales, awakening quality and psychometric tests were completed in the morning. The outcome of both tests showed that d-amphetamine caused a dose dependant impairment of sleep maintenance, but Modafinil did not. Here in lies a problem for those who have difficulty staying alert and awake during the day. The use of "classic" stimulants, such as amphetamines, threatens to reduce total sleep time and REM (rapid eye movement) sleep, and this will ultimately mean higher and higher doses are required, thus creating a vicious circle. On the other hand, Modafinil has not been shown to interfere with night time sleep, thus clearly indicating that the two compounds operate by different methods. Clinical Trials Numerous studies on animals since the mid 1980's, have confirmed the ability of Modafinil to increase awakeness and alertness, without serious side effects, or dependency. One animal test produced an interesting result. The researchers discovered that Modafinil decreased feeding and thereby reduced body-weight! The amounts of Modafinil required were not dose dependent and there was no alteration in the drink-feed ratio. Their conclusion was an interesting reference

to the possible mechanisms that underlie the relation between sleep, feeding and metabolism. Various human trials have concentrated upon Modafinil as a therapeutic agent to help maintain alertness and vigilance. A noteworthy conclusion of one clinical trial, compared Modafinil to amphetamine and described amphetamine as "vigilance increasing," but Modafinil as "vigilance promoting." This is an interesting comment because as we've seen, Modafinil won't prevent a person from sleeping if they want to, but if they wish to remain awake, they will do so with a far greater degree of alertness. This was borne out in one clinical trial, where volunteers were subjected to 60 hours of sleep deprivation! During their continued wakefulness, their vigilance was assessed using questionnaires; analogue visual scales and sleep latency tests. The subjects received either 200 mg. Modafinil, or a placebo every 8 hours. The Modafinil group sustained a satisfactory level of vigilance with an absence of sleep episodes, unlike the placebo group who gradually declined and slipped into "micro-sleep" episodes, (as one might expect when awake for longer than 24 hours). A French study, conducted over 3 years, discovered that Modafinil reduced drowsiness in 83% of hypersomniac patients and 71% of narcoleptics. Modafinil did not produce peripheric side effects, disturb night sleep and it was never responsible for drug dependence. Another French study conducted with 149 patients over a period of 2 years, treated patients with narcolepsycataplexy at 200 mg. to 400 mg. daily. The patients were then asked to score the benefit of Modafinil themselves, from 0 (no affect) to 3 (excellent affect), 64.1% of the subjects scored it as excellent. An earlier study, with 123 patients, (which also included those suffering with hypersomnia), the physicians evaluated the patients on a scale of 1 (no affect) to 4 (excellent affect). The results? 17% excellent response, 63% good response, 17% fair and 3% no affect. The incidence of side effects were minimal, (14 of the 123 patients had side effects, 11 of whose side effects disappeared when doses were reduced). These results have been repeated in a number of smaller clinical trials, and confirm Modafinil's excellent response to treating individuals suffering with narcolepsy, hypersomnia and cataplexy. Its efficacious use, in conjunction with a virtual absence of side effects, noncontraindication with normal sleep patterns and lack of drug dependence, certainly indicates that eugeroics are a breed apart from conventional stimulants. The Military Involvement It is fascinating to see that several countries armed forces have studied, and indeed use Modafinil for military operations. The use of stimulants to keep troops awake and alert is not a new one. It is known that British troops used them during the Falklands conflict and that USAF aircrews took amphetamines during the Libyan air strikes. The French government even admitted that its crack Corp- The Foreign Legionused Modafinil for covert operations inside Iraq, during the Gulf war. In-fact, Professor Michel Jouvet, an authority on sleep, claimed during an international defense meeting in Paris that, "Modafinil could keep an army on its feet and fighting for three days and nights with no major side effects." Not surprisingly then, we have heard that Modafinil is in use in some sections of the Belgian, Dutch and US airforces. Dosages and Side Effects Side effects in continuous studies of therapeutic dosages of Modafinil, lasting 3 or more years have been minimal and usually noted as nothing more than headache or nausea. In rare cases there has been hyper-salivation and moderate tachycardia (increased pulse rate), this probably accounts for Modafinil's instruction sheet, which states that those suffering from a heart condition must consult their physician before use. However, blood and pulse rates usually remain unchanged at normal therapeutic doses. In-fact, a suicidal 21-year-old female who ingested 4500 mg. demonstrated the relative safety of Modafinil. Her side effects were limited to tachycardia, excitation and insomnia. As is common with most drugs, Modafinil should not be used by those with serious liver disorders, nor those women who are pregnant or nursing. The official insert for the Modafinil package remains unclear as to the precise drug contraindications, but as it is an alpha-adrenergic agonist, those drugs that represent alpha-adrenergic antagonists should be avoided. These include prazosin, phentolamine or beta-blockers such as propranolol (Inderal). Furthermore, drugs that enhance norepinephrine activity, (such as yohimbine) should only be used with care, as there may be a synergistic affect. Adrafinil is noted as contraindicated with epilepsy and has been shown to enhance the potency of anti-epileptic drugs such as phenytoin, (Dilantin/ Epanutin). Although this isn't mentioned in any of the Modafinil literature we have read, it is best being advised of.

As for dosages, there seems to be little difference in doses and more importance is placed upon regular administration. Those who simply wish to remain awake and alert will benefit from a single 100 mg. dose. To remain "on-call" all day, 100 mg. in the morning and another 100 mg. in the afternoon is probably all that is required. For those wishing to remain awake during the night, then another 100 mg. in the late evening, (in other words approximately every 8 hours). Dosages for those suffering from narcolepsy, hypersomnia and cataplexy are 100 mg. to 200 mg. morning and afternoon. Some of the doses in trials have been as high as 600 mg. to 700 mg. daily, although doses over 500 mg. daily may cause euphoria, motor excitation or even insomnia! Adrafinil and Modafinil Compared Adrafinil was the world's first eugeroic, and developed some years before the introduction of Modafinil. At first glance there may appear to be little difference between the two. It is true that Modafinil is more potent, "average" doses of Adrafinil are in the region of 600 mg. to 1200 mg. daily, compared to Modafinil's 200 mg. to 400 mg. daily and this is self evident by the respective tablet sizes (300 mg. for Adrafinil and 100 mg. for Modafinil).

We were going to reiterate and condense the above article into a few lines, but the conclusion of the Aerospace Medical Association article on Modafinil was so pointed, we will leave the last words to them. "The development of Modafinil brings to light a crucial social question. What would be the impediment for its use, if a compound such as Modafinil is more like caffeine than amphetamine in terms of safety, and yet, as effective as the amphetamines?" However, if one were to compare Adrafinil and Modafinil on price alone then Adrafinil would win hands down! So the question must be asked, why did Lafon produce an analogue of Adrafinil? The answer probably lies in two parts. Firstly, it is thought that Adrafinil may not be such a highly selective alpha-1 adrenergic as originally thought, and may also affect other alpha receptors (albeit in a minute fashion). Secondly, Adrafinil is attributed to some other possible side effects that have not been associated (to date) with Modafinil, including, stomach pain, skin irritations, inner tension and in long term use, (over 3 months usually), an increase in liver enzyme levels, (which is reversible by reduction or withdrawal). It is our guess that it is the last potential side effect that lead to the development of Modafinil. Clearly, continuous Adrafinil use would require regular blood testing to monitor liver enzyme levels, and this is obviously inconvenient to those patients who need to use eugeroics on a regular basis. As none of the clinical reports we have read indicate any such problem with Modafinil, it would appear that the analogue has achieved its desired aim.

All of our skin is affected by age without exception, but some segments are more vulnerable than others and components of skin age at different rates and degrees. This is due to individual genetic variations, as well as external factors and influences such as sun exposure, diet and pollution. The skin is the largest organ in the body covering an area on average of about 1.49 square meters. Its main aim is to protect the body from the external world. Another of its functions is to provide sensory information about temperature, touch, pain and vibration etc. The skin is also a secretory organ which eliminates toxins through perspiration. Components of the Skin The epidermis is the outermost layer of the skin which protects it from injuries. When it comes to treatment, the problem is that it may be difficult for nutrients and pharmaceutical agents to penetrate the epidermis, and reach the inner layers of the skin. Scientists have invested a great deal of research in finding effective ways of delivering nutrients where it matters, deep into the skin. Liposomes are one such way, making use of tiny particles surrounded by lipid molecules which are able to penetrate the outer layers of the skin, carrying the nutrient or other chemical with them.

The main constituents of the dermis, (the inner layer of skin) are collagen and elastin. The collagen and elastin infrastructure keep the skin resilient, elastic and firm. Collagen fibres are strong protein molecules which form a scaffolding to support other skin structures. Elastin fibres also form a mesh or net allowing the skin to maintain its elasticity and firmness. These two types of molecules can be considerably weakened by age-related processes, such as glycosylation and oxidation. Photo-damage, (i.e. damage caused by the UV radiation of the sun) is also a major cause of collagen and elastin destruction.

Apart from the collage/elastin network, other components of the dermis are cells, (such as fibroblasts), fatty tissue, nerves, blood vessels, sweat glands, hair follicles and sensory structures. A major component is the intracellular matrix. This is formed by a mixture of protein/carbohydrate molecules surrounded by water. The main functions of the intracellular matrix are to facilitate the nutrition of the various skin components and to protect the skin against external insults. The fluid nature of the intracellular matrix gives the skin its plump, youthful and rounded appearance.

Within the matrix, moisture is retained by various substances such as NaPCA, a natural moisturiser. The problem is, the concentration of NaPCA diminishes with age and this causes progressive loss of moisture from the skin. However, the aging of the skin is a complicated process involving several interrelated factors. How Age Affects the Skin Strictly speaking, there are two types of skin aging. The intrinsic (or chronological) aging accounts for about 10% of all skin aging and is characterised by thinning of the skin, loss of elasticity and a decreased metabolic activity. The other 90% of skin aging happens as a result of photoaging, which causes damage to the skin followed by an exaggerated repair reaction resulting in collagen and elastin degeneration, dilatation of the small vascular vessels, deposition of abnormal elastin and collagen, and increased skin pigmentation. Intrinsic skin aging is due to genetic factors and endogenous influences such as oxidation and glycosylation, whereas extrinsic aging is due to external factors such as UV radiation, stress, pollution, and other environmental insults. Free radicals are the main culprits commonly implicated in causing skin aging. Oxygen and nitrogen free radicals are produced by cigarette smoking, environmental pollution, inadequate nutrition, pesticides, food chemicals, and most importantly, by UV action'upon the skin. When UV radiation interacts with the skin, it produces an excessive number of free radicals which destroy collagen, elastin and other proteins in the skin. UV radiation also interacts with the DNA inside skin cells causing further damage by allowing faulty DNA to replicate. In this way, imperfect DNA transcription and defective protein formation is perpetuated. Chromophore skin cells are specialised cells which absorb much of the UV radiation, but with age, their activities diminish resulting in more UV radiation reaching the inner parts of the skin. Years of this process will result in skin losing its moisture and support, appearing less shiny, wrinkly and droopy, with other noticeable imperfections. The risk of skin cancer is also considerably increased. This is one of the reasons that protection from the sun is paramount to maintain healthy, youthful looking skin for the longest time. A glycosylated protein may interact with another causing crosslinking, which is an irreversible union between two molecules. When this happens, neither of the molecules is free to perform its ordinary functions, and this contributes to further overall damage. An additional problem is that the cross-linked unit may then interact with other oxidised molecules, resulting in large and harmful molecules called AGEs (Advanced Glycosylation End products). These perpetuate the damage to the skin collagen, elastin and intracellular matrix, and increase the death rate of the fibroblasts. Glycosylation may also be caused by other toxic by-products and is connected with various inflammatory pathways, stimulating production of inflammatory chemicals such as tumour necrosis factor (TNF) and nuclear factor kappa b (NFkB). These inflammatory agents attack other skin cell material resulting in chronic skin micro-inflammation, which not only makes the skin appear red, rough and itchy, but causes wrinkles and skin blemishes. Therefore, the combined action of oxidation, glycosylation and chronic inflammation results in the visible signs of skin aging such as wrinkles and rough skin. Apart from the visible sings of aging, there are other less visible changes. For example, the sweat and sensory elements of the skin become affected with age. Sweating becomes more problematic and less effective. As a consequence, elimination of toxins from the skin declines, and toxins may thus accumulate in the body. (An exception to this is the increased sweating sometimes

Preventing and Treating Skin Aging Coenzyme Q10 Studies show that Coenzyme Q10 applied directly to the skin helps reduce the appearance of wrinkles and fine lines, it also promotes the repair of fibroblasts. But after the age of 30-35, the body becomes less able to create enough Coenzyme Q10 to meet its energy needs. This is made worse by bad eating habits, stress, infection and certain drugs. Coenzyme Q10 can significantly suppress the expression of collagenase, an enzyme which destroys collagen. In addition, it can penetrate the outer layers of the skin and exert antioxidant benefits deep in the dermis. One such agent is Aminoguanidine. It helps prevent glycosylationrelated collagen destruction and it is used in order to protect against skin roughness, age-spots, wrinkles and other blemishes. Specifically, Aminoguanidine works by blocking the expression of metalloproteinases. These are digestive enzymes which destroy the components of the matrix, resulting not only in skin aging but also in arterial and kidney disease. Metalloproteinases are induced by UV radiation and their action depends on the nitric oxide radical (NO). Scientists from the Division of Chemical and Biological Engineering, Konkuk University, Seoul, have shown that when Aminoguanidine is added to a solution containing metalloproteinases, it significantly reduces their activity. Translated into clinical terms, this means that Aminoguanidine has an additional benefit in protecting the intracellular matrix against age-related changes. It is known that the formation of AGEs contributes towards activation of metalloproteinases and enhances the degradation of collagen. However, Aminoguanidine, (which reduces AGEs) may have an important effect upon this process by reducing metalloproteinases as described above, and by protecting the intracellular matrix against age-related changes. Carnosine is a dipeptide, (a combination of the aminoacids alanine and histidine) which works in three different ways: 1. It prevents lipid peroxidation affecting the skin cell membrane. 2. It reduces the risk of glycosylationinduced damage. 3. It modulates agents causing microinflammation of the skin. Carnosine has been thoroughly investigated in terms of wound healing. It possesses several healing properties as an antioxidant, metal chelator and inflammation regulator. During wound healing there is an increased production of good quality collagen and proliferation of healthy fibroblasts. The balance between adequate and excessive formation is quite delicate and, if disturbed, it may result in excessive scarring. Carnosine can modulate the production of factors which regulate the equilibrium, thus preventing scar formation. The same is true for age-related skin damage where there is an attempt by the body to increase the formation of collagen and fibroblasts to replace those elements which had perished through lysis or though apoptosis respectively. Carnosine is an essential nutrient in this respect. In addition, carnosine can protect against UV radiation, so can be useful for individuals, who despite using sun screening lotions, are still liable to skin damage. DHEA A natural molecule, Dehydroepiandrosterone plays an important role against aging in general and against skin aging in particular. Taken in oral form or applied topically, DHEA increases the immune resistance of the skin, which in turn prevents inflammation-induced skin damage. It also stimulates the production of collagen and elastin and improves skin cell metabolism. In a classic experiment, 280 healthy volunteers aged 60-79 years were treated with 50 mg DHEA or placebo (in oral form), daily for a year in a double-blind, placebo-controlled clinical trial. The results showed that there was a restoration of the age-related decline of DHEA concentration back to youthful levels. Additionally, there was a small increase in testosterone and estradiol, especially in women patients. In the patients who were given DHEA, their skin exhibited significant improvement particularly in relation to hydration, epidermal thickness, sebum production, and abnormal discoloration. No side effects were reported. This demonstrates the promising effects of DHEA used specifically for skin health. DHEA is available in sublingual liquid as well as 25 mg and 50 mg capsules. A new medical discipline, called aesthetic endocrinology, is a study of how hormones can be used topically to help prevent age-related skin problems. Retin-A The retinoids, (derivatives of vitamin A) are a group of chemicals which have consistently and repeatedly been found to prevent and even reverse age-related skin damage. Retinoids work in a variety of ways. One of the most important is the antioxidant action, but not only do they prevent oxidation and lipid peroxidation, but they also interact with AGEs to limit glyco-oxidation, (damage caused by glycated material interacting with free radicals). Retin-A is an effective skin protector, which stimulates the expression of one of the commonest forms of collagen in the skin, type VI collagen. In particular, topical retinoids such as Retin-A influence glycosyltransferases (enzymes responsible for carbohydrate synthesis) and thus affect the rate of skin glycosylation. A day in the life of an antiaging clinic

American born Heather Bird is the President of HB Health, and is also a Director of the World Academy of Antiaging Medicine. Her London clinic, HB Health of Harley Street, focuses on internal antiaging medicine and preventative health, while her newly opened HB Beauchamp Place, specializes in external antiaging and beauty services.

AAM: Thank you Heather for taking the time out of your busy schedule to talk a bit about the operation of an anti-aging clinic. Perhaps you could tell our readers what made you personally get involved in preventative medicine? Heather: In my formative years I worked with the UN World Health Organization in Switzerland. During my time with them I got to visit many geriatric wards. A lot of what I saw there made me feel sad; knowing how different many of these people must have been in their youth, in some cases just a few years beforehand. I started to think that this kind of debilitating and degenerative aging is a disease, and that's when I became an ardent advocate of antiaging or anti-degeneration if you prefer.

AAM: I think we can all sympathise with that point of view. I've often said myself that if anyone is feeling down for any reason, they should pop along to the local hospital and visit the geriatric ward. They always welcome visitors to talk with the patients; but after an hour or two, it will soon become apparent that compared to them, one has no worries at all! Let me say Heather, you are in an interesting position, as an American living in Europe, how do you find the people of different countries compare with their view to health and beauty? Heather: There are not as many differences as most people might think, but on the whole I think that Americans embrace new ideas more readily than Europeans. It's often said that

Europe is 5-years behind new trends in America, and that's certainly true for antiaging medicine in general.

AAM: Personally I find that whilst Europe has a lot more drugs, and certainly has cheaper prices on the majority of them- compared to the States, that the opposite is true of nutrition. In its broadest sense, Europeans have less access to nutrition and certainly pay more for them. As the 1996 U.N Codex Alimentarius Commission launched a worldwide initiative to restrict access to natural medicines, and which now has been effectively been translated into law by the EU, it may be as early as August 2005 that the European Food Supplements Directive could become active. This is estimated to be able to remove over 5000 vitamin formulations from shops and dramatically cut the dosage of many others. In addition, the Herbal Medicines Directive- under consideration by the European Parliament, will prevent any new herbal formulas from ever coming onto the market and remove any herbal product which hasn't had a proven track record in the E.U. Meanwhile, the Pharmaceuticals Directive seeks to expand the scope of the drug classification and in its current wording even food or water can be included as a substance which- and I quote; restores, corrects, or modifies physiological functions! Heather: It's frightening stuff, but luckily there

AAM: I did notice with interest that you have split your operation into two different parts, one for internal antiaging medicine, the other for external. How has that helped you? Heather: Typically most people thinkincorrectly- of antiaging medicine as a branch of aesthetic medicine. In other words 'external' beautifying treatments, perhaps not necessarily of a cosmetic nature. Whereas 'internal' antiaging medicine is the more long-term, dare I say, 'serious' end of the spectrum, where we seek to slow down, arrest and hopefully reverse the aging process. It's not often that clients come in for both types at the same time, so our spa and clinic facilities have been separated to cater specially for each particular requirement.

Which is the best form of deprenyl to use? On paper the 5mg tablets are better value, and if high dosages of deprenyl are required, (i.e. as in Parkinson's disease), then the tablets make better economic sense. The tablets are in a hydrochloride base (an inorganic molecule) and due to the nature of tablets also contain fillers. In comparison the liquid is in a citrate base (an organic molecule) and small does of the liquid can be titrated, (e.g. 1 drop equals 1 mg.) This makes the liquid preferable for antiaging purposes where maybe only 1 to 3 mg. are required. There has been a dispute between the manufacturers of Gerovital-H3 ® and the Romanian government that effectively stopped production/ export of the original Gerovital-H3. There are two answers to this question. Firstly, if you want to take low dosages of Hydergine, then you can titrate as little as half a milligram with the liquid, whereas a quarter of a Hydergine tablet is equal to 1.125 mg. Secondly in rare cases the tablets can cause a stomach upset, which is less likely to occur with the liquid as some of the product is absorbed sublingual^.
