Individuals with special needs have special vision needs.

There are many conditions that affect a person’s development and make day to day activities more difficult.

Often an entire team of health care providers is required to help a person with special needs achieve their full potential. The visual system is frequently affected in these individuals, making a comprehensive eye examination a crucial component in the care of children and adults with special needs. Certain syndromes are associated with high prescriptions, while others affect eye focusing, eye teaming, or visual perception. Examples of syndromeassociated vision problems include: Autism Spectrum Disorder (ASD): ASD is expressed in a wide variety of ways, meaning no two individuals with autism are exactly the same. Variety is also observed in the multitude of visual conditions present in this population. Patients with ASD can have difficulties with eye teaming, eye focusing, eye tracking, and visual perceptual challenges. Some people may have a hyperactive visual system, which makes colorful and abrupt visual stimuli disturbing. Others may demonstrate

a hypoactive visual system and are drawn to colorful and bright visual stimuli.

Cerebral Palsy: The majority of patients with cerebral palsy have “cortical visual impairment�, or damage to the visual tract in the brain resulting in reduced vision that cannot be corrected with glasses. It is import that vision loss due to cortical visual impairment be identified so that appropriate recommendations can be made for a modified learning environment. Individuals with cerebral palsy also have difficulty focusing their eyes at near, resulting in blurry vision. Blurred vision can make educational material or augmented communication devices difficult to see.

Down Syndrome (Trisomy 21): Down Syndrome is the most common chromosomal condition in the United States. Individuals with Down Syndrome are more likely to have significant refractive errors and are more prone to cataracts at a younger age. continued on page

The prevalence of vision conditions in this population make comprehensive eye examinations very important.

These are just a few of the many examples of how the eyes can be affected in individuals with special needs.

Early detection of vision conditions is especially impor-

tant in young children with special needs as any vision deficit can affect overall development and learning.

Comprehensive eye examinations are important, but what happens when someone cannot sit through a standard eye examination, or is unable to provide responses to traditional eye tests?

Eye doctors with formal, advanced residency-training in pediatrics and special needs have both the knowledge and clinical experience to provide the elevated level of vision care required for individuals with special needs; regardless of their age. Depending on a person’s cognitive abilities and level of communication, modified testing may be required to evaluate the functional aspects of vision. It is important to ask if the doctor has both residency-training in pediatrics and special needs as well as the equipment and space required to provide the specialized vision assessments these patients need. Asking these questions will help ensure your loved one is working with a doctor who is familiar with the visual systems of those with special needs.

A Durable Power of Attorney for Healthcare allows you to determine a stand-in decision maker if you are unable to speak yourself. They will have the same power as you would. This is usually a spouse, child, or trusted friend who knows your wishes and will carry them out for you, should it become necessary. Again, an open and frank discussion ahead of time makes a crisis much easier to deal with.

Custom compounding is medication problem solving Most of the community enjoys the use of a traditional community pharmacy. Generally, this is a retail-oriented building with lots of remedies, vitamins and a secure area where one will find a staff of people working on packaging prescriptions together.

In a compounding pharmacy, all of these things are present, but if you look carefully, you would notice a compounding laboratory.

In this area, technicians and pharmacists are using specialized equipment and techniques to formulate, or “compound” medications that are not traditionally manufactured.

Now, one might ask why we need to compound medicines when we have so many other medications available for just about everything. Well, just about everything is missing about two percent of all the necessary drugs that our providers need to resolve patient’s medical needs.

There are many examples of compounding and I would like to discuss several of these so we all have an understanding of “pharmacy compounding.” In a hospital, a patient may require an IV, which is an 12

intravenous solution to be placed in a person’s vein. These are mixtures of special drugs in buffered solutions. A pharmacist has prepared this mixture from a formulary of compounds, usually Trissel’s Handbook of Injectable Drugs,” in a sterile environment.

These solutions are compounds.

In our compounding pharmacy, we formulate non-sterile compounds. The most common dosage forms are creams, liquids, suspensions, suppositories, capsules, sublingual tablets, liquid sprays, lollipops and powders.

Depending on the dosage form, we are able to incorporate medicines into these vehicles depending on the need.

For example, we have many patients under the age of 2 who require heart medicine. In our lab, we are able to take the adult heart medicine and dilute it to an infant dose and dispense it in a flavored liquid. The drug manufactures’ do not make these drugs in forms that young children may require. Other examples include medicines for pain, GERD, autism, dermatology, thyroid replacement, vaginal issues, and many others.

A very popular area of pharmacy compounding is bioidentical replacement therapy, or BHRT. Many providers prefer to solve hormone-related problems for both men and women with hormones that are identical to human hormones. The hormones most commonly prescribed are estriol, estradiol, progesterone, DHEA and testosterone.

We are able to place these hormones into special dosage forms for patient use. The most common is topical transdermal creams.

Hormones that are administered through the skin have the advantage of performing their task without being metabolized by the liver first. This allows for lower doses and less work on the liver and a more even affect. BHRT is a medical science that many providers specialize in.

Another area that uses a compounding pharmacy is veterinary medicine. We are able to compound all kinds of Dog and Cat medicine.

We can formulate pleasantly flavored medicines; the most popular would be grilled chicken, to treat high blood pressure. We compound transdermal creams that owners apply to the in-

side of the ears to treat thyroid disease. . We also compound formulations to treat seizures, bladder control, Heart disease, pain and many other conditions.

I hope this information has given us a general understanding of the needs of a compounding pharmacy in the community. I specialize in solving a patient need that cannot be resolved with traditional therapy. That two percent of the patients who may have an absorption problem, for example. Some patients cannot absorb medicine adequately from their digestive tract. We can prepare compounds that are absorbed through the skin, or alternate route therapy.

Some patients are allergic to an ingredient in their prescription. In the lab, we can compound a medicine without such an ingredient. Also, discontinued medications can be compounded.

Compounding pharmacists specialize in resolving difficult medication therapies with our skills and knowledge in the science of compounding.

The Myth of Seeing 20/20 Many people know that seeing 20/20 is a good thing, but what does that really mean? Being able to see 20/20 means that you can see the same size of letters that the average person should be able to see from 20 feet away.

What you may not know is that seeing 20/20 is only one aspect of “seeing”.

When you think about everything you ask your eyes to do throughout the day, you realize that seeing 20/20 tells you very little about how your eyes perform. 80% of what we perceive, comprehend and remember depends on the visual system.

Imagine if you were a kid again; learning to read,

playing sports, and going to movies with friends.

Now imagine how frustrating it would be if the words moved on the page or went double when you were trying to read. How much fun would baseball be if you could never seem to hit the ball? Would you want to go to a 3D movie if it gave you a headache?

25% of school-age children have vision problems that cause symptoms like these, yet many of these conditions go undiagnosed because the child can see 20/20 on the eye chart. It is important to know that seeing 20/20 or passing a vision screening does not mean that a vision problem is absent.

During a comprehensive developmental eye examination, the doctor is able to assess much more than the ability to see 20/20. He or she can evaluate eye health and how well the eyes focus, team, and track.

Evaluation of these vision skills is especially important in children. Eye tracking, teaming, and focusing deficits can interfere with development, academic progress, and athletic performance. Early diagnosis and treatment of these vision conditions lessens the negative impact they may have on the child’s development and learning.

Middle Ear Infections in Childeren Middle ear infection (otitis media) is the most common cause of preventable hearing loss among children. It is estimated that at least one-third of all children will suffer a middle ear infection before the age of 5.

Symptoms of an infection very from pulling on the ear to a feeling of pressure and blockage in the ear to severe earaches or drainage.

Because otitis media can lead to significant hearing loss if not treated promptly, parents should be aware of these symptoms.

Unfortunately, hearing loss is often overlooked by parents and is often detected only during rou-

Long-term hearing loss can lead to a learning disability.

Serous otis media is an infection in the middle ear and accumulation of fluid. It is caused when the Eustachian tube, which connects the middle ear with the back of the nose, becomes blocked. The blockage can be caused by a cold, allergy, infection in the throat, or enlarged adenoids.

Middle ear infections are common in children because the Eustachian tube is shorter, wider and more horizontal in children than in adults. Thus, it is more susceptible to blockage.

Left untreated, otitis media may result in permanent damage, hearing loss, and a cyst in the middle ear that can spread to other parts of the ear.

Nutrition, Exercise and Mental Wellness Nobody doubts the benefits of good nutrition and exercise for physical health. What isn’t as widely known or discussed is how essential it is to our mental well-being.

One of the most obvious, yet under-recognized factors in the development of major trends in mental health is the role of nutrition and exercise. The body of evidence linking diet and mental health is growing at a rapid pace.

Many of us already know that when we make healthy food choices we tend to feel better, and when we eat heavy, sugary foods we can feel tired or sluggish. Therefore, it should come as no surprise that there is considerable evidence to suggest that nutrition can have an impact on mental health as well as physical health.

A balanced mood and feelings of wellbeing can be protected by ensuring that our diet provides adequate amounts of complex carbohydrates, essential fats, amino acids, vitamins and minerals and water.

A healthy diet can be more expensive. Fish, fruit and vegetables can be particularly pricey. However, by cutting down on sugary drinks and snacks, takeaways and alcohol, you can save money so you can buy healthier foods. Frozen fruit and vegetables are often cheaper than fresh produce and are usually just as good nutritionally.

These are some tips to improve your mood through nutrition and exercise: Eat regular meals throughout the day to maintain blood sugar levels. Make sure you eat at least three meals each day. Missing meals, especially breakfast, leads to low blood sugar and this causes low mood, irritability and fatigue. If you feel hungry between meals you may need to include a healthy snack eg. fruit, nuts and cereals.

Aim to eat at least five portions of fruit and vegetables a day eg. 1 glass of orange juice or ½ grapefruit for breakfast, a banana or apple for a mid morning snack, salad at lunch time and then two types of vegetable (a portion is roughly two serving spoons) and piece of fresh or baked fruit for your evening meal.

Include protein at every meal to ensure a continuous supply of the amino

acid tryptophan to the brain. We all need to eat enough protein to maintain our skin, organ, muscle and immune function but recent research suggests that one particular component of protein, the amino acid tryptophan, can influence mood. Include some red meat and fish, as they are good sources of vitamin B12, another nutrient that seems to be associated with mood.

Eat a wide variety of foods to keep your diet interesting and to ensure you obtain all the micronutrients you need. The more varied your diet, the more likely you are to obtain all the nutrients you need. If you have bread at one meal, try cereal or potatoes, rice or sweet potatoes at the others.

Make sure you include at least 2 portions of different fruits and/or vegetables and a protein food at each meal. Include fish, especially oily fish, in your diet.

Depression affects different people in different ways. Some people lose interest in food or can’t motivate themselves to shop and cook, so therefore lose weight. Others find they want to eat more and gain weight when they are unhappy. Some medications can also increase or decrease your appetite – if you are concerned that the medication you are taking has made your weight problems worse, speak to your doctor.

Both excessive weight loss or weight gain can make your mood worse and should be avoided.

Weight loss and lack of good nutrition will deprive the brain of glucose and the other nutrients that control mood – you may need the advice of a dietitian to help you overcome this problem.

Putting on weight unintentionally or feeling out of control of your eating can increase your depression and can lead to yo-yo dieting, which leaves you further out of control.

If you are overweight, follow the advice on healthy eating but be extra careful to limit your fat and sugar intake (no fries, pies, cakes, puddings, sweets, chocolate or sweet drinks), use less fat in cooking, reduce your alcohol consumption, avoid sugary drinks,

and increase your exercise levels.

Not drinking enough fluid has significant implications for mental health. The early effects of even mild dehydration can affect our feelings and behavior.

Coffee, colas, some energy drinks and tea all contain caffeine, which some people use to boost energy levels. However, in large quantities caffeine can increase blood pressure, anxiety, depressive symptoms and sleep problems.

If you do take drinks with caffeine in them, try to limit yourself to just 3–4 cups per day and drink other fluids such as water, fruit juice and non-stimulant herbal teas at other times. Chocolate also contains caffeine and should be limited to an occasional treat.

Alcohol has a depressant effect on the brain and can result in a rapid worsening of your mood. It is also a toxin that has to be deactivated by the liver. During this detoxification process the body uses thiamin, zinc and other nutrients and this can deplete your reserves, especially if your diet is poor.

Thiamin and other vitamin deficiencies are common in heavy drinkers and can cause low mood, irritability and/or aggressive behavior, as well as more serious and long-term mental health problems.

Exercise leads to the release of endorphins – feel-good chemicals in the brain that help us to relax and to feel happy. Exercise is particularly important for people with depression as it also gives structure and purpose to the day.

Healthy Meals Made Fast Busy families are increasingly looking to free up time in their busy schedules. While certain shortcuts can make daily life more manageable, families sometimes pay too steep a price in their efforts to save time. Such is the case at the dinner table, where nutrition is often sacrificed at the expense of convenience. But quick and easy meals need not be void of nutrition and flavor.

North Americans have increasingly turned to fast food, convenience items and dining out as an alternative to homecooked meals. However, such processed foods are often unhealthy. According to the book, “Eat It to Beat It!”

by David Zinczenko, the average restaurant meal contains 58 grams of fat, or about 90 percent of the recommended daily fat intake for an average person.

What’s more, 70 percent of the average American’s caloric intake is derived solely from processed foods.

Moderation plays a crucial role when crafting a healthy diet. Families can enjoy the occasional meal out provided they are careful about what they eat during other times of the week or month.

Time-pressed families can employ a few strategies to help them eat better by enjoying more homecooked meals.

• Make a list of favorite family meals. Start by thinking about what foods 18

the entire household enjoys. Create a mealplanning schedule so that you can purchase all of the ingredients at once for the week so you aren’t making any time-consuming lastminute trips to the store.

• Shop off-peak hours.

Visit the supermarket during off-peak hours, such as early morning or late at night. Avoid Sunday afternoons when stores are often crowded. Shopping during off-peak hours can save you time.

• Separate bulk purchases into smaller portions.

Take the time to divide larger packages of meat or produce into smaller portions more appropriate for the family. Frozen meats will defrost more quickly, and it’s easy to grab just what you need to pull together a meal.

• Prep like a pro. When you have free time, wash and cut vegetables, then store them away. Cook larger quantities of foods you eat often, such as pasta or chicken. These ingredients can be used in many different meals.

Having them precooked or partially cooked cuts down on the cooking time during the week. Think ahead to which leftovers can be turned into new meals. Perhaps those grilled chicken breasts can be turned into fajitas or used as the base of a soup or stew later in the week.

When schedules overlap, have someone peel potatoes in the morning and set them into a bowl of water in the refrigerator to used that evening.

Another person can arrive home from school or work and start seasoning meat or poultry while someone else can be in charge of the actual cooking when he or she arrives home.

Working together cuts down on the amount of time any one person has to spend making meals.

• Embrace the slow cooker. Slow cookers are a boon to busy families.

Various ingredients can be placed inside of the crockpot and then set to cook slowly for several hours. The slow cooker uses a very low temperature to cook the foods for anywhere from 5 to 8 hours. The results are often flavorful, tender meals.

The Pros And Cons of Waiting to Start a Family

Present-day couples are opting to start families later than their parents and grandparents before them.

It is no longer uncommon for grown children to live with their parents well into their 20s, and many adults are opting to get married, and have children later in life, if they plan to have any children at all.

According to the U.S. Census Bureau, the average age for men to get married is 27 and for women 25.5. However, couples who come from more religious families in which living together before marriage is often frowned upon, may get married earlier. On the flip side, couples who live in communities and cities where cohabitating is readily accepted may wait until

their thirties before tying the knot. In such communities, starting a family is often put on hold. In the 1970s, many women were having children in their early twenties, whereas now women tend to have their first child in their mid- to late twenties. One survey of women from the United Kingdom found the average age for a woman to have a baby for the first time was 29.

Although there is no right and wrong answer as to when is the best time to start a family, there are some advantages and disadvantages to waiting to have children.

Disadvantages Starting a family later in life can seem like a good idea but may cause some

unnecessary obstacles.

One of the first challenges is biological. Aging may make it more difficult for women to conceive a child.

Not only is the number of unfertilized eggs a woman carries finite, but also there are only a set number of years in which a woman can conceive before reaching menopause. But men, too, also have varying degrees of infertility as they get older. One study conducted at Soroka University in Israel found semen quantity peaked for men between the ages of 30 and 35, while overall semen quantity was found to be lowest after age 55.
