Listeriosis, a serious infection usually caused by eating food contaminated with the bacterium Listeria monocytogenes, is an important public health problem in the United States and countries around the world. The disease primarily affects older adults, pregnant women, newborns, and adults with weakened immune systems. However, rarely, people without these risk factors can also be affected.

LISTERIA INFECTION Listeriosis can present in different ways. In older adults and people with immunocompromising conditions, septicemia and meningitis are the most common clinical presentations. Pregnant women may experience a fever and other non-specific symptoms, such as fatigue and aches, followed by fetal loss or bacteremia and meningitis in their newborns. Immunocompetent people may experience acute febrile gastroenteritis or no symptoms. Illness is either mild or quite severe, in what is sometimes referred to as a “bimodal distribution” of severity.

In case of mild symptom, listeriosis usually consists of the sudden onset of fever, chills, severe headache, vomiting, and other influenza-type symptoms. Along these same lines, infected individuals may develop fever, muscle aches, and sometimes gastrointestinal symptoms such as nausea or diarrhea. Most healthy adults and children who consume contaminated food experience only mild to moderate symptoms.

When pregnant, women have a mildly impaired immune system that makes them susceptible to Listeria infection. The illness appears as an acute fever, muscle pain, backache, and headache. Illness usually occurs in the third trimester, which is when immunity is at its lowest.

Infection during pregnancy can lead to premature labor, miscarriage, infection of the newborn, or even stillbirth.

22% of such infections results in stillbirth or neonatal death.

Newborns may present clinically with early-onset (less than 7 days) or late-onset forms of infection (7 or

more days). Early-onset listeriosis is most often acquired through trans-placental transmission. Late-onset neonatal listeriosis is less common than the early-onset form. Clinical symptoms may be subtle and include irritability, fever and poor feeding.

AT RISK POPULATIONS

Listeriosis can pose major risks for certain populations.

Pregnant women, older adults, and individuals with weakened immune systems are at greater risk.

Pregnant Women Pregnant women are approximately 20 times more likely than other healthy adults to get listeriosis. In pregnant women, it is typically a mild, flu-like illness. In the child, listeriosis can lead to miscarriage, stillbirth, or life-long health problems.

Older Adults As adults age, it is normal for their bodies not to work as well as they did when they were younger. Changes in their organs and body systems are expected as they grow older. These changes often make them more susceptible to contracting a foodborne illness such as Listeriosis.

Immunocompromised persons A properly functioning immune system works to clear infection and other foreign agents from the body. However, certain conditions including cancer and its treatments, diabetes, HIV/AIDS, and organ or bone marrow transplants can weaken the immune system – making the body more susceptible to many types of infections, including foodborne illness such as Listeriosis.

LISTERIA OUTBREAK

Outbreaks of Listeriosis linked a ranged of food, processed food in particular. In the Western world, wellcollected data and proper epidemiological investigation mean more information. On the other hand, lack of data does not mean lesser outbreaks but evidence. These are three latest records of outbreaks in the North America region. These cases are not about addressing outbreak itself, it is also about how to deal with it. A report by WHO suggested that an urgent effort is needed to fill in information on Listeria infections in developing countries, as countries accounting for 48% of the world’s population do not report Listeria illnesses.

A total of 19 people infected with the outbreak strain of Listeria were reported from nine states. Listeria

specimens were collected from July 5, 2015 to January 31, 2016. Ill people ranged in age from 3 years to 83. Of ill people, 74% were female. A person from Michigan died as a result of listeriosis. Listeria isolates from all 19 ill people showed that the isolates were closely related genetically.

This close genetic relationship means that people in this outbreak were more likely to share a common source of infection, such as a contaminated food. Whole genome sequencing of clinical isolates from ill people in Canada showed that the isolates were closely related genetically to Listeria isolates from ill people in the United States. Of 14 ill people who were asked about packaged salad, 13 (93%) reported eating a packaged salad. All of the 9 ill people who specified the brand of packaged salad eaten reported various kinds of packaged salad.

On January 21, 2016, the company reported that it had stopped production at the processing facility in Springfield, Ohio and withdrew packaged salads from this facility that were on the market at that time. On January 27, 2016, They voluntarily recalled all salad mixes produced in this facility.

Multistate outbreak of listeriosis linked to soft cheeses distributed by Karoun Dairies, Inc. Thirty people infected with one of the closely related Listeria strains were reported from 10 states since June 16, 2010. Ill people ranged in age from less than 1 year to 92, 70% of ill people were female. 6 of the illnesses were pregnancy-related, and one illness resulted in a fetal loss.

Three deaths were reported from California (2) and Ohio (1).

den Bakker et al. (2014) took samples of agricultural and natural environments in two US states (Colorado and Florida), yielding 18 Listeria-like isolates that could not be assigned to previously described species using traditional methods. Using whole-genome sequencing and traditional phenotypic methods, they identified five novel species, each with a genome-wide average BLAST nucleotide identity (ANIb) of less than 85% to currently described species. Five new species of Listeria are Listeria floridensis sp. nov., Listeria aquatica sp. nov., Listeria cornellensis sp. nov., Listeria riparia sp. nov. and Listeria grandensis sp. nov. Genomic and phenotypic data suggest that the novel species are non-pathogenic. www.thermofisher.com

FDA isolated Listeria monocytogenes from two environmental samples collected in September 2015 from the Central Valley Cheese, Inc. manufacturing facility in Turlock, California. Central Valley Cheese, Inc. manufactures cheese for Karoun Dairies. Whole genome sequencing showed that these isolates were closely related genetically to isolates from ill people. In addition, whole genome sequencing showed that five Listeria isolates collected in 2010 from the same facility were also closely related genetically to isolates from ill people.

On September 2015, Karoun Dairies, Inc. voluntarily recalled certain cheeses that the company distributes due to possible contamination with Listeria. The recall included several brands and types of cheeses that were distributed to retail outlets in the United States.

WHO STUDY EVALUATES GLOBAL LISTERIA BURDEN

In 2010, Listeria monocytogenes was estimated to infect 23,150 people worldwide. It killed 5,463 of them, or 23.6%. Of those who fell ill with Listeria in 2010, 20.7% were pregnant women. Among the pregnant women who suffered Listeria infections, 14.9% of the infections resulted in infant fatality. Researchers found that Listeria caused the highest burden on quality of life in Latin American region.

The least affected region was Eastern Europe. Other highly affected areas included Southeast Asia, Africa, Polynesia
