ABSTRACT Patient Tracker app by doctor is a project that helps doctor and patient keep their record on the server to find the best access point .To keep their record in this application doctor and patient must register according to their registration process after which they will get User id logged in as doctor and patient and after that they can continue. Nowadays the app that is available online all have a separate patient and physician platform so this project will provide one platform for doctor and patient to keep their record up to date. They do not need to carry their own reports with all the doctors and patients in the middle of which they can get patient reports and their appointments and all the doctors' appointments. The purpose of this app is basically to make a good user application and make it easy to use for everyone. Keywords: IDE, DBMS, MYOPD, Patient tracker App by Doctor.

I.

INTRODUCTION

The project “Patient tracker ” has been developed son Android SDK, Java, HTML, CSS, and MYSQL. Android Studio used for Android application development. The Project is an android application to assist doctor’s patient data management and viewing. The doctor can easily add new patient’s information as well as track the patient history at any given time all in his android phone. The system is a stand-alone system that can be installed on an android phone and can be used with valid login credentials. When Doctor installs this application and after successful login into the application, a doctor can add a new patient, his personal information, medical information, and also, the doctor can insert new medical appointments details into the existing patient’s medical appointments records and doctors can also insert various data of patients like patient name, contact, age, blood-group, gender, medical condition, medication provided and date of the last appointment which the system automatically takes through DBMS, etc. The system saves all the patient information in a database created on the android phone. The doctor can also view this data when needed. By this application, doctors can search patients by his name and it displays all his medical conditions and the medication-related details from recent visits. The application also has a feature where the doctor can pull out all the patients he attended for any particular date. To keep their record in this app doctors and patients have to registration according to their registration process after this they will get a user-id by which they log in as a doctor and a patient and after it, they can further proceed. It is a user-friendly application which used to easily accessible for users.

LITERATURE SURVEY

Traditionally the tracking systems available for Patient and Doctor that were Patient cloud, medical records App, patient communicator, MYOPD, list of my Patients, Pc360, True hub None of these provide a common platform for user and Patient. all are providing at a time only one’s data on the server and they didn’t contain centralized data of patients and Doctors. At a time, they contain one user’s data may be its patent, and maybe doctors. The “PATIENT TRACKER APP BY DOCTOR” provides a platform where users can keep all records on a single platform. Existing Systems are failing to keep centralized data of patients and doctors to keep records on a single platform. Processing very lengthy and time-consuming Takes www.irjmets.com

III.

PROBLEM STATEMENT

The Medical Center problem facing uses the hands-on process to provide health services, use of cards, files to track progressive patient records, and set records of new patients in the hospital. This procedure has a lot of inconsistencies such as patients losing their health cards or their files being misplaced. Therefore, the automated system helps to keep track of patient records and medical bills making it easier to determine new and ongoing status. This program improves the quality of the service in terms of the medical assistance provided by empowering physicians and administrators to be able to view the level of common diseases and their percentage. Managers can also determine the number of patients a doctor has visited over a set period of time.

PROPOSED SYSTEM

The doctor can easily add new patient details and track the patient's history at any time all on his android phone. The program is a standalone application that can be installed on an android phone and can be used with valid login details. When the Doctor installs this application and after successfully signing in to the app, the doctor can add new patient, personal information, medical details, and, the doctor can add new medical appointments to existing patient and doctor's appointment records and add various patient details such as patient name, contact, age, blood group, gender, medical status, medication prescribed and the last appointment date automatically used by the DBMS system, etc. The program saves all patient information in the database created on the android phone. The doctor can also view this data as needed. With this application, doctors can search patients by name and show all his medical conditions and medication-related details at recent visits. The application has a feature where the doctor can take all the patients to visit on any given day. To keep a record of this application doctors and patients must register according to their registration process after which they will receive the user id that they will enter as a doctor and patient and after that, they can proceed further. It is an easy-to-use application that was easily accessible to users. Significantly, it reduced cost for the composition of a college laboratory because it requires only the addition of the necessary software to the existing Informatics Laboratory. 

Mistakes of both tutors and learners cannot be proven catastrophic for both the lab and the participants' health.

The interaction with the virtual environment motivates the users, especially the learners more in regard to the real process.

It provides flexible access to students in which they can access through the internet and can do any experiment they have been asked.

Instant feedback can be provided by the process of compilation.

FRONT-END

In front-end coding, we use three languages that are HTML, CSS, and JavaScript. Html is used for creating webpages and CSS is used to control the layout of multiple web pages all at once and JavaScript is used for providing functioning to elements

BACK-END

PHP is used to create web applications, and the most popular web applications like Facebook, Yahoo, Wikipedia, Twitter, WordPress all has been developed in PHP, and you can also develop the next big thing by using PHP.

DATABASE

CONCLUSION

The proposed application is an easy-to-use application that helps users store their personal data on a server. Can be used in any Public Place. As it keeps patient and doctor records on one platform it is therefore easily reliable and efficient. This app is designed for a short time has many features that require the user according to their requirement. The app requires less space and is more powerful compared to other existing applications. There is an added range of improvements to this application for user requirements. More features can be added to this app in the future.
