WELCOME TO
HEALTHWORKS HealthWorks is your chance to come together to improve our health and happiness at work.
Poor health infrastructure and low levels of education in Cambodia mean that your workers are particularly vulnerable to infections and diseases. As a country, Cambodia spends the equivalent of 5% (US$848 million) of its GDP addressing malnutrition, poor sanitation and hygiene. Malnutrition costs the Cambodian economy US$400 million a year, of which US$90 million is spent on addressing poor maternal and child nutrition and US$130 million on stunted growth and anemia. Poor sanitation and hygiene costs Cambodia another US$448 million every year.
The good news is that many of the common health problems facing your employees can be prevented with some basic education and simple behavior changes. HealthWorks provides an easy and practical way for you to introduce your employees to information, services and activities that can lead to lasting improvements in their health, resulting in increased productivity with little cost to the factory itself. Your employees value their health and will value your efforts in helping them to improve their health.
SO, LET’S GET STARTED!

CARE CAMBODIA
CARE is an international development organisation that fights global poverty. One of CARE’s focus areas in Cambodia is working with factories to ensure that female garment workers have the knowledge and opportunities to develop as individuals. As part of this engagement, CARE provides workers access to information and services for:
2. HEALTH FACILITY REFERRAL SYSTEM:
4. Financial services including savings and remittances.
The health facility referral information aims to reduce absenteeism and staff turnover as a result of unplanned pregnancies by ensuring factory workers, particularly women, are able to make informed choices about where and when they can access reproductive and maternal health services. The referral system includes:
5. Improving the understanding of Cambodian labor laws to garment factory workers and factory managers.
• Health facility referral directory listing public, private and NGO clinics in Phnom Penh
6. Understanding and addressing Sexual Harassment
• Individual factory summary sheets and posters providing details of the closest public health facilities and other NGO or private health facilities near to the factory
1. Sexual and reproductive health and HIV/AIDS 2. Food hygiene and nutrition 3. Maternal and neonatal health
CARE also runs packaged training sessions, some of which are free. The packages are:
1. REPRODUCTIVE HEALTH Behaviour Change Package: The package aims to reduce absenteeism and staff turnover as a result of unplanned pregnancies by improving understanding of contraception and facilitating positive behaviour change. This is a full package of materials including: • Focused set of training sessions • Three episodes of a video drama • Mobile game • Supporting printed materials
3. TRAININGS ON NUTRITION AND FOOD SAFETY FOR WORKERS AND FOOD VENDORS: The trainings will enable workers to understand nutritional food and assist them in making informed food choices. The food vendor training is done in partnership with Municipal Health Department which equips food vendors with knowledge and skills on preparing nutritious, tasty food keeping in mind food safety and hygiene. The training includes:
• Lunch time worker awareness sessions about nutrition and food hygiene. • Vendor training sessions on food safety and hygiene
4. TRAINING ON HIV/AIDS: Training on HIV/AIDS is provided to workers to improve understanding on STIs and HIV/AIDS. This is done through lunchtime awareness sessions (flexible). Additionally, training provided to HIV/AIDS committees addresses the roles and responsibilities of the committee members. This training also equips the committee members to discuss and develop educational programs to improve workers awareness about STIs, HIV/AIDS and prevention of HIV transmission. CONTACT: MS. Here are some tips on how to get your employees to participate in HealthWorks activities HOLDING INFORMATION SESSIONS
PLACEMENT OF INFORMATION POSTERS
• Keep sessions short: Keep sessions to 30 minutes or less.
• Put posters and pamphlets in places where many employees pass through everyday and in areas where employees are likely to have time to stop and read the poster or take a pamphlet. Some good places to put materials are:
• Ensure privacy: Many of the health topics are sensitive. Choose a safe and private room to hold information sessions. • Separate males and females: You should separate information sessions for male and females when discussing sexual and reproductive health topics. • Sign Up: Always encourage employees to sign up if they want to attend a session so you know how many people to expect. • Warm up: Encourage participation by playing a short game or asking some questions at the beginning of the session. • Provide an incentive: Your employees aren’t used to attending information sessions in their free time and may need some incentives (e.g. food, water) to attend.
- Back of toilet doors - Hand washing areas - Water stations - Canteen - Eating areas - Areas where employees clock in and clock our - Factory clinic • You should print a minimum of 1 poster per 100 workers.
PROMOTION OF COMPETITIONS • Give employees at least one week notice of competition end dates. • Promote the competition through several sources: Advertise the competition through posters and loud speaker announcements. You can also ask HealthWork Champions and team leaders to promote the competition through word of mouth. • Provide an incentive for participation: This can be a small prize (a bag of rice, fruit basket) for winners or the provision of colour pencils or paper for entrants. You can also display winning entries and make posters or calendars from these entries. • Involve employee’s families: You can hold a children’s version of any poster or drawing competition. This is a good way to teach children about the health issues. • Always announce winners publically. This makes employees feel proud of their achievements.

GENERAL TIPS FOR RUNNING QUIZZES There are several quizzes in the Toolkit. You can use these quizzes in many different ways. Here are some ideas on how you could run the quizzes. Choose a method that works well for your factory site. • Print the quiz out and attach it to the payslip of every worker. Ask them to fill it in with their name and put in a box during the HealthWorks campaign. At the end of the Week, during lunchtime, read out the answers to workers. You can randomly draw out an entry and award prizes to the first names you draw out to encourage participation. • During lunchtime or a worker assembly, ask your workers to divide into teams of 5. Give them a quiz and a pencil. Read out each question and ask them to work in teams to circle the right answer. Ask them to swap completed quizzes with another team and mark each other. Read out the answers and provide a prize to the winner/s.
NUTRITION QUIZ
• Distribute one quiz to each production team and ask the team to fill it out and return to a manager. Read out the answers to all the employees during lunchtime or a worker assembly. Mark the quizzes and provide the results back to the team together with a prize for the winning team/s. • Use the quiz during a worker assembly. Read out the questions and ask employees to put their hand up if they think they know the answer. Pick someone to answer the question. Give them a prize if they answer correctly. Keep asking for volunteers to answer until you get the correct answer.

NUTRITION
HEALTHWORKS TOOLKIT PAGE 21
Nutrition has an enormous impact on our general mood, energy and physical health. The nutrients in food give our bodies the information and energy they need to function effectively. We spend up to 60% of our waking hours working and consume at least a third of our daily calorie intake at work, so the workplace is an ideal environment to positively influence eating habits.
KEY NUTRITION MESSAGES • Eat a nutritionally balanced diet. We should aim to have a diet high in protein, fruits and vegetables. • Eat more foods that are high in iron to improve your health. High iron foods include green vegetables, tofu, lentils, liver and meat.
Currently, most factory workers simply aren’t eating enough. The average garment worker consumes about 1600 calories, about half the recommended intake for those performing heavy work.
• Eat a rainbow diet. Colourful fruits and vegetables are high in essential nutrients.
Insufficient nutrition makes employees weak and susceptible to fainting. It also compromises their immune systems, making them prone to frequent illness. Common health complaints such as headaches and dizziness are also often a result of insufficient nutrients such as iron and protein.
• Provide a food supplement or allowance to employees.
The good news is that you can help to improve worker health and reduce sickness absence rates through a range of simple measures.
• Try to reduce access to processed foods and snacks with high fat and sugar content.
GOOD PRACTICE
• Share nutrition materials with outside food vendors and provide incentive for them to cook healthier options.
• If you provide food to workers, educate your canteen staff on healthy food preparation.

a. Fix the Posters to empty wall space near food vendors and in the common eating areas. Make sure to position them to be clearly seen.
Hold a drawing contest for employees where they are asked to draw a healthy, balanced meal. This will encourage employees to think about what makes a nutritious meal and encourage them to eat better. You can include a separate competition category for employee’s children. The top 12 entries can be turned into a company calendar. You could also frame the winning entries and place them around the factory.

3. COOKING DEMONSTRATION
Ask the factory chef or invite an external chef to conduct a lunchtime healthy cooking demonstration. You can ask the chef to demonstrate cooking a meal for 4 for $4, demonstrate one of the Healthy Recipes or a rainbow meal. A professional chef could also show employees how to involve kids in meal preparation and teach employees some tips and tricks to best stretch their food budget
INSTRUCTIONS: a. At least 4 weeks before HealthWorks, contact a professional chef or ask your factory chef to see if they can conduct a healthy eating demonstration. b. Find a suitable area in your site to hold the demonstration as well as someone to be a MC for the demonstration.
!
4. HEALTHY RECIPES
5. HEALTHY EATING QUIZ
Print out the Healthy Recipe flyers and distribute the employees. You could also compile the best recipes received in the “4 for $4” Cooking Contest and print them for distributing to employees. Alternatively, you can make these into a calendar to provide to employees. This will encourage employees to try more nutritious recipes.
INSTRUCTIONS:
The quiz contains interesting facts about everyday foods. It’s a great way for people to find out how much they really know about what they are putting in their bodies, enabling them to make healthier choices.
INSTRUCTIONS: a. Refer to General Tips for Running Quizzes for ideas on how to use quizzes.

7. NUTRITION VIDEOS AND SONGS These videos and songs about Nutrition can be played on the screens in the common areas during lunchtime. These are catchy tunes and videos that can be played during HealthWorks and will help the nutrition messages to stick in the minds of the employees. The Super Bean video captures several of the nutrition messages covered through HealthWorks in an entertaining way. It’s a video about garment workers that face many problems due to poor nutrition until Super Bean and Teddy Bear come to their rescue and teach them the benefits of a balanced diet, how to select foods at the market and the benefits of sharing food.
INSTRUCTIONS: a. Play the videos on the screens during lunch or other times when employees may be able to watch them. The audio messages and songs can be played at various times during the day.

HYDRATION
HEALTHWORKS TOOLKIT PAGE 27
HYDRATION Water plays many important roles within the body; water is required to transport nutrients throughout the body, regulate body temperature and remove waste. Many factors such as increased workload, stress, long journeys to and from work, dry air due to air-conditioning or heating environments can increase water loss even when we are not noticeably sweating. Dehydration can adversely affect worker productivity, safety and morale because mental and physical performance is reduced. Not being well hydrated during the working day can cause headaches, tiredness and loss of concentration. All of this impacts negatively on worker productivity and health. It is particularly important for your workers to drink an adequate amount of water given that they are engaged in labour intensive work in often hot conditions. In fact, 47% of employees surveyed as part of the HealthWorks Needs Assessment drank less than 5 glasses of water a day.
KEY HYDRATION MESSAGES • Drink 2 litres of water a day and at least 5 glasses of water a day at work. • Not drinking enough water can lead to headaches, tiredness and loss of concentration. • To avoid passing germs, make sure that your water bottle does not touch the water tap.
GOOD PRACTICE • Ensure all employees have their own water bottle, which is within easy reach of their work station. • Provide employees with quick access to safe drinking water. • Remind employees of the need for frequent water breaks via loudspeaker.

HYGIENE
HEALTHWORKS TOOLKIT PAGE 31
Our hands are constantly exposed to contaminated surfaces. We then, often unknowingly, touch parts of our face, which enables germs to enter our bodies through our eyes, mouths or noses. We may also touch other people, transmitting our germs to them. Germs can also spread when we sneeze, cough or rub our eyes and then touch someone else. Germs can also be spread when you don’t wash your hands after using the washroom and before handling food. In fact, 80% of infectious diseases are transmitted through touch. The simple act of washing our hands is the most powerful and effective means of combatting the spread of such diseases. Handwashing with soap has been shown to reduce the incidence of diarrhea by over 40%. 60% of employees surveyed as part of the HealthWorks Needs Assessment could not correctly identify hand washing as an effective means to stop the spread of disease. You can influence employees to stay healthier by encouraging them to wash their hands frequently and by teaching them the correct technique to do
so. Additionally, by educating employees about how germs spread through daily activities, such as sharing utensils when eating or not covering your mouth when coughing, you can go a long way in helping them to avoid contracting infectious diseases.
KEY HYDRATION MESSAGES
GOOD PRACTICE
• Wash your hands before you eat, before preparing meals and after going to the toilet to prevent the spread of stomach and respiratory illnesses
• Ensure that hygiene signage is placed in the relevant places to have maximum impact. For example, hand washing techniques near the sinks, hand washing reminders near toilets and water bottle hygiene signs near water refill stations.
• Wash your hands for 20 seconds to kill germs • Cover your mouth when you cough to prevent the spread of illnesses • If you are sick, use a clean spoon when sharing food to stop the spread of germs
• Provide soap at all hand washing stations. • Encourage regular hand washing through regular announcements, especially before eating and after using the washroom. • Provide employees with facemasks to wear if they have respiratory illnesses. • Encourage employees to take sick leave if they feel a cold or flu coming on so as not to infect their colleagues. • Remind employees of the need for frequent water breaks via loudspeaker.

2. PERFUMED SOAP
There are four hygiene awareness posters – The Hand Washing Awareness, Hand Washing Technique posters are designed to be used in hand washing areas, to remind employees to wash their hands after using the toilet, and before meals. The Don’t Spread Your Germs poster encourages employees to sneeze or cough into their sleeve to stop the spread of diseases. The Share Food, Not Germs poster lets employees know that germs can spread by sharing spoons, straws and chopsticks.
To encourage employees to wash their hands after using the toilets, you could supply perfumed soap during HealthWorks and place next to the washbasins. Employees will notice the new addition and be encouraged to wash their hands more frequently.
INSTRUCTIONS: a. Fix these posters to the back of toilet doors, handwashing basins and eating areas during the HealthWorks campaign.
INSTRUCTIONS: a. Place perfumed hand soaps near the wash basins. You can advertise this through HealthWork Champions or loudspeaker announcements.
Materials: Perfumed hand soap
This activity is a fun way to help employees visualise how germs spread and make them more aware of the importance of hand washing. The game involves passing around a powder-dusted balloon to imitate how quickly germs are passed around among people.
INSTRUCTIONS: a. Ask employees to form groups of about 15-25 people and give each group a balloon that has been dusted in flour. b. Explain the game to employees: ask employees to tap or throw the balloon to each other when the music starts. As the employees tap the balloon, they’ll see their hands picking up the powder. The powder represents how germs are spread from person to person.

6. WHAT DOES SOAP DO?
This activity effectively demonstrates the importance of using soap in washing your hands. It will help employees to remember the effects of hand washing. This activity will be best demonstrated in small, informal groups of about 20 people. You can do it at a table during lunch, so that employees who are passing by can watch. You may need to perform the activity several times so more people get to see it.
INSTRUCTIONS: a. Start by asking the group what they know about why we should use soap when we wash our hands. Then ask one of the employees to sprinkle a little black pepper in a bowl of clear water. A white bowl works best for this exercise. b. Ask the employees to see how the pepper floats on the water, just like germs on our skin. Ask one employee to put a finger into the bowl. They will see that the pepper sticks to their finger. c. Ask another employee to stick a finger in a bowl of dishwashing detergent. The next part happens very quickly so ask everyone to watch and pay attention to the big moment. Ask that employee to put their soapy finger into the bowl of pepper water. The pepper (or germs) quickly moves to the edge of the bowl. This represents how germs hate soap and are quickly repelled. Remind employees to wash their hands before they eat, before they cook and after they use the toilet. You can also tell employees to recreate this demonstration for their children at home.
Materials: One white bowl with clear water, one small bowl with dish-washing detergent, black pepper, a cloth for spills

7. HAND WASHING SONG Teach employees proper hand washing technique with a quick demonstration.
INSTRUCTIONS: a. During a worker assembly or lunchtime, talk to employees about the importance of hand washing. You should let them know that: • Washing hands regularly prevents 80% of illnesses.
e. To remind employees about proper hand washing techniques, play the Happy Birthday song over your factory sound system throughout HealthWorks and make announcements about hand washing.
• They should always wash their hands before and after eating, before preparing food and after going to the toilet.
Materials: Hand Washing Technique poster, Hand Wash Sticker, sound system, Happy Birthday music and music player (optional)
• They should wash their hands for about 20 seconds. b. Demonstrate the key steps for washing hands using the Hand Washing Technique poster: • Apply soap and rub your palms to lather. • Interlace fingers and rub.

SEXUAL HEALTH
HEALTHWORKS TOOLKIT PAGE 37
Your employees’ sexual health affects their mental, physical and emotional wellbeing. Symptoms of sexually transmitted infections (STI) vary depending on the infection, from headaches and fevers to tiredness and rashes. Untreated, STIs can cause long-term health problems including cancer, infertility, pregnancy problems, organ damage, and even death. Knowledge of and access to birth control empowers employees to take control of their reproductive health and wellbeing. An understanding of basic birth control is fundamental to the prevention of unwanted and unplanned pregnancies, which can have negative physical, psychological and social consequences for both mother and child You can play a role in helping your employees to understand how to take control of their reproductive health by helping them understand about family planning and STIs.
KEY SEXUAL HEALTH MESSAGES • Prevent the spread of STIs by using condoms • Use birth control to prevent unplanned pregnancies • Get free advice on birth control and sexual health from Marie Stopes or RHAC
GOOD PRACTICE • Ensure your factory clinic has a range of STI and family planning related information materials to provide to workers. • Have condoms on site to give or sell to workers. • Employees are likely to be embarrassed to seek advice on sexual health so it is important that you provide a private place for employees to talk to clinic staff. • Ensure sexual health related posters are placed in a visible place so that employees know that the clinic can provide counselling and advice on the matter.

!
5.STI & BIRTH CONTROL QUIZ
Encourage employees to ask questions about sexual health by holding a lunchtime Q&A session where they can submit anonymous questions. Ask either your clinic doctor or an external health clinic to run the session.
The STI and Birth Control Quizzes helps employees learn about key sexual health topics.
INSTRUCTIONS:
1. Refer to General Tips for Running Quizzes for ideas on how to use the Quiz.
a. Create a STI Q&A box: At least 1 week before the Q&A session, find a cardboard box (e.g. old shoe box), seal it and write Q&A questions on the box.
INSTRUCTIONS:
Materials: STI Quiz, Birth Control Quiz, papers, pencils, loudspeaker, cardboard box (optional)
b. Place the box in an accessible place (e.g. outside clinic, outside toilets, canteen) together with some pens and Q&A Forms. c. Publicise the information session and Q&A box using HealthWorks champions and the HealthWorks Empty Belly poster. d. Check the box every day. If there are not enough questions, ask your HealthWork Champions to encourage workers to write questions. e. Prepare answers: Take the questions out of the box a day before the information session. Ask your clinic doctor or external provider to prepare answers to the questions. If there are not enough questions, ask your doctor or external provider to make up questions and prepare answers for the Q&A session. If there are too many questions, pick the five best questions. f. Hold Q&A session: Hold Q&A session during lunchtime or morning assembly and keep it to less than 30 minutes.
Materials: Cardboard box, pens, Q&A Form, HealthWorks Empty Belly poster, sound system

MATERNAL HEALTH
HEALTHWORKS TOOLKIT PAGE 41
Maternal health has a real impact on the health and wellbeing of employees and their ability to work at their maximum capacity. 91% of garment factory employees are women, with the majority of childbearing age. Maternal health is particularly important in garment factories where pregnancy rates tend to be higher than average. The HealthWorks Needs Assessments found that between 3 to 11.5% of female employees were pregnant in pilot factories, compared to the national average of 3%. Adequate nutrition is particularly important for pregnant employees, who require 300-500 extra calories per day in the second and third trimester to support optimal foetal growth. More than 50% of pregnant women in Cambodia are anaemic and about 20% are underweight. By educating and supporting pregnant women to eat more nutritious foods, you can make sure that your employees can continue to work productively for longer. You can also help to encourage women to attend at least 4 antenatal check-ups during their pregnancy. Your employees will not only receive health checks during these visit, they will also receive nutritional counselling and supplements during visits to ensure that they are fit and healthy to work. You can support employees to attend
these check-ups by providing them with information about relevant services and giving them time to go to appointments during work hours. You could also consider asking your clinic doctors and nurses to acquire basic training on maternal health care so that they are in a strong position to provide support and advice to pregnant employees and new mothers.
KEY MATERNAL HEALTH MESSAGES
GOOD PRACTICE
• Pregnant women should aim to eat 300-500 more calories in the second and third trimester of pregnancy
• Encourage your workers to tell you that they are pregnant as soon as possible so you can provide support to them. You can do this by providing congratulatory gifts (e.g. bag of rice, lentils, sweet potatoes, eggs) to pregnant women.
• It is important for pregnant women to eat food high in iron, calcium and folic acid such as green vegetables, tofu, eggs, lentils, nuts, fish and milk • Pregnant women should attend a minimum of 4 check-ups during their pregnancy
• Once you find out that your employees are pregnant, provide them with the Maternal Health information materials. You might want to encourage them to book an appointment with the clinic nurse so they can ask questions. • Ensure pregnant workers know about their rights (e.g. maternity leave, breastfeeding room, child care room). This will help to alleviate any fears and anxiety they might have about the impact of their pregnancy on their job security.

Only 56% of women attend the minimum required 4 antenatal checks during pregnancy. Such check-ups are essential to make sure that mother and child are healthy. Your employees may find it difficult to take time away from work to attend such check ups. You can encourage more employees to have regular check ups by bringing qualified staff onsite to conduct antenatal check-ups as part of HealthWorks. If this is successful, consider making this a regular service. a. Contact a local health centre or RHAC to ask them if they can come for half a day to provide check-ups. b. Advertise the maternal health check-up service using the Maternal Health Empty Belly poster and encourage workers to sign up for 15 minute appointments using the HealthWorks Sign Up form.

EVALUATION GUIDE
Before you say goodbye to HealthWorks, it is important that you take time to ďŹ nd out what impact your hard work had on your employees health and on your business. Find out what employees learnt, what activities they took part in, what they enjoyed and what they would like to see more of! You should also see if your employees have suggestions for activities for the next HealthWorks or if any of them would like to be Champions. Enthusiastic employees should be encouraged to join the HealthWorks Committee.
HEALTHWORKS TOOLKIT PAGE 45
1. EVALUATION CARD We have developed Evaluation Cards for you to print off and distribute to employees. You can send these home with payslips or leave them in communal areas. You can provide a small incentive, for employees to fill in and return the Evaluation Card. You should tally up these results to get a better idea of the impact of HealthWorks.
2. LUNCHTIME FEEDBACK SESSION Organise a lunchtime feedback session for about 30 employees. You could provide lunch to those who come along to encourage more employees to attend. Ask employees what key things they learnt during HealthWorks, whether they have made any changes to their health and what they enjoyed the most about the health campaign.
3. MEDICAL AND ABSENTEEISM DATA Work closely with your clinic staff to monitor if there have been any changes to employee health after HealthWorks. Four weeks after HealthWorks, check your clinic and HR records to analyse changes to: • Number of employees complaining of headaches, dizziness, colds and stomach illnesses • The amount and type of medicine that your clinic has dispensed since HealthWorks • Average number of absences since HealthWork
4. EMPLOYEE REPRESENTATIVES/ HEALTHWORK CHAMPIONS Ask union or worker representatives or your 4 x HealthWork Champions to ask 6-8 employees for their feedback about HealthWorks and what they learnt. Using the evaluation card, ask each representative to spend 5 minutes speaking to each employee to gain a better idea of what employees thought of the initiative. This is a good approach if many of your employees have difficulty reading and writing.
