Dear Potential Summer Madness Volunteer, Thanks for your interest in volunteering with Summer Madness. This document provides all the forms that are needed to comply with standard vetting regulations for working with young people and vulnerable adults. If you have any difficulties or questions feel free to contact the Summer Madness office.

List of what needs done: 1. Fill in the Online Google Form first with your contact details, then print the application pack.

2. Complete the SM staff application documents and return them to the Summer Madness office. The 'Access NI' form needs to be filled in using black ink; and we like to get this sent off as quickly as possible since this aspect of the process can take 4-6 weeks to complete.

3. Give the Reference Request Forms to your two referees, asking them to fill it in and send back to us as soon as possible. You might want to check with them regularly that they have indeed returned them to us.

4. Attach a photograph or better still email a ‘JPG’ IMAGE to team@summermadness.co.uk Any size will do but we're looking for a clear head and shoulders shot. This will be used for your staff pass.

5. Send a photocopy of your driving license, passport or birth certificate AND 2 of the following: Marriage certificate, Bank statement, Utility bill, P45/60, TV license, Addressed payslip, National insurance card, Exam certificate or Letter from Head teacher We will be in touch once we have received all documents and the AccessNI form has been processed.

SUMMER MADNESS CODE OF CONDUCT Code of Conduct Staff and volunteers who come into contact with children and young people have a legal duty to help protect them from abuse or the risk of abuse. All staff are directed to ensure that their conduct is beyond question and should observe the full scope of the Child Protection training given and in particular should never: ! Spend excessive amount of time alone with children/young people away from others. ! Initiate or pursue any romantic relationships while at Summer Madness ! Allow children or leaders to use inappropriate language unchallenged ! Make sexually suggestive comments about or to a young person or leader, even in fun ! Allow an allegation made by a child/young person to go unchallenged or unrecorded Health and Safety ! No one should behave in such a manner or undertake any activity or operation that would put others or themselves in danger of injury or harm. ! All team members must follow out the Health & Safety instructions given by their team members and comply with instructions in the SM Health and Safety Policy Guide ! Heavy lifting: while staff may be required to help move items around the site everyone should follow our standard procedures for proper posture while lifting and not attempt anything beyond their normal capacity. ! If anyone has particular concerns about Health & Safety on site they should immediately inform their team leader or go directly to the Duty Officer. ! The site is a non-smoking site and alcohol free.

Child Protection Team If you have any concerns regarding the welfare of a young person at Summer Madness these should be passed immediately to the SM Safeguarding Trust Panel, who can be contacted via the Duty Officer, located in Comms. The Panel are available 24 hours per day throughout Summer Madness to ensure that any concern is appropriately addressed.

As a member of staff at Summer Madness, I am happy to adhere to the above guidance and will undertake the appropriate Child Protection training for SM volunteers before the festival.

GUIDANCE TO COMPLETE THE ACCESSNI FORM FIRSTLY - USE BLACK INK AND WRITE IN CAPITAL LETTERS For all applications – applicants must now ensure the following sections are completely correctly: B, D (E & F if necessary) & G ! Currently do not complete section C -!ISA part!

Provide previous 5 years address histories ! 1. Fill out part B Common Errors: B4 - name usually known by - you must fill this in. 3. Fill out part D Common Errors: D6 - remember to give the date at which you moved to your current address. This must be a specific date, not just a year or month.

4. Fill out part E - address history Part E must be filled out if you moved to your current address less than five years ago. Even if it is almost five years you must still account for your address history for a full five years.

Common Errors: E6/E12 - the dates you have lived at your previous addresses must match up exactly so that your address is accounted for down to the day. i.e. if you lived at address 1 until 20/8/1988 then the date you moved to address 2 must be 20/8/1988 5. Fill out part F - names history Part F must be filled out if you have had more than one surname throughout your life.

Common errors: F2/F4/F6/F8 - the dates of your surnames must fit exactly so that your surname can always be accounted for. i.e. if one surname ends on 20/8/1988 your next surname must begin on 20/8/1988.

6. Fill out part G You must cross yes or no for any previous convictions - this includes any driving offence.

If you cross yes for previous convictions you must include a separate piece of paper with your form detailing the conviction.
