Vet’s Corner

Robert N. Oglesby, DVM Stokesdale Veterinary Hospital Dr. Oglesby is a 1984 graduate of the University of Georgia School of Veterinary Medicine and the owner of Stokesdale Veterinary Hospital. He has been practicing in the Northwest corner of Guilford County for 31 years. Dr. Oglesby is shown here with his wife, Janette.

Hip Dysplasia, PennHip Testing, and the Eradication of Hip Dysplasia For decades now it has been recognized that some breeds of dogs are predisposed to the early development of a progressive arthritis of the hips and its concurrent debilitating lameness. The conformational weaknesses that were deemed to cause early hip arthritis were termed “Hip Dysplasia” (“HD”) and the number of breeds affected and the incidence within breeds is growing. It was thought that a shallow acetabulum, the “socket of the hip joint”, was the chief culprit. Recognizing there was a problem, the Orthopedic Foundation of America (OFA) was formed in 1966 to develop a database of radiographs to be used to help provide genetic counseling in an effort to reduce the incidence of HD and hip arthritis. In the late 1980’s, Dr. Gayle Smith of the University of Pennsylvania, and his coworkers, began investigating the differences in the hips of large breed dogs that were developing early arthritis with those that didn’t. To make a very long story short he found the chief correlating conformational factor leading to the development of early progressive arthritis was laxity in the soft tissues that make up the joint capsule of the hip. From this work he developed a method for measuring this laxity which became known as the PennHip Method. The PennHip method consists of three radiogaphic views: an extended hip view (same as the OFA), a compressed hip view and a distracted hip view. The compressed view is where the ball is pushed down into the socket and the distracted view is where the ball is pulled away from the socket. Neither procedure is dangerous to the dog’s health. These radiographs are then evaluated. An important component of this evaluation is measuring the range of motion of the head of the femur (the ball) between

the compressed and distracted view. This calculated distance is the “Distraction Index”. Many of the initial research projects were performed to find the range of Distraction Indexes (DI) within a breed, how the DI changed over time as the dog aged, and how well the DI correlated with the later development of arthritis. It was noted early on that different breeds had different ranges of DI, so databases of DI ranges were started based on individual breeds. The DI was found to be very consistent as dogs aged. Once sufficient numbers of DI’s were accumulated, and the dogs aged, it was found that dogs with greater ranges of motion when young were at high risk of developing arthritis. Those in the lower ranges were at low risk. Where a particular dog falls in the range of hip laxity for their breed is called the “Laxity Profile Ranking”. The next question was how hereditable was hip laxity. Though many studies showed the usefulness of this procedure, the largest and longest project was centered around the See Eye Inc. breeding project for guide dogs. While significant progress had been made using OFA principles, the chief geneticist was interested in further decreasing the incidence of hip arthritis in their breeding population. He introduced PennHip principles into the breeding program and monitored the results over 4 generations of dogs. Within just one generation he found further decreases in the incidence of hip arthritis and after several generations it became rare in their population of breeding dogs. See Eye Inc. now uses PennHip as the method for evaluating hip health. PennHip has proven itself to be a reliable method for the early prediction of the future onset of hip arthritis in dogs. It has also proven itself to be a reliable method of predicting the hip health of the next generation. Selecting on the basis of the top 40% tightest hips on the Laxity Profile Ranking reliably decreases the incidence of hip arthritis in the next generation. It does seem we now have a tool to all but stop the hereditable predisposition to arthritis in the hips, aka Hip Dysplasia.

Cover Story

Walk in Our Shoes This Weekend By Sharon Gray, Executive Director Animal Rescue & Foster Program

S

aturday morning has arrived. This is the beginning of the week for the Animal Rescue & Foster Program (ARFP). ARFP’s Next Step Adoption Center is buzzing by 9 a.m. Kitty cleaner volunteers are already scooping and cleaning for a day of visitors. Foster parents begin arriving at 10:40 as we open at 11 a.m. I glance out the front window and cars are already waiting for us to open. Our adoption center is almost 4000 square feet. We are open to the public Saturdays 11 a.m.-3 p.m., Sundays 1-4 p.m., and on Wednesdays 5-8 p.m. We are located at 711 Milner Drive in Greensboro, NC. This is our second adoption center, however it is only two houses away from our original center. We have been on Milner Drive a total of 13 years and it is home to our 23 year old organization. As I open the front door, I have to jump out of the way of bags tumbling inside. Visitors have dropped off bags of purses and shoes. Donations are so very much appreciated! People begin coming in to see kittens and puppies. Others have questions as to what to do with a mother cat that has decided to have kittens under their storage building. Meanwhile, someone has already cornered a volunteer asking about volunteer opportunities for their 16 year old son. I have a man from High Point coming at noon with a litter of seven puppies he found on his way home from work. At 12:30 two women are bringing in their mother’s small lap dog for me to evaluate. Their mother has entered assisted living and neither daughter can keep her little dog. At 2:30 someone is coming by with four kittens. ARFP helps a lot of people who have found homeless babies. LOTS of people. We will vaccinate, deworm, and take pictures of the seven puppies and the three kittens. This will make all of them more adoptable. The pictures can be used on social media and other internet sites. Based on our foster home availability, the health of the animals, and the overall socialization of the animals, we will intake as many as we have homes available.

Tangle Our goal for people that we help is to make them feel empowered, and therefore willing to hang on to the animals they have found until homes can be found. If we can network for them, provide some medical care, and offer them food, they feel as if they can actually see some light at the end of the tunnel. ARFP’s focus is helping to socialize puppies and kittens 12 weeks of age and younger that have been abandoned. We have three to four homes that will also foster small adult dogs under 15 lbs. We stay at capacity as much as we can. By 3 p.m. on Saturday we have taken in four puppies, leaving the man that found them three of the puppies to find homes for. He has been given shot records for his remaining three plus a timeline of when they are due again. We have pictures and his contact information to post on Facebook. He feels positive that he can now manage the remaining three puppies. The small lap dog ends up being a Papillion that we are hooking up with a Papillion Rescue. The sisters are thrilled and we are happy knowing that this dog will go to someone that knows the breed. The kittens come in right at 3 p.m. We are trying to lock up so that we can clean, go over adoption applications and give foster parents an update. All four kittens are healthy, have clear eyes and are very sweet. We take pictures, vaccinate and deworm the babies, and ask if they can hang on to them

until Wednesday when foster homes can be arranged. Knowing that we will take them on Wednesday, the people are happy and agree to keep them another four days. Volunteers are scooping the kitty litter, cleaning up the dog toys from our foster puppies’ playtime and washing water bowls. We have had almost 35 people walk through the front door today. Everyone was greeted by a volunteer and offered help. No disasters. It was a good day. Fast forward to Sunday at 11a.m. Volunteers are already at the center cleaning in the cat room. I start unlocking our outside gates for the foster pups coming in. Water bowls need to be filled and put outside along with toys and poop bags. Someone is banging on the front door at noon. A girl has found an adult Pit Bull mix and wants to know if we can take her. She appears healthy and is friendly. I try to educate her that a dog in this condition, wearing a collar, probably belongs to someone. I end up spending 45 minutes talking her into taking the dog by a veterinary office to be scanned for a chip and then taking it back to where she found it to try to find the owner. I tell her to watch Facebook and Craig’s List for lost and found ads. When she left I prayed that what I said did not fall on deaf ears. We are open from 1-4 p.m. on Sundays. Volunteers drop off their foster kittens and puppies a little before 1p.m. and pick them back up at 4 p.m. Some stay to help clean and talk with the public, others have errands to run. Volunteers who are puppy holders start showing up to hold the puppies whose foster parents have dropped them off. A local woman’s club member drops by with supplies they have collected such as Puppy Chow, Kitten Chow, paper towels and detergent! These are all things that we use every day and are always in need of. We get her club’s information so that we can send a thank you note. Two people come in to meet Harry Potter, our little Cerebellar Hypoplasia pup. He is always up for meeting people. He is always on the move but probably one of the most endearing dogs you will ever meet. A family of five comes by just to see which puppies


are available. They fall in love with several but since dad is not quite on the same page with getting a puppy, we encourage them to go home and talk. A couple that recently lost their 17 year old cat comes in. They want to donate his bed, bowls, etc…and to look at our cats for adoption. Within ten minutes we can tell it is too soon. One of them already had to go to the car while the other one has watery eyes. Towards the end of the day we have had no firm adoptions and in walks a man that adopted from us previously. He asks me which cat has been with us the longest. I take him into the cat room and he checks all the cats out while I tell him about their personalities, where they came from and how long we have had them. Low and behold he is ready for another family member and would like to apply for one of our female kitties who has been with us for almost a year. Everyone overlooks her because she is a black cat. The Way It Was... Eighteen years ago the Guilford County Animal Shelter and the Animal Rescue & Foster Program (ARFP) had this really amazing relationship. Charlie Porter with Guilford County EMS had reached out to ARFP and wanted to work together to save more puppies and kittens from euthanasia at the shelter. They had a new director, veterinarian Karla Brestle, and the goal was to get as many animals out of the shelter as possible through volunteers and rescues. We immediately bonded with these two people who had so much passion for animals. ARFP volunteers began volunteering at the shelter one Saturday a month. We would go out on Friday night and make sure every animal in all the adoption wards had name tags and bios. On

Saturday morning we would show up with balloons for the cages and plenty of volunteers to help people find the right match in a pet for their family. The staff looked forward to the Saturdays we would come out. Volunteers made their jobs easier in many ways and they always showed their appreciation. Dr. Brestle, the veterinary techs, and even the staff, would call us weekly asking if we could come out and look at special cases, a litter getting ready to be euthanized, or a geriatric that an employee had become attached to whose time was up. Eighteen years ago our relationship with the Guilford County Animal Shelter was one that we were very proud of. We haven’t given up hope that we can say that again in the near future.

The Animal Rescue & Foster Program (ARFP), founded in 1993, is a 501(C)(3) not-for-profit volunteer organization based out of Greensboro, NC. ARFP provides foster care for many abandoned animals through a dedicated volunteer base. Our main focus is puppies and kittens 12 weeks of age and under. ARFP’s Next Step Adoption Center is located at 711 Milner Drive in Greensboro. It is open to the public Saturdays 11-3pm, Sundays 1-4pm, and Wednesdays 5-8pm.

Vet’s Corner Dr. Cecilia Ho, DVM Dr. Cecilia Ho is a graduate of the College of Veterinary Medicine at Michigan State University. She decided to specialize in feline medicine two years into private practice as she found feline medicine to be more challenging and rewarding. She currently shares her home with five cats- all of which have her trained to their beck and call.

FELINE CARDIAC DISEASE Question: My cat was just at my veterinarian’s office for a routine physical exam and was diagnosed with a heart murmur. What does this mean for the health of my cat and should I be concerned about it? Answer: A heart murmur is caused by a disturbance of blood flow within the heart. This disturbance can be due to one or many factors. In most cases there is an abnormality of the muscle that makes up the walls of the heart which then leads to dynamic changes in the flow of blood when the heart contracts. As with all diseases, early detection and prompt treatment improve the chances of stabilizing and in some cases, even reversing some of the cardiac changes. Cats that have well controlled cardiac disease can lead full and long lives.

The age of a cat diagnosed with a heart murmur can help your veterinarian determine the cause for the murmur. In very young

cats, usually between the ages of 1 to 5 years, the reason is usually a congenital one. There are certain breeds that are predisposed to genetic mutations within their cardiac muscles that put them more at risk for heart disease. These include Maine Coons, Ragdolls, Persians and Himalayans. Fortunately, there are now genetic tests available that allow owners and breeders to screen their cats for these genetic mutations. If the cat is older when diagnosed with a heart murmur, the reasons are usually due to diseases that cats develop as they age. The most common disease that causes cardiac disease in older cats is hyperthyroidism. This disease results in the excessive production of thyroid hormone which causes the cat’s metabolism to rise. This in turn causes the heart to beat faster and work harder. Over time the stress of these changes on an aging heart results in cardiac disease. Once a heart murmur is diagnosed in a cat, your veterinarian can proceed with several additional diagnostic tests to determine the severity of the cardiac disease. A newly developed screening test called Feline Cardiopet Pro-BNP measures the amount of stretch and deformity of the cardiac muscle. This is a simple blood test. The greater the stretch and deformity of the muscle, which would indicate more severe cardiac disease, the higher the test result. If the Pro-BNP results are very high, indicating that there is significant cardiac disease, the next diagnostic step would be a cardiac ultrasound. This allows veterinarians to evaluate the interior structures of the heart, such as the heart valves and also the appearance and thickness of the heart walls. Once the correct diagnosis is made the cat can be started on the appropriate medical therapy. In the case of an older cat, diagnostics would also include a comprehensive blood panel, measuring the cat’s blood pressure, a urinalysis and perhaps also a heartworm test to determine the cause of the heart murmur. Treating cardiac disease in cats can be very rewarding. Lady

One of our volunteers did some work for the person who had Miss Ellie and asked if we could take her into the AARF foster program. It just so happens there is a couple in our foster program that is crazy about Beagles! Miss Ellie is unfortunately heartworm positive, but AARF will take care of her treatments. Chester is an 18 year old Tennessee Walking horse and draft mix. He is a gelding, stands approximately 16’1 hands and can be dominant with other horses. Training Your Pet

De-Stress Your Dog for the Holidays

Work with your dog to teach him to keep all four paws on the floor, and to keep his nose out of the trash. Be sure to keep tempting treats out of reach of your dog.

he holiday season can be a very busy and stressful time for us. Decorations, travel plans, visitors and special treats are all things we look forward to. Sometimes we don’t think about how the holiday season can affect our dogs. Anxious and reactive dogs are especially prone to holiday stress. Holiday decorations can be hazardous to dogs, or any pet. Christmas trees, tinsel, candles, poinsettias, ornaments and lights set a festive mood, but can be very dangerous if a dog chews or ingests any of these. Take time to teach a “leave it” command to your dog. This will allow your dog to understand that certain things are off limits. Holiday traveling can be extremely stressful for you and your dog. Air travel can be especially nerve-racking, with flight delays, cancelations and huge crowds at airports. Long car rides can stress some dogs. Using a crate helps confine a dog, and may help calm an anxious dog. Crates also provide a level of safety during an accident. Prior to your trip, take time to properly crate train and socialize your dog to the car. A “place” command can be very useful to minimize your dog’s stress at home. Our normal routine gets turned upside down during the holidays. We will have more house guests during this season. These visitors can make your dog extremely anxious. To keep your dog under control, train him to go “place” when your guests arrive. His “place” is his special location where he knows he has to stay until you release him. This will allow your guests to come inside, without your dog greeting them at the door. Tell guests (especially children) to leave the dog alone when he is on his place. This will allow the dog to truly feel at ease on his place. With all of the holiday meals and special treats, counter surfing and dumpster diving can cause lots of problems. Work with your dog to teach him to keep all four paws on the floor, and to keep his nose out of the trash. Be sure Bella learning “place”.

to keep tempting treats out of reach of your dog. This will save the Thanksgiving turkey, and help you avoid a trip to the vet. Avoid giving left over table scraps to your dog. Even though they will enjoy the tasty treat, their stomach may not be able to handle the rich, and fatty food. No matter how busy you get, don’t forget to spend quality time with your canine companion. Your dog still needs enough exercise, mental stimulation, and social interaction during the holiday season. A decrease in any of these three important things can lead to boredom, anxiety, and stress in your pup. If you plan on being away from home for an extended period of time, consider hiring a pet sitter, or send your pup to a reputable doggie daycare. The holiday season can be a wonderful time of year. With a little preparation, your dog can enjoy himself too.

THT: What led you into the insurance business? BB: When I graduated from college and moved to NC, I was working for a large department store. One of my co-workers also was working for the same department store and told me about a job opening in an insurance agency. My degree was in Marketing, so I wasn’t 100% confident I could learn insurance. Almost 14 years later, I did learn and continue to learn more every day. THT: As an insurance agent, what is the most important piece of advice you would give regarding homeowners, auto and

For auto, the state requires limits of 30/60/25 ($30,000 per person, $60,000 per accident and $25,000 property damage), however, we at Younts Insurance won’t write any limits less than 50/100/25. For home, auto or commercial insurance, do not just go with the cheapest rate. Make sure you have adequate coverage. THT: What are the things that impact insurance costs the most? Automobile? Homeowners? Commercial? BB: For auto, it could be previous claims, a person’s credit or an inexperienced operator. For home and commercial, it could also be claims and credit.


THT: Are there things a person can do to receive discounts on Automobile, Homeowners or Commercial Insurance? BB: If you put your home and auto (and umbrella, if you have one) together with one carrier, often times, the companies will give a discount. While commercial insurance doesn’t give a multi-car discount, there are sometimes credits available from carriers. THT: Have you seen changes in Automobile or Homeowners Insurance since you have been in this industry? BB: Yes, auto rates have stayed the same, or gone down some, where homeowners rates are increasing due to claims. THT: What are questions everyone should ask their insurance agent regarding Homeowners, Auto or Commercial policies? BB: Do I have enough coverage? Am I properly covered in the event of a loss? Do I have the correct drivers, vehicles on my auto policy? Do I have any rental property that could affect my umbrella that I need to let my agent know about? THT: Let’s switch topics. Can you tell us a little about your involvement with animal rescue? BB: I’ve been involved with animal rescue for many years, about 15-plus years! I started by fostering adoptable pets which led to foster failures, transports, and many lasting friendships. THT: How many pets do you have? BB: I am owned by 9 dogs and 2 cats. THT: Did you grow up with pets?

BB: I did grow up with pets. We had a Doberman named Katie and a medium mix named Susie. After they passed away, my dad said “no more dogs”. While he was out of town, my mom and I rescued a Boston Terrier/Pit Bull mix named Peppi! THT: What is your favorite rescue story? BB: I have so many, but my favorite is probably the story of Hope, a puppy mill rescue dog who was rescued along with 23 Schnauzers. She was in horrific shape when she came into our program. She was heartworm positive, less than 15 lbs. (should have been at least 30

Clara A Puppy Mill Survivor

ften times I am asked, whether being involved in rescue or a mom to nine dogs, three of which are puppy mill survivors, what my reward is? Reward meaning, does a survivor from a puppy mill have as much to “offer” as a dog from a shelter or even an owner surrender? In my honest opinion, I think they offer more. Being a part of a rescue organization and being able to go into a puppy mill, or as some call themselves “commercial breeders”, will forever change your life, change your heart. Tzu, also born into a puppy mill) and Addie Grayce (an English Springer Spaniel, surrendered by her owner), all played a part in helping us to know when the time was right for us to open our home to a new rescue. Upon the first initial meeting of “Sansa”, now named Clara Grayce, you would have never known she was a puppy mill survivor. You could tell that she was very attached to her foster mom, Mandy, but when I knelt down to pet her, Clara’s eyes showed me everything. She was terrified with fear and trembled. I gently scratched her under the chin and talked to her, but Clara wanted no part of me. Several minutes, which seemed like hours, went by and Mandy said, “She will be more comfortable in the car. I’ll put her on your lap and she will be OK.” I climbed in the backseat for the ride home and Mandy carefully sat the little girl onto my lap. Clara was stiff with fear and I thought her little heart would beat out of her chest. By the time we closed the door and she saw Mandy drive away, she looked up at me, climbed down off my lap and onto the floorboard and sat there for the next four hours as we drove home. I talked to her often and fed her treats to make her feel more secure. When we arrived home, it took 20 minutes to get her out of the car and into the house. I started introducing her to her new pack one by one. Most accepted her, some did not at first, but she didn’t care. Her eyes locked with our little abused and rescued Havanese, Shorty aka Short Man, and he has become her rock, her everything. While she is happy anywhere in the house with any of the others (she drives the senior dogs crazy), Shorty is her constant companion, they are inseparable. Today, Clara Grayce is a happy, healthy 16 month old. The days of

Clara with her BFF, Shorty her life in a puppy mill are over, but she will always be a shy and timid dog. Her days are spent hanging out with her pack, and most especially with her BFF, Shorty. With the help of a trainer, Clara is learning her socialization and people skills, and that not all people are going to hurt her. She loves the outdoors and playing ball. She also is a complete couch hog and enjoys watching television. These are all huge steps for Clara. So, when asked again, “Would you consider opening your home to a puppy mill survivor?” Me?
