Vet’s Corner Casey J. Clark, DVM Southwoods Animal Hospital I grew up out west and was always surrounded by animals – both wild and domestic. Through the years, my family primarily had cats and dogs but we even had a few other animals on various occasions. I was on course to become a human physician when the yearning for my pets became too strong when performing my undergraduate studies. I was in housing that did not allow animals and I realized that I could not live without the friendly wag of a tail when getting home or the comfort of a feline’s presence when staying up late studying for the next examination. This passion caused me to change gears and transfer out of the pre-med program and into a pre-veterinary program. I realized that as a veterinarian, I could not only help pets but people too. Before long, I found myself at Colorado State University in Fort Collins, Colorado. While in vet school, I had a Golden Retriever, a Great Dane and a cat that I adopted from the local animal shelter. After graduating veterinary school, I headed east to escape the cold and snow of the Rocky Mountains. I love North Carolina and originally worked in Winston Salem before working at Southwoods Animal Hospital in Greensboro. It is a great place to work and the team at Southwoods is my family away from home. I look forward to continue being a part of the Southwoods family for many, many years to come.

OSTEOARTHRITIS

A

s we head into fall, the weather will change and oftentimes, changes in our pets can be noticed as well. Cold weather can worsen the signs of orthopedic disease – disease affecting joints. Pet’s joints are formed where two bones meet, just like in humans. These joints involve bones that are covered in a tough cushion called cartilage. This material serves as a shock absorber and is critical in appropriate joint function. In healthy joints, the body is able to maintain a balance of cartilage production to match cartilage breakdown. While the following information will focus on dogs, it has a large degree of crossover to other species of animals. When cartilage is lost, it causes joint pain, decreased flexibility, and inflammation – this is known as osteoarthritis. This is the most common cause of chronic pain in dogs – affecting 1 in 5 individuals. This painful disease is more profound in older, overweight and large breed dogs. Other risk factors include inherited traits such as hip dysplasia, dogs with prior injuries to joints and individuals who are engaged in strenuous activities over long periods of time. One of the most important things that you can do is to keep your pet from becoming overweight. If your pet is overweight, please consult your veterinarian for weight loss solutions. Symptoms of osteoarthritis can oftentimes be subtle and are frequently disregarded as “slowing down” with age. One of the most

common symptoms is difficulty in performing activities that used to be easy – examples include: trouble getting up, decreased ability to climb stairs or jump, reluctance to play, falling behind on walks, and stiffness after activity. Pets are stoic and oftentimes will not vocalize or cry when they are uncomfortable. More commonly, they will rest or demonstrate withdrawn behavior. While osteoarthritis is not curable, it is very treatable and early treatment provides the greatest benefit. Treatments focus on decreasing inflammation, keeping cartilage healthy and decreasing its breakdown. Many treatments (often used together) can improve quality of life and slow the progression of osteoarthritis. Non-steroidal anti-inflammatory medications help control the inflammation present in osteoarthritis. By controlling inflammation, the cartilage destruction is slowed and pets feel less pain. There are numerous prescription drugs approved by the FDA and made specifically for pets (ex: Rimadyl). From a safety standpoint, it is best to avoid human non-steroidal drugs and you should always ask your veterinarian which product is best for your pet. These medications come in multiple forms (liquid, injectable and tablets) and varying palatability. I recommend bloodwork every six months to ensure that your pet is tolerating the medication appropriately. Joint supplements protect the cartilage in your pet’s joints. Most products will contain both glucosamine and chondroitin and when combined these ingredients have been shown to protect cartilage better than a single ingredient alone. Newer products (ex: Dasuquin) may contain ingredients from avocado/soybean and a significant amount of anti-oxidants. It is important to use a joint supplement with a strong reputation as there is no FDA regulation and label claims may not match the product sold. Lastly, many dog foods contain both glucosamine and chondroitin (which is great) but the levels are typically too low to be protective. Another useful medication to help slow the progression of osteoarthritis is a polysulfated glycosaminoglycan (ex: Adequan). This is an injectable medication used over the course of several weeks that helps prevent the wearing away of cartilage. This medication keeps cartilage intact by preventing its breakdown and helps restore the critical joint fluid that keeps cartilage healthy. Intact cartilage decreases the “bone-on-bone” contact in joints that causes significant pain. In addition to medications, laser therapy can decrease pain and inflammation and promote healing. Laser therapy is a noninvasive treatment that is surgery and drug free. This treatment uses a beam of laser light to penetrate tissues without causing harm. A series of treatments yields the greatest response and this form of therapy can often be adjunctive to non-steroidal anti-inflammatory medications and joint supplements. If you notice your furry family member slowing down, please discuss osteoarthritis with your veterinarian. While this can be a very challenging condition, it is quite rewarding when managed effectively. If your pet is not having problems, please keep them slim and promote a regular exercise program at home.

Some needed consoling while others just wanted food and water. Local residents were coming in and looking for their pets. If they were not found they would fill out a rescue request for a rescue boat to try to find their house and get their pet. At some point, when the water was down to about 4 feet, we had a request to rescue a Pit Bull from a single-wide trailer. Three of us on foot waded up to the trailer and, with crow bar in hand, busted a window out. We decided I had the most Pit Bull experience so I was shoved through the window. The trailer was drenched. I’ll never forget finding a nearly one year old Pit Bull behind a door, scared to death. As soon as I picked her up she melted as if to say thank you. There were so many stories and we remember all of them. Each night we would go back to our condo

and sit and stare at each other. Most nights we shared stories and cried. It was mentally and physically exhausting, but we would not have been anywhere else. About five days into the week we were able to take the boats and dock near many of the neighborhoods and get out to look for animals. Floyd had hit the poorest parts of Pitt County where people had nothing to lose and they lost what little they had. One day we docked on an island where there was a neighborhood of houses that were up on higher ground and the water had not touched a few of them. One resident had ridden out the storm, but desperately needed dog food for the neighboring dogs, as well as a few things for himself. He became one of our stops each day. Not too far from this stop was a very small trailer community where no one had come back for two larger red dogs. In the years to come, ARFP volunteers would attend more disaster trainings and respond to more disasters. There are too many stories to tell. We worked in Pitt

County helping with the unclaimed animals for many months on and off after Floyd. When the animals were finally released to rescue groups, we had favorites that we had all had a part in rescuing. We brought back a total of 52 cats and dogs to Greensboro to be fostered. They were all special. Three were returned to their owners once they got on their feet. On one of our trips back to Greenville I knew I needed to put some closure to the two larger red dogs. They tugged at me for some reason. We pulled into the run down trailer community which no one had come back to. There they were. I opened the back door of the van we were driving and none of us ever looked back. This is in memory of Rita the red dog. She came back to Greensboro and lived the life she should have thanks to a very special couple. I’ll always be grateful.

Vet’s Corner Susie Hodnett, DVM At Home Veterinary Housecalls This is Slick, my sweet 17 year old kitty, along with myself, Dr. Susie Hodnett. My husband and I live in Greensboro, and share our home with Slick and a little dog named Patrick. My housecall practice, At Home Veterinary Housecalls, is now twenty-four years old! Special interests include internal medicine, geriatrics, and dentistry. I love to share my passion for veterinary medicine; four of my previous employees are now practicing veterinarians. In my spare time, I enjoy wilderness camping and horseback riding, kayaking, and playing the Irish fiddle.

Old Age is Not a Disease! Just as we are living longer, healthier lives, so are our pets. We love them; they are our family. We want them with us as long as possible, happy and healthy, wagging their tails when we come home, purring on our laps in the evening. Time has a way of slipping up on us. The puppy you remember running around the yard is getting gray in the face; the kitten is sleeping on the bed rather than running up your pants leg. So how do we know when our pets reach “senior” status? Dogs’ longevity is generally based on size. Larger dogs (Great Danes, Mastiffs) are considered senior at ages five to six. Small breeds (Poodles, Yorkies) and cats can make it to age ten before becoming seniors. So how can we optimize our pet’s length and quality of life? They have the same basic requirements for a good life as we do: love, healthy and tasty food, exercise, and good health care. Love is the easy part of the equation; just look down at that sweet face. Choosing a healthy food can be more difficult. Senior pets need a high quality protein, restriction of calories and supplementation of various nutrients. Choose a good quality senior diet from an established pet food company. Your veterinarian may even suggest a prescription diet if health problems develop. Let’s talk about the amount to feed. Look at your pet. Can you feel the ribs with the flat of your hand, but not see them? Does he have a “waist?” If not, he is overweight. Reduce the amount fed (watch the table food), and consider adding canned cooked pumpkin, or various veggies for dogs. The higher protein in

canned food is ideal for cats (the “Catkins” diet). Exercise is an important key to a long, healthy life. It keeps the joints well oiled, and muscles strong. Arthritic problems worsen more quickly in the larger breeds. It is so very difficult to manage a large dog who cannot get up or walk well. Watch the amount of food we feed, and get those doggies outside! Play with your kitty; have them stalking the feather toy. Fun for everyone! Annual or semiannual visits to the veterinarian include a thorough physical examination. Heart disease, osteoarthritis, and skin masses are three common problems diagnosed in our senior pets. Blood work and a urinalysis should be done annually. Diabetes, kidney and liver diseases, bladder infections, and thyroid problems are commonly diagnosed with these tests. All of these diseases respond better to treatment if diagnosed early. As an example, if feline hypertension (high blood pressure) is not diagnosed early, the cat can experience a detached retina and loss of vision. Blood pressure medications will prevent this disastrous complication. Your veterinarian will examine your pet’s teeth to determine when a dental cleaning is needed. Improvements in anesthesia, as well as preanesthetic testing, makes dental work much safer. We routinely do dental procedures on senior, even geriatric pets. Owners often remark that their pets are much happier and more active after having dental problems fixed. What they thought was normal aging was actually painful dental disease! Untreated dental disease can cause kidney and heart disease, as well as a poor quality of life for your pet. Please do not neglect this most important piece of health care. Dental treats and brushing are helpful between cleanings. Older pets are being diagnosed in increasing numbers with osteoarthritis. Your pet does not have to live with this pain! Things you can do at home are exercise and weight control, of course. Fish oil capsules and joint supplements such as glucosamine can be helpful. Dasuquin is a supplement I use for both dogs and cats. Arthritic kitties seem to respond very nicely. Talk with your veterinarian; there are multiple pain medications now available.

We find that using multiple medications at the same time to control pain is very effective in managing osteoarthritis. Laser therapy and acupuncture are other options. Since I am writing this article, I must plug housecalls for senior pets (as well as the younger ones of course)! A housecall is beneficial in that your pet is examined in the comfort of their home, lowering their stress level. Older dogs do not have to get in and out of the car. Kitties of all ages appreciate not having to get in that cat carrier. A housecall offers an opportunity for long face time with your veterinarian, time spent individualizing recommendations for you and your pet. Above all, choose a veterinarian with whom both you and your pet feel comfortable. Ask questions; do your research. Work with your veterinarian as a partner in keeping your pet at your side for as long as possible.

The Pet Haven Project Violence in the home affects EVERYONE! October is Domestic Violence Awareness Month.

PETS AND DOMESTIC VIOLENCE by Scott Cunningham, Chief of Police, Town of Kernersville Often, people in domestic violence situations will remain with the abuser out of fear: fear of having nowhere to go, fear of their pets being injured, and general fear of significant change. Abusers attempt to control their victims by fear, force, and/or violence. The violence or threat of violence may be directed at the spouse/partner and/or it may be directed at children or pets. Getting away from a domestic violence situation is tough. Sometimes victims have to leave with little notice or preparation. Now options exist to help victims and their pets.

Pet Haven project

by Bruce Hennequin, Leadership Kernersville 2014 According to the American Humane Association, 71% of pet-owning women entering shelters reported the injuring, maiming or killing of family pets for revenge or psychological control. Of the 68% of battered women who reported violence towards their animals, they reported that 87% of the animal violence occurred in their presence and 75% occurred in the presence of children. Sadly, 25-40% of battered women are unable to escape abusive situations because they worry about what will happen to their pets and livestock if they leave. Statistics from the ASPCA provide similar numbers. A study of 11 U.S. cities revealed that a history of pet abuse is one of the four most significant indicators of who is at greatest risk of becoming a domestic violence batterer. A Texas study found that batterers who also abuse pets are more dangerous and use more violent and controlling behaviors. Children exposed to domestic violence were three times more likely to be cruel to animals. Finally, we learned that the ASPCA is assisting by helping to establish animal housing facilities in women’s shelters in order to keep family members together when leaving abusive situations. Lynne Garms, Executive Director of Next Step Ministries, has encountered many victims of

domestic violence that delay leaving a dangerous situation due to fear for family pets left behind. In an effort to help these victims, Leadership Kernersville spearheaded the Pet Haven Project, a part of Next Step Ministries, in the Fall of 2013.

Support for Next Step Ministries and Pet Haven Project

It costs between $17,000 and $20,000 per month to operate Next Step Ministries’ safe house and to provide the case management and services necessary to assist the victims who receive assistance. Proceeds are generated from the Next Step Ministries Thrift Store and grant funding is received from The Kernersville Foundation, The Winston Salem Foundation, HUD (Housing and Urban Development) and the NCCFW (North Carolina Council for Women funding). The ASPCA also provided grant funding for the Pet Haven Project. Local businesses providing help in one way or another, include: Trend Setters Salon and Spa, Laundromuttz, Dog-Gone Fun, Northwest Animal Hospital and Willow Run Kennels are just a few. Next Step Ministries greatly relies on donations from local businesses, churches, and individuals as well as volunteers from the local community to continue to offer the quality of service to our clients. Grateful for the support they have received over the past 15+ years, they have been able to not only save lives, but also make a positive difference and help break the cycle of violence for families living in our community. Every ten minutes police respond to an average of two DV calls. No one should have to live in an environment where they are consistently fearful of their safety. Neither should they have to fear for the well being of their children, family members or pets. Next Step Ministries is a 501C-3 non-profit organization that was founded in 1998 following the murder of a local woman as a result of a domestic dispute. Community leaders and pastors were devastated by the fact that this could happen so close to home that they came together and formed Next Step Ministries, Inc, a place where victims of domestic violence could go for food, shelter, guidance, and safety. Weebles wobble but they don’t fall down! That has been Weeble’s goal the past 5 months. Weeble came to Red Dog Farm about 5 months ago after her previous owner could no longer keep her. Weeble suffers from Metabolic Bone Deficiency, often found in reptiles caused by calcium deficiency, poorly balanced diet, and or lack of UVB lighting. Weeble has been in rehabilitation to gain her strength and balance and we feel that she is now healthy enough to find her forever home. She still has challenges walking and crawling normally as her legs are malformed. She sometimes flips herself over and cannot correct herself on her own. With time these changes may completely reverse or they may always remain an issue.

Putty is a beautiful, loving 3 year old Doberboy who came to us along with Shearer, possibly his mother. They came from some sort of wretched backyard breeder situation. Putty arrived here emaciated, probably because of intestinal Doberman Rescue of the Triad worms. He is a lovely, sweet boy who loves to play and prance around; he play bows, clacks his teeth and sports an unusual toothy grin when greeting, and it’s all just love and goodwill. He’s super friendly, good on a leash, jumps into and out of the SUV without difficulty (despite giving indication of some sort of past trauma to his hips/hind end that has given him a touch of arthritis).
