Adult Service Transition Coordinator Monadnock Developmental Services is looking for a Transition Coordinator Case Manager for young adults with disabilities. This new position is to join a team of approximately 15 to work with adults with disabilities. The position is based out of our Keene office for 37.5 hours a week. The caseload will be based out of Cheshire County and parts of Hillsborough County. Successful candidates will have strong advocacy and organizational skills, ability to work with a variety of teams in a fast-paced environment, network with the community, facilitate meetings, help develop dreams and problem solve. Candidates should be social, have the ability to multitask, and possess strong conflict management, computer and communication skills. The position entails monthly contact with individuals, families, and vendors on caseload and the development of new individual programs. Candidate would coordinate and facilitate team meetings and annual service agreements in accordance to regulatory guidelines.
