F | INDOOR SKATEPARK The Salvation Army Kroc Center Indoor Skatepark was added to the Kroc Center building plans at the request of a community youth committee. The Skatepark is filled with several elements to challenge both beginning and advanced skaters, including a full half-pipe ramp. F | FAMILY RESOURCE CENTER The Family Resource Center provides services through supportive case management. Applicants for services meet with Social Workers for a comprehensive assessment. Services include scholarships, emergency assistance, educational courses, and counseling. F | ADMINISTRATION OFFICES & COMMUNITY ROOM F | RJ’S PRESCHOOL ACADEMY A new Christian Preschool dedicated to providing quality early childhood education to prepare children for Kindergarden and beyond. Infant and Waddler care are also available, as well as extended care hours. E | RECREATION FIELD, CHALLENGE COURSE & ROCKWALL Suitable for soccer, flag football and other outdoor sports, the 55,000 square-foot field is bordered on the south by a challenge course consisting of several low and high rope course elements, and a 30-foot tall rock-climbing tower. E | GYMNASIUM & FITNESS CENTER A multi-use center, this 56,000 square-foot building contains a fitness area with weight and cardiovascular exercise equipment, a group exercise class area, and an NBA regulation basketball court. The gymnasium is ideally suited for volleyball, basketball, gymnastics, and badminton. There is also a family recreation area with foosball, air hockey and table tennis. 4

E | AQUATICS CENTER Water exercise, therapy, lessons and sports programs are available in three areas: 25 meter x 25 yard short course Competition Pool (80-82 degrees); Recreation Pool (84-86 degrees); indoor warm water Therapy Pool (92-96 degrees). D | KROC CAFÉ The brand new Kroc Café offers members and guests a convenient and friendly place to grab a coffee or bite to eat. Guests are encouraged to utilize our seating area to relax and enjoy food and fellowship. C | ICE ARENA The Kroc Center Ice Arena is the only ice arena centrally located in the heart of San Diego! A year round, NHL regulation sized ice surface, Kroc Center Ice offers public ice skating, learn to skate classes, learn to play hockey classes, drop-in hockey, youth in-house hockey, and adult hockey leagues. Host your next Birthday Party at the Kroc Center Ice Arena! B | PERFORMING ARTS CENTER The fully equipped 33,000 square-foot Performing Arts Center consists of the Joan B. Kroc Worship Theatre, dance studio, band and orchestra room, vocal and instrumental practice rooms and a large multipurpose conference room. The Joan B. Kroc Worship Theatre is used for our weekly church services, and Broadway-quality shows. It features seating for 600, a motorized orchestra pit, scene shop and dressing rooms. A | EDUCATION CENTER Adjacent to the Performing Arts Center, the Education Center features a youth library, visual arts studio, computer lab, study hall, classrooms and tutoring rooms.

Dear Friends, June 19, 2012 will be our tenth anniversary. Exciting celebrations are scheduled. The anniversary causes us to think again about Joan Kroc and the stupendous legacy she left this community. In the span of this past decade, if only there was a way to know how many swim lessons given prevented a future drowning, how many youth productively engaged avoided delinquency, and how many good souls escaped hopelessness through ministries such as Kroc Church and Celebration Recovery. If only we could know how many artistic and athletic skills have been gained, how many hours of fun have been experienced, how many new friends made, how many pounds lost, muscle mass gained, brains sharpened, songs sung and laughter enjoyed. You may not know that after Mrs. Kroc saw the Kroc Center in operation she gave another, even greater gift. That gift provided the means for additional Kroc Centers to be built in more than a score of communities across the U.S. In the western U.S., Kroc Centers are already open, or will in 2012, such as Coeur d’Alene, Salem, Phoenix, San Francisco, Suisan City and Honolulu.

The best way we can thank Mrs. Kroc is to keep her legacy going and growing. As the San Diego Kroc Center moves into its second decade of service to the community, we invite you to join our team as a volunteer. The opportunities are varied and many. A bus driver became annoyed with his job because he had to wait 7 minutes after every run near an open field which ‘litterbugs’ had made into an unofficial dump. He often thought that somebody should do something about that unsightly mess. One day he himself decided to get out and pick up some of the cans and other debris which were lying all around. This improved things so much that he soon was eager to complete his route and spend all his free moments in cleaning up the area. When spring came, he was so enthusiastic about this project that he decided to sow some flower seeds. By the end of the summer people were riding to the end of the line just to see what the bus driver had accomplished by doing what he and others had only talked about before. Perhaps you would like to join with us to make this a better community by volunteering. You can do so by calling 619.269.1424. Thanks to the generosity of Joan Kroc and each of us coming together for fun, recreation, learning and service, we can be assured that 2012 will be the best year yet at the Kroc Center.

Nearly 33 million Americans receive assistance from The Salvation Army each year through a wide array of social services that range from providing food for the hungry, relief for disaster victims, assistance for the disabled, outreach to the elderly and ill, clothing and shelter to the homeless and opportunities for underprivileged children. BRIDGING THE GAP BETWEEN POTENTIAL AND OPPORTUNITY Joan Kroc, wife of McDonald’s founder Ray Kroc, had a clear vision in mind when she gave The Salvation Army $87 million to build the flagship San Diego Kroc Center: build stateof-the-art recreational and arts facility in an underserved community to ensure that all individuals have equal opportunities to grow their natural gifts and talents. This 12.4 acre campus was built on a blighted abandoned shopping center lot, transforming the neighborhood, and bringing light to the community. The programs, as well as the building itself, were designed to stimulate the mind, body and spirit, to provide hope, and to transform the life of each and every member of the community.

Future When Mrs. Kroc passed away in October 2003, she left $1.5 billion - much of her estate - to The Salvation Army, by far the largest charitable gift ever given to the Army, and the largest single gift given to any charity at one time.

We are now offering FREE Introductory courses for adults and children in the following areas: Learn to Ice Skate Ballet Jazz Modern Contemporary Dance Skateboarding After School Sports (Drop in on first two classes of each session FREE) Percussion Swim Safety with Skill Evaluation (2 weeks) *

All courses are 4 weeks and 30 minutes in length unless otherwise noted. Please see the Athletics and Aquatics schedule for a full list of classes, schedules and locations. Courses will be titled INTRO, are FREE, and are available to General Family and Single Parent Family members only.

MEMBERSHIP PAYMENT PLANS

There is a $20 charge for declined automatic funds transfers. Memberships are deactivated until payment is received, and use of the Kroc Center is not permitted until payments are made current and all applicable fees are paid.

“PAY AS YOU GO” MONTHLY PAYMENTS Monthly membership dues can be paid via Electronic Funds Transfer (EFT) from a checking account or credit card. Dues for the first month and the required registration fee are due at the time of enrollment. To complete the membership process, the EFT Agreement must be completed. Membership dues will be automatically transferred from the account holders authorized account around the 20th of every month for the upcoming month.

CANCELLATIONS & CHANGES Membership Change/Cancellation requests must be submitted in writing and signed by the authorized Account Holder on file by the 5th of the month to effect the next draft. This includes changes to monthly draft Information. If a member cancels or if membership payments lapse for more than 30 days, the appropriate registration fee will be charged to reopen the membership account. Membership fees are non-refundable and nontransferable.

MEMBERSHIP POLICIES

Financial Assistance & Scholarship Program Individuals, families, and seniors who cannot afford Kroc Center membership or fees for youth camps and/or group classes can apply for a discount. Applicants for services will meet with FRC Associates for a comprehensive assessment to build upon strengths and assets and assist with meeting identified areas of need. Eligibility for this program is based on income level and address verification within the Kroc Center’s service area. Applications are available at any Guest Service Desk, in the Family Resource Center, and at www.kroccenter.org. Applications should be submitted at least six weeks prior to the start of the program requested.Additional information can be obtained by calling 619.269.1430 or e-mailing krocscholarships@usw.salvationarmy.org

NON-PAYMENT POLICY

ANNUAL PAYMENT: Annual payments may be paid in full by cash, debit card, or major credit card. Fees are equivalent to 12 monthly payments. The annual fee and required registration fee are due at time of enrollment.

LIABILITY WAIVER By signing our Membership Application and/or program enrollment form, members agree to the following: (1) member and any guests in his/her party will abide by the terms of this Agreement at all times during the period of membership and will comply with all rules and regulations posted or otherwise communicated to member, (2) in case of illness or injury, The Salvation Army Kroc Center is authorized to secure emergency medical treatment at the member’s expense, (3) The Salvation Army Kroc Center reserves the right to remove from the facility or terminate the membership of any member who fails to comply with any posted rules and regulations or otherwise breaches the terms of the Membership Agreement, in which case member will not be entitled to a refund of dues, (4) membership rights are not transferable, and (5) grant permission for The Salvation Army Kroc Center to make visual recordings of all individuals listed on this form for its responsible use. Also, that use of the facilities and equipment at The Salvation Army Kroc Center may involve risk of bodily injury or property damage and member agrees to assume any such risks. It is a member’s responsibility to consult physicians and other professionals to make sure they can safely participate in activities and events at The Salvation Army Kroc Center. By signing, members agree to give up their (or the minor for whom they sign) right to make any claim against The Salvation Army, its agents, employees and volunteers, including the right to sue them, for bodily injury or property damage or any other loss that might be suffered while using The Salvation Army Kroc Center facilities and services, except as limited by law.

REGISTRATION FEES Registration fees are used to provide major repairs and equipment replacement. This fee is payable when you join and is in addition to membership dues. There is no registration fee for Youth or Teen memberships. The registration fee will be waived for all renewing members. Memberships that have expired or been cancelled for more than 30 days are subject to repayment of the registration fee. Registration and membership fees are non-refundable and non-transferable. Please note: Membership fees, benefits, and payment structure are subject to change without notice.

MEMBERSHIP OPTIONS FAMILY MEMBERSHIP LIMITS Family memberships include: 2 adults and up to 6 dependent children ages 17 & under living in same household. Single-Parent Family memberships include 1 adult and up to 6 dependent children ages 17 & under living in same household. Dependents, ages 18-23, may be included in Family Memberships as dependent children, subject to verification of legal dependent status (current Federal Tax Return or proof of full-time student status). MEDICAL HOLD Members who are unable to use their membership due to medical reasons may put their membership on medical hold for up to 6 months. To place a membership on medical hold, a doctor’s note must be submitted with a completed Membership Change Form. A second doctor’s note is required to remove the medical hold. Monthly payment members will be drafted $1 per month while on medical hold. Annual pay members will not be charged a fee to place their membership on medical hold. Membership benefits are not active while a membership is on medical hold.

CLASS/PROGRAM CANCELLATIONS Cancellations five or more days prior to the first class: Full amount will be issued via Kroc Center Gift Card or you may choose to transfer to another class with no penalty. Cancellations less than five days prior to the first class: Full amount to a Kroc Center Gift Card or transfer to another class minus a $10 service charge. Cancellations after the start of a session: prorated Kroc Center Gift Card for the number of classes which have occurred (regardless of whether classes were attended), minus a $10 service charge.

CREDIT/REFUND POLICY Refunds are available for classes and leagues only. The purpose of our gift card/refund policy is to allow The Salvation Army Kroc Center to maintain quality programs and proper class ratios, while maintaining flexibility for our members and guests. Refunds are only issued for programs that are cancelled by the Kroc Center. Please allow a minimum of 2 weeks for reimbursement

DAY CAMP REFUND POLICY CANCELLATIONS GROUP MEMBERSHIP RATES We now offer special group rates to specific groups: residential group homes, businesses, and service organizations (Military, Fire & Police). Please contact the Membership Manager at 619.269.1501 for more details on this program and to see if your group qualifies. DORMANT HOLD (Automatic Draft Memberships Only) Members making monthly payments via EFT who cannot use their membership due to any nonmedical reasons (i.e. long vacations or out of town) may put their membership on ‘Dormant Hold’ for a minimum of 2 months and a maximum of 6 months. Dormant memberships are drafted $10 per month and membership benefits are not active while a membership is on Dormant Hold.

DISCOUNTS Member discounts are not transferable to other family members. In order to receive a member discount, the registering participant must be a member in good standing of the Kroc Center.

GIFT CARDS Gift Cards are available to pay for memberships, class and program fees. Gift Cards can be purchased at the Gymnasium Guest Service Desk. Gift cards cannot be used for class materials, theatre & performance tickets or vendor events. Gift Cards are applicable toward anything in The Salvation Army Kroc Center. Gift Cards do not expire. Gift Cards are not redeemable for cash. No Gift Cards or refunds are issued for membership, registration fees, gift certificates or class passes. The Kroc center is not responsible for lost or stolen Gift Cards.

VOLUNTEER OPPORTUNITIES The Kroc Center has many areas where volunteers provide valued support and service. PLAYCARE PlayCare is for members and guests involved in Kroc Center Programs. Use is restricted to one two-hour session daily. Space is limited; children are accommodated on a first-come basis. PlayCare is available in the Gymnasium and Education Center. Kroc Center PlayCare maintains a ratio of 1 associate for every 8 children and 1: 6 ratio if an infant is enrolled. BOOKING & CANCELLATIONS

In order to facilitate positive life experiences while at the Kroc Center, mutual respect between members, guests, and staff is required at all times. Failure to observe these policies will result in disciplinary action and/or termination of membership privileges. Center personnel have the right to refuse service to anyone deemed disruptive or abusive.

• Parties must be booked a minimum of 14 days in advance. • Full payment is required at the time of booking. • No cash refunds are given unless the party is cancelled by the Kroc Center. • Charges for any additional guests will be collected at the time of the party. • Food orders cannot be altered within 48 hours of the party. • If you cancel the party 7 or more days prior to the booked date, you will receive your full balance on a Kroc Gift Card, minus a $25 booking fee. • You can also transfer the date of your party for a $10 transfer fee. • Cancellations made less than 7 days prior to the event are not eligible for a refund or credit. • Parties with Inclement weather can be issued a rain check or option of different party area (based on availability). • Rain closes the Rock Climbing Tower and Bounce Houses. • Skateboarding parties only available on Sunday mornings. • For the Party Like a Kroc Star Package Only: May through September the Party area is on the Recreation Field and includes a shared Bounce House. October through April the Party area is in the Multi-Purpose room and includes an Arts and Crafts activity for all parties.

GENERAL KROC CENTER PARTY POLICIES • A maximum of 20 children (under 18 years of age) allowed per party. • There must be one (1) adult for every six (6) children (817 years old) with a minimum of 3 adults per party • No outside food or beverages are allowed at Kroc Center San Diego. Please inquire about special catering packages if additional food is needed. • Plastic tableware, table cloths, birthday candles, and serving utensils will be provided for each party. • Please arrive no less than 15 minutes prior to your parties start time to ensure enough time to check-in, receive wristbands and get your party set-up. • Approved table top decorations can be brought in to decorate. You will have access to your party area for set-up approximately 15 minutes prior to party start time. • Guests are responsible for removing gifts and personal items before the scheduled party is over. • Tacking decorations on the structures or any walls is not permitted. • Please no glitter or confetti. • Outside birthday cakes or cupcakes can be brought into the party area. Refrigeration is not available for ice cream cakes. No other outside food or beverages. • Every party guest participating in an activity must wear a wrist band provided by the party host. • In consideration of health and safety of all our guests it is our policy that no left over food from a party be allowed to leave the premises with the event host or any attendee. • Acknowledge and abide by general facility rules and safety guidelines as noted in the general acknowledgement form.

• Children under the age of 11 must be accompanied by an adult (18 years and over) during all activities. Children over the age of 5 must use gender-appropriate bath and locker rooms. • All visitors, guests and spectators must sign in. • Refrain from the use of foul language, abusive actions, and inappropriate behavior. • Refrain from smoking on Kroc Center grounds. • Lost membership cards must be reported. There is a $3 fee to replace a membership card. • Members must carry photo ID. • The Fitness Area is reserved for those age 16 and older, unless participating in a Kroc Center program or class. • Appropriate exercise attire is required (workout clothing and closed-toe athletic shoes). • Only light colored soft-soled shoes will be allowed on the Gymnasium floor. • A shirt and shoes are required at all times in the Fitness Center and Gymnasium. • Any logos or messages on clothing must be acceptable in a family setting. Individuals wearing gang-related attire will not be admitted to the facility. • With the exception of service animals, pets of any kind are not allowed in any Kroc Center venue.

SAFETY INFORMATION The Salvation Army is dedicated to keeping children and vulnerable adults safe from physical, emotional and sexual harm. We refer to our safety efforts as “Protecting The Mission.” Protecting The Mission guidelines ensure the Kroc Corps Community Center emphasizes safety in the following ways: • All Salvation Army personnel (officers, employees and volunteers) are trained in child abuse prevention and are mandated reporters of suspected child/vulnerable adult abuse. • All Salvation Army personnel undergo criminal background checks. • All adults accessing the facility may be screened against public sexual offender registry. Registered sex offenders are not permitted on campus. • The Kroc Center maintains video surveillance of the facility 24 hours a day. • All instructors are trained in appropriate interactions with Kroc members to enhance learning by creating a safe and inviting environment. • Programs involving children are closely monitored to ensure adequate supervision and safe interactions between Kroc personnel and children. • Programs that require solitude, such as private music lessons, are held in open and visible spaces that are monitored by regular walk-throughs by other personnel. • The Kroc Center constantly adapts and enhances safety protocols to adjust to needs.

Members and guests are encouraged to report suspicious behavior. Forms for anonymous reporting, called Confidential Notices of Concern, are available at any Guest Service Desk. The Salvation Army Kroc Center reserves the right to remove from the facility and terminate the membership of any member who fails to comply with any posted rules and regulations or otherwise breaches the Code of Conduct, in which case member will not be entitled to a refund of dues.

SKATEPARK RULES & REGULATIONS • The Skatepark is for skateboarding and rollerblading only. All other uses are prohibited. Sorry, no bikes or scooters. • All skaters must have a signed liability waiver on file. For skaters under 18 years of age, a parent or guardian must sign waiver in person, or have waiver notarized. • Minimum age for skaters is six years old. All children under the age of 11 must be accompanied by an adult (18 years & older) • All skaters must wear the appropriate safety gear, including: helmet, elbow pads, knee pads, shirt and appropriate shoes. In addition, wrist guards are strongly recommended. Failure to wear protective gear may result in loss of use privileges. • All skaters must skate safely at all times, and avoid collisions with others. Fighting or other aggressive, disruptive, or rude behavior will result in ejection from the Park. Ejected skaters must obtain Manager approval prior to returning. • All equipment used by skaters must be in good condition, and must not damage the Park or its structures. • Spectators are not allowed in the skating areas. • All forms of graffiti, tagging or other means of defacing Center property is strictly prohibited. Such activity may result in closure of the facility until it can be cleaned and/ or repaired. Violators will be ejected and not permitted back without the approval of the Manager. Payment of repair costs will be required to return. • Food, drinks, or glass containers are not allowed in skating areas. • Drugs, alcohol, weapons, or profanity are not allowed. • Loitering or skating outside the park, including sidewalks, parking lots, brickwork, planters, or other ornamental surfaces is not allowed. • Items such as benches, tables, wood materials or other objects may not be used as ramps or jumps and will not be allowed in the Skatepark. • Modifications to the Skate Park are not allowed.

The Salvation Army Kroc Center’s Family Resource Center provides supportive services to individuals, families, and seniors. Some assistance is based on predetermined guidelines and availability of services.

CASE MANAGEMENT: All Family Resource Center (FRC) services are provided through supportive and strengths-based case management. Applicants for services will meet with FRC Associates for a comprehensive assessment to build upon strengths and assets and assist with meeting identified areas of need. MEMBERSHIP AND PROGRAM SCHOLARSHIPS Individuals, families, and seniors who require financial assistance for Kroc Center membership or fees for youth camps and/or group classes may apply for a discount. Discounts are based on a sliding scale, taking into consideration household size** and income. Eligibility for this program is based on address verification within the Kroc Center’s Service Area.*

CHRISTMAS TOY AND JOY PROGRAM Each Christmas, The Salvation Army Kroc Center serves needy families within the Kroc Center’s Service Area*. Access to the fitness center is restricted to guests 16 years of age or older (id required), unless enrolled in a fitness center class or completion of the Teen Fitness Orientation

A fitness professional will help you determine body composition - the percentage of lean muscle mass versus body fat. BODY COMPOSITIONS are done by appointment only.

Teen members ages 14 & 15 may access the fitness center after completing a 1-hour teen fitness orientation.

ARE YOU A TEEN WHO WANTS TO USE THE FITNESS CENTER?

Members and guests are asked to carry identification and membership cards or day passes at all times. All guests must have a signed facility waiver on file. Towels are required for every person working out in the fitness center. Towels may be purchased at the gymnasium guest service desk for $4 (subject to availability). There is a 20-minute maximum usage time per machine during peak use periods. Classes are subject to cancellation due to low enrollment. Appropriate exercise attire required (workout clothing and closed-toe athletic shoes). Shirt and shoes are required at all times in the fitness center.

14 & 15 year olds can use the gym with successful completion of a 1-hour TEEN FITNESS ORIENTATION. Free with teen membership! Tuesdays, 6:00pm, $15 sign-up.

Ages 6 and over Our indoor Skatepark contains elements that challenge both the beginner and advanced skateboarder. For maximum safety, kneepads, elbow pads and a helmet are required for all skaters 17 years old and under. Helmets are required for skaters 18 years and older; elbow pads and kneepads are also recommended. A liability waiver signed by a parent or legal guardian is required prior to admission to the facility for participants under the age of 18.

SAINTS BASKETBALL LIFE OPTIONS K-9th Grade A youth basketball league designed to help improve children’s sports skills while emphasizing fun, sportsmanship, teamwork, and the development of self-esteem. DAY CAMP GENERAL INFORMATION CAMP ASSUMPTION OF RISK & LIABILITY WAIVER AND HEALTH HISTORY FORM A current form must be completed and on file with the Day Camp prior to your child’s first day of camp. COUNSELOR TO CAMPER RATIOS Kroc Center Day Camp ensures sufficient staff-tocamper ratios as recommended by the American Camp Association and will not exceed 1:10. A minimum of two staff are always present, regardless of the number of campers. DAY CAMP STAFF Our staff is as diverse as our campers. The majority of our staff is recruited from local colleges and universities. Most importantly, our staff is made up of people who love kids. They want to spend their time playing, teaching, and working with children. All Kroc Center Day Camp staff members are certified in basic First Aid & CPR and receive extensive training prior to the start of camp. Rest assured, we are hiring the best of the best. DISCOUNTS Member discounts are not transferable to other family members. In order for a camper to receive a member discount, the child must have a youth or family membership at the Kroc Center. DROP-OFF AND PICK-UP PROCEDURES Camper Drop-off and pick-up occurs at the Recreation Field during regular times. Extended Care drop-off occurs in the Gymnasium from 6:30 7:45 a.m. Parents arriving after 8:30 a.m. for sign-in or prior to 3:30 p.m. for sign-out will report to the Gymnasium Guest Service Desk and be instructed where to go to meet their camper’s group. Parents planning to take children home directly following the event, must sign him or her out at the Gymnasium Guest Service Desk prior to the event. Regular Sign-in & Sign-out times: Morning Drop-Off: 7:45 - 8:30 a.m. Afternoon Pick-Up: 3:30 - 4:00 p.m. Campers must be signed in and out each day by an authorized adult in the presence of Kroc Center Staff. A valid form of identification must be shown when a child is signed out of Day Camp. Valid forms of identification include current California State Identification Card; State issued driver’s license, Military Identification card, or Passport. All changes to the list of adults authorized to pick-up a child must be made on the Change of Authorization form by the parent who enrolled the child in camp. Please be prepared to show identification. Children must be 11 years of age or older to sign themselves out of camp. Parents wishing to give their children permission to do so may write the child’s name in the space provided on the waiver for authorized adults. In the event of inclement weather or irregular drop-off or pick-up, please consult the Gymnasium Guest Service Desk for Day Camp group locations.

EXTENDED CARE Additional supervision is offered FREE during Fall, Winter, and Spring Camps from 6:30 - 8:00 a.m. and from 4:00 - 6:00 p.m. FEES There is a $25 non-refundable deposit charged for each week ($10 for single-day registrations). To avoid forfeiture of deposit, balances are due in full one week prior to the start of the session. However, enrollment may be reinstated based on space availability but would be subject to a late fee of $10. FINANCIAL ASSISTANCE/ SCHOLARSHIP PROGRAM Families that cannot afford fees for Day Camp can apply for a discount. Eligibility for this program is based on income level and address verification within the Kroc Center’s service area. Applications are available at any venue Guest Service Desk or the Family Resource Center and should be submitted by the deadlines listed below. An immunization exemption waiver is required for parents who do NOT immunize due to religious beliefs. HOW TO REGISTER Registration can be completed on-site or by faxing the necessary registration forms. Registration must occur and all outstanding balances paid one week prior to the camp start date in order to avoid a $10 late registration fee. Register by Fax: 619.269.0272 (credit card payments only) INCLUSION The Salvation Army Kroc Center is a proud program partner of the National Inclusion Project in ensuring all children have access to recreat ion programs. The Kroc Center is committed to the inclusion of all children in our Day Camp Program. Parents of children with special needs should contact the Day Camp Manager at least two weeks prior to the desired session’s start date to determine whether the Kroc Center can provide appropriate accommodations and which camp(s) are best suited for him or her. Inclusion intake forms are available upon request or as a download on www.kroccenter.org/day-camp/

LATE PICK-UP A $1/minute late fee is charged for each minute a child remains at camp past 6:00 p.m. If the child is not picked up by 7:00 p.m. and we are unable to reach the parents, Child Protective Services will be contacted. MEDICAL NEEDS Please include all pertinent medical information on the waiver, including prescription and nonprescription medications and allergies. If your child needs medications administered during the camp day, a medical information sheet must be completed. Medication(s) must be in their original prescription container with the child’s name printed on the label, and placed in a plastic bag. Medications are secured by our staff in the camp office. Parents can download the Medical Information Form from www.kroccenter. org/day-camp/ or request a copy from the Day Camp Manager. Parents may sign the bottom of the Medical Information Sheet to authorize campers to carry and administer his/her medications needed for life-threatening conditions such as an epinephrine injector (Epi-Pen) and asthma inhalers. Prior approval is needed for other medications to be carried by campers. PARENT GUIDE TO CAMPER SUCCESS The Parent Guide to Camper Success provides information on camp policies & procedures. Parents are responsible for reading the Parent Guide and for reviewing camp policies with their child. Topics include signing in & out, behavior management, staffing, program contact information, and registration. Parents may receive advance copies of the guide during registration at the Gymnasium Guest Service Desk or on the first day of camp. The Parent Guide can also be downloaded from www.kroccenter.org/day-camp/ REFUND POLICY & CANCELLATIONS No cash or credit card refunds are issued unless a Day Camp session is cancelled by the Kroc Center. Cancellation credits are no longer stored as in-house credits. Rather, cancellations credits will be issued via a Kroc Center Gift Card and distributed directly to the participant. Cancellation at least one week prior to the start of the camp session: full credit on a Kroc Center Gift Card toward Kroc Center programs minus the $25 non-refundable deposit fee ($10 for a single-day registration). Cancellation less than one week prior to the start of the camp session: full credit on your Kroc Center Gift Card toward Kroc Center programs minus a $10 administrative fee and the $25 non-refundable deposit ($10 for a single-day registration). Kroc Center Gift Card Credit Request applications are available at any venue Guest Service Desk and the Day Camp office.
