It is the policy of the Illinois Council of Health-System Pharmacists (accredited provider) to ensure balance, independence, objectivity, and scientific rigor in all of its education programs. Please note: PTCB requires CPhTs to earn only pharmacy technician-specific (‘T-specific’) credit to qualify for recertification. ALL CPE credit earned must be T-specific. In addition, PTCB has reduced the number of allowable CPE hours earned through in-service training from 10 to 5.

Credit will be reported to CPE Monitor. You will have until Monday, May 15, 2017 to complete your online evaluations and submit your request for CPE credit. DEVELOPING YOUR LEADERSHIP PIPELINE TO FEED INNOVATION At the end of this presentation, pharmacist participants should be able to: 1. Compare and contrast replacement planning, succession planning and succession management in building a leadership pipeline for a pharmacy service. 2. Explain the benefits and challenges of succession management in building leadership continuity for an organization. 3. Identify both internal and external resources for developing leadership skills in an organization. 4. Describe the leadership skills and strategies necessary for stimulating innovation. 5. Discuss the linkage between leadership and becoming an innovative organization. At the end of this presentation, technician participants should be able to: 1. Mr. Mike Powell is the Executive Director of Pharmaceutical and Nutrition Care and the Nebraska Poison Control Center at Nebraska Medicine, as well as an Associate Professor and Associate Dean for Hospital Affairs in the College of Pharmacy at The University of Nebraska Medical Center. He also chairs the pharmacy committees for the Nebraska Health Network and the Enhance Health Network. Mr. Powell’s primary interests are in evaluating the effectiveness of Pharmacy and Therapeutics Committees and clinical outcomes of direct care by pharmacists. He directs research at Nebraska Medicine to quantify outcomes of formulary management strategies and pharmacist practice in the healthcare delivery system. His primary teaching interests are in postgraduate training programs in ethics, health systems policy and institutional pharmacy practice. After receiving his Bachelor of Science in Pharmacy degree from Ohio State University College of Pharmacy in Columbus, Mr. Powell went on to earn his Master of Science degree in Institutional Pharmacy from the University of Maryland at Baltimore. He completed his residency in Institutional Pharmacy at the University of Maryland Hospital in Baltimore, and was a Fellow at the Leonard Davis Institute of the Wharton School of Business at the University of Pennsylvania in Philadelphia. He also completed a Health Care Advisory Board Leadership Executive Program as a Fellow in Washington, DC. He is licensed to practice pharmacy in Nebraska and Ohio. Mr. Powell is an active member of the American Society of Health-System Pharmacists and the Nebraska Pharmacists Association. He has been a reviewer for several journals, and has authored numerous articles published in the American Journal of Health-System Pharmacy, Hospital Pharmacy, Hospital Formulary, American Journal of Hospital Pharmacy, and Clinical Pharmacy, among others.

POSTER SESSIONS Instructions for Receiving Credit for Poster Sessions Poster Session I, Friday, March 31, 2017 at 5:30-6:30pm (1.0 credit hour) Poster Session II, Saturday, April 1, 2017 at 8:00 - 9:00am (1.0 credit hour) To take advantage of important research, ICHP and MSHP are excited to provide the opportunity for continuing pharmacy education credit during the poster presentation sessions at the ICHP/MSHP 2017 Spring Meeting. Each session is worth 1 credit hour. You must review and discuss 4 posters in a single session to receive one (1) credit for that session. Please NOTE: 4 posters in Session I = 1.0 Contact hour / 0.1 CEU, no partial credit is available. 4 posters in Session II = 1.0 Contact hour / 0.1 CEU, no partial credit is available. Poster titles, presenter(s) and date of presentation are listed on pages 23-25. Detailed Poster Abstracts are available online at: ichpnet.org/events/spring_meeting/2017/program_handouts.php

Instructions on how to obtain CPE credit for the Poster Sessions: 1. Identify four (4) posters in a single session that interest you and that will expand your education, knowledge and continuing professional development (CPD) at the ICHP/MSHP Spring Meeting. 2. Add the poster ID # and title to your Poster Session CPE Verification Forms on the gold paper provided in your meeting folder. 3. Visit the selected posters and discuss them with the authors/presenters. When reviewing a poster, be sure to evaluate the model used for the initiative, how the data collected was assessed and what outcomes were observed. Learners are encouraged to ask questions of the poster presenters. 4. Please request any financial disclosures from the poster presenters and indicate the response in the Financial Disclosure box on the Poster Session CPE Verification Form. 5. Ask the presenter (or co-presenter) to sign your form, once you have reviewed your selected poster. When you have all four signatures for the Session, please bring the form to the Validation Station at the Meeting Registration desk. 6. Meeting staff will review and keep your documentation for ACPE poster attendance records. 7. Meeting staff will provide the Access Code for the Session(s) you attended. Write the code(s) on your CPE Attendance Record Sheet in the appropriate place. 8. You will then complete your CPE process by turning in a copy of your CPE Attendance Record sheet at the end of your time at the meeting, going to CESally.com and evaluating all the CPE program(s), including the Poster Sessions. See Syllabus pages 26 and 27 for detailed instructions on completing your CPE evaluation process for credit. Please complete the CPE credit process by end of day, Monday, May 15, 2017. CPE CREDIT INSTRUCTIONS

DEADLINE: MONDAY MAY 15, 2017

Instructions for obtaining CPE credit for ICHP accredited courses Please NOTE:You MUST complete your evaluation submissions by end of day Monday, May 15, 2017. Please READ all information carefully so you understand the deadlines and how to access credit statements. After the meeting, all attendees will need to access ICHP’s online CPE system at CESally.com, and complete and submit all evaluations to have your credit automatically submitted to CPE Monitor. Please honor the deadlines! Do NOT Delay in completing your CPE processing. If you encounter problems, we will need time to assist you before the final deadline. To obtain CPE credit for each ICHP accredited course attended, attendees must complete the following requirements.

QUICK OVERVIEW 1. Fill out your CPE Attendance Record (2-part white and yellow form in folder) during sessions. 2. If you attended a Poster Session, you will need to turn in your signed poster session form to receive the access code for your CPE Attendance Record (see page 22 for details). 3. Turn in BOTH the White and Yellow copies at Registration check out for signature. 4. Registration attendant signs BOTH copies and keeps the White top copy. Please SAVE your yellow copy! 5. Go to CESally.com and Login if you have an account; or “Sign Up!” to create an account if new to the site. Important! You must enter your NABP eProfile ID & birthdate as MMDD to set up your account and get credit. 6. Use the Search function to find all the Program Titles you attended and Add to your To-do List. 7. Go to your To-do List to complete program evaluations and submit. Follow the directions. 8. Once completed, you will see a submission confirmation message. 9. Go to your CPE Monitor account at NABP.net to print off official transcripts or statements of credit.

DETAILED STEPS ON HOW TO COMPLETE THE EVALUATION PROCESS –

Session Access Codes will be announced by the moderators during each session – write the codes on your “CPE Attendance Record” sheet (2-part white and yellow form). Please fill in your “Name” information on your CPE Attendance Record. At the end of your participation at the conference, please turn in BOTH the white and yellow copies of your Attendance Record at the Registration desk. Registration attendants sign both copies and keep the white copy. You will get the yellow copy back. Keep the yellow copy of the CPE Attendance Record for your file as you will need the Session Codes which you wrote down to complete the process of receiving your credit.

DETAILED INSTRUCTIONS TO COMPLETE EVALUATIONS ONLINE:

1. Go to www.CESally.com and click on “Sign Up!” Or log in with your existing account. For HELP at any point, click on the HELP tab or go to: https://www.cesally.com/help/. 2. Complete the Sign Up process and select a username and password. Important: Please store your username and password in a safe place. You will need to maintain a valid email address with CESally.com. 3. Enter your NABP eProfile ID and birthdate as MMDD when prompted. CESally.com will confirm in real time your eProfile information with CPE Monitor and let you know if there is an error, which you may then correct.

4. Once you have an account, you may use the Search Box in the upper right corner to find your activity by typing in the title, or type in 2017 ICHP-MSHP Spring Meeting to see all the sessions. You have several options for completing or saving for later.

Please pay CLOSE attention to the Title and the Pharmacist or Technician label in each title.

PHARMACISTS: You may NOT receive credit for Technician programs. TECHNICIANS: As of January 1, 2015, PTCB requires Technicians use only Technician-specific CPE

credit to recertify.

5. Identify the program(s) attended and choose either to: a) Click on that Activity title to open the information page, and you will see your options in the right hand column on the information page.

b) OR Click on the checkbox inside the small information box, then go to the bottom of the page and see your options there. 6. Choose from either a) or b) above, to Complete Now, Save for Later, OR ADD to To-do List.

7. When you are ready to complete your CPE credit process, follow directions to EVALUATE the programs. • You will verify your attendance, provide the session ACCESS code that was given to you during the program, and complete an evaluation of the activity and the speaker(s). • POSTER SESSION Note: If you have comments for a particular poster presenter, please note the name and make your comments in the Feedback section of the Evaluation form. We will forward your feedback to the presenters and speakers. 8. Click on “Report CE”. Your CPE credit will be uploaded to CPE Monitor automatically. You will see a submission confirmation message when you have successfully completed the process. 9. If you have any questions contact us at members@ichpnet.org. 10. Go to www.NABP.net to download an official copy of your CPE transcripts. Click on the CPE Monitor link and log in to access your CPE Monitor records. Important: Per ACPE requirements, official transcripts and statements of credit may now be accessed ONLY through CPE Monitor. PLEASE Note: You will have until the end of day Monday, May 15, 2017 to complete your online evaluation and submit it for your CPE credit. ICHP policy requires that CPE evaluations be completed within a specific deadline to ensure timely submission of your CPE credit to your CPE Monitor account, and to meet the ACPE deadlines. Anything submitted after the ACPE deadlines will be rejected by CPE Monitor. If you miss the deadline you will lose your credit. Remember - if you experience any problems, we will need time to assist you. Please do not delay! Important Notice: Only registered attendees to the CPE presentations are eligible to receive credit. Meeting registration lists and CPE attendance records are used to verify that those who process credit on CESally.com attended the live programming.
