MESSAGE FROM THE CHAIR Welcome to the first edition of the University of Kentucky College of Medicine Department of Orthopaedic Surgery and Sports Medicine update. A lot has changed over the last five years, and we are glad to be able to communicate with our past and current residents, faculty, and all of those associated with our program about our progress and current status. UK Orthopaedics has developed a wide range of subspecialties that rank very highly in the region. The University of Kentucky has always been a destination center for orthopaedic trauma and complex orthopaedic problems. Still, in addition to this, we have also become the go-to source for all orthopaedic problems. What you will notice in the following pages is that we have faculty of diverse backgrounds and a full breadth of orthopaedic services. You will also see how our research has increased dramatically as we continue to solve problems for the Commonwealth and orthopaedics as a whole. We hope you will be proud of our orthopaedics department that you have been part of building, whether as a resident, faculty, patient, or prospective patient. We look forward to sharing our plans for expansion in the future.

OVERVIEW The UK College of Medicine Department of Orthopaedic Surgery and Sports Medicine includes world-renowned, board-certified orthopaedic surgeons, primary care sports medicine physicians, podiatrists, and other orthopaedic specialists who are consistently named to the Best Doctors in America list. Because our providers participate in clinical trials and research studies, patients can experience groundbreaking care options from leaders in the field of orthopaedics. We are proud to be the official team physicians for many of the schools and universities across the Commonwealth of Kentucky. Orthopaedic surgery and sports medicine involves the art and science of healing bones and joints and restoring human beings to a functional life.

The orthopaedist requires technical operative proficiency, a mastery of an ever-expanding body of knowledge, and clinical skills which all combine to creatively solve complex musculoskeletal issues. We have 40 board-certified orthopaedic surgeons specializing in other orthopaedic subspecialties including, hip and knee replacement, foot and ankle, hand surgery, pediatric orthopaedics, sports medicine, spine, and musculoskeletal cancer. Our team of experts also includes three board-certified sports medicine primary care physicians. Caring for patients in Kentucky is the reason we are here, and our faculty take pride in treating the next generation of orthopaedic surgeons.

MISSION Our group of highly trained subspecialists provides comprehensive orthopaedic coverage for the state of Kentucky and the surrounding region. We offer complete musculoskeletal care to all age groups, educate future surgeons, research new techniques, and provide community outreach. In association with Shriners Children’s Lexington, Kentucky Children’s Hospital (KCH) is ranked 42nd in the country for pediatric orthopaedic care. While they are separate entities, Shriners and KCH work closely together to provide seamless pediatric orthopaedic care.

HAND AND UPPER EXTREMITY The UK HealthCare Hand Center was designed to meet the needs of our patients. Our physicians are orthopaedic surgeons who have pursued additional subspecialty training at the fellowship level to provide the most up-to-date and comprehensive treatment of the whole upper extremity from acute injury to every day wear and tear. HIP AND JOINT RECONSTRUCTION The orthopaedic adult reconstruction staff is internationally recognized for expertise in the fields of adult hip and knee care. Continuously evolving techniques and innovations make these replacement surgeries uniquely beneficial to patients who suffer from arthritis and other painful hip and knee conditions. These procedures can alleviate problems caused by osteoarthritis, rheumatoid arthritis, fractures, congenital deformity, and other diseases which restrict an adult patient’s active lifestyle. To achieve optimal patient outcomes, a team approach is essential. UK orthopaedic surgeons, nursing staff and rehabilitation specialists work together in pre-operative planning and post-operative evaluation to assure the highest quality of care.

ORTHOPAEDIC ONCOLOGY Comprised of experts in the fields of medical and pediatric oncology, musculoskeletal radiology, surgical pathology, radiation oncology, and orthopaedic oncology, the UK Musculoskeletal Oncology team treats both pediatric and adult patients suffering from bone and soft tissue tumors along with tumor-like conditions that affect the musculoskeletal system. In addition to clinical care, this team of physicians and researchers is pursuing research in cancer immunotherapy, advanced tumor imaging, biomedical engineering of bone-like substances, and clinical trials - all dedicated to improved patient-centered cancer care.

FACULTY Patrick O’Donnell, MD, PhD, Director of the Sarcoma Program at Markey Cancer Center, Associate Professor Dr. O’Donnell specializes in orthopaedic oncology and advanced orthopaedic reconstruction. SPINE The UK HealthCare Orthopaedic Surgery Spine Center specializes in comprehensive care of the spine. Our physician staff is committed to providing the highest level of care to patients suffering from pain, numbness, or weakness anywhere from the neck to the lower back. Many of the procedures performed are outpatient procedures or allow the patient to go home the following morning. A facility planning process for a new site at UK HealthCare is underway to provide more comprehensive spine care in concert with UK neurosurgery, interventional pain, and physical medicine and rehabilitation providers.

FACULTY R. Carter Cassidy, MD, Professor Dr. Cassidy specializes in complex cervical spine (neck) surgery, spine deformities such as scoliosis and kyphosis, and spine trauma. TRAUMA The orthopaedic trauma service is an integral part of the multispecialty trauma services that provide a Level 1 Trauma Referral Center for central Kentucky. The orthopaedic trauma team provides comprehensive 24-hour service specializing in the patient with multiple system injuries. The service emphasizes early stabilization of fractures and aggressive rehabilitation of victims of major trauma. Specialized surgical techniques include closed intramedullary fixation, combinations of internal and external fixation, and complex fracture treatment of the pelvis, acetabulum, and extremities.

SPORTS MEDICINE Our sports medicine division is comprised of board-certified, fellowship-trained Orthopaedic Surgeons who are committed to providing the highest quality sports medicine care to all active individuals in Kentucky. Sports medicine is an ever-changing medical profession, which requires all professionals involved in the treatment and care of athletic injuries to stay attuned to the latest prevention and treatment methods. Our sports medicine physicians and certified athletic trainers have more experience and provide care to more athletes than any other group in this area. Our multidisciplinary team of sports medicine professionals provides injured athletes with a physician-directed program of treatment, rehabilitation, and conditioning. We offer the state’s largest group of fellowship-trained sports medicine physicians. The CME-accredited course was designed to deepen attendees’ understanding of the mechanisms and treatments of knee injuries. Under the leadership of course director and assistant professor Austin Stone, MD, PhD, a distinguished panel of speakers discussed advanced surgical techniques, moderated live surgeries, highlighted hands-on interactive track and laboratory time, and debated some of today’s pressing topics in the treatment of sports-related injuries. Our esteemed faculty include orthopaedic sports medicine and shoulder surgeons, primary care sports medicine physicians, physical therapists, and athletic trainers. Discussions will focus on initial management, advanced surgical techniques and reconstructions, and optimal strategies to return patients to high performance. The course is expanded this year to include medical sports injuries and both operative and nonoperative upper extremity pathologies. ATHLETIC TRAINING PROGRAM We employ the region’s largest certified athletic training staff. Three of our athletic trainers have won Athletic Trainer of the Year in the State or District, and one of our athletic trainers is in the Kentucky Athletic Trainers Hall of Fame. Our certified athletic trainers are specially trained in the prevention, assessment, treatment, and rehabilitation of all types of sports-related or physically active injuries and illnesses. Prevention, treatment, and rehabilitation programs are developed based on medical, exercise, and sports sciences. They provide athletic training services at schools, clubs, tournaments, special sporting events, and all Kentucky High School Athletics Association (KHSAA) state events. Our athletic trainers are also involved in educational events, including lectures, community seminars, or health fairs. Our certified athletic trainers are directly involved with patient care, providing patients with education on injuries, treatment and rehabilitation protocols, pre- and post-operative care, return-to-play guidelines, and the fitting of braces. Our mission is to educate each student-athlete on their injury and give them a specific, comprehensive treatment plan so they can attain their return-to-activity goals.

Examining the Importance of Athletic Trainers

After the physician and certified athletic trainer have explained the treatment plan to athletes and parents seen in the clinic, the certified athletic trainer will call that player’s coach and/ or certified athletic trainer to inform them of any restrictions or return-to-play guidelines. Communication is vital for an athlete to return to sports in a safe and efficient manner. The certified athletic trainers use this same philosophy at their contracted high schools when evaluating and treating athletic injuries. There is constant communication between the physician, certified athletic trainer, parents, and coaches regarding all injuries.

SPORTS REHBILITATION/ PHYSICAL THERAPY UK Sports Rehabilitation is committed to the pillars of academic health care through research, education, and clinical care. Dedicated to the health and recovery of athletes, we will provide the most advanced patient care. We will strengthen and continuously work to improve the delivery of sports rehabilitation to the patient through our partnering with the UK HealthCare Sports Medicine team. We will support the UK College of Medicine’s education and research needs by offering cutting-edge services on par with the nation’s best providers.

LEXINGTON VA HEALTH CARE SYSTEM UK HealthCare and the Lexington VA Health Care System share a long-standing relationship. As part of this collaboration, select UKHC Orthopaedic surgeons, including adjunct faculty, provide optimal patient care for the Veterans in Kentucky. In addition, Orthopaedic Surgery Residents from UK HealthCare dedicate 10 weeks of their third and fifth years to rotations at the VA Hospital. RESEARCH Researchers in the UK College of Medicine Department of Orthopaedic Surgery and Sports Medicine have a shared mission to improve outcomes after musculoskeletal injury. Our clinicians and scientists have expertise in three key areas: • Surgical techniques to reduce post-operative complication/re-injury and improve patient outcomes. • Treatment options to potentially reduce recovery time while also decreasing the risk of re-injury. • Mechanisms and treatment options for post-traumatic osteoarthritis to ensure long-term joint health after sports-related injury. We collaborate with experts in rehabilitation science, advanced imaging, muscle physiology, nutrition, and pharmacology, among others. This teamwork allows us to study musculoskeletal injury from molecular, joint, and whole-person perspectives to improve health-related quality of life within the Commonwealth and beyond.

HIP/JOINT RESEARCH In joint reconstruction, researchers are involved in clinical research investigating implant design, surgical techniques, and treatment options for patients undergoing complex hip and knee arthroplasty. Additionally, researchers have collaborated with investigators with expertise in imaging and biomarkers to investigate underlying biochemical profiles in patients undergoing reconstructive procedures. Researchers have strong partnerships with Dr. Michael Samaan and Dr. Bruno Menarim.

SPORTS MEDICINE RESEARCH In sports medicine, researchers are involved in clinical research projects investigating underlying mechanics and treatment options for post-traumatic osteoarthritis, and surgical indications to optimize treatment following various sport-related injuries. Additionally, researchers collaborate with investigators with expertise in biomechanics and muscle biology to describe muscular and functional changes after injury and surgery. Researchers have strong partnerships with Dr. Brian Noehren, Dr. Chris Fry, and the Sports Medicine Research Institute.

TRAUMA RESEARCH In trauma, researchers are involved in clinical research investigating infection prevention and treatment, non-opioid pain control, and treatments to optimize patient’s return to function. EDUCATION Our department is committed to training the next generation of orthopaedic surgeons and sports medicine providers. What started in 1997 as 10 faculty and three residents per year has grown to over 40 full and part-time faculty, five residents per year, six orthopaedic surgical fellows and four non-operative sports medicine fellows. With the recruitment of top-notch faculty, we are now providing specialty care in the areas of foot and ankle, general orthopaedics, joint reconstruction, orthopaedic oncology, pediatric orthopaedics, spine, sports medicine, trauma, and hand and upper extremity.

Residents in years 2 through 5 follow a program that repeats the complete lineup of orthopaedic subspecialties as junior and senior resident. These rotations include orthopaedic trauma, hand, sports medicine, foot/ankle, spine surgery, hip/knee reconstruction, shoulder/elbow, and pediatric orthopaedics.

This recruitment not only provides the citizens of Kentucky and surrounding states with exceptional care but also enables us to provide the education and training necessary for graduating outstanding orthopaedic residents along with sports medicine, trauma, and joint reconstruction fellows.

In addition to the clinical rotations, numerous additional learning experiences support the education of the UK orthopaedics resident. The department has vastly increased its research capacity, grants, and productivity, with the residents participating to a significant extent in this growth. Additionally, there are opportunities for education and leadership development through various conferences and growth in clinical skills by serving as team physicians for area sports teams.

The UK College of Medicine’s orthopaedic surgery residency program desires to prepare surgeons with the character, intellect, and surgical discipline required to become outstanding clinicians and leaders in patient care. The program’s strength is the dynamic group of individuals that form the resident team. Their comprehensive education occurs through surgical training in all the orthopaedic surgical sub-specialties, didactic and simulation, and advancing the field through research. The residents experience their surgical training through a local, diverse set of hospitals on UK’s campus and within the city of Lexington: University of Kentucky Chandler Medical Center, UK Good Samaritan Hospital, Kentucky Children’s Hospital, Veteran’s Administration Hospital, Lexington Shriners Hospital for Children, and two local private hospitals. ORTHOPAEDIC FELLOWSHIPS HIP AND KNEE

TRAUMA

The UK HealthCare is the main tertiary referral center of central and eastern Kentucky for joint reconstruction. This large referral network allows fellows to participate in primary to complex joint reconstructions as well as a large revision arthroplasty practice. The fellow will get exposure to direct anterior and miniposterior approaches to the hip in addition to unicompartmental, bicruciate retaining, and routine total knee arthroplasty. Hip preservation will serve as a third of the fellowship year with exposure to hip arthroscopy, pelvic and femoral osteotomies, and congenital deformities. All patient care will be performed under the guidance of experienced, full-time academic faculty interested in joint reconstruction.

UK Albert B. Chandler Hospital is the flagship provider of multidisciplinary trauma care for the Commonwealth of Kentucky. A large referral network includes the central and eastern Kentucky regions, as well as outof-state referrals. Fellowship participants will be exposed to all aspects of orthopaedic trauma care with an emphasis on complex fractures and multiply-injured patients. All definitive trauma management is provided by the orthopaedic trauma faculty. All patient care will be performed under the guidance of experienced, full-time academic faculty interested in trauma care of the highest quality.

SPORTS MEDICINE The University of Kentucky Sports Medicine Fellowship provides a well-rounded look at the world of sports medicine, including sideline team coverage, training room evaluation, and operative and non-operative treatment of common musculoskeletal problems that we encounter in a sports medicine practice. The sports medicine fellowship provides an opportunity for the fellows to participate in the direct care of University of Kentucky athletes as well as other professional, collegiate, and high school athletes in central Kentucky.

EXPANDING OUR EFFORTS TO ADDRESS THE NEEDS OF KENTUCKY Over the past year, we have expanded our clinical footprint by creating a joint reconstruction presence in Georgetown, Ky., offering a weekly outpatient clinic within our established family medicine practice. We also began offering a five-day-per-week general orthopaedics walk-in clinic at UK HealthCare Turfland. Additionally, four new athletic training contracts were added to our welldeveloped athletic training program, as well as significant collaboration with the Lexington Sporting Club, making UK Sports Medicine the official health care provider for the Lexingtonbased soccer club.

LOOKING FORWARD WITH A PURPOSE

The new UK Markey Cancer Center will be the home for the UK Complex Spine Institute. This will be a joint effort with Orthopaedic Surgery and Neurosurgical spine services and will include interventional pain, physical medicine & rehab and related therapies.

Anticipated to open in 2027 a new outpatient cancer treatment center and advanced ambulatory complex. The new complex will be the future home of the UK Markey Cancer Center in addition to other ambulatory services. These include but are not limited to, minor procedures, endoscopy, diagnostics and imaging services, pharmacy, retail, outpatient clinics, meeting spaces, and necessary support spaces. Also included is a 2,131-space parking structure and the installation of new roadways and utility infrastructure to support the new complex. The new UK Markey Cancer Center will be the home for the UK Complex Spine Institute. This will be a joint effort with Orthopaedic Surgery

In 2029, UK HealthCare will cease hospital operations at Good Samaritan. Inpatient services provided at Good Samaritan will be relocated to the Chandler Hospital campus, where a new patient tower is planned to replace the Good Samaritan beds and add 300 more.

Looking ahead, the UK Board of Trustees endorsed plans to continue to establish new smaller ambulatory sites, specifically to serve UK’s families closer to their homes as well as to address critical needs for medically underserved populations in Fayette County. These sites will include orthopaedic surgery and sports medicine clinical services within Fayette County and surrounding areas.
