This one’s for the boys...

A
s a woman writing the publisher's letter for the “Men’s Edition,” I find myself not quite sure where to start. I’ve considered myself one of the guys in many circumstances. I was a tomboy as a child—one of the first picked, along with the boys, as one of the best kickball players on the playground. I was the only female with the on-air staff at several radio stations where I worked as a young adult. I could hang with the guys. Ultimately however, I’ve always embraced my womanhood and had a certain respect for things distinctly masculine. Maybe I felt a certain envy of my male counterparts' freedom to be and do the things that back in my day were considered inappropriate for females. Men – the empowered! As fate would have it, I had only one child, my son Jason. He is the light of my world. This somehow seems appropriate as my ability to embrace all things girlie has never come easily. Jason’s first obsession was with big trucks. He was fascinated with HeMan and the Masters of the Universe, the Ninja Turtles and later Mortal Combat. He would play outside until he was pure dirt, then run inside to scream, “Mom! Love you!” then run back to rejoin his friends. He was...all boy. I went to that boy’s wedding last month. He will now have a family and will be the man. This made me ponder the qualities that I wish for my son that I know will serve him and his family. A man should respond. It takes discipline to filter oneself. Boys react, men respond. Men have a cause. Jason doesn’t have to end world hunger, but he can fight to be a better version of himself. Men take action while boys complain. And finally, my son Jason, men, and all humans should be free to be unashamedly themselves! Please enjoy this edition dedicated to men. You’ll meet several that are making a difference and taking action to change the fields in which they play.

TWILIGHT CRITERIUM COMING TO BOISE JULY 11TH Inaugurated in 1987, the Downtown Boise Twilight Criterium is a fast-paced, high stakes cycling race in the heart of downtown Boise. Fascinated by an anatomy and physiology course in seventh grade, Andrea Axtell, M.D., decided she wanted to become a doctor. As an internist practicing in a traditional outpatient clinic, Axtell experienced first hand the inefficiencies and excessive costs of our healthcare system and decided to do something about it. She opened Alliance Concierge Care on State Street in Boise last July. “My experience with the traditional healthcare system drove me to find a more patient-centered practice, which I found in direct primary care,” states Axtell. “Direct primary care (DPC) is a new concept for Idaho, and my biggest challenge is educating individuals on how DPC can work with insurance to save individuals time and money.” A physician for eight years, Axtell is board certified in internal medicine and offers primary care services to patients 18 and older. She wants to restore the time honored patient-physician relationship, without the hassles or influences from insurance companies and health care systems. She provides an alternative to patients with personalized medical treatment plans. “As members of Alliance, patients can still use their insurance to cover medications, tests and imaging,” Axtell points out. “It is recommended that patients have medical insurance to cover catastrophic events, such as hospitalizations. “Alliance Concierge Care offers services to all individuals, with or without insurance,” explains Axtell. “Individuals may submit paperwork to their insurance companies for reimbursement. They may also cover some services under flexible spending and health savings accounts.” “Greater availability to your physician has been shown to decrease visits to specialists, urgent care centers, and the emergency room, thereby decreasing overall healthcare costs,” notes Axtell. When was the last time you had a house call from a doctor? If you need it, it’s available, whether you are a member or a pay-asyou-go patient. What about un-rushed visits with your doctor? “I typically spend an hour with my patients, while the average

visit time is less than eight minutes," Axtell says. "I am able to spend more time with my patients because I do not follow the fee-for-service payment plans offered by insurance companies. under the fee-for-service model, doctors are paid based on the number of patients they see in one day—the more patients they see, the more they get paid. Since insurance companies are lowering reimbursements to physicians, the response is to increase the number of visits per day to maintain profits. Another reason I can spend more time with patients is that I am limiting the number of petients that I take care of. This allows for greater access, including same day appointments." Membership for ages 18-64 is $49/month or $59 a month for those 65 and older. Axtell provides health care to her patients with the emphasis on care. Patients are guaranteed same day appointments for urgent issues and 24/7 access to their doctor’s cell phone. “Ideal patients are those individuals who are interested in establishing a meaningful, long-term relationship with their physician,” shares Axtell. They say the key to any successful project starts with the prep work, and the same is true with a bathroom remodel project. A bathroom remodel is no place for improvisation, which typically leads to costly overruns and poor choices you may later regret. Before ripping out the first tile, think hard about how you will use the space, what materials and fixtures you want, and how much you’re willing to spend. The National Kitchen and Bath Association (NKBA) recommends spending up to six months researching, evaluating, and planning before beginning work. Create a roadmap that will guide decisions, even the ones made under remodeling stress. Once work has begun — a process that averages two to three weeks — resist changing your mind. Work stoppages and alterations add costs. Some contractors include clauses in their contracts that specify premium prices for changing original plans. If planning isn’t your strong suit, hire a designer. In addition to adding style and efficiency, a professional designer makes sure contractors and installers are scheduled in an orderly fashion. A professional designer charges $100 to $200 per hour, and spends 10 to 30 hours on a bathroom project.

Establish the scope of work you want accomplished down to the exact details of what materials you want by manufacturer and model number. Leave no margin for interpretation or error. Better yet, if the contractor will allow you to go to the fixture showrooms and order the materials under his account, then do it. Validate each new material selection with the contractor in the context of your budget. Clearly establish a budget or a not-to-exceed number, and share it with your contractor. They will provide you with construction cost information that you will need to understand how to develop your budget. Do not let daydreaming or well-meaning advisers—that know nothing about the budget— sway you. W
hen Nick Schlekeway graduated from Boise State in 2007, he took with him way more than just the knowledge gained from a degree in business management. As a three-year starting defensive end and team captain for the Boise State Broncos—part of their first Fiesta Bowl win—he also picked up some serious leadership and motivational skills. “Being involved on the leadership side of a program like that was an invaluable learning experience in terms of motivating and directing people,” says Schlekeway. “I came to understand it’s not as much about the playbook and the schemes as it is about getting the players to buy in to what you’re doing. If you can get that you’re going to win football games. And it’s the same when it comes to attracting and retaining top talent in business.” Of course, it didn’t hurt that he was an Academic All-American who graduated magna cum laude with a 3.8 GPA. After spending four years as a City of Boise firefighter, Nick changed course to 18

real estate. A short time later he became the designated broker at Boise Premier, where he met Steve Caporale. It didn’t take long for the two to realize they had very similar ideas about what they thought a brokerage firm should provide its clients, and a few months later they left to form their own company. “Steve and I immediately recognized the shared vision of a firm that was focused on quality representation and excellence in marketing,” explains Schlekeway, “But beyond that, we also wanted to be involved in local charities and have associates who very much wanted to give back to the communities where they live and sell.” During his days as a Bronco, Nick was introduced to that world through the team’s involvement with the Make-A-Wish Foundation. He was particularly impressed with the bravery he saw in two young cancer survivors, a boy named Amherst and a girl named Madison. When Nick and Steve were brainstorming on a name for their new company late one night that

story came up, and they decided instantly that Amherst Madison was the perfect fit. “It’s one thing to say that you want to give back to your community, but we like to think it’s ingrained in our business model,” says Schlekeway, “Even to the degree that it’s part of our name.” Call it good recruiting or just plain community spirit, but every one of the agents now at Amherst Madison have bought in to that idea. “It’s not required, it’s just really strongly emphasized," Schlekeway says. "We make it very clear to people that it’s part of our company culture and that it’s very important to us. And because of that we tend to attract people who care about it.” They demonstrate their dedication to this company tenet primarily through an organization called The Black Door. Started in 2014 by founding partner Nikele Wood, this non-profit organization works with multiple charities, including Full Circle Exchange, Make-A-Wish, and local refugee groups, to facilitate events and fundraising activities.
Last year, over a dozen Amherst Madison agents volunteered to help out with a Thanksgiving dinner The Black Door put on for 75 refugees. “The fact that The Black Door supports multiple organizations gives us the ability to do that as well,” Schlekeway says. “And they also provide for anonymous giving, which is becoming more and more important to donors these days.” Both The Black Door and Amherst Madison have deep roots in Idaho and are looking to sink those roots deeper in the Boise area. Amherst Madison just celebrated the opening of their new Boise office, a 3,000-square-foot building at Emerald and Garden streets that’s sporting a new, half-million dollar facelift. “Because of our name and our image, a lot of people assume we’re not from here, but we’re a locally owned, locally managed company,” Schlekeway says with pride. “I grew up here, went to Eagle High School, played for Boise State, was a Boise City firefighter. In less than two years, Amherst Madison has grown to 40 agents in two offices representing some $40 million in real estate. All this didn’t just drop in their laps. They spent a full year in development, making sure their transaction and marketing models were set up just right before they began recruiting. “We learned our lessons working with other brokers who didn’t do that, and we didn’t want to be in a position where we couldn’t grow and expand if and when we wanted," Schlekeway says. "We had opportunities to expand outside Idaho very early on, but we really wanted to focus on Boise and the needs of our local community. It was vital to us that we build a model that could be scaled, but in a sensible and targeted manner.” One of the unique things about Amherst Madison’s business model is their two-tiered structure. Agents start out as associates. Then, based on time and experience, they have the chance to move up to partner and, if they continue their development, to senior partner. These designations carry with them a great amount of pride and subsequent reward, as well as real opportunities for career advancement. “Brokerages who don’t offer those kinds of opportunities often have a difficult time retaining talent,” Schlekeway says. “People hit these ceilings and they can get stagnant. If you have talented people, you have to give them someplace to go, a way to get beyond those ceilings, or they’re going to go away. If an item could tell it’s own history what could one discover? Linda Borup, owner of Linda’s Loft, loves learning the history behind the pieces she stocks in her store. THE HOPELESS CHEST
The year is 1939. A young woman purchases a hope chest with a bright eye turned toward the future. She fills the chest with embroidered pillow cases, carefully pressed tablecloths, perhaps even some baby shoes. Then World War II breaks out. A generation of young men lost too soon. The young woman’s hopes dim. The years pass while the hope chest gathers dust in a corner. Finally as an old woman, who has never married, she gives the chest away. No longer is it a hope chest. As she gives the chest away, she calls it her “hopeless chest.” THE OLD STOVE

The Basque only use 7 humble seasonings. Olive oil, garlic, salt, saffron, lemon, and paprika. This selection of spices is traditional to take really good food and enhance it, not change it. What is produced is very savory food flavors. One misnomer is everyone associates lamb with Basque cuisine. That is really an American Basque cuisine evolved from the adaptation of what was available to cook when settling in Idaho. The flavors that were used with the plethora of seafood from the Bay of Biscay were now adapted to lamb, beef, chicken, and pork. So although lamb is available on the menu in many recipes, utilizing fresh fish brings back true Basque authenticity. Always offered are halibut, shrimp, cod, and ink fish (squid with black ink sauce). But one should inquire about what is the evening seafood special. All meals are served in traditional courses with a choice of Basque Red Bean soup or their daily rotating soup, salad with Epi’s vinaigrette, the entrée with two sides, and bread. Is Your Heart Telling You Something? HEART SKIPPING A BEAT? IT COULD BE MORE THAN JUST A COINCIDENCE. ARTICLE MARGOT VLOKA, MD | PHOTOGRAPHY PROVIDED
A
ll too often, I see patients in my office who say they wish they had "come in earlier" or "paid more attention" to their irregular heartbeat. Atrial Fibrillation, also known as A-Fib, is the most common form of heart rhythm disturbance. Currently, it affects 6-7 million Americans nationwide and more than 10,000 people in the Treasure Valley alone. A lot of my patients with A-Fib first experience a rapid and chaotic heartbeat, shortness of breath, dizziness, chest discomfort, fatigue and difficulty in performing vigorous exercise or just maintaining daily activities. On the other hand, some patients have minimal or no symptoms at all; their atrial fibrillation is often only detected during their visit to the doctor's office. Unfortunately, too many people wait too long to address these symptoms. They don't realize that an irregular heartbeat doesn't just mean that they are out of shape, anxious or stressed out. An irregular heartbeat due to atrial fibrillation is associated with a two-fold increase in mortality– stemming from clots in the heart which may migrate to the brain and cause a stroke, as well as increased risk of developing heart failure. Not surprisingly, some factors that increase the risk of coronary artery disease also increase the risk of developing atrial fibrillation– including high blood pressure, diabetes, high cholesterol, obesity, high salt intake, excessive consumption of alcohol and caffeine, and others. Snoring and irregular breathing when asleep may also contribute to development of atrial fibrillation, if it's caused by ob-

structive sleep apnea. Approximately one in four patients over the age of 45 will develop atrial fibrillation over their lifetime. There are several treatment options for A-Fib, ranging from medication to surgery. One frequently selected treatment is a procedure called ablation, which can eliminate the triggers of atrial fibrillation. The procedure's success depends on several factors, including how long the patient has had A-Fib. Generally speaking, the earlier we can diagnose A-Fib, the better our chances are of treating it successfully. If you're experiencing an irregular heartbeat, it could be more than a minor problem. Atrial fibrillation is one of the leading causes for stroke and its diagnosis is associated with increased risk of dying, especially if treatment is not instituted in timely fashion. We can help. o who actually read the waiver before they signed it?” the instructor asked. Sheepish grins crossed my entire class of some 10 people. “Exactly,” he continued, “because we know if you had, not one of you would still be here.” The group erupted in laughter. “Now we are going to watch a video explaining exactly what the document did say.” Welcome to skydiving. Ever since I was a child jumping down stairwells onto carpets with my best friend wearing red capes pretending we were Superman, I've wanted to fly. More than once, I’ve dreamed of soaring like a bird above the earth. Over the years, I’ve been hang gliding, bungee jumping and even flew through the jungles of Costa Rica on canopies like Tarzan. But through it all–nothing has compared to jumping out of an airplane 14,000 feet high, free falling for what seemed like eternity, and relying on nothing more than a bunch of vinyl to ensure my safety to reality. The year was 1996. I was a mere 27 years old. Having just finalized a divorce, I was looking to experience all life had to offer to do something crazy, insane and even dangerous. My destination was Hinckley, Illinois- about three hours outside Chicago. I chose to skydive tandem, which meant I would be attached to clips which would be attached to an expert parachutist. It would be the expert’s parachute that would save us from certain doom. By going tandem, I would now have to “free fall” for two minutes. If I went solo, a mechanism on the airplane would release my chord the moment I left the airplane. Free fall baby! 34

Next, I signed numerous waiver forms. I actually did start to read one, but after the first mention of “death,” I stopped reading and just signed. I responded several times to workers’ questions with, “Whatever.” Then came the video. A religious looking man in black attire with a long white beard spent 15 minutes saying something to the effect of: “What you are doing is very dangerous. You could really, really die. You could really, really be permanently paralyzed. You could have brain damage. But if you do, you won’t sue. If you can’t walk, you won’t sue. If you die and can’t walk, no relatives will sue…..Have a nice day.” After the completely ignored message and a quick lesson in terms of what would actually happen, I dressed in what looked like an orange prison jump suit with a helmet and goggles (like that would save me). Then my guide, Bill, clipped me to his gear. We boarded the airplane. Before I knew it, I leaped to my death–I mean jumped out of the airplane. I was flying! I beamed. “Fun, huh?” Bill, said as we hurled more than 100 mph through the sky. “Okay, pull the chord.” I panicked. I couldn’t find it. “Pull it!” I still struggled. “If you don’t pull it in the next second, I have to,” he said, with worry now. Success. After we catapulted another 100 feet up, we drifted for another five minutes. The sky never looked so blue. The sun never looked so yellow. Life has never been the same.
