LUNG CANCER Screening program Saves Lives EARLY DETECTION IS KEY ARTICLE MELISSA ROONEY, MD PHOTOGRAPHY PROVIDED

It

has now been 50 years since the surgeon general’s report on tobacco, describing the causal role between cig-

arette smoking and lung cancer. Surprisingly, many people -- smokers and non-smokers alike -- do not know that lung cancer is the leading cause of cancer death for both men and women. More surprisingly, many people do not realize lung cancer remains the number one cause of cancer related death in the United States in women ‒ more than breast, ovarian and uterine cancer combined. It is estimated that in 2015, more than 221,200 new cases of lung cancer will be diagnosed with an estimated 158,040 deaths. The high death rate associated with lung cancer can in part be attributed to the fact that it is often found after it has spread. Lung cancer can be caused by any number of factors: smoking (including second-hand smoke), genetics, radon exposure, occupational hazards, pre-existing lung disease and air pollution. Cancer screening can save lives by identifying lung cancer at early stages when treatments work best. The single best way to prevent lung cancer deaths, in addition to the myriad of other health benefits, is smoking cessation. A large multi-center study published in the New England Journal of Medicine (NEJM) validated lung cancer screenings for high risk individuals with use of annual low-dose computed tomography (LDCT). In this study, lung cancer screening decreased the risk of lung cancer death by 20%. LDCT is a non-invasive test that is able to produce three-dimensional images of the lungs, allowing for detection of lung abnormalities. LDCT scans allow expert clinicians to examine the shape and size of suspected lung nodules and determine

their likelihood of being cancerous. The dose of radiation is significantly lower than standard doses of a CT scan. Given the encouraging findings of recent lung cancer screening studies, specialty physician and medical organizations (American College of Chest Physicians, American Society of Clinical Oncology, the National Comprehensive Cancer Network and the US Preventative Task Force) are now recommending lung cancer screening for high risk individuals.

Criteria for screenings of high‑risk individuals include: • Current or former smokers 55 or older • A smoking history of at least 30 pack‑years (meaning one pack a day for 30 years; 2 packs a day for 15 years; etc.) • If a former smoker, stopped within the last 15 years • No current or past treatments for lung cancer

Patients will receive a letter with the results of their screening exam. If the scan shows one or more nodules, they will be referred for further testing and consultation. A nurse navigator is available to assist in coordinating care between patients and their doctors. Medicare and most insurance plans cover lung cancer screening for patients who meet the criteria.

Sights& Sounds of Switzerland A Country of Beautiful Contrasts

R

ecently, I had the opportunity to take in the sights and sounds of Switzerland; it is a country of beautiful contrasts -- high stone

mountains, clear lakes and waterways, medieval castles and churches, modern city landscapes and vineyards dating to the 11th century. Traveling exclusively by trains, boats and my own two feet, I traversed the French-speaking cities of Geneva, Morges, Lausanne, St-Saphorin and Villeneuve on Lac Léman (Lake Geneva), the Swiss-German-speaking Lucerne and Mount Pilatus, in central Switzerland. I estimate that I walked approximately 75 miles during the trip, helping to burn off the calories I consumed during fantastic two-hour meals, each ending with my favorite dessert-- crème brûlée. After boarding the train in Geneva I traveled to Lausanne, my home for two weeks. Lausanne is home to the Olympic Committee and is on the northern, Swiss shore of Lac Léman. It is also home to Cafe de Grancy, a high-quality, neighborhood restaurant and one of the city's only restaurants open on Sunday nights. Train travel is efficient and is always punctual; as my time in Switzerland extended, I was known to make a quick dash for the train in the moments before leaving the station. The city of Morges, a 10-minute train ride west from Lausanne, was my working base during the trip and has a

quaint port filled with active sailboats. On land, pedestrians enjoy lakeside flowers, walkways and art. On the best weather days, you can look across the lake and see Europe's largest mountain, the towering and snow-capped Mont Blanc, in the distance. The contrast of sailboats and snow is an incredible sight to see. I traveled east from Lausanne to the village of Villeneuve to visit Chillon Castle, the inspiration for Lord Byron's The Prisoner of Chillon. On my way back to Lausanne, I stopped at St-Saphorin, a 375-person village in the UNESCO World Heritage site of the Lavaux Vineyard Terraces. The terraces were established in the 11th century and they contain an elaborate system of water and erosion control to produce grapes while protecting the landscape. I walked through the quaint town on cobblestone streets, upwards to the terraces that overlooked the village and the lake. It was quiet and the air had a temperate

and hospitable quality, perfect for nurturing grapes. One morning, I took a two-hour train ride northeast to Lucerne-- a popular tourist destination on Lake Lucerne, filled with shops and restaurants surrounded by towering stone mountains, one of them Mount Pilatus. I took two gondola rides to the summit, where I was greeted by breathtaking scenery and alphorn musicians playing traditional music. I could also hear cowbells very clearly and I walked a few hundred meters, down from the summit, to visit a small herd of friendly cows, grazing on patches of grass in the rocky landscape. To descend, I took the world's steepest rack railway down to the lake; to complete the popular "Golden Round Trip," I took a boat back to Lucerne and the Chapel Bridge.

Before traveling back home, I spent a day in Geneva-- where my trip to Switzerland had started. Though the famous Geneva Water Fountain was not operating while I was there, I did walk through Geneva's version of Rodeo Drive (rue du Rhône) and experienced Geneva Old Town, Switzerland's largest historical site. St. Peter's Cathedral dominates the landscape and Switzerland's mastery of watch making is evident at every turn. Geneva is also home to the second largest United Nations office and the Allée des Nations, a lawn comprised of all 193 member nations' flags. Switzerland is truly one of the most gorgeous places I have visited; it is clean, peaceful and chock full of amazing sights and unique sounds that are etched in my memory forever.

A

fter a decade in the St. Louis real estate investing and brokerage business, I find the market to be quite interesting and to have some definite idiosyncrasies. I have a slightly different view than many other investors might; I have had the opportunity to work a wide range of markets: buying investment properties to rehab and sell, listing and selling homes in the million dollar range and owning rental properties in the St. Louis area. I own more than 40 rental homes in the area; my experiences as a landlord allow me to share some words of wisdom with you. CHOOSE LOCATION WISELY

When you see high “cash on cash” returns on a rental property in economically obsolescent areas, understand the amount of work that comes with obtaining those returns. Economically obsolescent areas require a manager who understands the risks that inherently come with a property in an area where the socioeconomics are challenging-- not to mention the high police presence in many of these areas. My point is, it’s not all about the high returns; sometimes it’s better to take a lower return and get more sleep at night. You should always invest in areas you feel comfortable visiting in the middle of the day. HAVE A PLAN

Normally, nothing comes easy. Real estate investing looks great on television; the reality is when you get toward the middle of the month and your rent check has not come in yet and

the tenant won’t answer your calls-- it’s not a good feeling. This is why you need to have a plan in place before these situations arise. I always try to work with my tenants, reach out to them and communicate. If that communication is not going anywhere you should call an eviction attorney right away. Every new landlord I have worked with tries to handle it themselves and by the second problem tenant scenario they call that eviction attorney immediately-- unless of course the tenant is openly communicating with them and is willing to work through the challenge in terms of a repayment plan, for instance. MAINTAIN RELATIONSHIPS

It’s not all about you. Truly successful landlords engage their tenants and check in with them once every month or two, at least. Check in with them and let them know you are still around and that you care about their residential experience; ask them how everything is going. Don’t fix frivolous things for overly demanding tenants; if there are real issues, you want to make sure they are being addressed promptly. You will be surprised how much this time and attention will improve your profitability and overall happiness with your business model. your neck.” So, when I entered this dusty deathtrap (with my Dad’s supervision, of course, despite the years that had passed since my childhood), I tried to take in an attic full of items I had never seen before. Some things immediately caught my eye. Two old steamer trunks,

each with faded canvas and hardened leather on the outside and original peeling paper lining inside. As I painstakingly restored the trunks, it unleashed an avalanche of questions. To whom did they belong? What possessions were carried in them? How did the trunks' owners feel leaving family behind they knew they would most likely never see again? What hopes for the future did they hold for a land they had never traveled to before? Of course, this is not the type of travel many of us enjoy or dream about today. Travel for pleasure was a luxury reserved for a select, wealthy few. Real travel was usually a once in a lifetime experience for the purpose of moving to a New World destination. Reasons for leaving native lands varied, but were remarkably similar to reasons people leave today—crowded cities, war-torn regions or the hope of turning economic hardships into opportunity. The trunks, it turns out, came from Germany with my great-greatgreat-grandmother in 1850. These trunks had crossed an ocean before arriving in New Orleans, a major port of entry for many immigrants; most didn’t stop at their point of arrival in the United States. These trunks continued with my family up the Missouri River to the growing community of Chesterfield, one of many like it emerging throughout the country. A great number of immigrants as well as citizens living in the East ventured even further west. In the “Wild West” or along one of the many westward migration paths, such as the Oregon Trail, travel could be even more dangerous and isolated. Similar to vacationers today, many travelers overstuffed their trunks and wagons, although probably more from a fear of being caught in a truly desolate land with no nearby stores for hundreds of miles rather than a trivial fear of being caught without the right shoes, ten extra shirts or an umbrella in case of a few drops of rain. So, it stands to reason that folks would bring anything with them they thought they may need and of course, they weren’t planning on returning anytime in the future either. However, if you were depending on horses, mules or oxen

to pull all of your worldly belongings, as well as the weight of your family, some items might prove impractical. In fact, the Oregon Trail is famously known for being strewn with furniture, hardware and other personal goods that homesteaders had packed for the journey, but discarded when the items became too big a burden. So, what objects did these voyagers choose to pack? They usually brought items important to their lives or vital to their existence.

ome travelers are hesitant to leave their beloved four-legged family members behind when they go on vacation; now, it’s increasingly possible to bring them along to share in the fun. This is great news for many travelers; it eliminates worry about leaving pets behind so trips can be free of this added stress. Airports, airlines and hotels have made it easier to take your dog or cat on a trip; it will require some advance planning, a careful reading of regulations to make sure your pet can travel safely and an additional expenditure. When comparing to the cost of boarding your pets or hiring a pet-sitter to care for your pets in your absence, travel expenditures may be considerably less. You’ll often find the welcome mat rolled out even before boarding. Under federal regulations designed to make them accessible to passengers traveling with service dogs, airports are required to have animal relief areas; those areas are open to all travelers. Atlanta’s airport, for example, offers a fenced-in dog park landscaped with flowers, grass, rocks and benches and biodegradable bags for waste collection. New York’s JFK Airport is building a state-of-the-art terminal for animals that’s scheduled to open next year.

Airlines will accept pets on board for a fee; the rules regarding how your pet can travel will vary. There are different regulations depending on the size and breed of the animal, the type of plane, the destination and even the time of year. For example, Delta Airlines does not accept pets as “checked baggage” from May 15 to September 15. Delta’s Pet First service allows small dogs, cats and household birds to travel in the cabin if they’re at least 10 weeks old and fit in a kennel stored underneath a seat. Other animals, including larger dogs and cats, guinea pigs, hamsters and rabbits, can fly as checked baggage. For some international destinations, pets, with the exception of service animals, must travel as cargo. United Airlines allows cats, small dogs, rabbits and birds in the aircraft cabin on most domestic flights for a service charge of $125 each way. Restrictions apply based on the aircraft, cabin and seat. Passengers with pets cannot be seated in the bulkhead or emergency exit row and only a certain number of pets are permitted on each flight, so it’s important to reserve a spot early. Once you’ve booked a flight for you and your pet, the next step is finding a hotel. Like airlines, major hotel chains have varying policies. Starwood Hotels & Resorts leaves it up to individual brands or properties to decide whether pets can be accommodated. Generally, hotels that accept pets allow cats and smaller dogs--those under 50 pounds--although hotel managers have some discretion. Hotels will charge either a daily fee or a cleaning fee to have a pet in a room. Some require guests to sign a waiver agreeing to pay for the cleanup or repair of any damage caused by their pet. Usually, pets cannot be left in a room unattended; some chains, including Fairmont Hotels & Resorts, offer pet daycare. Hotels that allow pets will often make an effort to pamper your pooch. Some Hyatt properties provide a dog bed and bowl as well as a dog-walk area with clean-up stations. For help planning a vacation with your pet, contact your travel agent; your agent will be knowledgeable on the various pet policies and point you in the right pet-friendly direction. With a little extra planning, your vacation can truly be one for the whole family to enjoy--even the furry family members.

TIMING IS EVERYTHING WORDS ROBERT ALLEN

hether we like it or not, we can all agree that timing and traveling are intertwined. Being “on time” is of the utmost importance--on time to book your flight, on time to check-in for your boarding pass; we all know the differences between “A” and “C” boarding passes as they relate to convenient, comfortable seating. Recently, we had the opportunity to get away to Crested Butte, Colorado, for a family visit, sightseeing and relaxation. Unlike business travel, where your time schedule is at the forefront of all itineraries and decisions, this “schedule-free” trip was a rare luxury. Yet, it was on this trip that we came to appreciate the “timeliness” of travel and realize how lucky we were as to when we scheduled this excursion. Timeliness is defined as “occurring at a suitable time;” I would like to share instances of how timing made our trip special and appreciated. A quick visit with our son and his friends in Denver found us in a locally known pub called The Elm. It was there that the topic of "best green chile breakfast burrito” came up. With a little historical perspective of the many years of immigration and migrant workers coming to Colorado, I knew that some of the greatest green chile dishes were in Denver and the breakfast burrito was very popular. After discussing everyone’s favorite, we pared it down to The Burro King; it was local, small and the breakfast burritos were fantastic. A perfect decision made on an unlikely, yet timely conversation. While the drive to Crested Butte is quite spectacular, especially for a "flatlander," it dawned on us we had picked the perfect time to see the Aspens turn their splashy golden best. This golden explosion happens only for about a week at various times throughout the state. It also brings out many travelers stopping on roadsides for pictures and views which are breathtaking. Future travelers, the best color and the greatest turn of the Aspens may be on County Road 12, from Crested Butte to Kebler Pass. 34

Often, a central focus of traveling is dining out. How often do we get to a restaurant and ask for the soup du jour? We find out it is chicken noodle (pass, we can make it at home), broccoli cheddar (standard), or how about cream of rutabaga with cauliflower? Such was my lucky day at the Coal Creek Grill in Crested Butte. Couple that with the chance evening table by the babbling Coal Creek and you have a beautiful setting. Earlier, a lunch stop at the Eddyline Restaurant and Brewery in Buena Vista brought out a soup du jour of pork pozole. On that day, at that time, I felt fortunate of the timing to taste this long simmering, Mexican inspired stew of pork, chiles and hominy. Ever notice at breakfast when one person orders eggs Benedict with soft yolks and the server waits for the other orders before bringing everything to the table? Please give the waiter approval to bring the eggs Benedict when ready, no need to let the eggs continue cooking under the warming lights…your spouse will appreciate it. Not all timing is positive, rest assured. So an evening reservation at a great restaurant, the Soupcon Bistro, allowed us the chance to sample a four star rated establishment after rave reviews from our hosts and others. Here is the deal…poor timing gets you seated next to the coveted, regular customer. We felt like rookies next to the long time veteran. The waitress had a hard time even noticing we were there until the timely order of a $100 bottle of red. Dinner was delicious, the evening turned out fine, and here is a great tip...wear flannel. I'm really not the kind of guy who assigns extraordinary weight to every little occurrence or tries to tie it into a zenlike meaning of life; no, I am just a guy with a happy countenance and appreciation for the positive. I appreciate when timeliness is on my side, especially when I am traveling and that same timeliness allows me to experience things I may not otherwise have had the opportunities to experience.
