he year is on a roll and so are we with our first edition for 2016. Our medical focus is on Dr Ndiviwe Mphothulo, South Africa’s 2015 Rural Doctor of the Year. We also have a look at physiotherapy and Francois Rossouw lymphedema. We also have some interesting holiday destinations in Zanzibar, Limpopo, Northen Cape and KZN. The Lamborgini and the Lamborgini Huracan is in the spotlight in this issue with their beautiful super car in our motoring section page 10. Fashion make-overs are the order of the day and Shabby Chic show us how it is done. Ann Lee Couture shows off her cocktail dresses in our fashion section on page 34. MESSAGE FROM THE PRESIDENT

Pharmacies today are at a significant point between the conventional drug-dispensing business of the past and a healthcare provider not forgetting the retail sector where pharmacies are not standalone businesses anymore. If you look at the trend is South Africa over the last few years, it is clear that a major mind shift has taken place. The financial survival and wellness of pharmacies relies on the retail element selling fitness, health and beauty products alongside the traditional medicine dispensing counters of pharmacists. While they are subject to legislation, regulation and oversight for their medicine and drug dispensaries, they are also influenced by the same trends and successes that shape the leading national retail pharmacy companies such as Pick n Pay, Dischem, SPAR, Shoprite Checkers and the Clicks Group to mention a few. The trend of healthcare medical services industry to shift their key focus and service offering preventative healthcare approach by promoting a healthier lifestyle through primary care. Community and retail pharmacies are following suit with many adding primary care nursing clinics in their stores, in their quest to offer unparalleled access to innovative, high-quality, affordable health and wellbeing services. In order to remain competitive and loved by their communities, pharmacies are fast adopting to offer total holistic healthcare services compared to before when most visits to the pharmacy was to collect prescriptions. The result of these changes is good news for pharmacists and nurses due the large increase of staff. This is also good news for us in the locum industry where demand is outstripping supply. Proper use of medications is essential to meeting that goal, particularly for those with chronic conditions requiring complex medication therapies. Even a small improvement in chronic disease management would result in substantial savings for your patients. Pharmacists bring comprehensive knowledge on the use of medications for the treatment, management and prevention of diseases to the health care team. As pharmacies become more involved in the health care system patients will no longer need to make an appointment with their general practitioner or doctor for simple tests related to chronic diseases, patient assessments, and disease management. Lamborghini History

Italian Ferruccio Lamborghini founded the company in 1963 with the objective of producing a refined grand touring car to compete with offerings from established marques such as Ferrari. The company’s first models were released in the mid1960s and were noted for their refinement, power and comfort. He was born in Italy in 1916. Lamborghini first manufactured tractors and as Italy’s economy grew, demand for his high quality tractors started to grow. He began building his own tractor engines, but he loved cars as well. Lamborghini decided there was nothing Ferrari was doing he could not exceed. He decided to build his

own car with a V12 engine. For the design he used an engineer named Giampaolo Dallara who had previously worked on a Ferrari V12 engine. His first car was the Lamborghini 350 GT. The Miura was on shown in November 1965 at the Turin Auto Show, represented by Ferruccio Lamborghini himself with great success. In 1973 at the Geneva Auto Show Lamborghini surprised the world with his revolutionary LP400 Countach. After this many Lamborghini supercars followed. On August 4 1998, in a series of complex transactions, Audi AG became the sole owner of Automobili Lamborghini.

YOUR STRESS AND LIFESTYLE CHOICES

ARE YOU FLIRTING WITH BURN-OUT? “Health is a state of physical, mental and social well-being and not merely the absence of disease or infirmity.” This statement is made by the World Health Organisation but I would add the word, emotional, to the above definition too. We are realising (and science is proving) that we are WHOLE and that our mind, emotions and physiology are inextricably linked. The health of the one affects the other. Our mental, emotional and physical health all need to be nourished to ensure overall health and well-being. (a.k.a. A BEING that is WELL) Our level of health affects the way we deal with daily challenges and opportunities. It affects our thinking, resilience, relationships, performance and enjoyment of life. I love the word “nourish”. It feels wholesome, nurturing and healthy. It got me thinking about how we “feed” ourselves every day. My invitation to you is to do a personal audit and start noticing… What are you feeding your mind? How are you nourishing your body? In what ways do you sustain your relationships and community? What conversations and activities fill you up with nutrients? How can you make healthier choices for mind, body and emotional health? Does it serve you to work 15 hour days, 7 days a week? If so, these are “Red Zone” conditions

where adrenalin is released constantly and your cortisol (the stress hormone) levels spike due to your body thinking it is under attack. Your body is “built” for short, quick spikes of cortisol for survival purposes - to outrun the lion chasing you. It is not designed to outrun the imaginary lion (stress) forever by flooding your system with adrenalin 24/7. The body is designed to spend most of its time in the “Green Zone” where we stay healthy, recover, refuel and find homeostasis. Adrenalin is only supposed to be activated when you are in real danger. Increased cortisol can lead to systems within the body shutting down (e.g. reproductive, digestive and respiratory systems) and because your body thinks it is in constant danger, a whole host of symptoms can arise (e.g. IBS, asthma, rashes, hypertension etc). Word on the street, “burnout.” You might win the pitch, get the business but you might run the risk of spending the money you make on therapy, chiropractors, alcohol, television, shopping or endless quick fixes to try and restore the unbalanced you (and your relationships and goals) back to health or escape the illness. How can you find a way to feed all parts of youself so that you feel nourished and healthy? I am not saying therapy, chiros and even the odd glass of wine doesn’t have its place

in promoting health and the enjoyment of life. Just ask yourself, do they form part of how you nourish yourself –how you stay happy and healthy? Or does it only treat the symptoms of an “unhealthy” lifestyle resulting in unhealthy choices which leads to a “not so fun” experience of life? The term “health” is relative. Some people are vegan and others don’t do takeaways during the week. Some people cut out sugar or run marathons. Some people stretch, others meditate or play golf. Some people participate in a book club. We all have different ideas of what health means and what makes us feel good as a holsitic being in body, mind and emotions. My advise to you: “Feast on your Life” You are the head chef in YOUR kitchen. FEED YOURSELF and others with NOURISHING words, food, activities and interactions and spend more time in the GREEN ZONE. olopo Kalahari Lodge is a gem in the Green Kalahari. Situated 190km north of Upington, towards the Botswana and Namibian border on the way to the Kgalagadi Transfrontier Park. Everyone likes a tax break! Bev van Nijkerk, segment specialist for the young professional market at Sanlam says there’s just a month to go before the end of the 2016 tax year and a great opportunity to benefit from a tax deduction by investing as much as you can in a retirement annuity (RA). As a locum professional, you do not have the luxury of working for a big company which will give you the peace of mind of a stable pension or provident fund contribution each month. Furthermore, funds accumulated in a RA are exempt from estate duty and are protected from creditors.

But with the proposed changes in the wings, it is important that professionals get expert advice from an experienced financial adviser and to review their retirement planning portfolio against this. Lymphoedema Tarde

L

ymphoedema, a rare, untreatable condition which could end in amputation of the affected limb is difficult to detect and my diagnosis was made after a long journey visiting various specialists, surgeons and general practitioners. At first I was told by a vascular surgeon that I have vascular oedema, however, after seeing

the lymphoscintigraphy he apologised, confirming the diagnosis as Lymphoedema and suggested “Debulking Surgery�, where the areas affected by Lymphoedema are cut away. While the lymphoscintigraphy showed that over a period of 24 hours there was limited movement of lymph fluid in the lymph nodes of my

right hand and arm, he recommended a pump, which I needed to import but in the same breath he added that he did not have much faith in the device. No doctor would monitor my progress unless I agreed to the debulking surgery. After terminating my treatment with a physiotherapist, which was


simply too painful, I had to face the diagnoses of Lymphoedema: •It is permanent, with NO cure. •It is a sentence, not a diagnosis. •The end of this road is amputation. •The chances of developing cancer in the next 10 years is increased by 95% •My risk to contract cellulitis is increased Despite all of this, I remained hopeful and since the diagnosis, I started reading a lot about Lymphoedema and could not find other patients in South Africa who could give me some insight and advice. A support network with other patients is very important to help one cope and give hope for recovery but in my case there was no one to turn to. I even phoned the medical board of South Africa without success. I discovered an American-based support group on the web, called lymphedemapeople.com, that gave me a lot of valuable information. I contacted Pat O’Connor through his Facebook group to try and find a therapist in South Africa specialising in Lymphoedema. He referred me to Thea van Schoor, a Registered Nurse Practitioner. Thea invited me to a Lymphoedema symposium that was being offered in Cape Town. I was so relieved to at last have somebody who might be able to help me. I was introduced to the

course by an experienced Lymphoedema therapist. She demonstrated massage treatment to the student therapists on me. This treatment was not painful and did not only focus on the right arm but worked on the rest of my body as well. The massage made my skin tingle, this was followed by a process called, bandaging, where specific bandages are wrapped around the entire arm, my arm felt heavy and stiff but I was told that it is temporary. After the bandages were taken off the following day, my fingers were swollen and looked like Russian sausages. The therapist said my hand looked better and felt softer, but my thought was, ‘OK. You probably have to say that’ To me, my fingers felt stiff and my knuckles looked as if they had a ridge of fat. I was very upset. I phoned home and told my husband that I would rather have the amputation, because the pain was unbearable but as a last resort, I decided to complete the treatment. As the day progressed the new bandages felt much better. My arm continued to feel heavy but the pain lessened. I realized I had to be strong and that evening was the first time in a long time that I slept well. To my surprise, treatment was a roller coaster ride with the results fluctuating daily, one day it looked and felt much better, just to be knocked down the next day by intense heaviness, swelling and unbearable pain. There is

also the emotional struggle of staying strong and remaining positive, while people came across as insensitive; they stare or made unwelcome remarks about my right arm that was much bigger than my left arm. Having Lymphoedema in South Africa is difficult. There is little awareness of this illness and much ignorance and thoughtless cruelty, even from medical professionals. A few days ago I fell and tore a ligament in my ankle. In the emergency room, I asked them to use my left arm to measure my blood pressure. The nurse complained that I was being difficult, that the cuff would do no damage to my arm. When I said that it would reduce the risk of injury the nurse accused me of being paranoid and over-protective. The doctor agreed, adding that nothing can be done for Lymphoedema. THE OUTLOOK ON LYMPHOEDEMA HAS TO CHANGE The negative stigma that Lymphoedema is untreatable together with the negative attitude of medical practitioners towards this condition, needs to change. I manage to live with this disease (without amputation) and perhaps there is no medication that can assist with this condition but there are definitely ways to manage it. She answered all my questions and empathized with me. And of course Pat, who cared enough to make sure I got connected with a therapist in South Africa. The diagnosis of Lymphoedema made me stop and think about what matters in life: my wonderful supportive husband, my four loving children, my mother who is always willing to help and my religion, that carries me through each day. Lymphoedema is scary and has been hard work, but I’ve learnt that it is MANAGEABLE. Even though I’ve been told that it is a life sentence, for me it acts as a daily reminder to STOP and count my blessings. I know that I will have my “off-days”, but then I look at my arm and thank God that He gave me a reminder to focus on the good things in my life.

My search in finding answers has brought me into contact with some remarkable individuals. Thea is one of them. She opened the way for me to receive treatment and also gave me hope that life can go on with Lymphoedema. I’ve learned that there are people who

care and I even realised that animals instinctively know when a person needs help and in a strange way offer more compassion and care than a lot of fellow human beings at times. Jane, my therapist, and Pat have meant a great deal to me, aiding me psychologically and emotionally. My therapist has been so patient with me, even though I moaned about the bandages. Speaking to Jane who also has Lymphoedema, made me realize that I am not alone.

Rural Doctor of the Year 2015 By: Monique Warden Coordinator Rural Doctors Association of Southern Africa (RuDASA)

O

ver the 2015 heritage day weekend health professionals from across the country gathered in Dullstroom, Mpumalanga, for the annual Rural Health Conference organised by the Rural Doctors Association of Southern Africa (RuDASA), Rural Rehab South Africa (RuReSA), Professional Association of Clinical Associates (PACASA) and the University of KwaZulu Natal. One of the highlights of the conference is the Pierre Jacques award for Rural Doctor of the Year which has been awarded to rural doctors working at the coal face since 2002. This year the Rural Doctor of the Year award was presented to Dr Ndiviwe Mphothulo from Taung District in North West Province. Dr Mphothulo’s dedicated work in the service of TB patients and the fight against TB in Taung district is truly outstanding. It all began in 2003 when this Soweto born and bred junior doctor was placed in Taung District hospital for his community service year. Since then he’s never looked back and his passion for public health has taken him to the forefront in the fight against TB in the community, starting with the TB ward at the hospital. Dr Mphothulo has been instrumental in developing protocols, setting up a TB/HIV co-infection treatment program with more than 95% collaboration, providing telephonic advice to local clinics and medical officers from other hospitals in the district and liaising with the South African Social Services Agency (SASSA) for grant applications and home-based care groups on patient followup. Dr Mphothulo also took up the challenge of decentralising MDR-TB treatment by managing the satellite unit at Taung District Hospital, which went from 20 beds to 33 beds in January 2011 and is close to achieving the goal of curing 100 patients by the end of this year. In addition to this, he is also the author of a

Dr Ndiviwe Mphothulo book titled, “TB cases from Taung – A perspective from a rural district hospital” which highlights the management of difficult TB cases in a rural setting with scarce resources. In his acceptance speech, Dr Mphothulo was gracious in acknowledging that his achievements would not have been possible without a supportive team of health care workers and the political will of the district health services. He reminded us that health care is a long supply chain that begins with the Constitution, passes down to the President and Ministers and then to MECs and health facilities before finally making a difference in the lives of patients. ow does physiotherapy work and what is the evidence for these claims? What makes your body move, breathe, feel, function, do all the things that come naturally to a living body, from walking to giving birth to singing to climbing stairs? Physiotherapists are experts at ensuring your body – comprising muscles, bones, joints and nerves, and even your lungs and heart – is able to function at the best possible level. To achieve this, physiotherapists use a wide range of skills, techniques and knowledge acquired and honed over four years of academic and clinical study. One leg of physiotherapy treatment is passive mobilisation of the joints, muscles and neural structures of your body, after they have assessed each individual patient and developed a treatment plan. These techniques by themselves have a measurable demonstrated effect on conditions such as chronic low back pain, for example, as demonstrated in a study by Aseer (2013): ‘Effectiveness of Integrated Soft Tissue Mobilization on the Functional Outcome in Chronic Low Back Pain Patients’. However these passive treatment techniques are only one part of the physiotherapist’s job. While assessing and treating the patient, the physiotherapist will be devising and supervising an exercise

plan, which the patient can carrie out at home. These will include carefully planned exercises that strengthen muscles, improve endurance and mobilise joints. A combination of passive treatment techniques and exercise, tailored to the patient’s condition, has been proven to be effective in relieving pain and restoring function in a wide range of conditions, like patello-femoral pain (Barton et al 2015), osteoarthritis of the knee (Deyle et al 2005), neck pain (Miller et al 2010), low back pain (Aure et al 2003; Geisser et al 2005), and many more. Physiotherapists understand the value of holistic exercise. Exercise isn’t just a tool to strengthen and stabilise the body, and it isn’t just about improving the function of muscles, tendons and joints! Physiotherapy training includes insight into the role exercise plays in reducing pain,


fear, anxiety, depression and many other psychosocial issues. Exercise improves your mood, and research shows that exercising with a physiotherapist could be a very effective treatment for depression. (Dunn et al 2005). The final tool in the physiotherapy tool-kit is education. Perhaps the physiotherapist’s most important role is in that one-onone interaction, where they help patients understand themselves and their pain or debility. “The impact of our words on the patient’s nervous system is much more relevant and important, than the passive treatments we do,” says Dr Ina Diener, president of the South African Society

of Physiotherapy (SASP). “If you don’t make a therapeutic connection with your patient, so that they trust you and the information you provide and understand their condition – or even worse, if your explanation engenders fear of movement in them – it doesn’t matter how good your passive treatments and your exercise programme is, your outcomes will not be good. Physiotherapists have learned that the connection you have with your patient is everything.” Psychosocial factors have been demonstrated to play an important role in the onset and progression of chronic pain. Pain-related fear contributes

It is therefore important to explain a patient’s pain in terms of the neurophysiology of pain (Louw et al 2014), rather than in bio-anatomical terms. Physiotherapists are trained to do Therapeutic Neurophysiological Pain Education (Louw, Diener et al 2014). Physiotherapists may have extended skills in particular fields like Womens Health, Sport Injuries, Cardiopulmonary Rehabilitation, Neuro-rehabilitation, Orthopaedic Manual Therapy, Pain Management, Animal Health, Aquatic Exercise, Paediatrics or Geriatrics. “As first-line practitioners, who can be consulted without referral from another medical practitioner and who work in primary, secondary and tertiary healthcare settings, we must be ready for every patient who walks through our doors, from the earliest to the final phase of healing” says Dr Diener. “A 70-year-old pensioner recovering from a total knee replacement, a middle aged person who’s had chronic low back pain for some years, a woman struggling with stress-incontinence, or a young football player who pulled a hamstring two days ago – they all need a tailored combination
